A case of signet ring cell gastric carcinoma initially presenting with disseminated bone marrow metastases

Abstract
Bone metastases from gastric carcinoma are reported in up to 17% of cases. However, disseminated bone metastasis is very rarely the first symptom of gastric carcinoma. We present a 63-year-old male who had disseminated bone metastases as a symptom of signet ring cell gastric carcinoma. He came to the clinic complaining of back pain and dizziness that had started 6 months earlier. Blood tests showed pancytopenia, and the alkaline phosphatase (ALP) was elevated 10 times higher than normal. Magnetic resonance imaging of the thoracic and lumbar spine and the entire skeleton was subsequently performed, and severe bone marrow infiltration was found. A bone marrow biopsy showed metastatic infiltration of signet-ring-shaped carcinoma. We performed positron emission tomography to find the primary site, and hypermetabolism on the stomach wall implied that the primary site was the stomach. (Korean J Med 75:S787-S791, 2008)
