Iatrogenic Kaposi’s sarcoma in a patient with idiopathic thrombocytopenic purpura following corticosteroid therapy

Abstract
A 58-year-old man presented with a several-day history of multiple asymptomatic black-purplish papules and plaques on both forearms and thighs. He had been taking prednisone 80 mg daily, for 2 months for idiopathic thrombocytopenic purpura. He had no history of other medical problem, except diabetes mellitus, or high-risk sexual contact. Physical examination showed multiple, ill-defined, variable-sized, black-purplish papules and plaques on both forearms and thighs. He was HIV-seronegative. Skin punch biopsy showed spindle-shaped cells that delineated abnormal, slit-like vascular spaces that contained erythrocytes. A diagnosis of iatrogenic Kaposi’s sarcoma was made. Whole-body PET/CT revealed ill-defined nodules in both lungs, hard and soft palate, and right inguinal region. Prednisolone was stopped and chemotherapy with etoposide was given for Kaposi’s sarcoma. After the third course of chemotherapy, the lesions show marked improvement in size and number.
