A case of granulomatous hepatitis probably due to primary hepatic tuberculosis |

Abstract |
Primary hepatic tuberculosis is a very rare manifestation of tuberculosis. The diagnostic criteria for primary hepatic
tuberculosis include evidence of a recent tuberculosis infection, demonstration of tuberculosis in the liver and the absence
of old or current tuberculous lesions elsewhere, with clinical and radiological exclusion of extrahepatic tuberculosis. We
experienced a patient who presented with a fever of unknown origin. Laparoscopic liver biopsy showed noncaseating
granulomas and stains for AFB, and the results of PCR for tuberculosis were negative. We examined the possible causes
of hepatic granulomas but could not find evidence of any other cause. Considering the high prevalence of tuberculosis
and positive PPD tests in Korea, we tried to treat our patient with an antituberculous agent.
After the treatment, his fever and other symptoms rapidly subsided and the abnormal findings on liver tests returned
to normal. After about 3 months of treatment, a percutaneous liver biopsy was taken, and the histology revealed the
disappearance of hepatic granulomas.(Korean J Med 73:S925-S929, 2007) |
