A case of diabetic ketoacidosis associated with long-term risperidone use

Abstract
The atypical antipsychotics used to treat patients with schizophrenia may cause many metabolic problems, such as weight gain, hyperlipidemia, diabetes mellitus (DM) and, rarely, diabetic ketoacidosis (DKA). We report the first case of new-onset type 2 DM and DKA associated with long-term risperidone use in Korea. A 42-year-old man visited our hospital complaining of polydipsia, polyuria, polyphagia, generalized weakness, and weight loss. He had been taking risperidone (4 mg/day) to treat schizophrenia for 7 years. Based on laboratory tests, dyslipidemia and new-onset type 2 DM accompanied by DKA were diagnosed. He was treated immediately with intravenous hydration and insulin therapy, and recovered completely. When treating patients with risperidone, it is important to monitor their weight, blood pressure, fasting blood glucose, and lipid profile carefully because risperidone may lead to type 2 DM or, rarely, DKA.
