A case of tuberculosis empyema necessitatis presented as a cutaneous fistula of the breast and chest wall mass

Abstract
Empyema necessitatis is a collection of inflammatory tissue that usually extends directly from the pleural cavity to the thoracic wall, forming a mass in the extrapleural soft tissues. Mycobacterium tuberculosis is responsible for about 70% of empyema necessitates cases. We report here the case of a 31-year-old female who presented with a purulent discharge from a cutaneous fistula of the breast and an enlarging, painful mass in the left posterior chest wall. A chest CT scan revealed a necrotic cavity and multi-loculated pleural effusion in the left hemithorax with several destroyed bones. Empyema necessitatis caused by tuberculosis was confirmed bacteriologically and histologically by the demonstration of chronic granulomatous inflammation with extensive caseous necrosis in the empyemal sac through thoracotomy. Anti-tuberculous medication was initiated, followed by empyemectomy and bone reconstructive surgery.
