A case of gastric granulomatous inflammation in scrub typhus

Abstract
Scrub typhus, an acute febrile illness caused by Orientia tsutsugamushi-induced vasculitis, is common in Korea and Asia. Endoscopic mucosal lesions have rarely been reported with Scrub typhus. We experienced gastric mucosal erosion by Scrub typhus in a 51-year-old male patient. The patient was admitted to another hospital with a fever and myalgia over the past 3 months, and his disease was diagnosed as Scrub typhus. During admission, the patient presented with epigastric discomfort. An esophagogastroduodenoscopy (EGD) showed gastric mucosal erosion. A chest X-ray, abdominal computerized tomographic scan, and a colonoscopy were unremarkable. After the patient improved with doxycycline administration, the patient visited our hospital complaining of epigastric discomfort. An EGD showed erythematous mucosal erosion at the midbody of the stomach, and a biopsy revealed a non-caseating granuloma. The M. tuberculosis polymerase chain reaction and acid fast bacilli staining of the biopsy specimen were negative. A follow-up EGD 8 months later showed no mucosal lesion at the stomach midbody.
