Two cases of tsutsugamushi disease complicated with acute acalculous cholecystitis

Abstract
We report two cases of acute acalculous cholecystitis associated with scrub typhus. The patients exhibited eschar and general rash with high titer of anti-Rickettsia (R)-tsutsugamushi antibody, fever, myalgia, severe right upper quadrant abdominal pain, nausea, and dyspepsia. Whole abdomen ultrasonography and computed tomography revealed eccentric gall bladder wall thickening, but did not reveal a stone. The patients were successfully treated with appropriate antibiotic therapy following early diagnosis with physical examination and radiology studies. Operation was not required. Acute acalculous cholecystitis is not a common complication in patients with scrub typhus. We suggest that scrub typhus also be included as one of the causes of acute acalculous cholecystitis and report other cases of acute acalculous cholecystitis caused by scrub typhus.
