A case of primary pericardial effusion lymphoma |

Abstract |
Primary effusion lymphoma (PEL) is a recently-defined, rare type of non-Hodgkin's lymphoma that is characterized by involvement of body cavities without a definite mass or lymphadenopathy. Human herpes virus-8 (HHV-8) and Epstein-Barr virus (EBV) have been recognized as playing a pathogenic role in the development of PEL in immunocompromised patients such as human immunodeficiency virus (HIV) patients or the elderly. Prognosis is very poor. We report the case of a 71-year-old man with dyspnea for 4 days who showed a large amount of pericardial effusion, without organ involvement, on his chest CT. Pericardial fluid cytology showed B-cell non-Hodgkin's lymphoma expressing CD45 and CD20. PET-CT scan after pericardiocentesis revealed no abnormal findings. The result of serologic analysis for HIV was negative. He was treated with 6 cycles of CHOP (cyclophosphamide, adriamycin, vincristine, prednisolone) chemotherapy. Follow-up PET-CT scan and chest CT scan 27 months later showed complete response and absence of pericardial effusion. |
