Management of a fractured basket and bile duct stone by percutaneous transhepatic cholangioscopy

Abstract
One of the unusual complications of endoscopic retrograde cholangiopancreatography (ERCP) is breaking the basket traction wire. We report here on a case of basket traction wire breakage along with an impacted large common bile duct (CBD) stone in a 72-year-old woman. The patient was previously admitted to another hospital with choledocholithesis and endoscopic removal of stone was attempted, but the basket traction wire was broken during ERCP and the basket with impacted stone was retained in the extrahepatic bile duct. Emergency percutaneous transhepatic biliary drainage (PTBD) was done and the patient was transferred to our hospital. The ERCP image and contrast enhanced computed tomography showed the retained basket in the extrahepatic bile duct and a large impacted CBD stone. After dilatation of the PTBD tract, we attempted percutaneous transhepatic cholangioscopy (PTCS) for removal of the basket and the impacted CBD stone. The basket and impacted stone were successfully removed.(Korean J Med 71:S989-S993, 2006)
