- Botryomycosis는 과립상 물질을 형성하는 만성 화농성 육아종 감염 질환이고 세균이 원인이며 조직학적으로 진균종, 방선균균종, 방선균증과 감별된다. Botryomycosis의 발생에 있어 세균의 독성에 대한 숙주의 국소적인 면역체계 이상이 원인이 될 수 있으나 아직 명확하게 밝혀진 것은 없다. 내장형 botryomycosis는 비특이적인 증상을 보이고 특징적인 방사선 소견이 없어 종양으로 오진되기도 하므로 진단에 주의를 요한다. 지금까지 전 세계적으로 사람에서 발생한 botryomycosis는 200예 이상의 보고가 있었으나 비장을 침범한 사례는 2예밖에 없었으며 우리나라에서 보고된 증례는 없었다. 이에 저자들은 면역 기능이 정상인 53세 여자에서 위암 진단 후 시행한 위절제술에서 합병된 Streptococcus mitis에 의한 botryomycosis의 비장 침범 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

- Abstract
- Botryomycosis is an uncommon chronic suppurative bacterial infection of the skin and visceral organs seen primarily in immunocompromised patients. Here, we report a case of splenic botryomycosis caused by Streptococcus mitis in a 53-year-old immunocompetent woman with a history of distal gastrectomy for advanced gastric cancer.
