- Abstract
- The hepatic cytochrome P450 (CYP3A4) converts clopidogrel into an active thiol metabolite that binds specifically and irreversibly to the P2Y12 receptor, effectively inhibiting ADP-induced platelet aggregation. Current percutaneous coronary intervention (PCI) guidelines recommend the obligatory use of dual antiplatelet therapy (clopidogrel and aspirin) in patients undergoing PCI. The relationship between ticlopidine and liver injury has been widely reported, while clopidogrel-induced hepatic toxicity is very rare. We report the case of a 51-year-old woman with stable angina who presented with clopidogrel-induced hepatotoxicity.
