- Abstract
- The gastrointestinal stromal tumor (GIST) is the most common mesenchymal tumor of the gastrointestinal tract; it
expresses the C-kit protein. GIST of the stomach is usually not accompanied by specific symptoms or such as abdominal
pain or upper GI bleeding. However, rarely hemoperitoneum due to tumor rupture occurs. We treated a case of
hemoperitoneum caused by a ruptured stomach GIST in a 47-year-old woman that presented with abdominal pain. An
abdominal CT demonstrated a ruptured submucosal mass of the stomach with high attenuation of a fluid collection in
the peritoneal cavity, suggesting a hemoperitoneum. Esophagogastroduodenoscopy showed a protruding submucosal mass
with intact overlying mucosa and submucosal hemorrhage at the gastric cardia. Endoscopic ultrasonography revealed a
submucosal tumor with necrosis originating at the second or third layer of the stomach wall. A radical total gastrectomy
was performed. Evaluation by histopathology and immunohistochemical examination showed hemorrhagic necrosis,
normal gastric mucosa and proliferation of the C-kit positive spindle-shaped cells. Therefore, we diagnosed the
hemoperitoneum as a ruptured GIST of the stomach.(Korean J Med 73:S866-S870, 2007)
