A case of intraarterial thrombolytic therapy of acute renal infarction

Abstract
Acute renal artery thromboembolism is a relatively common cause of acute renal failure, but the appropriate therapy remains controversial. Recent reports have suggested that infusion of intra-arterial thrombolytic agents can be used for the treatment of acute renal artery thromboembolism, but the ultimate value of this procedure remains uncertain. A 71-year old woman with mitral stenosis and atrial fibrillation was admitted for treatment of right flank pain and fever, but the flank pain did not subsided with antibiotics. An abdominal computed tomography (CT) scan demonstrated a wedge shaped low density of the right kidney, and renal angiography disclosed filling defects in the right renal artery. Urokinase 500,000 IU was infused through the catheter; the arteriogram done after thrombolysis demonstrated disappearance of the filling defects. Low molecular weight heparin was administered until an adequate oral dose of warfarin could be achieved. The flank pain was relieved and the serum creatinine level returned to normal. The patient has remained free of symptoms and she has preserved renal function during a 8-month follow-up period.(Korean J Med 71:S1049-S1053, 2006)
