A case of ileal lipoma resulting in intussusception

Abstract
Lipomas in the small bowel are rare and they occur with the third frequency as benign small bowel tumors. Most of them are asymtomatic until significant complications develope such as intussusception, obstruction or hemorrhage. As it is not distinct from carcinoma in the clinical picture, making the preoperative diagnosis is very important so as to avoid unnecessary radical resection in high risk patients. For an alternative surgical method, if the patient does not have any risk factors such as carcinoma or gangrenous bowel change, then intussusception in the small intestine can be treated with initial reduction and resection guided by a laparoscope. In this study, a 28-year-old male with intermittent severe abdominal pain visited the ER, and he was diagnosed as having ileo-ileal intussusception caused by submucosal lipoma with a characteristic low fat density, according to CT. In this case, gangrenous change had already progressed, as noted on the laparoscopic exam, so laparoscopic reduction could not be applied. Therefore, the procedure was converted to open abdominal surgery and small bowel segmental resection was done. The patient was discharged on the fourteenth postoperative day. There were no postoperative complications.(Korean J Med 71:S945-S950, 2006)
