요약

목적

혈소판의 수와 특성은 항혈소판제의 혈소판 반응성에 영향을 미칠 수 있다. 본 연구는 클로피도그렐과 티카그렐러 치료를 받는 관상동맥 질환 환자를 대상으로 혈소판 지표들과 혈소판 반응성과의 관계를 비교하였다.

방법

관상동맥 질환으로 중재시술을 받는 환자들을 대상으로 클로피도그렐군(63명), 티카그렐러군(65명)으로 나누어 약물을 투여하였다. 약물 투여 전, 시술 후 48시간째, 30일째 혈액을 채취하여 혈소판 수, 평균혈소판용적, 혈소판분포폭, 혈소판 거대세포 비율, 미숙혈소판분획 그리고 혈소판 반응성을 측정하였다. 높은 치료 중 혈소판 반응성(HPR)은 자동 혈소판 응집 측정장치에서 측정값 47 U 이상인 경우로 정의하였다.

결과

기저 혈소판 반응성은 두 군 간의 차이가 없었으나 48시간째, 30일째 혈소판 반응성은 티카그렐러군에서 유의하게 낮았다. 30일째 혈소판 반응성과의 상관관계는 클로피도그렐군에서 백혈구 수, 혈소판 수, 평균혈소판용적, 혈소판분포폭, 혈소판 거대세포 비율 그리고 미숙혈소판분획 모두에서 유의한 상관관계를 보였으나 티카그렐러군에서는 상관관계가 없어지거나 작아졌다. 전체 환자에서 클로피도그렐의 사용(HR 4.1, 95% CI 1.4-11.9, p = 0.010)과 혈소판 수 (HR 9.7, 95% CI 2.9-32.7, p = 0.001)가 30일째 HPR을 예측하는 독립적인 위험 요인이었다. 클로피도그렐군에서는 혈소판 수가 독립적인 위험 인자였으나 티카그렐러군에서는 위험 인자가 되지 못하였다.

결론

혈소판 수와 특성들은 혈소판 반응성과 연관성이 있으나, 티카그렐러로 치료받는 환자에서는 영향을 주지 못한다.

Abstract

Background/Aims

Platelet counts and characteristics can influence platelet reactivity during antiplatelet therapy. We compared the effects of both platelet count and indices on platelet reactivity between patients who were treated with either clopodogrel or ticagrelor.

Methods

Patients with coronary artery disease who underwent percutaneous coronary intervention were randomly assigned to either the clopidogrel (n = 63) or ticagrelor (n = 65) groups. Platelet count, platelet indices (including mean platelet volume, platelet distribution width, platelet large cell ratio, and immature platelet fraction), and platelet reactivity were measured before intervention, and 48 hours and 30 days post-intervention. High on-treatment platelet reactivity (HPR) was defined as ≥ 47 unit as assessed by multiple electrode platelet aggregometry.

Results

Baseline platelet reactivity was similar between the two groups; however, at 48 hours and 30 days, platelet reactivity was significantly lower in the ticagrelor group than in the clopidogrel group. Platelet count, mean platelet volume, platelet distribution width, platelet large cell ratio, and immature platelet fraction were significantly correlated with platelet reactivity in the clopidogrel group; however, these correlations were attenuated in the ticagrelor group. The use of clopodogrel (hazard ratio [HR] 4.1, 95% confidence interval [CI] 1.4-11.9; p = 0.010) and platelet count (HR 9.7, 95% CI 2.9-32.7; p = 0.001) were independent predictors for 30 day HPR. Platelet count was an independent predictor of HPR in the clopidogrel group but not in the ticagrelor group.

Conclusions

Platelet count and indices are significantly correlated with platelet reactivity. However, antiplatelet treatment with ticagrelor could overcome these associations.

서 론

혈소판 P2Y12 수용체 억제제에 대한 비반응성, 즉 높은 치료 중 혈소판 활성도(high on-treatment platelet reactivity, HPR)가 급성관상동맥증후군이나 관상동맥 중재시술을 받은 환자의 불량한 예후와 관계가 있다고 알려지고 있다[1-4]. HPR에 관련된 요인으로는 급성관상동맥증후군, 심부전증, 만성 콩팥병과 당뇨병 등의 임상적 요인이 알려져 있다[5,6]. 그러나 혈소판 자체의 숫자나 여러 가지 지표들, 즉 평균혈소판용적(mean platelet volume), 혈소판분포폭(platelet distribution width), 미숙혈소판분획(immpature platelet fraction)등이 혈소판 반응성에 영향을 미칠 수 있다[7,8].

티카그렐러(Brilanta^®, AstraZeneca, Cambridge, UK)는 최근 사용되는 강력한 항혈소판제로 대규모 임상 시험을 통해 출혈의 위험은 높이지 않으면서 클로피도그렐(Plavix^®, Sanofi, Paris, France)에 비해 주요 심혈관 사건의 발생을 줄이는 것으로 나타난 약물이다[9]. 따라서 저자들은 강력한 항혈소판제 치료가 혈소판 특성에 따른 혈소판 반응성의 차이를 극복할 수 있을 것으로 가정하였다. 본 연구는 관상동맥 중재시술을 받은 환자들을 대상으로 클로피도그렐과 티카그렐러를 투여하고 30일째 혈소판 반응성과 혈소판 수, 혈소판 지표들을 비교하였다.

대상 및 방법

대상

본원에서 관상동맥 질환으로 스텐트 시술을 받는 환자를 1:1로 무작위 배정하여 클로피도그렐과 티카그렐러를 투여하였다. 시술 전 각각의 약물을 부하 용량으로 투여하였고, 이후에는 1일 유지 용량을 투여하였다(clopidogrel 75 mg/day, ticagrelor 180 mg/day). 스텐트 시술을 하지 않은 환자, 기존에 혈소판 P2Y12 수용체 억제제를 복용하고 있는 경우, 80세 이상, 뇌출혈의 기왕력이 있는 경우, 혈액 질환으로 치료받는 환자, 혈색소가 10 g/dL 이하, 혈소판 수치가 140,000/μL 이하 또는 400,000/μL 이상인 경우는 제외하였다. 모든 환자는 기관생명 윤리위원회의 승인을 얻은 동의서에 서명한 이후 치료에 임하였고, 30일 동안 추적 관찰되었다(201603-HRE-031).

각 군당 70명의 환자가 등록되었으나 30일 이전에 주요 심장 사건이 발생한 경우 2명, 동의 철회 1명, 추적 소실 4명, 30일째 혈액 검사 거부 1명이 있어 최종적으로 클로피도그렐군 63명, 티카그렐러군 65명의 환자에서 연구를 종료할 수 있었다.

혈소판 지표와 혈소판 반응성 측정

혈액 채혈은 ethylenediamine tetraacetic acid 용기를 이용하여 항혈소판제 부하 용량 투여 전, 시술 후 48시간, 30일째 시행하였다. 혈소판 지표는 자동화된 혈구측정기(XE2100, Sysmex, Kobe, Japan)를 이용하여 혈소판 수, 평균혈소판용적, 혈소판분포폭, 혈소판 거대세포 비율(platelet large cell ratio) 그리고 미숙혈소판분획을 측정하였다.

혈소판 반응성은 자동 혈소판 응집 측정장치(multiple electrode platelet aggregometry [MEA], Multiplate analyzer, Roche Diagnostic GmbH, Mannheim, Germany)를 이용하여 시행하였다. 항혈소판제 투여 전, 시술 후 48시간, 30일째 adenosine diphosphate에 의한 혈소판 응집 정도를 unit (U)로 표시하였다. 기존 보고에 의해, 47 U 이상을 HPR로 정의하였다[10].

통계 분석

기존 연구에 따라 혈소판 수, 평균혈소판용적 그리고 미숙혈소판분획의 표준편차를 10%로 가정하였고, 1종 오류 0.05, 2종 오류 80%로 가정하였다[8]. 따라서 혈소판 지표들의 중앙값 또는 삼분위 값에 따른 혈소판 반응성의 차이를 알아보는데 최소한 군당 60명의 환자가 필요하였다. 측정 값들은 평균 ± 표준편차 또는 수(%)로 표기하였다. 통계 분석을 위해 SPSS 소프트웨어 version 19.0 (SPSS Inc., Chicago, IL, USA)를 사용하였다. 각 군 간의 비교는 t-test 및 chi-square test를 통해 비교하였고, 혈소판 지표와 혈소판 반응성 간의 상관관계는 pearson correlation test를 이용하여 비교하였다. 30일째 HPR을 예측하는 독립적인 위험 인자를 알아보기 위해 다변량 분석을 시행하였다. 단변량 분석에서 p< 0.1인 변수를 이용하여 시행하였으며, 연속변수의 경우 상위 삼분위수를 이용하였다. p 값이 0.05 미만인 경우 통계적으로 유의한 값으로 판단하였다.

결 과

대상자들의 특성

전체 환자의 기저 혈소판 반응성은 85.5 ± 35.7 U, 48시간째 32.4 ± 22.2 U, 30일째 34.0 ± 20.8 U였다. 치료군 간의 나이, 성별, 위험 인자, 진단명은 차이가 없었다(Table 1). 기저 혈소판 반응성은 두 군 간의 차이가 없었으나 48시간째, 30일째 혈소판 반응성은 티카그렐러군에서 유의하게 낮았다. 따라서 30일째 HPR은 티카그렐러군에서 클로피도그렐군에 비해 낮았다(28.6% vs. 13.8%, p= 0.052).

혈소판 반응성과 혈구 지표들과의 상관관계

전체 환자에서 혈소판 수는 혈소판 반응성과 유의한 상관관계를 보였다. 백혈구 수, 평균혈소판용적, 혈소판 분포 폭, 혈소판 거대세포 비율 그리고 미숙혈소판분획은 검사 시점에 따라 혈소판 반응성과 상관관계를 보이기도 하고 그렇지 않기도 하였다.

30일째 혈소판 반응성과의 상관관계는 클로피도그렐군에서 백혈구 수, 혈소판 수, 평균혈소판용적, 혈소판분포폭, 혈소판 거대세포 비율 그리고 미숙혈소판분획 모두에서 유의한 상관관계를 보였다(Table 2). 그러나 티카그렐러군에서는 상관관계가 없어지거나 작아졌다.

30일째 혈소판 수와 혈소판 지수들을 삼분위하여 혈소판 반응성과 비교하였을 때 오직 혈소판 수만이 클로피도그렐군에서 유의한 관계를 보였다(Fig. 1). 티카그렐러군에서는 모든 지표가 삼분위에 따라 증가하여도 혈소판 반응성에 영향이 없었다.

30일째 HPR에 영향을 미치는 인자에 대한 다변량 분석

전체 환자에서 클로피도그렐의 사용(hazard ratio [HR] 4.1, 95% confidence interval [CI] 1.4-11.9, p= 0.010)과 혈소판 수(HR 9.7, 95% CI 2.9-32.7, p= 0.001)가 30일째 HPR을 예측하는 독립적인 위험 요인이었다(Table 3). 그러나 각 군 간의 분석에서 클로피도그렐군에서는 혈소판 수가 독립적인 위험인자였으나 티카그렐러군에서는 위험 인자가 되지 못하였다.

고 찰

본 연구에서 혈소판 수와 특성들이 혈소판 반응성과 연관이 있었다. 특히 혈소판 수가 30일째 HPR의 독립적인 위험인자였다. 그러나 티카그렐러로 치료받은 환자에서는 연관성이 없었고, 혈소판 수가 독립적인 위험 인자도 아니었다.따라서 강력한 항혈소판 치료가 중재시술을 받은 환자에서는 필요할 것으로 생각된다.

급성관상동맥증후군 및 스텐트 시술 후 치료의 필수적인 요소는 항혈판제 치료이다. 과거 아스피린과 클로피도그렐의 병합요법이 표준으로 사용되었고 최근에는 프라수그렐이나 티카그렐러와 같은 새로운 약제들이 소개되고 있다. 항혈소판제 치료에도 불구하고 충분히 혈소판 억제가 일어나지 않는 경우가 보고되고 있는데 이를 HPR 현상이라 한다[1-4,11]. HPR 현상은 클로피도그렐 사용시 약 20-30%까지도 보고되고 있으며, 이는 스텐트 혈전증이나 심장 관련 사건의 발생과 관련이 있다. 최근의 연구에 의하면 HPR을 보이는 환자들은 더 심하고 광범위한 동맥경화성 질환을 가지고 있으며 병변의 형태도 취약한 동맥경화반인 섬유죽종의 형태를 보인다고 하였다[12-14]. 또한 흡연자, 당뇨병, 만성콩팥병을 가진 환자들에서 HPR을 보이고 있어 환자의 기본특성이 위험 인자가 많고 불량한 예후를 갖는 경우가 많다[5,6]. 새로이 개발된 강력한 항혈소판제를 사용할 경우 HPR의 빈도는 감소하는 것으로 알려져 있으나 완전히 위험이 없어지는 것은 아니다. 따라서 HPR의 위험 인자를 파악하고 맞춤형 치료를 시행하는 것이 매우 중요하다.

혈소판은 그 자체로 혈전 형성에 관여할 뿐만 아니라 동맥경화의 발생과 진행에 중요한 역할을 한다[15]. 병적인 상황에서 혈소판이 매우 증가하는 환자들에서 혈전성 질환의 위험이 증가하는 것은 잘 알려져 있다. 그러나 정상 범위의 혈소판 수치를 가지는 환자에서 혈소판 숫자가 혈전 형성과 관계가 있는지는 논란이 있다. 최근의 보고에 의하면 오히려 혈소판의 특성을 나타내는 지표들, 즉 평균혈소판용적, 혈소판분포폭, 혈소판 거대세포 비율, 미숙혈소판분획 등이 혈전생성이나 급성관상동맥증후군 환자의 주요 심장 사건의 발생과 연관이 있다고 한다[16-19].

혈소판 지표들과 혈소판 반응성과의 관계는 아직 논란이 있다. Kim 등[7]은 클로피도그렐 치료를 받는 환자들을 대상으로 평균 혈소판 용적이 혈소판 반응성과 관련이 있다고 보고하였다. Guthikonda 등[8]도 클로피도그렐 치료를 받는 환자들에서 미숙혈소판비율과 혈소판 크기가 혈소판 반응성에 영향을 미친다고 보고하였다. 그러나 클로피도그렐 치료를 받는 환자에서 혈소판 거대세포 비율은 혈소판 반응성과 무관하다는 보고도 있으며[20], 티카그렐러로 치료받는 환자에서는 미숙혈소판비율이나 혈소판 크기는 혈소판 반응성에 영향을 미치지 않는다는 보고도 있다[21,22]. 본 연구에서는 혈소판 숫자뿐만 아니라 평균혈소판용적, 혈소판분포폭, 혈소판 거대세포 비율, 미숙혈소판분획과 같은 혈소판 지표들이 모두 혈소판 반응성과 연관성이 있었다. 그러나 치료 약제에 따라 구분하였을 때 클로피도그렐군에서만 연관성이 나타났고, 티카그렐러군에서는 연관성이 없어지거나 약해졌다. 일부 연구에서 프라수그렐로 치료받은 환자에서는 미숙혈소판과 혈소판 반응성 간의 연관성이 유지된다고 보고하고 있는 바 치료 약제에 따라 연관성이 다르게 나타날 수 있다[23].

본 연구의 제한점은 비록 통계적 방법을 사용하였으나 표본 수가 적다는 것이다. 통계적 방법은 실제 임상 상황을 모두 반영할 수 없으며, 혈구 숫자는 임상 상황에 따라 많은 변이를 보일 수 있기 때문에 충분히 많은 숫자의 환자가 필요할 것이다. 또한, 환자의 진단이 통일되지 않고 안정형 협심증부터 심근경색증까지의 모든 환자가 포함되었다는 것이다. 그러나 저자들은 진단에 따른 변이를 줄이고자 30일째 혈소판 반응성을 연구 종결점으로 정하였다. 또한 30일째 혈소판 반응성이 장기 예후와 어떤 관련이 있는지 알지 못한다. 혈소판 수나 지표들이 혈소판 반응성과 연관되어 예후에 영향을 미치는지, 항혈소판제 치료가 이를 반영하여 실제 예후를 개선하였는지는 본 연구로는 알 수 없다.

결론적으로 혈소판 수와 특성들은 30일째 혈소판 반응성과 연관성이 있었으나 티카그렐러 치료는 이 연관성을 약화시켰다. 티카그렐러 치료가 혈소판 반응성을 개선시켜 장기예후를 개선시키는지에 대해 더 많은 연구가 필요할 것이다.
