A case of reactive arthritis with osteitis condensans ilii

Abstract
Reactive arthritis that develops after infections outside a joint can manifest as asymmetrical arthritis of the lower extremities or inflammatory back pain. Osteitis condensans ilii (OCI) is characterized by intermittent low back pain or buttock pain, but the diagnosis is based on radiographic findings that mimic the radiographic changes of spondyloarthropathy. We describe a 28-year-old female suffering from reactive arthritis. Her back pain was intermittent and was not relieved by exercise. Bilateral sclerotic changes of the iliac bone adjacent to the sacroiliac joint suggested that her symptoms were related to reactive arthritis. Since the back pain was not inflammatory, we performed computed tomography in the hope of finding another disease. The absence of joint space narrowing and erosion confirmed that the radiographic change was not the result of reactive arthritis, but the result of OCI. (Korean J Med 75:S926-S929, 2008)
