A case of metastatic sarcomatoid carcinoma presenting as a small polypoid lesion of the duodenum

Abstract
Sarcomatoid carcinoma consisting of carcinomatous and sarcomatous elements is a rare malignant neoplasm. Various terms have been applied in reference to these neoplasms and include carcinosarcoma, pseudosarcomatous carcinoma, pseudosarcoma, spindle cell carcinoma, and others. In the gastrointestinal tract, sarcomatoid carcinoma has been described in the oral cavity, pharynx, and esophagus. However, below the esophagogastric junction, this tumor is extremely rare. Sarcomatoid carcinoma has a worse prognosis compared with adenocarcinoma of the gastrointestinal tract, which is much more common. We report a case of a 55-year-old man with epigastric pain and nausea. Upper gastrointestinal endoscopy revealed a small polypoid lesion in the second portion of the duodenum, predominantly Yamada type II. Histologic examination of biopsy specimen confirmed sarcomatoid carcinoma. Abdominal CT scan revealed multiple metastatic nodules in the liver and pancreatic tail.
