A case of pseudohyperbilirubinemia due to multiple myeloma

Abstract
Paraproteins are abnormal monoclonal immunoglobulins that occur in patients with plasma cell disorders. Paraproteins can interfere with chemical measurements when they form precipitates during the testing procedure.Paraproteins that interfere with the Roche total bilirubin assay are rare and probably idiosyncratic. We experienced an artifactually increased total bilirubin concentration in a patient with multiple myeloma. We performed tests with using the Roche total bilirubin assay on a Modular analyzer (Roche Diagnostics). If spurious hyperbilirubinemia is suspected, then we recommend a visual check for a lack of icterus in the sample, and we recommend measuring the total bilirubin with using a different method.(Korean J Med 72:S304-S308, 2007) Key Words : Multiple myeloma, Paraprotein, Hyperbilirubinemia
