A case of progressive IgA nephropathy with pulmonary hemorrhage

Abstract
Pulmonary renal syndrome is defined as a combination of diffuse pulmonary hemorrhage and glomerulonephritis. This condition includes Wegener's granulomatosis, Churg-Strauss vasculitis, systemic lupus erythematosus and IgA nephropathy. Although IgA nephropathy is the most common glomerulonephritis, extrarenal manifestations, including pulmonary hemorrhage, are rare complications. Pulmonary renal syndrome requires intensive treatment with corticosteroid and immunosuppressive agents. We report here on a case of Ig A nephropathy with pulmonary hemorrhage. A 17-year-old man was admitted because of hemoptysis. Chest X-ray showed bilateral pulmonary infiltration in the lower lung fields. He had dyspnea and oliguria. He was treated via hemodialysis and intensive therapy with pulse methylprednisolone and cyclophosphamide. Renal biopsy showed IgA nephropathy (grade V). Although the respiratory manifestation was improved, renal failure progressed to end-stage renal disease that required maintenance hemodialysis.(Korean J Med 71:1060-1065, 2006)
