A case of propylthiouracil-resistant Graves’ disease

Abstract
Graves' disease is an autoimmune disease characterized by diffuse hyperplasia and hypersecretion of the thyroid gland, infiltrative ophthalmopathy and localized myxedema. Antithyroid drugs have been a mainstay of treatment Graves' disease. Propylthiouracil (PTU) has been used for hyperthyroidism and it has an effective reaction in most cases. In the general use of PTU for hyperthyroidism, any resistance or negative reaction to the medication is rare and several possible mechanisms for this have been considered. The first is that patients with more severe disease and larger thyroid gland respond to antithyroid agents more slowly. The second is the malabsorption of drugs from the gastointestinal system. The third is the rapid turnover or metabolism of antithyroid drug within the thyroid itself. Finally, the compliance of patients must be questioned in any patients who fail to respond to medication. We experienced a case of PTU-resistant Graves' disease and report this case with review of the literature.(Korean J Med 71:S1035-S1038, 2006)
