<trans-abstract xml:lang="en"><p>Hepatic fibrosis is a critical factor in predicting hepatocellular carcinoma occurrence and prognosis in patients with liver disease, making its accurate assessment essential for managing chronic liver disease. Traditionally, liver biopsy has been the standard method for evaluating hepatic fibrosis. However, due to its invasive nature and the limitation of sampling only a small portion of the liver, it is not suitable for widespread use in chronic liver disease patients. Recently, there has been growing interest in noninvasive tests for assessing hepatic fibrosis, including imaging techniques and serum biomarkers. These noninvasive tests are being continuously developed and are increasingly being utilized to evaluate the prognosis of chronic liver disease patients and to inform clinical practice. This paper reviews the recent trends in noninvasive methods for assessing hepatic fibrosis in patients with chronic liver disease, based on studies published to date.</p></trans-abstract>

<title>서 론</title>
<p>간섬유화는 간질환 환자의 간암 발생 및 예후를 예측하는데 가장 중요한 인자로 이를 정확하게 평가하는 것은 만성 간질환 환자의 진료에 필수적이다. 전통적으로 간섬유화 평가는 간 조직 검사를 통해 이루어졌으나 이 방법은 침습적이고 평가 가능한 범위가 제한적이라는 단점이 있다. 이러한 한계를 극복하기 위하여 최근에는 비침습적 영상학적 검사와 혈청표지자들이 개발되었으며 이를 활용한 만성 간질환 환자의 예후 평가와 진료 적용에 대한 연구가 활발히 진행되고 있다. 그러나 의료진에게는 비침습적 검사법에 대한 실질적 정보가 부족하여 이에 대한 진료 지침의 필요성이 대두되었다. 이에 따라 2024년 대한간학회는 만성 간질환에서 간섬유화 평가를 위한 비침습적 검사 진료 가이드라인(이하 2024 진료 가이드라인)을 제정하였다&#x005B;<xref ref-type="bibr" rid="b1-kjm-99-5-232">1</xref>&#x005D;. 본 논문에서는 2024 진료 가이드라인을 중심으로 비침습적 간섬유화 검사의 활용과 최근 연구 동향을 고찰하고 임상적 적용 가능성에 대해 논의하고자 한다.</p>

<sec>
<title>본 론</title>
<sec>
<title>간섬유화 평가를 위한 비침습적 검사의 종류</title>
<p>간섬유화를 평가하기 위한 비침습적 검사는 크게 두 가지로 나눌 수 있다. 첫째는 혈액을 활용한 혈청표지자 검사이다. 이는 간 손상, 간 내 염증, 또는 간기능의 변화를 반영하는 간접 섬유화 지표와 섬유 형성이나 세포외기질 재구성 과정에서 혈중으로 방출된 성분을 측정하는 직접 섬유화 지표로 구분된다&#x005B;<xref ref-type="bibr" rid="b2-kjm-99-5-232">2</xref>&#x005D;. 간접 섬유화 지표로는 대표적으로 aspartate aminotransferase (AST), alanine aminotransferase (ALT), apolipoprotein A1, platelet count, total bilirubin, prothrombin time, gamma-glutamyl transpeptidase, haptoglobin, &#x003b1;2-macroglobulin, cholesterol 그리고 asialo &#x003b1;1-acid glycoprotein 등이 있다&#x005B;<xref ref-type="bibr" rid="b3-kjm-99-5-232">3</xref>-<xref ref-type="bibr" rid="b5-kjm-99-5-232">5</xref>&#x005D;. 이러한 지표들은 간섬유화 진단에 도움을 줄 수 있으나 일반적으로 간섬유화 진단능이 단독으로는 제한적이어서 여러 지표를 조합하여 사용하는 경우가 많다. 대표적인 조합 지표로는 AST-to-ALT ratio, AST-to-platelet ratio index (APRI), BARD score, fibrosis-4 index (FIB-4), non-alcoholic fatty liver disease (NAFLD) fibrosis score 등이 있다&#x005B;<xref ref-type="bibr" rid="b6-kjm-99-5-232">6</xref>-<xref ref-type="bibr" rid="b10-kjm-99-5-232">10</xref>&#x005D;. 직접 섬유화 지표로는 procollagen III C-terminal propeptide, procollagen III N-terminal propeptide, chitinase-3-like protein 1, matrix metalloproteinase 2, hyaluronic acid, tissue inhibitor of metalloproteinase 1, YKL-40, Mac-2 binding protein glycosylation isomer 등이 대표적이다&#x005B;<xref ref-type="bibr" rid="b11-kjm-99-5-232">11</xref>-<xref ref-type="bibr" rid="b13-kjm-99-5-232">13</xref>&#x005D;. 이러한 직접 섬유화 지표들도 단독으로 사용되기보다는 여러 지표를 조합하여 활용되는 경우가 많으며 대표적으로 enhanced liver fibrosis test (ELF), FibroTest<sup>&#x000ae;</sup>, ADAPT, FIBC3, NIS4<sup>&#x000ae;</sup> 등이 있다(<xref rid="t1-kjm-99-5-232" ref-type="table">Table 1</xref>) &#x005B;<xref ref-type="bibr" rid="b14-kjm-99-5-232">14</xref>-<xref ref-type="bibr" rid="b16-kjm-99-5-232">16</xref>&#x005D;.</p>
<p>둘째, 장비를 이용하여 간 내 탄성도를 측정하는 방법으로 는 순간 탄성 측정법(vibration-controlled transient elastography, VCTE), 횡파 탄성 초음파(shear wave elastography, SWE), 자기 공명 탄성 검사(magnetic resonance elastography, MRE) 등이 있다. VCTE는 비교적 빠르고 간편하게 간섬유화를 평가할 수 있는 장점이 있으며 다양한 만성 간질환에서 우수한 진단능을 보였다&#x005B;<xref ref-type="bibr" rid="b17-kjm-99-5-232">17</xref>,<xref ref-type="bibr" rid="b18-kjm-99-5-232">18</xref>&#x005D;. SWE는 점 SWE와 이차원 SWE로 구분되며 간의 해부학적 구조를 초음파로 확인하면서 간섬유화를 평가할 수 있는 장점이 있다&#x005B;<xref ref-type="bibr" rid="b19-kjm-99-5-232">19</xref>,<xref ref-type="bibr" rid="b20-kjm-99-5-232">20</xref>&#x005D;. MRE는 비침습적 검사 중 가장 높은 진단능을 보이는 방법이지만 측정 비용이 상대적으로 높고 검사 가능한 의료기관이 제한적이라는 단점이 있다(<xref rid="t2-kjm-99-5-232" ref-type="table">Table 2</xref>) &#x005B;<xref ref-type="bibr" rid="b21-kjm-99-5-232">21</xref>&#x005D;.</p>

<sec>
<title>만성 간질환에서 비침습적 검사의 활용</title>
<p>만성 B형간염 환자에서 APRI, FIB-4, VCTE는 특이도가 높아 의미 있는 간섬유화를 배제하는 데 유용하다. 또한 VCTE를 활용하면 만성 B형간염 환자에서 의미 있는 간섬유화와 간섬유화증을 높은 민감도와 특이도로 진단할 수 있다. SWE와 MRE 또한 만성 B형간염 환자에서 의미 있는 간섬유화와 간경변증을 진단하는 데 우수한 진단능을 보여준다. 의미 있는 간섬유화와 간경변증의 진단 정확도를 높이기 위해 혈청 표지자와 VCTE의 순차적 혹은 동시 검사를 고려할 수 있다&#x005B;<xref ref-type="bibr" rid="b1-kjm-99-5-232">1</xref>&#x005D;.</p>
<p>비침습적 검사는 만성 B형간염 환자의 고위험군을 선별하는 데도 활용할 수 있다. 만성 B형간염 환자 중에는 혈청 hepatitis B virus DNA와 ALT가 자연 경과 중 특정 단계에 부합하지 않는 회색지대가 존재하며 이러한 환자는 전체 만성 B형간염 환자의 약 30%를 차지한다&#x005B;<xref ref-type="bibr" rid="b22-kjm-99-5-232">22</xref>,<xref ref-type="bibr" rid="b23-kjm-99-5-232">23</xref>&#x005D;. 이 회색지대에 해당하는 환자들은 기존에 알려진 면역 관용기와 면역 비활동기보다 간세포암종 발생 위험도가 높은 것으로 보고되고 있다. 따라서 이들 가운데 항바이러스제 치료가 필요한 고위험군 환자를 잘 감별할 필요가 있다. 간 조직 검사를 통해 간섬유화를 평가할 수도 있으나 VCTE나 MRE와 같은 비침습적 검사를 활용하여 간섬유화를 평가하고 고위험군을 선별할 수 있으며 결과에 따라 항바이러스제 치료를 고려할 수 있다&#x005B;<xref ref-type="bibr" rid="b24-kjm-99-5-232">24</xref>&#x005D;.</p>
<p>만성 C형간염 환자에서 비침습적 검사의 유용성은 많은 연구를 통해 입증되었다. 특히 VCTE에 대한 연구 결과가 가장 많이 보고되었으며 이외에도 혈청 표지자, SWE 그리고 MRE 등의 검사가 만성 C형간염 환자의 간섬유화를 진단하는 데 유용한 것으로 알려져 있다&#x005B;<xref ref-type="bibr" rid="b25-kjm-99-5-232">25</xref>,<xref ref-type="bibr" rid="b26-kjm-99-5-232">26</xref>&#x005D;. 알코올 관련 간질환 환자에서는 진행된 간섬유화를 선별하거나 배제하기 위해 VCTE를 사용할 수 있다. 또한 알코올 관련 간질환 환자에서 ELF, VCTE, FIB-4, SWE 등의 비침습적 검사를 활용하여 간섬유화를 진단하는 데 도움을 받을 수 있다&#x005B;<xref ref-type="bibr" rid="b27-kjm-99-5-232">27</xref>,<xref ref-type="bibr" rid="b28-kjm-99-5-232">28</xref>&#x005D;. 원발담도담관염, 자가면역간염, 원발경화담관염과 같은 희귀 간질환 환자에서도 VCTE와 같은 비침습적 검사의 활용이 간섬유화 진단에 유용하다고 알려져 있다&#x005B;<xref ref-type="bibr" rid="b29-kjm-99-5-232">29</xref>-<xref ref-type="bibr" rid="b31-kjm-99-5-232">31</xref>&#x005D;.</p>

<sec>
<title>특수한 상황의 간질환 환자에서 비침습적 검사의 활용</title>
<sec>
<title>간세포암종 발생 예측</title>
<p>만성 간질환 환자에서 간세포암종의 발생은 예후를 결정하는 가장 중요한 사건으로 간세포암종 발생 가능성이 높은 고위험군을 선별하고 면밀히 추적 관찰하는 것은 만성 간질환 환자의 진료에서 가장 중요한 부분이다. 따라서 비침습적 검사를 활용하여 만성 간질환 환자의 간세포암 발생 가능성을 예측하고자 하는 다양한 연구들이 수행되었다. 혈청 표지자 중 간세포암종 발생 예측력에 대한 연구가 비교적 많이 이루어진 것은 FIB-4이다. 국내 연구에 따르면 FIB-4 &#x0003c; 1.25인 환자들에 비해 1.7 &#x02264; FIB-4 &#x0003c; 2.4인 환자의 간세포암종 발생 상대 위험비는 4.57, FIB-4 &#x02265; 2.4인 환자의 상대 위험비는 21.34로 높았다&#x005B;<xref ref-type="bibr" rid="b32-kjm-99-5-232">32</xref>&#x005D;. 또한 간경변증이 있는 만성 B형간염 환자에서 항바이러스 치료 후 1년째 FIB-4의 감소는 간세포암종 발생 위험도의 감소와 관련이 있었다&#x005B;<xref ref-type="bibr" rid="b33-kjm-99-5-232">33</xref>&#x005D;. 장비를 활용한 검사 중에서는 VCTE를 이용한 간세포암종 발생 예측에 관한 연구가 가장 많이 이루어졌다. 만성 B형간염 환자를 대상으로 한 국내 코호트 연구에서 VCTE는 유의하게 간세포암종 발생을 예측하였으며 유의한 간경직도 절단값은 8.0-14.1 kPa로 보고되었다&#x005B;<xref ref-type="bibr" rid="b34-kjm-99-5-232">34</xref>&#x005D;. 최근 발표된 메타 분석에 따르면 10개 연구, 18,150명의 만성 간질환 환자들이 포함되었으며 VCTE에서 간경직도 11-13 kPa 이상인 경우 간세포암종 발생 위험비는 3.33이었다. 진단 area under the ROC curve (AUC)는 0.74, 민감도와 특이도는 각각 61%, 78%였다&#x005B;<xref ref-type="bibr" rid="b35-kjm-99-5-232">35</xref>&#x005D;.</p>

<sec>
<title>간세포암종의 근치적 치료 이후 예후</title>
<p>예측 완치를 목적으로 간세포암종을 수술하는 환자에서 합병증 및 간세포암종의 재발은 예후를 결정짓는 중요한 요인이다. 그러나 이를 예측할 만한 검사와 추적 관찰에 대한 명확한 기준은 아직 마련되어 있지 않다. 최근 간섬유화를 비침습적으로 확인할 수 있는 검사들이 개발되면서 간세포암종으로 간 절제술을 시행받는 환자들을 대상으로 수술 전 비침습적 검사를 통해 수술 후 합병증 발생 및 간세포암종의 재발을 예측하려는 연구들이 지속적으로 보고되고 있다. 최근 연구에 따르면 간세포암종을 진단받고 간 절제술을 시행받는 환자들을 대상으로 수술 전 VCTE를 시행한 8개의 연구를 메타 분석한 결과 VCTE에서 높은 결과값을 보인 고위험군이 낮은 결과값을 보인 저위험군에 비해 간 절제술 후 합병증 발생 위험이 현저히 증가함을 확인할 수 있었다(위험비, 8.3). 다만 연구마다 고위험군 선정 기준이 되는 VCTE 절단값(12.0-25.6 kPa)이 다양하고 간 절제술 후 합병증 발생 위험도(3.3-21.2) 또한 연구마다 차이가 있는 한계점이 있었다. 포함된 연구의 민감도는 76% (95% 신뢰구간, 55-89), 특이도는 85% (95% 신뢰구간, 73-92)였다. 간 절제술 후 간세포암종 재발을 연구한 메타 분석에서는 VCTE에서 높은 결과값을 보인 고위험군에서 간 절제술 후 간세포암종의 재발률이 유의하게 증가함을 확인할 수 있었다(위험비, 2.1). 다만 연구에 포함된 모든 연구가 아시아에서 진행되었으며 간세포암종의 병인으로 만성 B형간염인 환자들이 주로 포함되어 연구가 진행되었다는 점은 연구의 한계점으로 생각된다&#x005B;<xref ref-type="bibr" rid="b36-kjm-99-5-232">36</xref>&#x005D;.</p>
</sec>
<sec>
<title>비대상성 간경변증 발생</title>
<p>비대상성 간경변증 발생 예측에 있어 혈청 표지자의 역할은 제한적이며 주로 각 원인 간질환에서 후향적 코호트를 활용하여 보고되었다. 따라서 비대상성 간경변증 발생에 대한 혈청 표지자의 활용에 대해서는 추가적인 후속 연구가 필요할 것으로 생각된다. 반면 VCTE는 비대상성 간경변증 발생 예측에 대한 유용성이 다양한 원인 간질환에서 많이 연구되었다. 만성 간질환 환자를 대상으로 한 17개 연구를 포함한 메타 분석에서 VCTE로 측정한 간경직도의 증가는 비대상성 간경변증 발생과 상대 위험비 1.07로 유의한 상관관계를 보였다. 유럽의 전향적 연구에서는 VCTE가 비대상성 간경변증 발생 예측에서 간 문맥압 측정 검사(hepatic vein pressure measurement, HVPG)와 대등한 성능을 보였으며 간경직도 절단값 21.1 kPa와 HVPG 절단값 10 mmHg 이상 모두 음성 예측도 100%를 기록하였다&#x005B;<xref ref-type="bibr" rid="b37-kjm-99-5-232">37</xref>&#x005D;. VCTE가 기존의 간 조직 검사나 HVPG 검사를 대체할 수 있다는 근거는 아직 부족하지만 비대상성 간경변증 발생 예측에 있어 유용한 검사로 평가된다. 반면 SWE와 MRE는 만성 간질환 환자에서 비대상성 간경변증 발생 예측에 도움이 되는지에 대한 근거가 부족하며 이를 명확히 하기 위해서는 다양한 원인의 만성 간질환 환자를 대상으로 추가적인 연구가 필요하다.</p>

<sec>
<title>사망</title>
<p>비침습적 검사 중 사망 예측에 가장 많이 연구된 혈청 표지자는 FIB-4이다. 만성 간질환이 없는 국내 46,456명의 성인을 대상으로 한 연구에서는 FIB-4 &#x0003e; 2.67 집단의 총 사망 및 간 관련 사망 위험비가 각각 1.64 및 10.50으로 불량한 생존율과 연관되어 있음을 확인하였다&#x005B;<xref ref-type="bibr" rid="b38-kjm-99-5-232">38</xref>&#x005D;. 또 다른 연구에서는 국내 만성 B형 간염 환자에서 FIB-4 &#x0003e; 2.67이 높은 간 관련 사망과 연관되어 있음을 보고하였다&#x005B;<xref ref-type="bibr" rid="b39-kjm-99-5-232">39</xref>&#x005D;. 그 외 혈청 표지자 중 APRI는 간세포암종 환자 대상 메타 분석에서 사망 위험비 1.62로 불량한 생존율과 연관되어 있음을 보여주었다&#x005B;<xref ref-type="bibr" rid="b40-kjm-99-5-232">40</xref>&#x005D;. 장비를 활용한 비침습적 검사 중에서는 VCTE를 이용하여 만성 간질환 환자의 사망과의 관련성을 조사한 연구가 가장 많다. 여러 메타 분석 결과에 따르면 VCTE에서 높은 경도 결과를 보인 환자들은 간 관련 사망과의 상대 위험비가 높았으며 추적 관찰 동안 간경직도가 20% 이상 증가할 때 간경변증 합병증 발생 및 간 관련 사망 발생이 증가함을 확인하였다&#x005B;<xref ref-type="bibr" rid="b41-kjm-99-5-232">41</xref>&#x005D;. 따라서 VCTE는 만성 간질환 환자에서 간세포암종 발생, 비대상성 간경변증 발생뿐만 아니라 사망 예측에도 유용할 수 있다. 또한 SWE와 MRE도 만성 간질환 환자의 사망 예측에 도움이 될 수 있다.</p>
</sec>
<sec>
<title>문맥압항진증의 진단과 예후 예측</title>
<p>문맥압항진증은 간경변증에 의해 문맥 및 그 지류들의 혈압인 문맥압이 비정상적으로 상승하는 상태로 간경변증의 주요 합병증 중 하나이다. 문맥압 항진증은 간경변증과 관련된 다양한 합병증의 직접적인 원인이 되기 때문에 간경변증 환자에서는 문맥압 항진증의 평가와 정기적인 추적 검사가 필수적이다. 현재 문맥압 항진증의 표준 검사는 간정맥에 풍선 카테터를 직접 삽입하여 측정하는 간정맥 압력차 측정법을 사용하고 있으나 이 방법은 침습적이며 특정 시설과 전문가의 감독이 필요하여 임상에서의 적용에 제한이 많다. 이러한 한계를 극복하기 위해 비침습적 검사를 이용하여 문맥압 항진증을 진단하고 예후를 예측하려는 연구들이 활발히 진행되고 있다. 특히 VCTE의 문맥압항진증 진단에 대한 유용성에 관한 연구가 많이 진행되었으며 전반적으로 우수한 진단능을 보이고 있다. 최근의 메타 분석 결과에 따르면 11개 연구와 1,451명의 환자를 대상으로 한 연구에서 VCTE의 임상적으로 의미 있는 문맥압항진증 진단의 hierarchical summary AUC는 0.90, 민감도는 87.5%, 특이도는 85.3%였으며 summary HVPG-liver stiffness measurement (LSM) 상관계수는 0.783으로 높았다&#x005B;<xref ref-type="bibr" rid="b42-kjm-99-5-232">42</xref>&#x005D;. 그러나 문맥압항진증의 진단 AUC는 간질환의 원인이나 연구 조건에 따라 다를 수 있으며 간정맥 압력차가 12 mmHg 이상으로 심해지는 경우에는 문맥압과의 상관성이 낮아지는 경향이 있다&#x005B;<xref ref-type="bibr" rid="b43-kjm-99-5-232">43</xref>&#x005D;. 이러한 제한점을 극복하기 위하여 VCTE에 다른 혈청 또는 영상학적 표지자를 결합하여 진단능을 향상시키려는 연구가 진행되고 있다.</p>
</sec>
<sec>
<title>식도정맥류 고위험군 선별</title>
<p>간경변증 환자에서 식도정맥류의 1차 예방을 위한 감별 진단으로서 상부 위장관 내시경 검사가 전통적으로 활용되어 왔다. 최근에는 비침습적 간섬유화 평가 방법들이 발전함에 따라 이러한 비침습적 검사법을 이용하여 임상적으로 유의한 식도정맥류를 예측하려는 연구가 활발히 진행되고 있다. 체계적 문헌 고찰 및 메타 분석에 따르면 VCTE를 이용한 식도정맥류 진단 AUC는 0.84로 보고되었으며 민감도는 87%, 특이도는 53%로 나타났다. 또한 큰 식도정맥류의 경우 진단 AUC는 0.78, 민감도는 86%, 특이도는 59%로 보고되었다&#x005B;<xref ref-type="bibr" rid="b44-kjm-99-5-232">44</xref>&#x005D;. Baveno VI 기준에 의하면 간경직도가 20 kPa 미만이고 혈소판 수가 150,000/mL를 초과하는 환자에서는 상부 위장관 내시경 검사를 피할 수 있는 기준으로 제시되었다. 이 기준은 다양한 연구에서 검증되었으며 일반적으로 약 20%의 환자에서 내시경 검사를 피할 수 있었고 치료가 필요한 식도정맥류 환자의 4% 미만이 누락되는 것으로 확인되었다&#x005B;<xref ref-type="bibr" rid="b45-kjm-99-5-232">45</xref>,<xref ref-type="bibr" rid="b46-kjm-99-5-232">46</xref>&#x005D;. 결론적으로 비침습적 검사 방법은 식도정맥류의 고위험군을 선별하는 데 효과적이며 불필요한 상부 위장관 내시경 검사를 줄이는 데 기여할 수 있다. 그러나 일차 출혈 예방 치료가 필요한 고위험군 환자에 대해서는 여전히 선별적 내시경 검사가 필요하다.</p>
</sec>
<sec>
<title>대사이상 관련 지방간 환자의 진료 지침</title>
<p>최근 비만과 함께 대사이상 관련 지방간질환(metabolic dysfunction-associated steatotic liver disease)의 유병률이 증가함에 따라 이들 환자에서의 간섬유화 평가의 중요성이 부각되고 있다. 2024년 진료 가이드라인에서는 NAFLD 환자의 관리에 관한 지침을 제시하고 있으며 이를 통해 비침습적 검사와 혈청 표지자를 활용한 간섬유화 평가의 접근 방식을 안내하고 있다. 일차 의료기관에서는 NAFLD가 의심될 경우 우선 환자의 병력 청취와 혈액 검사를 통해 바이러스 간염, 알코올 관련 간질환 및 기타 원인에 의한 간질환 동반 여부를 확인하여야 한다. 기본적인 혈액 검사와 임상 정보를 통해 얻을 수 있는 혈청 표지자, 특히 FIB-4는 NAFLD 환자의 위험도를 평가하는 데 유용하다. FIB-4가 1.30 미만인 경우 저위험군으로 분류하며 1.30 이상일 경우 추가적인 위험도 분석을 위해 VCTE를 시행하거나 간질환 전문가에게 의뢰할 수 있다. FIB-4가 2.67을 초과하면 간섬유화 진행의 가능성이 높으므로 간전문의에게 바로 진료를 의뢰할 수 있으며 1.30과 2.67 사이인 경우에는 VCTE와 같은 비침습적 검사 결과에 따라 의뢰 여부를 결정할 수 있다. 간경직도가 8 kPa 이하일 경우 저위험군으로 분류하여 중등도 이상의 위험군 가능성을 배제할 수 있다. 중등도 이상의 위험군으로 분류된 환자는 간섬유화에 대한 정확한 평가와 적절한 관리를 위해 간전문의에게 의뢰해야 하며 간전문의는 환자의 병력과 간섬유화 위험도를 종합적으로 검토한 후 필요에 따라 MRE, ELF, Agile score 등 추가 검사를 시행하고 간섬유화 진행 여부에 따라 추적 관찰 및 치료를 시행한다(<xref rid="f1-kjm-99-5-232" ref-type="fig">Fig. 1</xref>) &#x005B;<xref ref-type="bibr" rid="b47-kjm-99-5-232">47</xref>,<xref ref-type="bibr" rid="b48-kjm-99-5-232">48</xref>&#x005D;.</p>

<title>결 론</title>
<p>만성 간질환 환자에서 간섬유화는 간암 발생을 포함한 예후를 예측하는 데 중요한 인자로 이를 적절한 시기에 적절한 방법으로 평가하고 변화 추이를 모니터링하는 것은 필수적이다. 최근 도입된 비침습적 간섬유화 평가 방법들은 이러한 필요성에 대응하기 위한 중요한 도구로 자리 잡고 있으나 임상에서의 활용에는 여전히 실질적인 정보의 부족으로 인한 한계가 존재하였다. 이를 해결하고자 2024년 대한간학회는 비침습적 검사에 대한 진료 가이드라인을 발표하였다. 이 가이드라인은 만성 간질환 환자의 진료 및 추적 관찰에 중요한 기여를 할 것이며 궁극적으로 환자의 예후 개선에도 긍정적인 영향을 미칠 것으로 기대된다. 더불어 비침습적 검사에 대한 지속적인 연구와 기술 발전이 이루어지면서 기존 검사 방법들의 단점을 보완하고 진단 능력을 향상시킬 새로운 방법들이 개발되고 있다. 이러한 발전은 만성 간질환 환자에서 비침습적 검사 방법의 다양한 활용 가능성을 제시하며 향후 임상 적용에서의 역할이 더욱 확대될 것으로 예상된다.</p>
