<trans-abstract xml:lang="en"><p>Monoclonal gammopathy (MG) encompasses a diverse group of disorders characterized by the secretion of monoclonal immunoglobulins or their light-chain components. The incidence of multiple myeloma (MM) in South Korea is rapidly increasing, and it is important to be aware of its initial clinical presentations and the most efficient laboratory algorithms for early detection. Serum protein electrophoresis (SPE) and urine protein electrophoresis (UPE) are the primary screening tests for patients with clinically suspected MM or amyloid light-chain amyloidosis; these tests are reimbursed in South Korea. We reviewed clinical studies that applied national and international guidelines to evaluate test panels for early detection of MGs, including MM. The serum free light chain (sFLC) with SPE panel is recommended for the initial work up for diagnosis of MGs. In the case of a normal SPE, sFLC should be measured subsequently, so as not to miss the presence of M-protein. Use of this screening panel avoids medical expenses related to delayed diagnosis. Guidelines and recommendations suggest that no single method (SPE, serum immunofixation electrophoresis, sFLC, or UPE) should be used to exclude a diagnosis of MM. We believe that a screening test panel comprising SPE plus sFLC will increase the rate of early and accurate diagnosis of MM and related disorders.</p></trans-abstract>

<title>서 론</title>
<p>단세포군감마글로불린병증(monoclonal gammopathy, MG)은 일반적으로 단클론 면역글로불린(monoclonal immunoglobulin, M-Ig) 또는 단클론 유리경쇄(monoclonal free light chain, M-FLC)의 분비를 특징으로 하는 질병군이다. 여기에는 다발골수종(multiple myeloma, MM), 발덴스트룀고분자글로불린혈증(Waldenstr&#x000f6;m&#x02019;s macroglobulinemia, WM), 형질세포성 백혈병(plasma cell leukemia, PCL), 아밀로이드 유리경쇄 아밀로이드증(amyloid light-chain amyloidosis, AL amyloidosis), 경쇄침착질환(light chain deposition disease, LCDD), POEMS 증후군(POEMS syndrome) 및 무증상 다발골수종(smoldering multiple myeloma, SMM)과 의미불명 단세포군감마글로불린병증(monoclonal gammopathy of undetermined significance, MGUS) 등이 속한다&#x005B;<xref ref-type="bibr" rid="b1-kjm-96-5-371">1</xref>&#x005D;. 또한 광범위큰B세포림프종(diffuse large B-cell lymphoma) 등의 비호지킨림프종(non-Hodgkin lymphoma, NHL)에서도 MG를 동반하는 경우가 있다&#x005B;<xref ref-type="bibr" rid="b2-kjm-96-5-371">2</xref>,<xref ref-type="bibr" rid="b3-kjm-96-5-371">3</xref>&#x005D;.</p>
<p>다발골수종의 경우, 전체 종양의 1% 정도를 차지하고 혈액종양 중에서는 NHL에 이어 두 번째로 흔한 악성 혈액종양이다&#x005B;<xref ref-type="bibr" rid="b4-kjm-96-5-371">4</xref>,<xref ref-type="bibr" rid="b5-kjm-96-5-371">5</xref>&#x005D;. 2012년 한국의 국가 암통계에 따르면&#x005B;<xref ref-type="bibr" rid="b6-kjm-96-5-371">6</xref>&#x005D;, 2012년까지 10년 동안 연령표준화발생률이 2배 증가하였고, 증가율은 20년 전에 비해 10배 높은 수치이다&#x005B;<xref ref-type="bibr" rid="b7-kjm-96-5-371">7</xref>&#x005D;. 이러한 급격한 증가는 M-단백을 검출하는 검사법의 발전과 노인 인구의 증가로 설명될 수 있다. 일부 환자는 진단 후 몇 주 이내에 사망할 수 있지만, 어떤 환자들은 수년간 &#x00022;무증상&#x00022;인 경우도 있다. 환자는 뼈의 통증과 골절, 극심한 피로감이나 빈혈, 신장 기능 저하, 급성 및 만성 감염을 동반할 수 있으며, 형질세포종(plasmacytomas)에 의한 뼈나 연조직 침범이 발생할 수도 있다. 많은 경우 진단 후 적절한 항-골수종 치료가 필요하다.</p>
<p>MM 및 AL 아밀로이드증을 비롯한 MG 동반 질환의 진단이 지연되면 합병증이 더 많아지고 무병생존율이 감소하는 것으로 잘 알려져 있다&#x005B;<xref ref-type="bibr" rid="b8-kjm-96-5-371">8</xref>&#x005D;. 따라서 질병의 초기 단계에서 진단하기 위한 효율적인 검사 알고리즘을 확립하여 조기 진단을 하는 것이 무엇보다 중요하다. MG를 선별하는 검사법은 오랫동안 혈청 단백전기영동검사법(serum protein electrophoresis, SPE)과 소변 단백전기영동검사법(urine protein electrophoresis, UPE)이 이용되어 왔다. 또한 10여 년 전에 혈청유리경쇄 검사(serum free light chain assay, sFLC)가 국내에 도입됨에 따라(Freelite<sup>&#x000ae;</sup>) 소변에서 검출하였던 M-FLC을 혈청에서 &#x003ba;-FLC와 &#x003bb;-FLC를 측정하고 FLC &#x003ba;/&#x003bb; 비를 통해 M-FLC의 정량적 검출이 가능해졌다.</p>
<p>SPE의 검출민감도가 &#x0007e;500 mg/L인 점을 감안할 때, M-Ig 다발골수종(intact immunoglobulin multiple myeloma) 환자를 진단하는 데 적합하다고 할 수 있다. 그러나 경쇄 다발골수종(light chain multiple myeloma, LCMM), 미량의 M-단백을 분비하거나 분비하지 않는 비분비성 다발골수종(nonsecretory multiple myeloma, NSMM), AL 아밀로이드증 등의 M-FLC 관련 질환이 있는 환자의 경우 SPE나 UPE에 의해 M-단백이 검출되지 않을 수 있으며&#x005B;<xref ref-type="bibr" rid="b9-kjm-96-5-371">9</xref>-<xref ref-type="bibr" rid="b22-kjm-96-5-371">22</xref>&#x005D;, 이로 인하여 진단의 지연으로 이어지기 쉽다. 따라서, 국제골수종연구그룹(International Myeloma Working Group, IMWG) &#x005B;<xref ref-type="bibr" rid="b23-kjm-96-5-371">23</xref>&#x005D;, 영국 국립보건임상평가연구소(The National Institute for Care and Health Excellence, NICE) &#x005B;<xref ref-type="bibr" rid="b24-kjm-96-5-371">24</xref>&#x005D; 등의 임상 진료 지침에서는 많은 연구들을 근거로 SPE, UPE나 혈청이나 소변의 면역고정전기영동(immunofixation electrophoresis, IFE) 단독 검사 결과만으로 의심 환자에서 MM 및 MG 동반 질환을 배제하지 않도록 권고하고 있다. MM 및 MG 동반 질환 의심 환자에서 여러 가지 선별 검사를 시행하는 것은 조기 진단, 치료 효과 증가, 무병생존율 증가 등을 고려해 볼 때 증가될 의료비용을 고려하더라도 중요하다고 판단된다. 본 저자들은 MM 및 MG 동반 질환의 조기 진단을 위한 가장 효과적인 패널에 대하여 임상 연구 및 임상 진료 지침 등의 문헌 검토를 통하여 파악하고자 하였다.</p>

<title>본 론</title>
<sec>
<title>다발골수종 조기 진단을 위한 선별 검사 패널</title>
<p>오랫동안 전기영동법을 이용한 SPE, UPE가 MM을 비롯한 MG 동반 질환 의심 환자에 대한 선별 검사로 이용되어 왔다. SPE 검사는 혈액 내의 M-Ig를 검출하는 데 검사의 낮은 민감도로 일부 환자에서는 M-단백이 검출되지 않을 수 있다. 소변 M-FLC의 검출을 위하여 24시간 소변에서 UPE 검사를 시행할 수 있지만, 민감도가 충분하지 못하여 많은 환자에서 소변 M-FLC가 검출되지 못하는 경우가 있을 뿐만 아니라 의심 환자에게서 24시간 소변을 수집하여 검사를 진행하는 과정에 어려움이 많다. 2007년과 2008년에 영국에서 시행한 MG에 대한 검토에서 의심 환자에서 24시간 소변을 수집하여 검사를 진행할 수 있었던 환자는 14%에 불과하였고&#x005B;<xref ref-type="bibr" rid="b19-kjm-96-5-371">19</xref>&#x005D;, 이러한 24시간 소변 검체의 가용성 문제는 영국 NICE 가이드라인의 최근 업데이트에서도 지적된 사항이다&#x005B;<xref ref-type="bibr" rid="b24-kjm-96-5-371">24</xref>&#x005D;. 영국 NICE 가이드라인에서는 24시간 소변 검체를 얻지 못하면 잠재적인 MG 진단 실패를 초래할 수 있고, SPE 단독으로는 M-FLC의 검출을 놓칠 가능성이 있기 때문에 sFLC가 MG가 임상적으로 의심되는 환자에서 시행하는 선별 검사 패널에 포함되어야 한다고 강조하고 있다.</p>
<p>10여 년 전에 혈청 유리경쇄를 정량적으로 측정하는 검사(Freelite<sup>&#x000ae;</sup>)가 처음 개발되어 MM 및 MG 동반 질환의 진단, 예후, 치료에 대한 반응 모니터링, 잔존 질환 검출, 재발의 빠른 확인에서 sFLC의 임상적 유용성에 대한 많은 연구들이 발표되었다. 특히, MM 및 MG 동반 질환 의심 환자에서 시행하는 선별 검사로서의 진단 민감도 향상에 sFLC가 미치는 영향에 대한 많은 연구들이 수행되었다(<xref rid="t1-kjm-96-5-371" ref-type="table">Table 1</xref>).</p>
<p>선별 검사의 성능을 평가하는 연구에서 sFLC에 의하여 추가로 검출된 MG 동반 질환은 다양하였다. 대부분이 오직 M-FLC만을 동반하는 질환(LCMM, NSMM, AL 아밀로이드증 및 LCDD)이었다. Hill 등&#x005B;<xref ref-type="bibr" rid="b12-kjm-96-5-371">12</xref>&#x005D;은 2명의 LCMM 환자를 추가로 진단하였고, Piehler 등&#x005B;<xref ref-type="bibr" rid="b15-kjm-96-5-371">15</xref>&#x005D;은 5명의 LCMM 환자, 1명의 NSMM 환자 및 1명의 AL 아밀로이드증 환자를 추가로 보고하였으며, McTaggart와 Kearney &#x005B;<xref ref-type="bibr" rid="b22-kjm-96-5-371">22</xref>&#x005D;는 2명의 추가 환자를 확인하였는데, 한 명은 LCDD 환자였고 다른 한 명은 다중 형질세포종 환자였다.</p>
<p>현재까지 보고된 가장 큰 연구인 Katzmann 등&#x005B;<xref ref-type="bibr" rid="b11-kjm-96-5-371">11</xref>&#x005D;의 보고에서는 MG의 검출을 위하여 서로 다른 잠재적 선별 검사 패널의 각 조합별 진단 민감도를 비교하였다. MG로 새롭게 진단된 1,877명의 환자 검체에 대하여 5가지 선별 검사(SPE, UPE, serum IFE, urine IFE 및 sFLC)를 시행하였다. 시행된 검사법 중 어느 것도 독자적인 단일 검사법으로 사용하기에 충분한 민감도를 보이지 못하였다. 5가지 검사를 모두 시행할 때 가장 높은 진단 민감도를 얻을 수 있었는데, 모든 대상 검체 중 98.6%에서 MG를 검출하였다. 그러나, AL 아밀로이드증, 형질세포종 및 골수 외 형질세포종(extramedullary plasmacytoma)의 일부에서는 5가지 검사로도 MG가 검출되지 않았다. SPE와 sFLC의 2가지 검사로 간소화된 검사 패널로는 94.3%의 민감도를 보였고, MM (100% 민감도), WM (100% 민감도) 및 SMM (99.5% 민감도)과 같이 종양의 양이 많은 MG의 경우 SPE와 sFLC의 2가지 선별 검사가 진단에 매우 효과적인 것으로 평가되었다.</p>
<p>한국인 환자에서 이러한 검사법에 따른 민감도의 검증은 Jeong 등&#x005B;<xref ref-type="bibr" rid="b21-kjm-96-5-371">21</xref>&#x005D;이 보고하였는데, M-단백의 검출을 위한 최적의 선별 검사 패널을 찾는 다양한 진단 알고리즘이 비교되었다. 210명의 MM환자를 대상으로 M-단백 검출을 위한 5가지 검사법(SPE, sIFE, UPE, uIFE 및 sFLC)의 진단 민감도를 확인할 수 있었다. SPE와 sFLC의 조합은 100% 민감도를 보였다(<xref rid="t2-kjm-96-5-371" ref-type="table">Table 2</xref>). 저자들은 sFLC와 SPE 검사의 2가지 패널이 MM 의심 환자의 선별 검사법으로 사용될 수 있으며, MM진단에 소요되는 시간을 단축하여 비용효율적이라고 결론지었다.</p>
<p>Park 등&#x005B;<xref ref-type="bibr" rid="b20-kjm-96-5-371">20</xref>&#x005D;이 실시한 원인불명의 신장기능장애로 신장내과를 방문한 환자 471명을 대상으로 한 후향적 연구에서는 신장 질환 환자에서 sFLC &#x003ba;/&#x003bb; 비의 조정된 정상참고범위의 진단적 유용성이 SPE, UPE와 함께 평가되었다. 총 110명의 환자(23.4%)가 MM으로 진단되었고, 346명이 &#x00022;non-MM&#x00022;으로 분류되었으며, 나머지 15명은 다른 형질세포 연관 질환이나 림프증식성 질환으로 진단되었다. sFLC (민감도 100%) 검사는 SPE (민감도 58.6%) 또는 UPE (민감도 69.0%)와 비교하여 LCMM의 선별 진단에 있어 특히 효과적이었다(<xref rid="t3-kjm-96-5-371" ref-type="table">Table 3</xref>). 또한, 저자들은 24시간 소변 검체 수집의 어려움으로 선별 검사 패널에 sFLC를 도입한 이후 추가로 9.5% (24/252)의 만성 신질환(chronic kidney disease, CKD) 환자가 MM으로 진단될 수 있었다고 언급하였다. 저자들은 신장 질환 환자에서 SPE와 sFLC (sFLC &#x003ba;/&#x003bb; 비의 조정된 정상참고범위 적용)이 MM 검출을 위한 최적의 선별 검사 패널이라고 결론지었다.</p>
<p>IMWG에서는 MG의 선별 검사 패널과 관련하여 sFLC를 SPE 및 sIFE와 함께 사용하면 높은 민감도를 보여주므로, AL 아밀로이드증을 제외한 형질세포 연관 질환에서는 진단을 위하여 시행해오던 24시간 소변을 이용한 검사를 대체할 수 있음을 강조하였다&#x005B;<xref ref-type="bibr" rid="b23-kjm-96-5-371">23</xref>&#x005D;.</p>
<p>MM 진단은 골수에서 단클론 형질세포의 존재가 확인되면서 관련 장기 또는 조직 손상(고칼슘혈증, 신부전증, 빈혈 또는 골병변; CRAB 기준으로 통칭)이 확인되어야 한다. 그러나, 2014년에 개정된 IMWG 가이드라인&#x005B;<xref ref-type="bibr" rid="b25-kjm-96-5-371">25</xref>&#x005D;에서는 매우 비정상적인 sFLC (involved/uninvolved sFLC 비율이 100 이상으로 정의됨), 골수내 형질세포 60% 이상, magnetic resonance imaging 영상 검사에서 1개를 초과하는 병변을 포함하는 추가적인 &#x00022;생체표지자(SLiM 기준이라고도 함)&#x00022;를 MM 진단 기준에 추가하여 정의하였다. 이러한 개정은 이전에 SMM으로 진단되어 치료없이 경과 관찰만 하던 환자 중 비교적 빠른 시간 안에 MM으로 진행하는 고위험군 환자를 MM으로 진단할 수 있게 됨으로써 잠재적으로 비가역적 장기손상이 발생하기 전에 조기에 적절한 치료를 고려할 수 있으므로 중요한 의미를 갖는다.</p>
<p>IMWG 가이드라인과 유사하게 sFLC는 National Comprehensive Cancer Network (NCCN) Guidelines<sup>&#x000ae;</sup>에서도 의심되는 MM 및 관련 형질세포 질환이 의심되는 모든 환자의 선별 검사에서 권장된다&#x005B;<xref ref-type="bibr" rid="b26-kjm-96-5-371">26</xref>&#x005D;. SPE 및 sIFE와 함께 sFLC를 선별 검사에 이용하는 것이 MM 진단까지의 시간 단축(159일에서 45일)과 MM 진단율의 증가(연간 949건에서 967건)를 가져올 것으로 예측되었고, 진단 검사 비용에 37,680 달러가 추가로 필요하겠지만, 조기 진단으로 인한 합병증 관련 비용으로 연간 74,135 달러를 절약할 수 있을 것으로 평가하였다.</p>
<p>NICE 가이드라인은 의료전문가와 환자가 정보에 입각한 선택을 할 수 있도록 돕기 위해 임상 및 비용효율성에 대한 근거를 기반으로 발간된다. 2016년에 NICE에서 발표된 MM 가이드라인에서는 MM이 의심되는 환자에서 SPE 및 sFLC 검사를 사용하여 진단 혹은 배제할 것을 권장한다&#x005B;<xref ref-type="bibr" rid="b24-kjm-96-5-371">24</xref>&#x005D;. 4,054명의 환자를 대상으로 한 3개의 연구에서 얻은 근거에 따르면 sFLC와 SPE를 병합하면 MM및 관련 형질세포 질환의 진단 민감도가 향상되어 94% (72-99%)의 추정치를 얻을 수 있었다. MM의 진단을 배제하기 위해 단일 검사법의 결과만을 사용해서는 안 된다고 주장한다.</p>
</sec>
<sec>
<title>신장 손상 환자에서 다발골수종 진단 및 배제를 위한 검사 패널</title>
<p>FLC은 정상인의 경우, 골수와 림프절에서 하루에 약 500 mg이 생성되며 주로 신장에 의해 빠르게 대사된다. 사구체로 여과되는 대부분의 FLC는 하루에 10-30 g의 단백질을 흡수하는 근위세뇨관에서 재흡수 되기 때문에 소변에서의 M-FLC의 측정은 신장 기능에 의존적일 수밖에 없다. 따라서 질환 초기 혹은 미량의 FLC를 분비하는 환자에서 UPE 검사에서 검출되지 않을 수 있다.</p>
<p>혈청 &#x003ba;-FLC 및 &#x003bb;-FLC의 농도는 생산과 제거 간의 균형에 의존한다. sFLC는 신장에 의해 빠르게 제거되고 대사된다. 약 25 kDa 크기의 단량체인 &#x003ba;-FLC는 사구체 여과율의 40%에서 2-4시간 내에 제거된다. 약 50 kDa의 이량체인 &#x003bb;-FLC는 사구체 여과율의 20%에서 3-6시간 내에 제거된다&#x005B;<xref ref-type="bibr" rid="b27-kjm-96-5-371">27</xref>,<xref ref-type="bibr" rid="b28-kjm-96-5-371">28</xref>&#x005D;. 정상인의 경우 하루에 1-10 mg의 FLC가 소변으로 배출된다. 신장 손상 환자의 경우 정상인에서보다 FLC가 원활하게 제거되지 못하여 FLC의 반감기가 길어져 혈청에서의 농도가 증가하게 된다. 신장 기능의 이상 정도에 따라 FLC 제거율의 감소 속도는 &#x003ba;가 &#x003bb;보다 영향을 더 받게 되어 sFLC &#x003ba;/&#x003bb; 비가 다소 증가한다&#x005B;<xref ref-type="bibr" rid="b29-kjm-96-5-371">29</xref>&#x005D;. 따라서, sFLC &#x003ba;/&#x003bb; 비의 일반적인 정상 참고범위(0.26-1.65)가 아닌 신장 기능 이상 환자에게 조정된 sFLC &#x003ba;/&#x003bb; 비의 참고범위(0.37-3.1)를 적용하면 특이도를 증가시킬 수 있다.</p>
<p>MG에 의한 신장 손상의 양상은 상당히 다양하고, pH 또는 국소 단백질 분해와 같은 환경 요인뿐만 아니라 개별 M-FLC, 특히 가변 도메인의 구조적 특성에 의하여 영향을 받는다&#x005B;<xref ref-type="bibr" rid="b30-kjm-96-5-371">30</xref>&#x005D;. MG와 관련된 신장 질환은 사구체 또는 세뇨관-간질(tubulo-interstitial)의 두드러진 손상에 따라 두 가지 주요 그룹으로 분류될 수 있다. MM 환자에서 주로 관찰되는(&#x0007e;90%) 신손상의 형태는 원주 신장병증(cast nephropathy)이며, 신세뇨관의 위축이나 세뇨관-간질의 섬유화가 특징적인 소견이다&#x005B;<xref ref-type="bibr" rid="b31-kjm-96-5-371">31</xref>&#x005D;.</p>
<p>급성 신손상 환자에서 MM이 의심되는 경우에 주로 SPE, sIFE와 UPE 검사를 수행해왔다. 그러나, 더 간단하고 신속한 접근법은 SPE와 함께 sFLC를 수행하는 것이다&#x005B;<xref ref-type="bibr" rid="b32-kjm-96-5-371">32</xref>&#x005D;. Cook 등&#x005B;<xref ref-type="bibr" rid="b33-kjm-96-5-371">33</xref>&#x005D;이 발표한 경제성 평가 연구에서는 SPE와 sFLC를 함께 시행하는 것이 진단 및 치료 단계에서 경제적 이득이 있다고 보고하였다. sFLC를 알고리즘에 포함시키는 이 접근법은 진단 및 치료에 소요되는 시간을 줄여 신장 기능 회복 및 생존율 향상을 가져왔다.</p>
<p>Koo 등&#x005B;<xref ref-type="bibr" rid="b34-kjm-96-5-371">34</xref>&#x005D;은 신장 생검을 받은 한국인 환자에서 M-단백을 선별하기 위한 검사로 sFLC의 진단 민감도를 다른 검사들과 조합하여 평가하였다. 신증후군(nephrotic syndrome, NS) 환자의 경우, sIFE, uIFE와 FLC 검사 패널은 MM 및 신장에 의미 있는 단세포군감마글로불린병증(monoclonal gammopathy of renal significance, MGRS)에 대하여 100%의 진단 민감도를 보였다. 만성 사구체 신염(chronic glomerulonephritis) 환자의 경우, sFLC와 SPE 검사 패널은 MM에 대하여 100% 민감도를 보여주었다. 저자들은 CKD 환자의 경우 M-단백의 확인을 위한 선별 검사로 sIFE와 sFLC를 고려해야 하며 두 검사가 모두 음성인 경우 NS를 동반한 환자에게는 uIFE를 추가할 수 있다고 제안하였다.</p>
</sec>
<sec>
<title>다른 MG 질환의 조기 진단을 위한 선별 검사 패널</title>
<p>전신성 또는 AL 아밀로이드증은 M-FLC 또는 그 단편이 기능적&#x000b7;구조적 장기 손상을 일으키는 세포 외 불용성 amyloid fibril이 축적되는 것을 특징으로 하는 질환이다. AL 아밀로이드증은 여러 기관에 영향을 미치지만 대부분의 경우 신장(74%), 심장(60%), 간(27%), 말초 신경계(22%) 및 자율신경계(18%)에 기능 저하를 유발한다&#x005B;<xref ref-type="bibr" rid="b35-kjm-96-5-371">35</xref>&#x005D;. &#x003ba;형 AL은 일반적으로 위장관과 간에 영향을 미치는 반면, &#x003bb; 형 AL은 신장에 손상을 유발하여 신 증후군 단백뇨가 높은 비율에서 관찰된다. 조직 분포는 개별 FLC의 구조적 특성과 관련이 있을 수 있다&#x005B;<xref ref-type="bibr" rid="b36-kjm-96-5-371">36</xref>,<xref ref-type="bibr" rid="b37-kjm-96-5-371">37</xref>&#x005D;. AL 아밀로이드증의 조기 진단은 효과적인 화학요법을 빠르게 적용하여 비가역적인 장기 손상이 발생하기 전에 아밀로이드 형성 FLC의 생산을 억제할 수 있으므로 매우 중요하다. IMWG와 British Committee for Standards in Haematology 가이드라인에서는 AL 아밀로이드증이 의심되는 환자에서 sIFE, uIFE와 함께 sFLC를 함께 시행할 것을 권고하였다&#x005B;<xref ref-type="bibr" rid="b23-kjm-96-5-371">23</xref>,<xref ref-type="bibr" rid="b38-kjm-96-5-371">38</xref>&#x005D;. AL 아밀로이드증에 대한 선별 검사에서 sFLC의 진단 민감도와 전기영동검사법을 비교한 많은 연구가 보고되었다&#x005B;<xref ref-type="bibr" rid="b23-kjm-96-5-371">23</xref>&#x005D;. Katzmann 등&#x005B;<xref ref-type="bibr" rid="b11-kjm-96-5-371">11</xref>&#x005D;은 MM, AL 아밀로이드증 및 관련 형질세포 질환이 의심되는 환자에서 MG를 확인하기 위한 선별 검사 패널을 비교하였다. 이 연구에서 AL 아밀로이드증으로 진단된 581명을 대상으로 sFLC의 민감도는 88.3%를 보였으며, sIFE를 포함하면 97.1%로 증가하였다. 또한 혈청 검체를 이용한 검사 패널에 uIFE를 추가하면 민감도가 98.1%로 증가하나 AL 아밀로이드증 환자의 매우 소수의 환자에서만 소변 검체를 이용한 검사로 M-단백이 추가로 검출될 수 있음을 확인하였다. Palladini 등&#x005B;<xref ref-type="bibr" rid="b39-kjm-96-5-371">39</xref>&#x005D;은AL 아밀로이드증으로 진단된 환자 121 명에 대한 전향적 연구 결과에서 단일 검사만으로 진단을 배제해서는 안되며, sFLC와 함께 sIFE, uIFE를 포함하는 선별 검사 알고리즘이 AL 아밀로이드증에 대하여 100% 민감도를 보여준다고 결론지었다. 지금까지 발표된 많은 연구에서, sFLC는 AL 아밀로이드증이 의심되는 환자에서 효과적인 선별 검사임이 입증되었다&#x005B;<xref ref-type="bibr" rid="b36-kjm-96-5-371">36</xref>,<xref ref-type="bibr" rid="b40-kjm-96-5-371">40</xref>-<xref ref-type="bibr" rid="b44-kjm-96-5-371">44</xref>&#x005D;. 2016년 미국심혈관학회 연례 모임에서 아밀로이드증 연구 컨소시엄에서는 모든 심부전 환자에게 sFLC를 시행하여 AL아밀로이드증 동반 여부에 대한 감별 진단을 조기에 할 것을 권고하였다&#x005B;<xref ref-type="bibr" rid="b45-kjm-96-5-371">45</xref>&#x005D;.</p>
<p>고립형질세포종(solitary plasmacytoma)은 골수 형질세포증가증(bone marrow plasmacytosis)이 없거나 10% 미만이면서 클론성 골수 형질세포(clonal bone marrow plasma cells)의 단일 종양이 있고, 1차 병변과 관련된 증상을 제외하고는 다른 증상이 없다는 특징이 있다. 형질세포종은 MM으로 진행될 수 있다. 뼈의 고립 형질세포종(solid bone plasmacytoma, SBP) 환자의 약 절반은 전형적으로 약 5 g/L 정도의 소량의 혈청 M-단백이 검출되며&#x005B;<xref ref-type="bibr" rid="b46-kjm-96-5-371">46</xref>&#x005D;, 가장 일반적인 M-단백의 유형은 IgG이며 그 다음이 FLC이다. SBP 환자의 40% 이상이 sIFE와 uIFE에서 음성을 보이지만 sFLC에서는 M-단백이 검출된다&#x005B;<xref ref-type="bibr" rid="b46-kjm-96-5-371">46</xref>,<xref ref-type="bibr" rid="b47-kjm-96-5-371">47</xref>&#x005D;.</p>
<p>PCL은 MM의 희귀하고 공격적인 변이형으로 형질세포 관련 질환의 2-4%를 차지한다. 말초혈액에서 형질세포가 20%를 초과하여 존재하거나 형질세포의 수가 2 &#x000d7; 10<sup>9</sup>/L를 초과하는 것으로 정의된다&#x005B;<xref ref-type="bibr" rid="b48-kjm-96-5-371">48</xref>&#x005D;. MM의 증거가 없이 처음부터 원발성으로 발생할 수도 있고(primary PCL, 60-70%), 진행성 및 불응성 MM 환자의 1-2%에서는 이전의 MM에서 백혈병 양상으로 변형되어 발생할 수도 있다(secondary PCL, 30-40%) &#x005B;<xref ref-type="bibr" rid="b49-kjm-96-5-371">49</xref>&#x005D;. M-단백은 대부분의 PCL 환자에서 검출이 되나, 일부 환자에서는 M-단백이 SPE 검사로 검출되지 않을 수 있지만, sFLC 검사에서는 확인이 된다&#x005B;<xref ref-type="bibr" rid="b50-kjm-96-5-371">50</xref>&#x005D;. MM과 마찬가지로, SPE와 sFLC 검사의 조합은 PCL에 대한 효과적인 선별 검사 패널로 밝혀졌으며&#x005B;<xref ref-type="bibr" rid="b11-kjm-96-5-371">11</xref>&#x005D;, sFLC는 PCL가 의심되는 환자에서 조기 진단을 위한 검사로 시행되어야 한다&#x005B;<xref ref-type="bibr" rid="b51-kjm-96-5-371">51</xref>&#x005D;.</p>
<p>M-단백의 존재는 MG의 특징이지만 만성 림프구백혈병(chronic lymphocytic leukemia, CLL)과 NHL과 같은 다른 B세포 악성 종양에서도 검출될 수 있다&#x005B;<xref ref-type="bibr" rid="b2-kjm-96-5-371">2</xref>,<xref ref-type="bibr" rid="b3-kjm-96-5-371">3</xref>,<xref ref-type="bibr" rid="b52-kjm-96-5-371">52</xref>,<xref ref-type="bibr" rid="b53-kjm-96-5-371">53</xref>&#x005D;. 비정상 sFLC는 CLL과 악성 림프종의 세부 아형의 종류에 따라 다른 양상을 보인다(<xref rid="t4-kjm-96-5-371" ref-type="table">Table 4</xref>) &#x005B;<xref ref-type="bibr" rid="b53-kjm-96-5-371">53</xref>-<xref ref-type="bibr" rid="b70-kjm-96-5-371">70</xref>&#x005D;. 예를 들어, 비정상 sFLC &#x003ba;/&#x003bb;비는 호지킨림프종(Hodgkin lymphoma, HL) 환자의 5-7%에서 발견되지만&#x005B;<xref ref-type="bibr" rid="b54-kjm-96-5-371">54</xref>-<xref ref-type="bibr" rid="b56-kjm-96-5-371">56</xref>&#x005D;, 맨틀세포림프종(mantle cell lymphoma) 환자에서는 36-77% &#x005B;<xref ref-type="bibr" rid="b53-kjm-96-5-371">53</xref>,<xref ref-type="bibr" rid="b60-kjm-96-5-371">60</xref>,<xref ref-type="bibr" rid="b61-kjm-96-5-371">61</xref>&#x005D;, WM 환자에서는 70% &#x005B;<xref ref-type="bibr" rid="b63-kjm-96-5-371">63</xref>-<xref ref-type="bibr" rid="b65-kjm-96-5-371">65</xref>&#x005D;까지 검출된다.</p>

<sec>
<title>골병변을 동반한 환자에서 다발골수종 선별 검사의 중요성</title>
<p>다발골수종은 형질세포의 골수 침윤으로 통증, 병적 골절(pathologic fracture), 척수압박증후군(spinal cord compression) 등의 뼈 병변이 주 증상으로 나타나는 질환으로, 약 90%의 환자에서 골용해성병변(osteolytic lesion)이 나타난다&#x005B;<xref ref-type="bibr" rid="b71-kjm-96-5-371">71</xref>&#x005D;. 이러한 뼈의 통증은 진단 시 2/3에서 나타나는 주요한 증상이다&#x005B;<xref ref-type="bibr" rid="b8-kjm-96-5-371">8</xref>&#x005D;. 진단된 환자의 약 80%에서 X선 검사에 이상 소견이 관찰되며, 주된 침범 부위는 척추(65%), 늑골(45%), 두개골(40%), 어깨(40%), 골반(40%), 장골(30%) 순이다&#x005B;<xref ref-type="bibr" rid="b71-kjm-96-5-371">71</xref>&#x005D;. 따라서 뼈의 통증은 MM의 진단에 가장 중요한 신호로 여겨질 수 있지만, MM이 매우 드문 질환이라는 인식으로 효과적인 선별 검사가 시행되지 않아서 진단이 지연되는 경우가 많다. MM에 대한 효과적인 치료 약제의 개발로 많은 환자들이 장기 생존이 가능해지고 있으므로 뼈 등 장기 손상이 진행되기 전에 빠른 진단을 받는 것이 환자의 생존 기간을 연장하는데 중요하다&#x005B;<xref ref-type="bibr" rid="b8-kjm-96-5-371">8</xref>&#x005D;. 앞서 언급된 MM의 효과적인 선별 검사 패널을 고려할 때, 원인불명의 뼈 통증이 있는 환자에서 SPE와 더불어 sFLC를 시행하는 것은 MM 진단을 앞당기는 데 도움이 된다.</p>

</sec>
<sec sec-type="conclusions">
<title>결 론</title>
<p>수도권의 한 지역 MGUS 유병률을 조사한 연구에 의하면 우리나라 MGUS 유병률은 65세 이상에서 3.3%로 보고된다&#x005B;<xref ref-type="bibr" rid="b72-kjm-96-5-371">72</xref>&#x005D;. 우리나라의 유병률과 유사할 것으로 보이는 일본인의 유병률을 참고할 때 50세 이상에서 2.0-2.4%로 추정할 수 있다. 2020년 우리나라 50세 이상의 인구 약 21,000,000명을 고려할 때&#x005B;<xref ref-type="bibr" rid="b73-kjm-96-5-371">73</xref>&#x005D;, 한국인의 MGUS 유병 환자수는 420,000명에서 500,000명으로 추산된다. 매년 약 1%의 MGUS 환자가 MM, AL 아밀로이드증 등과 같은 악성 형질세포 질환으로 진행한다는 연구 결과를 볼 때&#x005B;<xref ref-type="bibr" rid="b74-kjm-96-5-371">74</xref>-<xref ref-type="bibr" rid="b77-kjm-96-5-371">77</xref>&#x005D;, 매년 4,200명에서 5,000명의 환자가 MM 등 악성 형질세포 질환으로 진단될 수 있음을 추정할 수 있다. 그러나, 실제 진단되는 MM 환자는 2012년 1,272명으로 보고되었고, 과거 10년간 두 배 정도 발생빈도가 증가한 증가율을 고려하여&#x005B;<xref ref-type="bibr" rid="b78-kjm-96-5-371">78</xref>&#x005D;, 2021년에는 약 2,000명의 환자가 새로 진단될 수 있다고 추정할 수 있다. 즉, 추가로 MM 및 악성 형질세포 질환으로 진단될 수 있는 2,000명에서 3,000명의 환자는 정확한 진단을 받지 못하고 관련 증상에 대한 치료만 받다가 사망에 이른다고도 볼 수 있다. 보건의료빅데이터개방시스템&#x005B;<xref ref-type="bibr" rid="b79-kjm-96-5-371">79</xref>&#x005D;에 등록된 MG 환자수는 2019년 10,870명으로 연 증가율을 고려할 때 2021년에는 13,000명이 넘을 것으로 추정된다. 따라서 MM, AL 아밀로이드증 등 형질세포 질환 의심 증상을 보이는 고령의 환자에서 효과적인 선별 검사를 시행하는 것은 조기 진단뿐만 아니라 환자의 치료 효과, 생존율 향상에 크게 기여할 것으로 보인다.</p>
<p>MM을 비롯한 MG의 조기 진단을 위한 가장 효과적이고 효율적인 선별 검사 패널에 대한 많은 연구들이 이뤄져 왔고, 각국의 임상 진료 지침 또한 개정되어 왔다. 보건의료빅데이터개방시스템&#x005B;<xref ref-type="bibr" rid="b79-kjm-96-5-371">79</xref>&#x005D;에서 SPE와 sFLC 청구된 환자수를 분석한 결과에서는 2021년 MM 등 MG가 의심되어 SPE가 의뢰된 환자는 약 100,000명 정도가 될 것으로 추산되나, sFLC가 의뢰된 환자는 이 중 1/4 정도에 불과할 것으로 추산된다.</p>
<p>SPE와 sFLC는 대부분의 임상 진료 지침에서 MG 진단을 위한 선별 검사 패널에 기본적으로 포함된다. 관련 증상을 보이지만 원인이 분명하지 않은 고연령의 환자에서는 반드시 SPE와 sFLC 선별 검사를 통하여 MM, AL 아밀로이드증 등의 MG질환을 진단 혹은 배제해야 한다. 따라서 임상적으로 의심되지만 SPE에서 M-단백이 검출되지 않은 경우, sFLC는 M-단백의 존재를 놓치지 않기 위해 반드시 추가로 시행되어야 한다. 이러한 선별 검사 패널이 진단 지연으로 인한 의료 비용 증가를 고려해 볼 때 비용 효과적이고 의료비용 절감에 도움이 된다는 것이다&#x005B;<xref ref-type="bibr" rid="b12-kjm-96-5-371">12</xref>,<xref ref-type="bibr" rid="b17-kjm-96-5-371">17</xref>,<xref ref-type="bibr" rid="b22-kjm-96-5-371">22</xref>,<xref ref-type="bibr" rid="b24-kjm-96-5-371">24</xref>,<xref ref-type="bibr" rid="b33-kjm-96-5-371">33</xref>,<xref ref-type="bibr" rid="b80-kjm-96-5-371">80</xref>&#x005D;.</p>
<p>2019년 12월 1일에 신설되어 고시된 sFLC 보험급여 인정 기준에서는 MM, AL 아밀로이드증 등 형질세포 질환과 악성림프종이 의심되는 경우 진단을 위하여 시행하는 경우가 포함되었다&#x005B;<xref ref-type="bibr" rid="b81-kjm-96-5-371">81</xref>&#x005D;. 고칼슘혈증, 신장 기능 이상, 빈혈, 원인불명의 뼈 통증 등의 MM 관련 증상을 보이는 고령의 환자들에 대하여 조기 진단을 위한 선별 검사 패널로서 SPE와 sFLC를 함께 고려함으로써 정확한 진단이 조기에 이루어져서 많은 환자들이 적절한 시기에 최선의 치료를 받을 수 있게 되기를 바란다.</p>
