<trans-abstract xml:lang="en"><p>Microscopic colitis (MC) is a chronic idiopathic inflammatory bowel disease presenting with chronic watery diarrhea. Epidemiologic studies from Western countries have demonstrated that it is almost as common as other classic inflammatory bowel diseases, such as Crohn&#x02019;s disease and ulcerative colitis. Histological examination can confirm the diagnosis and differentiate between the two main subtypes of MC: collagenous colitis and lymphocytic colitis. The pathophysiology of MC remains unknown; however, possible etiologies include genetic predispositions, autoimmunity, inflammatory responses to luminal factors such as certain drugs or bacteria, and myofibroblast dysregulations. The aim of MC therapy should take into account the severity of symptoms, impact on quality of life, and evidence from clinical trials of available medical treatments.</p></trans-abstract>

<title>서 론</title>
<p>현미경적 대장염(microscopic colitis)은 만성 재발성 설사를 일으키는 염증성 장질환이다. 이 질환은 1980년 Read 등&#x005B;<xref ref-type="bibr" rid="b1-kjm-89-6-663">1</xref>&#x005D;이 최초로 보고한 후 만성 설사를 호소하는 환자에서 드물지 않은 원인으로 알려지고 있다. 대장의 육안적 소견은 정상에 가까우며 다른 검사에서도 이상소견이 발견되지 않는다. 진단은 병리학적으로 하며 조직소견에 따라 두 가지 아형으로 분류할 수 있다. 림프구성 대장염(lymphocytic colitis)은 1989년 Lazenby 등&#x005B;<xref ref-type="bibr" rid="b2-kjm-89-6-663">2</xref>&#x005D;이 보고하였으며 상피내 림프구(intraepithelial lymphocyte) 증가 및 상피하 점막 고유판에 만성 염증성 세포의 침윤을 보이고, 교원성 대장염(collagenous colitis)은 1976년 Lindstr&#x000f6;m 등&#x005B;<xref ref-type="bibr" rid="b3-kjm-89-6-663">3</xref>&#x005D;이 보고하였고 상피하 콜라겐 층의 비후가 특징이다. 현미경적 대장염의 두 가지 아형은 조직학적 유사성이 있고 한 아형에서 다른 아형으로 전화할 수 있어 각 아형을 개별적 질병으로 나누는 데 이견이 있으며&#x005B;<xref ref-type="bibr" rid="b4-kjm-89-6-663">4</xref>&#x005D;, 임상적으로 아형에 따른 차이는 없다&#x005B;<xref ref-type="bibr" rid="b5-kjm-89-6-663">5</xref>&#x005D;.</p>
<p>본고에서는 현미경적 대장염의 역학, 증상, 진단 및 치료에 대해 살펴보고, 원인 인자 및 병인론에 대해 논하고자 한다.</p>

<sec>
<title>역 학</title>
<p>스웨덴과 미국의 연구에서 현미경적 대장염의 유병률은 인구 십만 명당 123-219명으로 보고하였다&#x005B;<xref ref-type="bibr" rid="b6-kjm-89-6-663">6</xref>,<xref ref-type="bibr" rid="b7-kjm-89-6-663">7</xref>&#x005D;. 스웨덴, 스페인, 아이슬란드를 포함한 유럽의 단일기관 연구에서 교원성 대장염의 발생률은 연간 인구 십만 명당 1.1-5.2명, 림프구성 대장염은 3.1-4.0명&#x005B;<xref ref-type="bibr" rid="b8-kjm-89-6-663">8</xref>-<xref ref-type="bibr" rid="b11-kjm-89-6-663">11</xref>&#x005D;, 미국 및 캐나다에서도 각각 3.1-4.6명, 5.4-5.5명으로 유사하였다&#x005B;<xref ref-type="bibr" rid="b12-kjm-89-6-663">12</xref>,<xref ref-type="bibr" rid="b13-kjm-89-6-663">13</xref>&#x005D;. 유럽 및 미국에서 증가 추세를 보였으나&#x005B;<xref ref-type="bibr" rid="b11-kjm-89-6-663">11</xref>,<xref ref-type="bibr" rid="b12-kjm-89-6-663">12</xref>&#x005D;, 최근 발생률이 안정화된 것으로 보인다&#x005B;<xref ref-type="bibr" rid="b7-kjm-89-6-663">7</xref>&#x005D;. 이전의 발생률 증가의 원인으로 임상 의사의 관심 및 대장내시경 건수의 증가와 원인 약제 사용의 증가 등이 제시된 바 있다&#x005B;<xref ref-type="bibr" rid="b12-kjm-89-6-663">12</xref>,<xref ref-type="bibr" rid="b14-kjm-89-6-663">14</xref>,<xref ref-type="bibr" rid="b15-kjm-89-6-663">15</xref>&#x005D;.</p>
<p>현미경적 대장염의 진단율은 서구에서 만성 설사를 호소하는 환자의 9.5-10.2% &#x005B;<xref ref-type="bibr" rid="b9-kjm-89-6-663">9</xref>,<xref ref-type="bibr" rid="b16-kjm-89-6-663">16</xref>-<xref ref-type="bibr" rid="b18-kjm-89-6-663">18</xref>&#x005D;, 국내 단일기관 연구에서는 만성 설사를 호소하는 환자 중 대장내시경 소견이 정상인 경우 15% &#x005B;<xref ref-type="bibr" rid="b19-kjm-89-6-663">19</xref>&#x005D;, 다기관 전향적 연구에서 22%였다&#x005B;<xref ref-type="bibr" rid="b20-kjm-89-6-663">20</xref>&#x005D;.</p>
<p>고령에서 호발하며 발병 당시 평균 나이는 53-69세이지만, 45세 이하 젊은 연령이 25%를 차지한다&#x005B;<xref ref-type="bibr" rid="b21-kjm-89-6-663">21</xref>&#x005D;. 성비는 3:1-9:1로 여성에서 호발한다&#x005B;<xref ref-type="bibr" rid="b8-kjm-89-6-663">8</xref>,<xref ref-type="bibr" rid="b9-kjm-89-6-663">9</xref>,<xref ref-type="bibr" rid="b13-kjm-89-6-663">13</xref>,<xref ref-type="bibr" rid="b22-kjm-89-6-663">22</xref>&#x005D;.</p>
<p>현미경적 대장염과 대장암의 직접적인 연관성은 없는 것으로 보인다&#x005B;<xref ref-type="bibr" rid="b12-kjm-89-6-663">12</xref>,<xref ref-type="bibr" rid="b22-kjm-89-6-663">22</xref>,<xref ref-type="bibr" rid="b23-kjm-89-6-663">23</xref>&#x005D;. 대규모 후향적 코호트 연구에서는 대장 선종 및 암의 발생률이 현미경적 대장염 환자에서 더 낮았다&#x005B;<xref ref-type="bibr" rid="b24-kjm-89-6-663">24</xref>&#x005D;.</p>
</sec>
<sec>
<title>병태생리</title>
<p>현미경적 대장염의 원인 및 병태생리는 명확하지 않으며, 유전적 소인과 자가면역성, 다양한 장관내 인자의 자극으로 인하여 촉발된 염증 반응 및 섬유화 과정의 이상 등이 복합적으로 관여하는 다인성(multifactorial)으로 추정한다(<xref rid="f1-kjm-89-6-663" ref-type="fig">Fig. 1</xref>) &#x005B;<xref ref-type="bibr" rid="b5-kjm-89-6-663">5</xref>,<xref ref-type="bibr" rid="b25-kjm-89-6-663">25</xref>-<xref ref-type="bibr" rid="b27-kjm-89-6-663">27</xref>&#x005D;.</p>
<p>가족내 발병 및 유전적 소인과의 연관성은 분명하지 않지만&#x005B;<xref ref-type="bibr" rid="b28-kjm-89-6-663">28</xref>-<xref ref-type="bibr" rid="b30-kjm-89-6-663">30</xref>&#x005D;, HLA-DQ2, matrix metalloproteinase (MMP) 유전자 변이가 있을 수 있다&#x005B;<xref ref-type="bibr" rid="b31-kjm-89-6-663">31</xref>,<xref ref-type="bibr" rid="b32-kjm-89-6-663">32</xref>&#x005D;. 자가면역성 질환 중 celiac 병, 갑상선염, 제 1형 당뇨병, 류마티스관절염 등이 연관된 것으로 알려져 있다&#x005B;<xref ref-type="bibr" rid="b22-kjm-89-6-663">22</xref>,<xref ref-type="bibr" rid="b33-kjm-89-6-663">33</xref>,<xref ref-type="bibr" rid="b34-kjm-89-6-663">34</xref>&#x005D;. Celiac 병과의 연관성 연구에서 림프구성 대장염 환자의 15%에서 celiac 병이 진단되었으며&#x005B;<xref ref-type="bibr" rid="b35-kjm-89-6-663">35</xref>&#x005D;, celiac 병과 연관된 HLA-DR3-DQ2 haplotype이 현미경적 대장염과도 연관되어 공통적인 자가면역성 병인을 추정할 수 있다&#x005B;<xref ref-type="bibr" rid="b36-kjm-89-6-663">36</xref>&#x005D;. 교원성 대장염 환자에서는 면역글로불린 M 및 항핵항체(antinuclear antibody)가 증가될 수 있으나 임상적으로 유용한 자가항체는 없다&#x005B;<xref ref-type="bibr" rid="b37-kjm-89-6-663">37</xref>&#x005D;.</p>
<p>일시적인 환상 회장루(loop ileostomy) 수술 후 증상과 조직학적 호전을 보이는 것은 장관 내강내의 인자가 병인으로 작용할 수 있음을 시사하며&#x005B;<xref ref-type="bibr" rid="b38-kjm-89-6-663">38</xref>&#x005D;, 약물, 담즙, 세균 및 독소 등 다양하게 제시하고 있다. 비스테로이드성 항염제, 양성자 펌프 억제제, 선택적 세로토닌 재흡수 억제제, ranitidine, ticlopidine, acarbose, statin 등의 연관성이 알려져 있으며, carbamazepine, flutamide, paroxetine 등의 약제와의 연관성이 보고되었다(<xref rid="t1-kjm-89-6-663" ref-type="table">Table 1</xref>) &#x005B;<xref ref-type="bibr" rid="b5-kjm-89-6-663">5</xref>,<xref ref-type="bibr" rid="b14-kjm-89-6-663">14</xref>&#x005D;. 네덜란드의 대규모 환자 대조군 연구에서 양성자 펌프억제제(adjusted odds ratio &#x005B;OR&#x005D; 10.6; 95% confidence interval &#x005B;CI&#x005D; 1.8-64.2)와 비스테로이드성 항염제(adjusted OR 5.6; 95% CI 1.2-27.0)가 현미경적 대장염의 위험인자였다&#x005B;<xref ref-type="bibr" rid="b39-kjm-89-6-663">39</xref>&#x005D;. 25개의 역학 연구에 대한 메타 분석에서는 양성자 펌프 억제제(OR 2.68; 95% CI 1.73-4.17)와 선택적 세로토닌 재흡수 억제제(OR 2.41; 95% CI 1.64-3.53)가 연관되었다&#x005B;<xref ref-type="bibr" rid="b40-kjm-89-6-663">40</xref>&#x005D;. Lansoprazole, omeprazole, esomeprazole 등의 양성자 펌프 억제제와 림프구성 대장염과의 연관성이 보고된 바 있다&#x005B;<xref ref-type="bibr" rid="b41-kjm-89-6-663">41</xref>-<xref ref-type="bibr" rid="b43-kjm-89-6-663">43</xref>&#x005D;. 비스테로이드성 항염제를 사용하였던 현미경적 대장염 환자와 과민성장증후군 또는 대장 게실을 가진 환자를 대조군으로 한 연구에서 현미경적 대장염 환자는 6개월 이상 장기간의 비스테로이드성 항염제를 사용한 경우가 많았으며&#x005B;<xref ref-type="bibr" rid="b44-kjm-89-6-663">44</xref>&#x005D;, 스웨덴의 후향적 연구에서 교원성 대장염 환자의 34%가 비스테로이드성 항염제를 사용한 과거력이 있었다&#x005B;<xref ref-type="bibr" rid="b21-kjm-89-6-663">21</xref>&#x005D;. 이 약제를 중단 및 재투여한 후 설사 증상이 호전 및 재발하는 경과를 보인 것은 약제와의 연관성을 시사한다&#x005B;<xref ref-type="bibr" rid="b44-kjm-89-6-663">44</xref>&#x005D;. 그러나 이러한 약제의 높은 사용 빈도에 비하여 현미경적 대장염의 발생률이 낮은 것은 약제에 대한 특이반응(idiosyncratic reaction)일 수 있으며 약제 자체의 부작용일 가능성이 있고&#x005B;<xref ref-type="bibr" rid="b45-kjm-89-6-663">45</xref>&#x005D;, 약제 중단 후 임상적, 조직학적 호전 및 재투여시 증상 발생 등을 통한 인과관계의 입증과 발병 기전에 대한 추가연구가 필요하다&#x005B;<xref ref-type="bibr" rid="b25-kjm-89-6-663">25</xref>&#x005D;.</p>
<p>세균 감염 및 독소에 대한 연구로는 세균의 독소와 결합하거나 활성도를 저하시킬 수 있는 cholestyramine이나 bismuth 제제를 사용한 후 호전되거나&#x005B;<xref ref-type="bibr" rid="b46-kjm-89-6-663">46</xref>,<xref ref-type="bibr" rid="b47-kjm-89-6-663">47</xref>&#x005D;, 항생제 치료 후 호전된 환자들에서 병인으로 추정하였다&#x005B;<xref ref-type="bibr" rid="b21-kjm-89-6-663">21</xref>&#x005D;. 계절적으로 림프구성 대장염이 여름과 가을에 호발하는 것이 감염성 원인을 시사할 수 있으며&#x005B;<xref ref-type="bibr" rid="b48-kjm-89-6-663">48</xref>&#x005D;, <italic>Yersinia</italic>나 <italic>Clostridium difficile</italic> 감염과의 연관성도 제시된 바 있다&#x005B;<xref ref-type="bibr" rid="b49-kjm-89-6-663">49</xref>,<xref ref-type="bibr" rid="b50-kjm-89-6-663">50</xref>&#x005D;. 담즙산 흡수장애는 분비성 설사의 한 원인으로 대장 점막의 자극인자가 될 수 있는데, 동물실험에서 담즙산을 관류시킨 쥐의 대장에서 수분 분비가 증가하고 염증세포의 침윤과 콜라겐 침착 등 현미경적 대장염과 유사한 조직학적 변화를 보였다&#x005B;<xref ref-type="bibr" rid="b51-kjm-89-6-663">51</xref>&#x005D;. 교원성 대장염 환자를 대상으로 한 연구에서 27명 중 12명(44%)에서 담즙산 흡수장애 소견을 보였으며, 전체 환자 중 21명(78%)이 cholestyramine, colestipol 등 답즙산 흡착제를 사용한 후 증상의 호전을 보였고, 담즙산 흡수장애를 보인 12명의 환자 중 11명(92%)이 약제 복용 후 호전되었다&#x005B;<xref ref-type="bibr" rid="b52-kjm-89-6-663">52</xref>&#x005D;. 그러나 이 연구에서 담즙산 분비 검사가 정상인 환자에서도 증상 호전이 있었으며, 호기검사를 이용한 다른 연구에서는 담즙산 흡수장애의 소견이 없었다&#x005B;<xref ref-type="bibr" rid="b53-kjm-89-6-663">53</xref>&#x005D;.</p>
<p>현미경적 장염의 주된 조직병리 소견은 염증 세포의 침윤 및 콜라겐 침착이며, 염증 반응 및 섬유화 과정의 이상이 병인으로 제시되었다. 약제, 세균, 담즙산 등 다양한 장관내 인자의 자극이 이러한 반응을 촉발하는 것으로 추정 가능하지만 명확하지 않다. 면역조직화학염색을 통한 연구에서 점막 고유층의 CD4&#x0002b; T 림프구 및 상피내 CD8&#x0002b; T 림프구 증가, 상피세포의 제 2형 주조직적합성 복합체(major histocompatibility complex)의 이상발현이 있었다&#x005B;<xref ref-type="bibr" rid="b54-kjm-89-6-663">54</xref>&#x005D;. 교원성 대장염 및 림프구성 대장염 환자의 조직에서 공통적으로 nitric oxide (NO)가 증가되어 있으며&#x005B;<xref ref-type="bibr" rid="b55-kjm-89-6-663">55</xref>-<xref ref-type="bibr" rid="b57-kjm-89-6-663">57</xref>&#x005D;, inducible NO synthase (iNOS)가 과 발현되어 있었고 iNOS의 길항제인 NG-monomethyl-L-arginine (L-NMMA)로 처리하였을 때 수분 분비가 저하되어 NO의 증가가 조직의 증가된 iNOS에 연유한 것으로 보인다&#x005B;<xref ref-type="bibr" rid="b57-kjm-89-6-663">57</xref>&#x005D;. Budesonide 치료 후 iNOS mRNA는 감소되는데 이는 환자의 조직학적 소견 및 증상 호전과 연관되어 있다&#x005B;<xref ref-type="bibr" rid="b58-kjm-89-6-663">58</xref>&#x005D;. Interferon (IFN)-&#x003b3;, tumour necrosis factor (TNF)-&#x003b1; 등 제1형 도움 T 세포(type 1 T helper cell, Th1)의 활성화와 관련된 사이토카인이 증가되어있으며 상피세포와 고유층에 존재하는 단핵구의 NOS의 증가와 연관되었다&#x005B;<xref ref-type="bibr" rid="b59-kjm-89-6-663">59</xref>&#x005D;. IFN-&#x003b3;, TNF-&#x003b1; 등에 의한 NOS의 증가는 nuclear factor &#x003ba;B의 상향조절로 인한 것으로 보인다&#x005B;<xref ref-type="bibr" rid="b60-kjm-89-6-663">60</xref>&#x005D;. 최근 연구는 점막내 Th17/Tc17, Th1/Tc1 세포와 관련된 사이토카인 및 IFN-&#x003b3;, IL-12, IL-17A, IL-21, IL-22 mRNA 증가를 보고하였다&#x005B;<xref ref-type="bibr" rid="b61-kjm-89-6-663">61</xref>&#x005D;. 이러한 연구 결과는 염증 반응에 관여하는 여러 매개물질이 증상 발생의 원인이 될 수 있다는 것이며, 이 과정의 조절기능 이상 또는 이를 촉발하는 다른 원인에 의한 증상 발생을 설명하는데 도움이 될 수 있다.</p>
<p>교원성 대장염의 특징인 상피하 콜라겐 침착에 착안하여, 콜라겐의 생성, 분해 등에 관여하는 근섬유아세포(myofibroblast)의 기능적 변화도 가능한 병인으로 연구되었다. Transforming growth factor-&#x003b2;1 (TGF-&#x003b2;1)은 조직의 콜라겐 침착 및 섬유화에 관여하며, 교원성 대장염 환자에서 침윤된 호산구의 TGF-&#x003b2;1 mRNA가 대조군보다 증가되어 있었다&#x005B;<xref ref-type="bibr" rid="b62-kjm-89-6-663">62</xref>&#x005D;. 면역조직화학염색을 이용한 연구에서는 섬유화에 관여하는 vascular endothelial growth factor 및 tenascin이 상피 및 고유층에 증가되어 있었으며&#x005B;<xref ref-type="bibr" rid="b63-kjm-89-6-663">63</xref>&#x005D;, 세포 기저막의 콜라겐을 생성하고 장 점막의 정상 구조를 유지하는 역할을 하는 장 상피하 움(crypt) 주변부의 근섬유아세포초(myofibroblast sheath)의 크기와 세포충실도가 교원성 대장염 환자에서 증가되어 있었다&#x005B;<xref ref-type="bibr" rid="b64-kjm-89-6-663">64</xref>&#x005D;. 또한, 콜라겐 분해에 중요한 효소인 MMP의 RNA 발현이 감소하고 MMP의 내인성 길항제인 tissue endogenous inhibitors of metalloproteinase의 발현이 증가되어 있는 것은 이러한 섬유화와 분해 기전의 이상이 관여할 수 있음을 시사한다&#x005B;<xref ref-type="bibr" rid="b65-kjm-89-6-663">65</xref>&#x005D;.</p>
<p>연구 결과를 종합해보면 현미경적 대장염의 병태생리에 대해 다양한 가능한 원인인자가 제시되었으나, 증상 발생 및 조직학적 변화와의 직접적인 인과관계는 밝혀지지 않았으며, 다양한 요인이 복합적으로 작용하는 것으로 보인다.</p>
</sec>
<sec>
<title>증 상</title>
<p>주 증상은 만성 수양성 설사로 재발성 경과를 보이며, 탈수와 전해질 이상은 드물다. 염증으로 인한 흡수 장애, 분비 증가 및 콜라겐 층이 확산 장벽(diffusion barrier)으로 작용하여 설사를 일으키는 것으로 추정한다. 복통과 체중감소를 동반할 수 있으며 반복되는 설사와 변실금으로 삶의 질이 저하된다&#x005B;<xref ref-type="bibr" rid="b66-kjm-89-6-663">66</xref>&#x005D;. 기질적 설사를 의심할 수 있는 야간 증상이 27-50%에서 있을 수 있고&#x005B;<xref ref-type="bibr" rid="b21-kjm-89-6-663">21</xref>,<xref ref-type="bibr" rid="b26-kjm-89-6-663">26</xref>&#x005D;, 지방변, 혈변, 발열 등이 있다면 다른 질환을 고려해야 한다&#x005B;<xref ref-type="bibr" rid="b5-kjm-89-6-663">5</xref>,<xref ref-type="bibr" rid="b26-kjm-89-6-663">26</xref>&#x005D;.</p>
<p>특징적인 증상은 없으며 과민성 장증후군의 증상을 보이는 경우가 56%로 높고 현미경적 대장염 환자의 33%는 과민성 장증후군으로 진단할 수 있으므로 주의를 요한다&#x005B;<xref ref-type="bibr" rid="b67-kjm-89-6-663">67</xref>&#x005D;. 현미경적 대장염 환자에서 과민성 장증후군의 로마 진단기준을 충족하는 경우는 43%이며&#x005B;<xref ref-type="bibr" rid="b68-kjm-89-6-663">68</xref>&#x005D;, 미국의 전향적 연구에서 과민성 장증후군 환자의 1.5%가 현미경적 장염으로 진단되었다&#x005B;<xref ref-type="bibr" rid="b69-kjm-89-6-663">69</xref>&#x005D;.</p>
<p>관절통, 갑상선질환, 건선, 쇼그렌 병 등 자가면역성 질환이 동반될 수 있으며&#x005B;<xref ref-type="bibr" rid="b21-kjm-89-6-663">21</xref>,<xref ref-type="bibr" rid="b22-kjm-89-6-663">22</xref>,<xref ref-type="bibr" rid="b33-kjm-89-6-663">33</xref>,<xref ref-type="bibr" rid="b70-kjm-89-6-663">70</xref>&#x005D;, 드물게 염증성 장질환 환자에서 현미경적 대장염이 병발하거나 현미경적 장염 환자에서 염증성 장질환이 발생한 예가 보고된 바 있다&#x005B;<xref ref-type="bibr" rid="b71-kjm-89-6-663">71</xref>,<xref ref-type="bibr" rid="b72-kjm-89-6-663">72</xref>&#x005D;. 서구에서는 celiac 병과 연관될 수 있어 글루텐 제한 식이에 반응이 없다면 감별 진단해야 한다&#x005B;<xref ref-type="bibr" rid="b73-kjm-89-6-663">73</xref>&#x005D;.</p>
</sec>
<sec>
<title>진 단</title>
<p>환자는 만성 수양성 설사로 내원하게 되므로 다양한 질병의 가능성을 고려하여 포괄적으로 접근하게 되는데, 이러한 진단 과정에서 항상 본 질환의 가능성을 염두에 두어야 한다. 질환 특이 증상 및 질병 표지자가 없고 임상 양상이 기능성 질환과 유사할 수 있어 주의를 요한다.</p>
<p>내시경 소견은 대부분 정상이거나 경한 발적 및 부종 등 비 특이적인 소견이다&#x005B;<xref ref-type="bibr" rid="b74-kjm-89-6-663">74</xref>&#x005D;. 내시경 소견이 정상이어도 15-22% 정도에서 현미경적 대장염이 있을 수 있으므로&#x005B;<xref ref-type="bibr" rid="b20-kjm-89-6-663">20</xref>&#x005D;, 조직 검사를 통해 본 질환을 배제하려는 노력이 가장 중요하다. 교원성 대장염에서 종주 궤양(longitudinal ulcer) &#x005B;<xref ref-type="bibr" rid="b75-kjm-89-6-663">75</xref>&#x005D;, 혈관상 과다(hypervascularity) &#x005B;<xref ref-type="bibr" rid="b76-kjm-89-6-663">76</xref>&#x005D;, 정상 혈관상의 소실&#x005B;<xref ref-type="bibr" rid="b77-kjm-89-6-663">77</xref>&#x005D; 등 내시경 소견 상의 특징을 보고한 바 있으며, 림프구성 대장염에서는 혈관상 과다, 삼출성 출혈(exudative bleeding) 등의 소견이 있을 수 있다&#x005B;<xref ref-type="bibr" rid="b78-kjm-89-6-663">78</xref>&#x005D;.</p>
<p>확진은 조직 검사를 통한 병리 소견으로 한다. 림프구성 대장염의 특징적인 소견은 상피내 림프구 증가이며, 점막 고유층에 만성 염증성 세포의 침윤이 혼재된 양상을 보일 수 있다&#x005B;<xref ref-type="bibr" rid="b79-kjm-89-6-663">79</xref>&#x005D;. 최근 활동성 움염(cryptitis), Paneth 세포 화생, 움 구조의 불규칙 등 염증성 장질환의 조직학적 특징과 유사한 소견을 국소적으로 보이는 경우도 보고하고 있다&#x005B;<xref ref-type="bibr" rid="b80-kjm-89-6-663">80</xref>&#x005D;. 교원성 대장염에서는 상피하 콜라겐 층이 두꺼워지는데, 두께는 7-100 &#x003bc;m로 다양하다&#x005B;<xref ref-type="bibr" rid="b8-kjm-89-6-663">8</xref>,<xref ref-type="bibr" rid="b27-kjm-89-6-663">27</xref>,<xref ref-type="bibr" rid="b81-kjm-89-6-663">81</xref>&#x005D;. 림프구성 대장염과 유사하게 점막 고유층에 염증성 세포가 증가할 수 있으며 활동성 움염, 움농양(crypt abscess) 등 염증성 장질환과 유사한 소견을 보일 수 있고&#x005B;<xref ref-type="bibr" rid="b80-kjm-89-6-663">80</xref>&#x005D;, 표층 상피세포의 손상이 동반될 수 있다&#x005B;<xref ref-type="bibr" rid="b2-kjm-89-6-663">2</xref>,<xref ref-type="bibr" rid="b27-kjm-89-6-663">27</xref>&#x005D;. 최근 유럽의 합의안에는 림프구성 대장염에서 상피내 림프구가 100개의 표층 상피세포 중 20개 이상, 교원성 대장염에서는 상피하 콜라겐 층의 두께가 10 &#x003bc;m 이상을 진단기준으로 정하였다&#x005B;<xref ref-type="bibr" rid="b82-kjm-89-6-663">82</xref>&#x005D;.</p>
</sec>
<sec>
<title>치 료</title>
<p>세밀한 병력청취를 하여 원인이 될 수 있는 약제나 음식을 중단한다. 흡연의 연관성이 제시된 바 있어 금연을 권한다&#x005B;<xref ref-type="bibr" rid="b83-kjm-89-6-663">83</xref>,<xref ref-type="bibr" rid="b84-kjm-89-6-663">84</xref>&#x005D;. 증상이 경하다면 loperamide 등의 지사제를 사용해 볼 수 있다&#x005B;<xref ref-type="bibr" rid="b21-kjm-89-6-663">21</xref>,<xref ref-type="bibr" rid="b33-kjm-89-6-663">33</xref>&#x005D;. Bismuth subsalicylate는 부분적인 치료 반응을 나타내지만&#x005B;<xref ref-type="bibr" rid="b33-kjm-89-6-663">33</xref>&#x005D; 소규모 연구에서 8주간 투여하였을 때 장기 관해도 보고된 바 있다&#x005B;<xref ref-type="bibr" rid="b47-kjm-89-6-663">47</xref>,<xref ref-type="bibr" rid="b85-kjm-89-6-663">85</xref>&#x005D;. Cholestyramine과 colesevelam 등의 담즙산 흡착제도 일차적으로 사용해 볼 수 있다&#x005B;<xref ref-type="bibr" rid="b46-kjm-89-6-663">46</xref>&#x005D;.</p>
<p>스테로이드 제제는 가장 많은 연구가 이루어진 약제로, 다른 치료에 반응이 없는 중증의 환자를 대상으로 한다. Budesonide는 prednisone과 동등한 효과를 내고 부작용이 적어 선호하며, 교원성 대장염 환자를 대상으로 한 무작위 대조군 연구에서 임상증상 및 조직학적 호전을 보였으며&#x005B;<xref ref-type="bibr" rid="b86-kjm-89-6-663">86</xref>,<xref ref-type="bibr" rid="b87-kjm-89-6-663">87</xref>&#x005D;, 유럽의 다기관 연구에서도 좋은 치료 반응과 적은 부작용을 보였다&#x005B;<xref ref-type="bibr" rid="b88-kjm-89-6-663">88</xref>&#x005D;. 림프구성 대장염 환자에서도 임상증상 및 조직소견의 호전을 보였다&#x005B;<xref ref-type="bibr" rid="b89-kjm-89-6-663">89</xref>&#x005D;. 10개의 무작위 대조군 연구에 대한 메타분석에서 80% 이상 높은 반응률을 보였으며 림프구성 대장염의 number needed to treat은 3명, 교원성 대장염은 2명으로 좋은 효과를 보였다&#x005B;<xref ref-type="bibr" rid="b90-kjm-89-6-663">90</xref>&#x005D;. 초치료 반응률은 높지만 약제 중단 후 재발이 많은데, 51명의 교원성 대장염 환자를 대상으로 한 연구에서 budesonide 치료 후 33명(80%)의 환자에서 임상적 관해에 도달하였으나, 약제 중단 후 16개월간 추적하였을 때 20명(61%)에서 재발하였다&#x005B;<xref ref-type="bibr" rid="b91-kjm-89-6-663">91</xref>&#x005D;. 저용량(3-6 mg/day) 유지요법은 장기 관해유지 및 재발방지에 효과적이었다&#x005B;<xref ref-type="bibr" rid="b92-kjm-89-6-663">92</xref>,<xref ref-type="bibr" rid="b93-kjm-89-6-663">93</xref>&#x005D;. 환자는 만성 재발성 경과를 가질 수 있기에 스테로이드 제제의 사용에 있어 부작용을 최소화할 수 있는 용량과 적응 대상환자 선별에 신중해야 한다.</p>
<p>난치성이거나 스테로이드 의존성인 경우 azathioprine, 6-mercaptopurine, methotrexate 등 면역조절제를 고려해 볼 수 있으며&#x005B;<xref ref-type="bibr" rid="b94-kjm-89-6-663">94</xref>-<xref ref-type="bibr" rid="b96-kjm-89-6-663">96</xref>&#x005D;, 소규모의 후향적 연구에서 azathioprine은 28%의 환자에서 관해 유지에 효과적이었다&#x005B;<xref ref-type="bibr" rid="b97-kjm-89-6-663">97</xref>&#x005D;. Infliximab과 adalimumab 등 항 TNF 제제를 시도해 볼 수 있지만 근거는 미약하다&#x005B;<xref ref-type="bibr" rid="b98-kjm-89-6-663">98</xref>,<xref ref-type="bibr" rid="b99-kjm-89-6-663">99</xref>&#x005D;.</p>
<p>내과적 치료에 반응이 없는 소수의 환자에서는 회장루술, 대장절제술 등의 외과적 처치가 필요할 수 있다&#x005B;<xref ref-type="bibr" rid="b100-kjm-89-6-663">100</xref>,<xref ref-type="bibr" rid="b101-kjm-89-6-663">101</xref>&#x005D;.</p>
</sec>
<sec>
<title>예 후</title>
<p>치료에 대한 반응과 자연경과에 대해 연구마다 다양하게 보고하고 있으나 대체적으로 양호한 경과를 보인다. 25명의 현미경적 대장염 환자를 추적관찰하였을 때 86%에서 설사 증상의 호전을 보였고 32%의 환자만이 약물 치료가 계속 필요하였으며 47개월 후 모든 환자에서 증상의 호전을 보였고 약물 치료가 필요한 환자는 29%였다&#x005B;<xref ref-type="bibr" rid="b102-kjm-89-6-663">102</xref>&#x005D;. 27명의 림프구성 대장염 환자를 평균 37.8개월 동안 추적한 결과 93%에서 설사 증상의 호전 및 82%에서 조직학적으로 정상화되었다&#x005B;<xref ref-type="bibr" rid="b103-kjm-89-6-663">103</xref>&#x005D;. 교원성 대장염 환자를 10년간 추적한 연구에서는 48.9%에서 설사 증상의 관해를 보였으나 이 환자들 중 대부분이 약물 치료를 필요로 하였으며 19.2%에서 재발하였다&#x005B;<xref ref-type="bibr" rid="b104-kjm-89-6-663">104</xref>&#x005D;. 다른 연구에서는 3.5년간 추적하였을 때 48%에서 약물 치료 후 관해에 도달하였으며 자연 관해된 경우도 15%였다&#x005B;<xref ref-type="bibr" rid="b105-kjm-89-6-663">105</xref>&#x005D;.</p>
</sec>
<sec sec-type="conclusions">
<title>결 론</title>
<p>현미경적 대장염은 임상적으로 만성 설사를 주소로 내원한 환자에서 생화학 검사, 내시경 검사 등에서 특이소견을 보이지 않지만 조직 검사를 이용하여 진단할 수 있다. 증상은 과민성 장증후군을 포함한 기능성 질환과 유사할 수 있기에 본 질병의 가능성을 고려하여 감별 진단하려는 노력이 중요하다. 병인으로 만성 염증, 섬유화 기전 이상, 자가면역, 약제, 세균, 담즙산 등 다양한 원인이 관여할 수 있다. 보존적 치료, 지사제, 스테로이드, 면역억제제 등을 사용하지만 질병의 중증도와 약제의 부작용 및 근거를 고려하여 환자의 증상과 삶의 질을 호전시킬 수 있는 적절한 치료 전략을 수립해야만 한다(<xref rid="f2-kjm-89-6-663" ref-type="fig">Fig. 2</xref>) &#x005B;<xref ref-type="bibr" rid="b106-kjm-89-6-663">106</xref>&#x005D;. 대부분 경한 예후를 보이지만 일부 환자에서 설사의 재발 및 만성 경과를 나타낼 수 있다.</p></sec></body>
