<p>크립토코쿠스에 의한 피부 감염은 비교적 흔하지만 심부연조직 감염은 매우 드물다. 저자들은 의인성 쿠싱증후군으로 진단된 72세 여자 환자에서 외상 후 <italic>Cryptococcus neoformans</italic>에 의한 연조직염과 근육근막염을 진단하여 fluconazole을 12개월간 투약 후 호전된 1예를 경험하였기에 문헌고찰과 함께 보고한다.</p></abstract>
<trans-abstract xml:lang="en"><p>The skin is the third most common site of cryptococcosis after infections of the central nervous system and lungs. However,
cryptococcosis rarely involves deep soft tissue. Here, we report a case of cryptococcal cellulitis and myofascitis in a 72-year-old
woman with iatrogenic Cushing&#x02019;s syndrome. Despite the administration of antibiotics, the patient continued to suffer from
aggravated wounds and pain. Cultures of pus and an excisional biopsy of the skin revealed <italic>Cryptococcus neoformans</italic>. She was
treated successfully with intravenous fluconazole (400 mg/day) for 14 days followed by oral fluconazole (400 mg/day) for

<title>서 론</title>
<p><italic>Cryptococcus neoformans</italic>는 환경에 널리 존재하는 효모균으로 조류 분비물에 오염된 토양과 부패된 나무, 과일 등에서 발견된다. 크립토코쿠스증(cryptococcosis)은 기회감염증으로 후천성 면역 결핍증, 장기이식, 종양, 당뇨, 사르코이드증, 장기간 코티코스테로이드를 사용한 환자에서 잘 발생한다고 알려져 있다[<xref ref-type="bibr" rid="b1-kjm-86-1-102-18">1</xref>]. 폐와 중추신경계 감염이 가장 많지만 피부, 부신, 전립선, 눈, 뼈, 복막, 비뇨기계, 심장 등도 침범할 수 있다[<xref ref-type="bibr" rid="b1-kjm-86-1-102-18">1</xref>]. 국내에서 스테로이드 사용자와 의인성 쿠싱증후군 환자에서 일차 피부 크립토코쿠스증(primary cutaneous cryptococcosis)은 2예가 보고되었다[<xref ref-type="bibr" rid="b2-kjm-86-1-102-18">2</xref>,<xref ref-type="bibr" rid="b3-kjm-86-1-102-18">3</xref>]. 그러나 근육염에 대한 보고는 당뇨 환자에서 폐 크립토코쿠스증과 동반되어 수술과 항진균제 사용으로 호전된 1예밖에 없었다[<xref ref-type="bibr" rid="b4-kjm-86-1-102-18">4</xref>]. 저자들은 의인성 쿠싱 증후군(iatrogenic Cushing&#x02019;s syndrome) 환자에서 외상 후 <italic>C. neoformans</italic>에 의한 크립토코쿠스 연조직염과 근육근막염이 발생하여 수술적 치료 없이 항진균제만으로 호전된 1예를 경험하여 문헌고찰과 함께 보고한다.</p>
</sec>

<sec sec-type="cases">
<title>증 례</title>
<p><bold>환 자:</bold> 김&#x03007;&#x03007;, 72세, 여자</p>
<p><bold>주 소:</bold> 내원 3주 전 발생한 피부 병변</p>
<p><bold>현병력:</bold> 양쪽 무릎의 퇴행성 골관절염으로 약 10년 전부터 일차 의료기관에서 코르티코스테로이드로 추정되는 약물을 투여 받던 환자로 내원 6개월 전부터 피부가 쉽게 벗겨지고 멍이 잘 들기 시작하였다. 내원 3주 전 계단에서 넘어져 양쪽 무릎이 멍이 들고 피부가 벗겨지는 상처가 생겼다. 내원 2주 전부터 병변에 고름이 나와 일차 의료기관에서 cefazedone과 netilmicin을 투여받았으나 호전이 없어 본원으로 전원되었다.</p>
<p><bold>과거력:</bold> 6년 전 상행결장암과 위암으로 우반결장절제술과 원위부 위 절제술을 시행받았다. 5년 전부터 고혈압으로 valsartan/hydrochlorothiazide를 당뇨병으로 sitagliptin/metformin을 복용 중이었다.</p>
<p><bold>신체 검사 소견:</bold> 내원 당시 활력징후는 혈압 150/90 mmHg, 맥박 수 91회/분, 호흡 수 20회/분, 체온 36.8&#x02103;이었다. 키는 151 cm, 몸무게는 53 kg이었다. 급성 병색을 보였고 의식은 명료하였다. 전신 피부는 얇고 건조하였다. 양측 정강이에 불규칙한 다발성의 미만과 궤양, 농성 삼출이 있었고 인접 피부에는 종창과 발적이 있었다. 가장 큰 궤양의 크기는 우측 하지는 10 &#x000d7; 10 cm, 좌측 하지는 3 &#x000d7; 3 cm였다(<xref rid="f1-kjm-86-1-102-18" ref-type="fig">Fig. 1</xref>A).</p>
<p><bold>검사실 소견:</bold> 말초혈액검사에서 백혈구 13,500/mm<sup>3</sup>, 혈색소 9.2 g/dL, 혈소판 562,000/mm<sup>3</sup>였다. 혈청생화학검사에서 AST 26 IU/L, ALT 6 IU/L, 총 빌리루빈 0.4 mg/dL, BUN 19 mg/dL, Cr 0.7 mg/dL, Na 135 mmol/L, K 3.3 mmol/L, Cl 96 mmol/L, CRP는 2.28 mg/dL였다. HIV 항원/항체검사는 음성이었다.</p>
<p><bold>방사선 소견:</bold> 흉부 단순촬영은 정상이었다. 자기공명영상에서 양측 하지의 피하조직에 심한 종창과 그물 모양의 조영증강이 있었으며 표재근막과 심부근막을 따라서 조영증강이 있었다. 우측 전방구획 근육 및 좌측 후방구획 근육에 조영이 증강되어 있었다. 이는 연조직염(cellulitis)과 근육 근막염(myofascitis)에 합당한 소견이었다(<xref rid="f2-kjm-86-1-102-18" ref-type="fig">Fig. 2</xref>A).</p>
<p><bold>치료 및 경과:</bold> 급속 ACTH 자극검사 결과 250 &#x003bc;g cosyntropin 직전, 30분 뒤, 60분 뒤 혈청 cortisol 측정치가 각각 0.46, 3.4, 4.3 &#x003bc;g/dL으로 부신기능저하증에 합당하였다. 의인성 쿠싱증후군에 의한 이차 부신피질기능저하증으로 판단하고 경구하이드로코르티손 10 mg을 투여하였다.</p>
<p>무균적으로 농을 채취하여 세균 배양을 시행하였으며 피부병변에서 절개생검을 시행하여 일반 세균, 진균, 항산성균 배양을 시행하였다. 외부병원에서 항생제 사용 중에도 피부병변 악화를 보였기 때문에 정확한 균 동정을 위하여 경험적 항생제는 사용하지 않고 배양 결과를 기다리기로 하였다. 입원 일주일째 배양 검사에서 <italic>C. neoformans</italic>가 동정되었다. 신경학적인 증상과 징후는 없어 뇌척수액 검사는 시행하지 않았다. 혈액과 요 배양 검사에서는 분리된 균이 없었다. 혈청 내 크립토코쿠스 항원은 양성이었으나 역가는 측정하지 못했다. 조직생검의 H-E 염색에서 진피층에 림프구와 조직구, 거대세포의 침윤으로 이루어진 육아종성 병변과 다수의 염증세포가 관찰되었다. 효모균은 Gomori&#x02019;s methenamine silver 염색에서 흑색으로(<xref rid="f3-kjm-86-1-102-18" ref-type="fig">Fig. 3</xref>A), Mucicarmine 염색에서 적색으로 관찰되었다(<xref rid="f3-kjm-86-1-102-18" ref-type="fig">Fig. 3</xref>B).</p>
<p>병변의 범위와 깊이가 너무 커서 수술적인 처치를 시행하지 못하였고 항진균제 투약을 하면서 경과를 관찰하기로 하였다. Fluconazole 400 mg 하루 한 번 정맥주사로 2주간 투약하면서 점차 호전되어 퇴원 후 fluconazole 400 mg 하루 한 번 경구 투약을 지속하였다. Fluconazole을 9개월 간 복용 후 양 하지의 병변(<xref rid="f1-kjm-86-1-102-18" ref-type="fig">Fig. 1</xref>B)과 자기공명영상(<xref rid="f2-kjm-86-1-102-18" ref-type="fig">Fig. 2</xref>B)에서 호전을 보였다. 환자는 총 12개월간 투약한 후 치료를 종결하였다.</p>

<sec sec-type="conclusions">
<title>고 찰</title>
<p>크립토코쿠스에 의한 피부 감염은 중추신경계, 폐 감염에 이어 세 번째로 흔한 침범 부위이다. 후천성 면역 결핍증, 코티코스테로이드 투여, 장기 이식, 혈액암, 결합조직병, 당뇨 등이 크립토코쿠스증의 위험인자이며 특히 고형장기이식을 받은 환자와 tacrolimus를 투여받는 환자에서는 피부와 연 조직, 골관절의 크립토코쿠스증이 더 잘 발생한다고 알려져 있다[<xref ref-type="bibr" rid="b1-kjm-86-1-102-18">1</xref>].</p>
<p>우리나라에서 일차성 피부 크립토코쿠스증은 쿠싱증후군 환자에서 두 개의 보고가 있다. 한 환자는 fluconazole 1주 정주 후 itraconazole을 경구로 10주 투여 후 호전되었고[<xref ref-type="bibr" rid="b2-kjm-86-1-102-18">2</xref>], 다른 환자는 외과적 변연 절제술과 함께 fluconazole 10일 정주 후 경구로 3개월간 투약 후 치유되었다[<xref ref-type="bibr" rid="b3-kjm-86-1-102-18">3</xref>]. 더 깊은 부위의 감염으로 당뇨 환자에서 발생한 근육염 보고가 있었으나 혈행성으로 폐 감염이 동반된 증례였다[<xref ref-type="bibr" rid="b4-kjm-86-1-102-18">4</xref>]. 이 환자는 근육염 부위를 수술적 배농을 하면서 amphotericin B를 1달간 정주 후 fluconazole을 경구로 1년간 투약하여 치료가 되었다.</p>
<p>외국에서도 파종 감염(disseminated infection)의 증거 없이 근육에 침범한 증례 보고가 두 개가 있었다. 첫 번째 증례는 후천성 면역 결핍증 환자로 외상 없이 하지의 피부와 근육에 발생하여 수술적 처치 없이 항진균제 투약만으로 호전이 되었다[<xref ref-type="bibr" rid="b5-kjm-86-1-102-18">5</xref>]. 두 번째 환자는 림프종 환자로 척추주위 근육에 발생하여 fluconazole 투약만으로 호전이 되었다[<xref ref-type="bibr" rid="b6-kjm-86-1-102-18">6</xref>]. 이번 증례 보고의 환자는 의인성 쿠싱 증후군 환자로 외상 후 피부와 연조직염으로 시작하여 진단이 늦어지면서 근육근막염까지 진행한 것이다. 범위와 깊이가 너무 커서 수술적으로 병변을 제거하지는 못하였고 항진균제 투약만으로 호전이 되었다.</p>
<p>이 증례 보고의 제한점은 뇌척수액 검사와 혈청 크립토코쿠스 항원 역가 측정을 시행하지 못하여 다른 장기로의 파종 여부를 구분하지 못한 점이다. 그러나 발열, 경부강직, 신경학적 징후 등 뇌수막염을 시사할 만한 증상, 징후가 없었기 때문에 중추신경계 감염 동반 가능성은 낮을 것으로 생각된다. 일반적으로 연조직 감염은 혈류 감염 동반율이 낮아 혈액 배양 양성률이 2% 정도로 알려져 있으며[<xref ref-type="bibr" rid="b7-kjm-86-1-102-18">7</xref>] Neuville 등[<xref ref-type="bibr" rid="b8-kjm-86-1-102-18">8</xref>]이 28명의 일차성 피부 크립토코쿠스증 환자를 대상으로 분석한 결과 7명의 환자에서 다른 장기 침범이 없이도 혈청 크립토코쿠스 항원에 양성이었고 HIV 감염이 없는 환자에서는 평균 역가가 1:40였다. Hafner 등[<xref ref-type="bibr" rid="b9-kjm-86-1-102-18">9</xref>]도 장기간 코티코스테로이트 투여를 받은 환자에서 발생한 일차성 피부 크립토코쿠스증에서 혈청 크립토코쿠스 항원이 낮은 역가(1:64)로 양성일 수 있다고 보고하였다. 따라서 이번 증례 보고의 환자도 혈청 크립토코쿠스 항원이 양성이었지만 이전 증례보고들[<xref ref-type="bibr" rid="b8-kjm-86-1-102-18">8</xref>,<xref ref-type="bibr" rid="b9-kjm-86-1-102-18">9</xref>]처럼 낮은 역가의 양성이었을 것으로 추정된다.</p>
<p>크립토코쿠스 피부 연조직 감염과 근육염에 대해서는 치료 원칙이나 항진균제 사용 기간에 대해 잘 알려져 있지 않다. 2010년에 미국 감염학회에서 발표한 크립토코쿠스증의 치료지침에 따르면 면역저하자가 아닌 환자에서 중추신경계와 폐 이외의 한 부위에 발생한 경우 fluconazole 400 mg을 6-12개월 사용하는 것을 추천하고 있다[<xref ref-type="bibr" rid="b10-kjm-86-1-102-18">10</xref>]. 그러나 이 치료지침에서도 면역저하자에서 발생한 일차성 피부 연조직 감염의 치료에 대해서는 밝히고 있지 않으며 피부나 근육염에서의 수술 필요 여부 등에 대해서도 언급이 되어 있지 않다. Neuville 등[<xref ref-type="bibr" rid="b8-kjm-86-1-102-18">8</xref>]이 28명의 일차성 피부 크립토코쿠스증 환자를 대상으로 분석한 자료에 따르면 수술적 치료를 시행하지 않고 약물 치료만 한 환자 11명 중 9명은 완치되었고 2명은 호전되었다.</p>
<p>이번 증례는 의인성 쿠싱 증후군 환자에서 외상 후 피부와 연조직염으로 시작하여 진단이 늦어지면서 근육근막염까지 진행하였고 수술적 처치 없이 항진균제 12개월 사용으로 호전된 경우이다. 크립토코쿠스 감염의 위험인자가 있는 환자에서 경험적 항생제에 반응이 없는 피부 연조직 감염이라면 <italic>C. neoformans</italic>도 한 원인일 수 있으므로 이에 대한 적극적인 진단적 조치가 필요하다.</p>
