<abstract><p>저자들은 장점막의 손상을 일으킨 내시경 스텐트 삽입술 후 발생한 <italic>Kodamaea ohmeri</italic> 진균혈증을 amphotericin B으로 치료 성공한 1예를 경험하였기에 이를 보고하는 바이다. </p></abstract>
<trans-abstract xml:lang="en"><p><italic>Kodamaea (Pichia) ohmeri</italic> is a rare fungal pathogen that has recently been identified as an etiological agent of fungemia in immunocompromised patients. We report a case of <italic>K. ohmeri</italic> fungemia after colonoscopic stent insertion in a 73-year-old female who was successfully treated with a 2-week course of amphotericin B without stent removal. <title>서 론</title>
<p><italic>Kodamaea (Pichia) ohmeri</italic>는 식품의 발효를 위해 첨가물로 주로 사용되는 효모균으로, 사람에게 병원체로 작용하는 경우는 매우 드문 것으로 알려져 있다[<xref ref-type="bibr" rid="b1-kjm-85-1-106-19">1</xref>]. 지금까지 보고된 <italic>Kodamaea ohmeri</italic> 감염증 증례들은 당뇨, 미숙아, 반복적인 중심정맥관 삽입, 심한 면역저하와 연관성을 가지며 적극적인 감염원 제거와 항진균제 사용이 주된 치료였다[<xref ref-type="bibr" rid="b1-kjm-85-1-106-19">1</xref>,<xref ref-type="bibr" rid="b2-kjm-85-1-106-19">2</xref>]. 특히 <italic>Kodamaea ohmeri</italic>에 의한 진균혈증은 대부분 중심 정맥관을 통한 감염으로, 대장 스텐트 삽입 후 발생한 예는 보고된 바가 없었다.</p> 

<p>저자들은 내시경적 대장 스텐트 삽입술 후 발생한 <italic>Kodamaea ohmeri</italic> 진균혈증 환자를 스텐트를 제거하지 않고 amphotericin B로 치료 후 호전된 사례를 국내 처음으로 경험하였기에 문헌고찰과 함께 보고하는 바이다.</p>
</sec>

<sec sec-type="cases">
<title>증 례</title>
<p>73세 여자 환자가 3일 전부터 시작된 복부 통증을 주소로 내원하였다. 환자는 1년 전 직장암(T2N1M0, Stage III)으로 전위전방절제술 시행 후 6개월간 항암화학요법 시행받고 재발 없이 외래에서 추적관찰 중이었다. 내원 당시 혈압은 130/90 mmHg, 맥박수 103회/분, 호흡수 20회/분, 체온은 37.8&#x02103;였으며 장음은 감소되어 있었고 배 전반에 압통이 있었다. 복부 단순 촬영에서 전장의 확장된 소견이 보였고 복부 전산화 단층 촬영에서 직장연 상방 10 cm 부위에 협착 부위가 발견되었으며 전장에 걸쳐 기계적 장폐색이 보였다(<xref rid="f1-kjm-85-1-106-19" ref-type="fig">Fig. 1</xref>). 비위관 삽입 후 장의 감압을 유도하였고 내원 당시 열이 있는 상태로 수액 공급 및 경험적 정맥항생제(ceftriaxone, metronidazole) 투여하는 등 보존적 치료를 유지하면서 통증의 호전을 보여 입원 8일에 대장내시경을 시행하였다. 대장내시경에서는 항문피부선 10 cm 상방에 협착부위가 확인되었고 풍선확장술을 시행하였다. 협착부위에서 시행한 조직 검사 결과가 만성 염증으로 보고되었고 복부 전산화 단층 촬영에서 직장암의 재발을 시사하는 소견이 없었으며 전위전방절제술 시행을 받았던 과거력으로 비추어 보아 수술 후 협착으로 사료되었다. 시술 직후 38.5&#x02103;의 발열이 있어 경험적 항생제를 cefoperazone/sulbactam으로 교체하였고 당시 시행한 배양 검사에서는 동정된 균이 없었다. 환자가 안정된 후 입원 16일에 협착부위에 내시경적 스텐트 삽입술을 시행하였다(<xref rid="f2-kjm-85-1-106-19" ref-type="fig">Fig. 2</xref>). 스텐트 삽입술 시행 이틀 후 환자는 38.6&#x02103;의 발열 및 심한 오한을 호소하였고 혈압은 140/90 mmHg, 맥박수 100회/분, 호흡수 26회/분, 말초혈액검사에서 백혈구 8,220/mm<sup>3</sup> (호중구 80.8%, 림프구 8.3%), 혈소판 277,000/mm<sup>3</sup>였고, 혈청 생화학 검사에서 Na 137 mmol/L, K 2.5 mmol/L 혈청 총 단백 5.3 g/dL, 알부민 2.9 g/dL, AST/ALT 17/12 U/L, 총 빌리루빈 0.3 mg/dL, 혈액요소질소 11 mg/dL, 크레아티닌 0.4 mg/dL, C-반응성 단백질 27.94 mg/dL로 보고되었으며 혈액배양 3쌍 모두에서 <italic>Kodamaea ohmeri</italic>가 배양되었다. Fluconazole, amphotericin B, voriconazole에 모두 감수성을 보였고 fluconazole 400 mg/day로 정맥주사 투여하였다. Fluconazole을 5일간 사용하였으나 고열이 지속되고 전신 상태가 호전되지 않아 amphotericin B (1 mg/kg/day)로 교체하였고 스텐트 제거를 고려하였다. Amphotericin B 사용 5일 만에 열이 떨어지고 증상의 호전을 보이며 진균혈증 발생 7일 후 추적 혈액 배양에서 <italic>Kodamaea ohmeri</italic> 동정되지 않아 스텐트를 유지하고 amphotericin B 투여를 지속하였다. 2주간 총 amphotericin B 500 mg으로 치료하였으며 현재 12개월째 외래 추적관찰 중으로 재협착 및 진균혈증의 재발은 없는 상태이다.</p>

<sec sec-type="discussion">
<title>고 찰</title>
<p>전세계적으로 면역억제 환자가 늘어남에 따라 침습성 진균 감염의 수가 점차 늘어나고 있으며 Kodamaea (Pichia) ohmeri 역시 이러한 환자군에서 새로운 기회감염균으로 대두되고 있다. 1998년 Bergman 등[<xref ref-type="bibr" rid="b3-kjm-85-1-106-19">3</xref>]이 인체에 대한 감염증을 최초로 보고한 이후 현재까지 약 32예의 <italic>Kodamaea ohmeri</italic> 감염증이 보고되었다. 보고된 <italic>Kodamaea ohmeri</italic> 감염증 사례들을 살펴보면 진균혈증(25예) [<xref ref-type="bibr" rid="b1-kjm-85-1-106-19">1</xref>-<xref ref-type="bibr" rid="b7-kjm-85-1-106-19">7</xref>], 심내막염(3예) [<xref ref-type="bibr" rid="b4-kjm-85-1-106-19">4</xref>] 상처 감염(2예) [<xref ref-type="bibr" rid="b4-kjm-85-1-106-19">4</xref>], 요로감염(1예) [<xref ref-type="bibr" rid="b4-kjm-85-1-106-19">4</xref>], 악성외이도염(1예) [<xref ref-type="bibr" rid="b5-kjm-85-1-106-19">5</xref>], 복막염(1예) [<xref ref-type="bibr" rid="b8-kjm-85-1-106-19">8</xref>]이 있다. 특히 진균혈증 환자의 대부분은 중심 정맥관을 통한 혈액감염이 주 감염경로였다. 대부분의 환자에서 장기간의 당뇨, 장기간 입원병력, 광범위 항생제 투여 경력, 중심정맥관을 비롯한 인공도관, 인공판막과 인공심박동기 삽입, 면역억제제의 투여경력 등 침윤성 진균감염의 소인이 있었다.</p> 

<p><italic>Kodamaea ohmeri</italic> 감염증에 대한 최적의 치료는 명확하게 제시되어 있지 않으나 다른 진균 감염과 마찬가지로 감염원의 제거와 항진균 요법이 동반되는 경우가 치료 성공이 높은 것으로 알려져 있다. 현재까지 보고된 증례들에서 <italic>Kodamaea ohmeri</italic> 감염증에 의한 사망률은 약 40%로 기저질환의 악화인지 진균 감염의 치료 실패인지 뚜렷이 구분되지 않은 경우도 있지만 면역저하 환자에서 <italic>Kodamaea ohmeri</italic> 감염은 치명적일 수 있으며 치료가 쉽지 않음을 알 수 있다[<xref ref-type="bibr" rid="b1-kjm-85-1-106-19">1</xref>,<xref ref-type="bibr" rid="b4-kjm-85-1-106-19">4</xref>]. 따라서 혈액배양 검사에서 진균혈증이 의심되거나 진균감염의 위험요인을 가진 환자에서 이유 없이 악화될 경우 반드시 진균감염을 고려해야 하고 항진균제의 투여 및 선행 요인 제거가 늦어지지 않도록 해야 할 것이다.</p> 

<p>항진균제 측면에서 살펴보면 보고된 증례들에서 fluconzaole 또는 amphotericin B가 가장 많이 사용되었으며 아직까지 특정 항진균제가 더 추천되지는 않는다. 그러나 감염증을 일으킨 임상검체에서 동정된 <italic>Kodamaea ohmeri</italic>의 약제 감수성을 살펴보면 amphotericin B에 대해서는 모두 감수성이 있었으나 fluconazole에는 내성 또는 중간내성인 경우가 보고된 바 있어[<xref ref-type="bibr" rid="b1-kjm-85-1-106-19">1</xref>,<xref ref-type="bibr" rid="b8-kjm-85-1-106-19">8</xref>-<xref ref-type="bibr" rid="b9-kjm-85-1-106-19">9</xref>] fluconazole 사용에 주의가 필요할 것으로 보인다. 또한 fluconazole에 감수성으로 보고되어도 fluconazole 사용에 임상적 호전을 보지 못하여 amphotericin 또는 echinocandin 계열의 항진균제로 교체한 뒤 호전을 보인 증례들이 있었다[<xref ref-type="bibr" rid="b4-kjm-85-1-106-19">4</xref>-<xref ref-type="bibr" rid="b7-kjm-85-1-106-19">7</xref>,<xref ref-type="bibr" rid="b10-kjm-85-1-106-19">10</xref>]. Son 등[<xref ref-type="bibr" rid="b9-kjm-85-1-106-19">9</xref>]이 발표한 악성 외이도염 증례에서도 초기 fluconazole 치료 실패 후 amphotericin B를 사용한 뒤 호전이 있었으며 이후 fluconazole로 교체했을 때는 다시 점진적인 악화 소견을 나타내어 다시 amphotericin B로 치료하여 호전되었음을 보고한 바 있다. 따라서 환자의 상태가 중하거나 일차치료에도 불구하고 악화되는 진균혈증의 경우 선행원인의 제거와 함께 amphotericin B의 조기 사용이 고려되어야 할 것이다. 최근의 보고에서는 micafungin이나 caspofungin 등으로 치료에 성공한 증례들이 있어 신독성 등의 부작용 때문에 amphotericin B의 사용이 어려운 중증 환자에서 echinocandin 계열의 항진균제가 유용할 수도 있음을 제시하였다[<xref ref-type="bibr" rid="b1-kjm-85-1-106-19">1</xref>,<xref ref-type="bibr" rid="b10-kjm-85-1-106-19">10</xref>].</p> 

<p>본 증례의 경우 환자가 중심정맥관이나 인공삽입물을 가지고 있지 않았고 말초정맥관 삽입부에도 이상이 없었으며 대장 스텐트 삽입 시술 이후에 진균혈증을 보인 임상경과를 보아, 스텐트 삽입술을 통한 감염 외에 다른 원인을 생각하기는 어려울 것으로 생각한다. 환자는 직장의 협착이 심하고 이로 인한 통증과 변비를 호소하는 상태로 스텐트를 유지하는 것이 환자의 삶의 질에 최대한의 도움을 줄 수 있으리라 사료되었다. 혈액배양 검사 후 fluconazole을 우선 사용하였지만 호전이 없어 amphotericin B로 항진균제를 바꾸어 사용하여 결국 환자가 스텐트를 제거하지 않고 회복될 수 있었다.</p> 

<p>결론적으로 <italic>Kodamaea ohmeri</italic>는 주로 면역이 저하된 환자에게서 중심정맥 삽관을 통한 기회감염으로 인식되어 왔지만 본 증례와 같이 면역력이 정상인 환자에서도 시술 이후 감염되어 진균혈증으로 진행할 수 있으며 amphotericin B의 조기 사용이 환자의 예후에 도움이 될 수 있다. 이 진균에 의한 패혈증 치료는 아직 정립된 바가 없으므로 정확한 진단과 올바른 치료를 위한 더 많은 연구가 필요하리라 생각한다.</p></sec></body>
