Trans Abstract

The prevalence of obesity and functional gastrointestinal disorders (FGIDs) is increasing worldwide. Obesity has been linked to gastroesophageal reflux disease, irritable bowel syndrome, functional dyspepsia, and various FGIDs. However, the relationship between obesity and FGIDs remains unclear. The purpose of this paper is to evaluate the published studies on this topic and clarify the relationship between obesity and the pathophysiology of various FGIDs.

서 론

위식도역류 질환, 과민성 장증후군, 기능성 소화불량증 등과 같은 명확한 원인을 알기 어려운 소화기 기능성 질환(functional gastrointestinal disorder, FGID)은 높은 유병률을 보이며 최근 유병률이 점차 더 증가하고 있다. ROME IV 보고에 의하면 일반 인구에서 기능성 식도 질환은 20-40% [1], 기능성 위장관 질환은 10-30% [2], 기능성 장질환은 10-20% [3] 그리고 기능성 항문직장 질환은 7-15% [4] 정도의 유병률을 보이고 있다. 비만과 FGID가 모두 증가하고 있어 비만과 FGID와의 연관성이 제시되었고 이와 관련된 여러 연구가 꾸준히 진행되어 왔다. 현재까지의 연구에 의하면 복통, 과민성 장증후군, 복부팽만, 위식도역류 질환의 유병률은 과체중이나 비만한 사람에서 더 높으며[5-8], 체질량지수(body mass index, BMI)는 복통 및 설사와 양의 상관관계를 가지는 것으로 알려져 있다[9]. 하지만 많은 연구들이 비만보다는 위장관 증상을 호소하는 환자에 중점을 두어 진행되었고 연구마다 조금씩 다른 결과를 보고하고 있다. 이에 본고에서는 현재까지 발표된 연구들을 비교 분석하여 비만과 다양한 FGID의 연관성 및 이와 관련된 병태생리에 대하여 알아보고자 한다.

본 론

비만과 위식도역류 질환

서구뿐 아니라 우리나라에서도 비만과 위식도역류 질환의 유병률이 점점 증가하고 있으며 여러 연구에서 유의한 상관관계가 있었다[10]. 일반적으로 비만한 사람들에서 위식도역류 질환 유병률이 높고 위식도역류 증상이 BMI에 비례해서 증가한다고 알려져 있다[11-14]. 독일에서 7,124명을 대상으로 시행된 인구기반 연구에 의하면 비만한 사람들은 비만하지 않는 사람들에 비하여 위식도역류 질환이 발생할 교차비(odd ratio, OR)가 2.6으로 높았다[15]. 최근에는 복부비만에 대한 관심이 증가하고 있는데, 복부비만은 복압을 증가시키고 하부식도조임근의 압력을 감소시켜서 위식도역류 증상을 일으킬 수 있고 내장 지방은 여러가지 사이토카인들을 생성하여 식도와 위 운동 기능에 영향을 줄 수 있는 것으로 알려져 있다[10]. 특징적인 것은 BMI와 위식도역류 질환 간의 관계가 남성에 비하여 여성에서 더 강하게 나타나고 이는 비만 여성에서 에스트로겐 수치가 증가되기 때문으로 생각되며[16-18], 실제로 에스트로겐 호르몬 치료를 시행한 여성군에서 위식도역류 질환 증상이 더 빈번하게 발생하였다[16-18].

비만한 사람에서 위식도역류 질환이 증가하는 기전은 명확하지 않지만 현재까지의 연구들을 종합하여 다음과 같이 정리해 볼 수 있겠다[10]. 첫째, 비만한 사람에서 하부식도조임근 압력이 유의하게 감소되어 있다[19]. 둘째, 과체중군과 비만군이 정상 체중군에 비하여 식후 2시간 동안의 하부식도조임근 이완(transient lower esophageal sphincter relaxation, tLESR)의 빈도와 식도 산 노출 시간이 유의하게 증가되어 있고 이러한 증가들이 BMI와 연관이 있는 것으로 보아 tLESR이 또 다른 기전일 수 있겠다[20]. 셋째, BMI가 증가되면 위식도접합부의 압력 분포가 변화되어 식도열공 탈장(esophageal hiatal hernia)이 잘 발생할 것으로 추정하고 있으며[21], 실제 비만인에서 식도열공 탈장의 유병률이 높다[19]. 넷째, 비만한 사람에서 식도체부의 청소율이 더 감소되어 있다[22].

체중이 감소하게 되면 위식도역류 질환 증상이 호전되는지에 대해서는 보고 결과들이 일정하지 않았으나[23-25], 최근에 시행된 잘 디자인된 개입 연구에서 체중의 감소와 위식도역류질환 증상의 호전이 관찰됨[26,27]에 따라 체중의 증가와 감소가 위식도역류 질환 증상과 밀접한 관련이 있음을 알 수 있다.

비만과 과민성 장증후군

비만이 과민성 장증후군의 원인이 될 수 있다는 가능성이 제시되어 이와 관련된 몇몇 연구들이 진행되어 왔다. 하지만 호주에서 시행된 출생 코호트 연구에서는 과민성 장증후군과 비만의 관련성을 도출하지 못하였다[28]. 또한 중동 국가에서 시행한 과민성 장증후군과 비만의 관련성 연구에서는 복부비만이 있는 대상자에서 과민성 장증후군이 더 빈번하기는 하였지만 몇 가지 교란변수를 보정한 후에는 이 둘 사이의 관련성이 없는 것으로 나타났다[29]. 반면 다른 연구들에서는 비만한 사람에서 과민성 장증후군이 발생할 수 있는 OR을 1.16-2.6으로 다양하게 보고하고 있으며[8,9,30,31], 과민성 장증후군과 비만의 관련성을 시사하고 있다. 1980년부터 2012년까지 Medline과 Embase 검색을 통하여 과민성 장증후군과 비만과 관련된 9개의 성인 연구를 추출하여 분석한 결과 비만한 사람에서 과민성 장증후군의 유병률은 11.6%에서 24%로 다양하게 나타났다[32]. 이 연구에서는 대상 인구군에 따라 차이는 있겠지만 과민성 장증후군과 비만이 어느 정도의 관련성이 있을 것으로 결론지었다[32].

비만한 사람에서 과민성 장증후군이 더 빈번한 이유는 여러 가지가 있겠지만 다음과 같이 정리해 볼 수 있겠다[32]. 첫째, 비만한 사람에서 소장과 대장의 운동 변화가 나타날 수 있다. Basilisco 등[33]은 락툴로즈 호기 검사를 통하여 비만한 사람에서 경구-맹장 통과시간(orocecal transit time)이 지연됨을 보고하였다. 반면 Sadik 등[34]은 과민성 장증후군 환자에서 BMI와 대장 통과시간은 역비례 관계가 있으며 비만한 사람에서 하행결장과 구불결장/직장 접합부 통과시간이 더 빨랐다고 보고하였다. 비록 아직까지 데이터는 충분하지 않지만 이러한 소장과 대장 운동 변화의 결과들은 변비형 혹은 설사형 과민성 장증후군의 병태생리를 설명할 수 있는 근거가 될 수 있으며 향후 더 많은 연구가 필요하겠다. 둘째, 비만인 사람에서 저섬유질과 고탄수화물 식사가 과민성 장증후군의 증상과 연관이 있을 수 있겠다. 67% 이상의 과민성 장증후군 환자가 음식 불내성(food intolerance)을 호소한다는 보고가 있으며 이는 음식이 비만은 물론 과민성 장증후군의 증상과 연관이 있음을 시사해 준다[35]. 또한 과민성 장증후군 환자는 캔 음식, 가공육, 콩, 곡물, 과자류, 과일 등을 많이 섭취하는 것으로 알려져 있다[36]. 이러한 근거들을 종합해 볼 때 음식이 과민성 장증후군과 비만의 관계에 중요한 역할을 하는 것으로 생각할 수 있겠다. 셋째, 장내 미생물의 변화를 들 수 있다. 여러 연구에서 과민성 장증후군과 비만 모두에서 장내 미생물 변화가 관찰되었다. 소장 세균 과증식(small intestinal bacterial overgrowth)은 비만한 사람에서 과민성 장증후군 증상이 나타나는 잠재적인 기전일 수 있겠다. 실제로 비만 수술을 기다리는 환자에서 소장 세균 과증식의 비율은 41%로 나타났다[37]. 비만한 사람에서 장내 세균이 변화하는 기전은 명확하지 않지만 식이(특히 고지방 서양식 식이)는 장내 미생물 총에 영향을 미치는 것으로 보이며, 이는 비만한 사람의 과민성 장증후군 증상 발현에 영향을 줄 수 있겠다[38]. 최근 국내 연구에서는 과민성 장증후군 환자에서 일반 대조군에 비하여 대사증후군의 유병률이 더 높음이 보고되었으며, 이는 장내미생물군의 변화와 관련이 있을 것으로 추정하고 있다[39]. 하지만 현재까지 비만 환자의 장내 미생물 변화와 과민성 장증후군 증상 간의 인과관계를 확립할 수 있는 근거는 부족한 실정이다.

비만과 기능성 소화불량

프랑스에서 약 35,000명을 대상으로 시행한 대규모 연구결과 BMI와 FGID 간의 다양한 관련성이 제시되었는데, 그 중 주목할 것은 BMI와 여성 기능성 소화불량 환자에서 U자 모양 상관관계가 나타난 것이다. 즉, 여성 기능성 소화불량 발생의 OR은 BMI가 증가할수록 높아짐과 동시에 BMI가 감소할수록 역시 높아지는 특이한 결과를 보였다[40]. 기능성 소화불량의 일부 환자는 조기 포만감과 위저부 이완장애 등으로 식사를 잘 하지 못하게 되고 이로 인하여 저체중이 발생하는 반면, 일부 환자(위약에 반응하는 환자 등)에서는 정상 체중 혹은 과체중 상태를 유지한다는 것이다[41,42]. 이탈리아의 한 연구에서는 비만 수술이 필요할 정도의 심한 비만 환자와 정상 대조군 사이에 FGID의 유병률을 비교 분석하는 연구를 진행하였다. 기능성 소화불량의 유병률은 양쪽군에서 큰 차이는 없었으나 폭식을 하는 비만 환자와 식후 불편감 증후군(postprandial distress syndrome) 사이에는 유의한 상관관계가 있었다. 또한 폭식을 하지 않는 비만 환자에 비하여 폭식을 하는 비만 환자에서 명치부 포만감의 빈도와 강도가 현저히 높았다[43].

비만한 사람에서 소화불량이 더 빈번한 이유는 비만한 사람에서 위 운동과 이를 조절하는 각종 호르몬이 변화하기 때문이다[44]. 비만한 사람에서 위 운동의 변화를 살펴보면 높은 BMI, 높은 공복 위 부피는 포만감의 감소와 관련이 있다. 즉, 공복 위 부피가 50 mL 증가하면 약 144 ± 32 kcaL를 더 섭취하여야 최대 포만감에 도달할 수 있다는 것이다[45,46]. 후속 연구에서는 포만감 감소와 관련이 있으며 BMI가 5 kg/m2 증가할 때마다 포만감을 느끼기 전 50 kcaL를 더 소비함을, 또한 BMI가 높을수록 공복 위 부피가 커지고 고형과 액체의 위 배출이 가속화되었다[47]. 많은 위장관 호르몬이 위장관 운동과 관련이 있고 음식섭취와 관련되어 분비되며 포만감과 음식섭취의 조절에 중요한 역할을 한다[48]. 콜레시스토키닌, 소마토스타틴, 뉴로텐신, 위 억제 폴리펩티드(gastric inhibitory polypeptide), 펩티드 와이와이(peptide YY), 그렐린(ghrelin) 등이 연구되어 왔다. 대표적인 호르몬인 그렐린은 위에서 관찰되는 장 유래 펩타이드로 위 운동과 식욕의 조절에 중요한 역할을 한다[49]. 그렐린은 주로 위 배출을 촉진시키고 위 적응을 감소시키는 역할을 한다[50,51]. 비만한 사람들은 정상 체중인 사람에 비하여 혈중 그렐린 치가 낮으며 비만한 사람들의 체중이 줄게 되면 혈중 그렐린 치가 증가한다[52,53]. 그렐린은 위 운동을 항진시키고 위 배출을 촉진하며 위산 분비를 증가시킨다. 따라서 그렐린 수치가 증가하면 위 운동과 배출이 증가하기 때문에 증상이 호전될 것으로 추정된다. 반대로 체중이 증가하면 혈중 그렐린 치가 감소되어 소화불량 등의 증상들이 유발되기 쉽다[10]. 실제로 혈중 그렐린 치가 비정상적으로 낮은 기능성 소화불량 환자들은 위 배출이 지연된 경우가 많았다[54]. 179명의 기능성 소화불량 환자를 대상으로 한 국내 연구에서는 기능성 소화불량의 아형인 식후 불편감 증후군 그룹에서 혈장 아실 그렐린(acyl ghrelin) 치가 대조군이나 명치 통증 증후군 그룹보다 유의하게 감소되었고 Helicobacter pylori 제균 치료를 시행한 후에는 아실 그렐린치가 상승됨을 보고하여 아실 그렐린이 식후 불편감 증후군 증상 완화에 기여할 수 있음을 시사하였다[55].

비만과 기타 FGID

프랑스의 3차 병원에서 1,074명의 환자를 대상으로 시행한 연구에서 BMI는 여러 FGID와 관련이 있었다[56]. 삼킴 곤란의 유병률은 BMI가 낮은 사람에서 더 높은 반면 역류, 인후두 이물감(globus), 가슴쓰림(heartburn)과 식후 불편감은 과체중인 사람에서 더 높았으며 역류의 유병률은 비만한 사람에서 더 높았다. 특히 인후두 이물감과 역류의 OR은 BMI에 비례하여 증가하였으며 여성에서 더 뚜렷하게 나타났다[56].

BMI의 증가는 하복부 통증, 변비, 구역과 관련이 없으나 상복부 통증, 팽만감, 설사와는 유의한 관련이 있다는 보고가 있지만[6], 결과는 다양하다. 비만한 사람은 정상 체중인 사람에 비하여 설사의 유병률이 더 높다[57]. 2,660명의 대상자로 진행된 인구기반 연구 결과에서 설사의 유병률은 비만한 사람에서 30%이고 정상 체중인에서는 17% (OR 2.7)로 나타났다[6]. 프랑스에서 시행된 대규모 연구에서는 BMI와 여성 기능성 설사 환자의 양성 선형관계(OR 1.05)를 보고하였다[40]. 비만한 사람들은 흡수가 잘 되지 않는 당을 과다하게 섭취하는 경향이 있어서 삼투성 설사가 잘 유발될 것으로 추정되는데, 특히 과당 섭취가 과다한 것으로 보고되고 있다[31]. 이외에도 담즙산의 변화로 인한 담즙산 설사[58], 대장 통과 시간의 증가[59], 장점막 투과성의 증가[60,61] 등이 비만한 사람에서 설사가 더 빈번히 발생하는 기전으로 생각된다. 비만한 사람들이 위식도역류 질환이 잘 동반되어 프로톤펌프 억제제를 장기간 사용하는 경우가 많으며 프로톤펌프 억제제의 사용으로 위산 분비가 감소되어 장내세균이 증식되고 이로 인하여 복통, 팽만감, 설사 등의 증상이 발생할 수 있다는 가설도 있다[10]. 또한 비만인에게 빈번하게 처방되는 제2형 당뇨병 치료제인 메트포민이 설사의 원인이 될 수 있다는 연구결과도 있다[62].

비만과 변비와의 관련성은 아직까지 논쟁의 여지가 있다. 미국에서 시행된 지역사회 기반 역학 연구에서는 비만한 사람에서 변비의 유병률이 높았으나[63], 뉴질랜드의 인구기반 코호트 연구에서는 관련성이 없었다[8]. 반면 비만 어린이에서는 변비(변비 우세형 과민성 장증후군을 제외)가 더 빈번히 발생하였다[64].

비만이 불완전 직장 대변 배출과 관련이 있다는 보고도 있다(OR 1.64) [31]. 변실금은 높은 BMI와 관련이 있지만 관련성이 약하거나 통계적으로 유의한 결과를 보이지 않았다[44]. 비만은 변실금의 잠재적 수정 가능한 위험인자이며[65], 비만이 변실금 환자가 화장실을 사용할 때 어려움과 의존도를 증가시키는 중요한 위험인자라는 보고도 있다[66].

결 론

비만은 위식도역류 질환, 과민성 장증후군, 기능성 소화불량 그리고 기타 여러가지 FGID와 관련이 있다. 비만과 관련된 위장관의 운동의 변화, 비만과 관련된 각종 사이토카인 및 위장관 호르몬의 변화, 음식의 영향, 장내 미생물의 변화 등 다양한 병태생리학적 요소가 제시되고 있다. 하지만 더 명확한 관련성과 기저 병태생리를 규명하기 위해서는 향후 더 잘 디자인된 연구들이 필요하겠으며 비만 치료로 FGID의 증상이 호전될 수 있는지에 대한 장기간의 관찰 연구가 필요하겠다.
