Trans Abstract

Urticaria is a common cutaneous disease characterized by recurrent and transient wheals and pruritus, sometimes accompanied angioedema. The classification of urticaria is based on the duration of the disease and whether extrinsic triggers are identified or not. Acute urticaria is usually occurred by specific causes, such as drug, food, and infection, etc. Therefore, acute urticaria can be remitted within 6 weeks just by avoiding the exposure to the causes. However, chronic urticaria defined as repeatedly occurred itchy wheals and/or angioedema for at least 6 weeks, has a significant effect on patients’ quality of life. Chronic inducible urticaria can be triggered by various physical stimuli including dermographism, delayed pressure, cold, heat, cholinergic stimuli, sunlight, and exercise. Chronic spontaneous urticaria (CSU) is diagnosed when no specific extrinsic cause is identified in the patients. CSU due to autoimmune mechanism accounts for 30-50%, autologous serum skin test and anti-thyroid autoantibody can be evaluated. However, various physical stimuli, emotional or physical stress, drugs, particularly aspirin and non-steroidal anti-inflammatory drugs can exacerbate urticaria in 30-75% of patients with CSU. Allergic diseases and autoimmune diseases are more common in CSU patients than in general populations. To assess the severity of urticaria and to adjust treatment step, urticaria activity score over 7 days, calculated by the number of wheals and the severity of pruritus, is recommended by recent international guidelines.

증 례

48세 남자가 두 눈꺼풀의 심한 부종으로 내원하였다. 3개월 전부터 거의 매일, 특히 야간에 심한 피부 가려움과 팽진이 반복되던 중 내원 당일 아침 갑자기 두 눈꺼풀이 부어 올랐다고 한다(Fig. 1). 피부 가려움과 팽진은 2개월 전부터 개인의원에서 cetirizine, fexofenadine을 처방받아 복용 중이며, 가려움은 조금 나아졌으나 팽진이 지속되고, 증상이 너무 심한 날은 병원에서 스테로이드 주사도 맞았다고 한다. 하루 중에도 여러 차례 가렵고, 부푸는 증상이 특별한 자극 없이 나타난다고 하였다. 알레르기 비염이 있어서 간헐적으로 치료받았고, 약물 및 식품에 대한 알레르기는 없었다. 내원 2시간 전 두통으로 ibuprofen을 복용한 것 외에는 특별한 음식과 약물에 노출되지 않았다.

내원시 체온 37.2℃, 혈압 135/80 mmHg, 호흡수 20회/분, 청진상 천명음은 없었다. 압통과 열감이 없는 전반적인 눈꺼풀 부종과 몸통과 팔, 다리에 팽진과 주변 홍반이 관찰되었고, 팽진이 서로 뭉쳐서 갯수를 헤아릴 수 없는 것도 있으며, 가려움이 심하여 견딜 수 없을 정도라고 하였다.

두드러기의 분류

두드러기는 갑자기 발생하는 팽진, 혈관부종을 특징하는 질환으로 전체 인구의 약 20%가 일생에 한 번은 경험하는 것으로 알려져 있다. 두드러기는 팽진, 혈관부종, 피부 가려움 등 특징적인 증상의 지속 기간에 따라 급성과 만성으로 분류한다. 팽진이 발생한 후 6주 이내에 완전히 치료되거나 자연 소실되는 경우를 급성 두드러기로 분류한다. 주로 식품, 약물, 감염 등에 의하여 일시적으로 나타나는 급성 두드러기는 해당 원인이 소실되면 호전되어 더 이상 증상이 나타나지 않는다. 반면 팽진이 6주 이상 거의 매일 나타나는 만성 두드러기는 악화와 호전을 반복하며 그 경과를 예측할 수 없어 삶의 질을 떨어뜨린다. 팽진은 일시적으로 나타나며 대부분 24시간 이내에 소실된다. 팽진이 24시간 이상 동일 부위에 지속될 경우 피부조직 검사로 두드러기성 혈관염을 감별하여야 한다.

만성 두드러기의 분류 및 진단

만성 두드러기는 특별한 외부 유발요인에 의하여 반복적으로 발생하는 만성 유발성 두드러기와 외부 유발요인 없이 저절로 또는 이유 없이 반복되는 만성 자발성/특발성 두드러기로 분류한다[1]. 만성 유발성 두드러기는 주로 한랭, 햇빛, 묘기증, 압박, 운동, 체온 상승 등 다양한 물리적 자극에 노출될 때 팽진과 가려움이 발생하고, 자극이 없을 때는 증상이 나타나지 않는 상태가 6주 이상 지속될 때로 정의한다(Fig. 2). 자세한 병력청취를 통하여 확인할 수 있지만, 객관적인 유발 시험을 통하여 확진 및 진단 당시 자극에 대한 역치를 평가할 수 있다[2]. 만성 자발성 두드러기는 자가반응성 또는 자가면역성, 가성 알레르기, 음식물에 대한 불내성과 드물지만 악성 종양 등 유발 및 악화요인에 따라 다양한 아형으로 분류되며, 일부 환자에서는 여러 요인이 복합적으로 작용하기도 한다. 만성 자발성 두드러기 환자에서도 한 가지 또는 그 이상의 다양한 물리적 자극들이 악화요인으로 작용할 수도 있다. 최근 연구에서는 만성 자발성 두드러기 환자의 76%에서 물리적 자극에 의한 유발성 두드러기가 동반된 것으로 보고하였다[3].

두드러기의 진단은 문진과 진찰, 의심되는 음식, 약물, 물리적 인자에 대한 유발 검사를 포함한다(Table 1) [1,4]. 진단 절차는 환자의 병력과 두드러기의 분류에 따라 권장되는 검사의 항목이 다르다. 만성 두드러기의 진단을 위하여 다른 동반 질환 및 물리적 두드러기 여부를 확인하고, 문진을 통하여 가능한 원인을 파악해야 한다. 말초혈액 검사, 적혈구 침강속도, C-반응단백 검사를 통하여 감염 질환의 동반 가능성을 확인한다. B형 및 C형 바이러스 간염, 단순포진, Ebstein-Barr 바이러스, 기생충 등의 감염이 두드러기의 원인이 될 수 있다. 또한 갑상선 질환의 과거력이 있는 경우 자가항체 검사도 고려의 대상이 될 수 있다. 갑상선 질환 이외에도 여러 자가면역 질환이 있는 경우 두드러기 증상이 동반될 수 있으므로 문진에서 의심되는 경우 추가 검사를 시행해볼 수 있다. 두드러기 환자에서 자가항체가 흔히 발견되지만 이의 임상적 연관성이 뚜렷하지 않으므로 모든 환자에서 자가항체 검사를 시행하는 것은 추천되지 않는다. 팽진이 없이 혈관부종만 반복적으로 나타나는 경우 유전성 혈관부종을 의심하여 C4, C1 inhibitor를 측정한다. 만성 두드러기 환자의 30% 이상에서 아스피린 및 진통소염제 과민증이 동반되므로 의심되는 경우 경구유발 검사를 시행해볼 수 있다. Immunoglobulin E (IgE) 또는 고친화성 IgE 수용체에 대한 자가면역반응을 확인하기 위하여 자가혈청 피부반응시험을 시행할 수 있다.

Recommended evaluations for the diagnosis of urticaria

두드러기 중증도 평가

두드러기 중증도 평가는 두드러기 활성도 점수(urticaria activity score), 두드러기 조절 점수(urticaria control test, UCT), 항히스타민제 치료에 대한 반응 여부 등으로 평가한다. 두드러기 활성도 점수는 하루 평균 발생한 팽진의 갯수와 가려움의 심한 정도를 각각 0-3점까지 측정하여 두 항목 점수를 합하여 총 0-6점까지의 점수로 계산한다(Table 2). 특히 1주일간의 활성도를 평가한 UAS7 (0-42)이 가이드라인에서 권장되고 있다[1]. 최근 개발된 UCT는 지난 4주간의 두드러기 증상의 조절 정도를 환자가 평가하는 4문항으로 구성된 척도로 총 16점 중 12점 이상인 경우 잘 조절되는 두드러기로 평가할 수 있다[1,5]. 두드러기는 질병 활성도뿐 아니라 환자의 삶의 질 평가도 매우 중요하며 이를 측정하기 위한 평가 척도가 개발되었다[1,6].

UAS

혈관부종이 동반된 경우 두드러기 중증도가 더 높고 삶의 질 저하도 큰 것으로 나타나 문진시 이에 대한 확인도 필요하다. 또한 최근 가이드라인은 만성 두드러기에서 단계별 약물치료를 권장하고 있고, 1차적으로 졸리지 않은 2세대 항히스타민제를 4정 또는 4종까지 병용 또는 증량하는 것을 권고하고 있어 항히스타민제로 잘 조절되는 환자와 조절되지 않는 환자로 임상적 분류를 하고 있다. 약 50%의 환자는 항히스타민제로 조절되지 않는 두드러기로 분류된다.

결 론

두드러기는 유병 기간에 따라 급성과 만성으로 분류하며, 급성 두드러기의 경우 원인(약물, 식품, 감염 등)을 확인하여 회피하도록 해야 한다. 최근 국내외적으로 만성 두드러기의 유병률이 증가하고 있다[7,8]. 6주 이상 지속적으로 발생 또는 예측할 수 없게 악화되는 팽진/혈관부종과 가려움으로 삶의 질 저하는 심각하지만, 아직 질병에 대한 사회적 인식은 낮은 상태이다. 따라서 원인 확인, 조기 진단 및 중증도 평가를 통한 적절한 유지 치료에 대한 미충족 수요가 높다. 증례는 3개월 전부터 시작된 만성 자발성 두드러기에 진통소염제 과민증과 혈관부종이 동반된 환자로 진단할 수 있다. 진통소염제 복용으로 인한 급성 악화시 이에 대한 급성기 치료도 중요하지만, 원인 약물에 재노출되지 않도록 교육하고, 정보를 다른 의료진과 공유하는 것이 필요하다.

만성 두드러기는 다양한 원인에 의하여 발생할 수 있으며, 환자에 따라 악화인자도 다양하다. 자가면역기전에 의한 만성 자발성 두드러기가 30-50%를 차지하며, 물리적 자극에 의해서 유발되는 만성 유발성 두드러기 환자도 있다. 알레르기 질환 및 자가면역 질환이 일반 인구에 비하여 흔히 동반되며 물리적 자극, 가성 알레르겐, 약물, 식품 등이 악화요인으로 작용할 수 있어 환자의 임상 양상에 맞추어 관련 검사를 진행하고, 확인된 원인에 대해서는 철저한 교육이 필요하다.
