Trans Abstract

Mycobacterium abscessus is the second most important pathogen in pulmonary disease caused by nontuberculous mycobacteria (NTM), following Mycobacterium avium. Mycobacterium abscessus is classified into three subspecies: M. abscessus subsp. abscessus, M. abscessus subsp. massiliense, and M. abscessus subsp. bolletii. Mycobacterium abscessus is the most difficult to treat NTM due to its resistance to many antibiotics. Treatment should include an initial regimen of 2-3 injectable and oral antibiotics for several weeks or months, followed by inhaled amikacin and 1-3 oral antibiotics, depending on the subspecies and drug susceptibility patterns, including macrolide susceptibility. The continuation phase should be continued for a minimum of 12 months after culture conversion. Suitable injectable antibiotics include amikacin, imipenem, cefoxitin, and tigecycline, while oral antibiotics include macrolides (azithromycin or clarithromycin), clofazimine, linezolid, and moxifloxacin. Surgery can be a useful adjunctive therapy for some patients with refractory disease. However, the overall treatment prognosis is still unsatisfactory. Therefore, novel and more effective interventions are required for the treatment of M. abscessus pulmonary disease.

서 론

비결핵항산균(nontuberculous mycobacteria, NTM)은 결핵균(Mycobacterium tuberculosis complex)과 나병균(M. leprae)을 제외한 항산균을 지칭한다[1]. 현재까지 180여 종 이상의 균종(species)으로 분류되고 있으며, 자연수와 토양 등 자연환경에 널리 분포하고 있다[2,3]. NTM은 폐질환, 림프절염, 피부 및 연조직 골관절염 그리고 파종성 질환 등을 일으킬 수 있는데, 이 중 폐질환이 90% 이상을 차지한다[1]. NTM 폐질환은 전 세계적으로 발생률과 유병률이 증가하는 추세이며, 국내에서도 임상 검체에서 NTM이 분리되는 빈도, 발생률과 유병률 모두 빠른 속도로 증가하고 있다[3-5]. 국내 NTM 폐질환의 중요한 역학적 특징은 외국에서 상대적으로 드문 M. abscessus (MAB)가 두 번째로 흔한 원인균으로 전체의 20-30% 내외를 차지한다는 것이다[3,6-8].

NTM 폐질환의 가장 흔한 원인균인 M. avium complex와 비교하여 MAB 폐질환은 여러 항생제에 내성을 보이고, 효과적인 경구용 항생제가 부족하여 치료가 어렵고, 수주 혹은 수개월의 주사 치료를 포함한 장기간 항생제 치료에도 불구하고 치료 성적이 좋지 못하다[9-11]. 또한 최근까지 발표된 여러 진료 지침들 사이에 권고하는 내용이 차이를 보여, 진료 현장에서 MAB 폐질환을 치료하기에 여러 어려움이 있다. 본고에서는 MAB 폐질환의 치료 방법에 대하여 현재까지 발표된 진료 지침과 연구 결과를 살펴보고자 한다.

본 론

MAB 폐질환의 진단

MAB 폐질환의 진단은 임상적, 방사선학적, 미생물학적 기준에 따라 이루어지며, 2007년 미국흉부학회/미국감염학회의 기준이 전 세계적으로 가장 널리 사용되고 있다[1]. 이 진단 기준은 호흡기 증상을 가지고 있으면서, 흉부 X-ray 혹은 전산화단층촬영에서 공동 혹은 다병소의 기관지확장증과 동반된 다발성 소결절을 보이는 환자에 적용된다. 미생물학적으로는 첫째, 객담 검사에서 최소한 2회 배양 양성을 보이거나 둘째, 기관지세척액 1회에서 배양 양성인 경우 그리고 셋째, 경기관지 폐생검 등 조직 배양이 양성이거나 또는 조직 검사에서 육아종 등 항산균 감염의 병리학적 증거가 있으면서 1회 이상 객담 또는 기관지세척액에서 배양 양성이어야 한다[1].

MAB 폐질환은 다른 NTM 균종에 의한 폐질환과 마찬가지로 섬유공동형(fibrocavitary)과 결절 기관지확장증형(nodular bronchiectatic)의 두 가지 특징적인 방사선학적 형태로 구분된다. 섬유공동형은 주로 폐의 상엽에 공동이 관찰되며, 흡연력과 음주력이 있는 중년 이상의 남성에서 흔하고 만성 폐쇄성 폐질환, 폐결핵 등의 기저 질환을 갖고 있는 경우가 많다(Fig. 1A). 이러한 형태는 치료를 하지 않으면 수년 이내에 폐실질의 파괴와 사망으로 진행된다. 이에 반하여 결절 기관지확장증형은 기관지확장증에 동반된 다발성 중심소엽성 결절이 특징적으로 관찰되며 주로 우중엽과 좌상엽의 설상엽을 침범한다(Fig. 1B). 중년 이상의 비흡연자 여성에서 흔하고 기저 질환은 적다. 이 형태는 상대적으로 진행 속도가 느려서 일부 환자에서는 방사선학적 소견의 변화가 수년에 걸쳐 서서히 나타나기도 한다[12].

MAB의 명명법(taxonomy)

고체배지 계대배양(subculture)에서 7일 이내에 집락을 형성하는 항산균을 신속 성장균으로 분류하는데, 인체 감염을 일으키는 신속 성장균 중 MAB는 가장 대표적인 균종으로 M. chelonaeM. fortuitum 등과 함께 약 80%를 차지한다[1,13]. MAB에 의한 인체 감염 질환이 최초로 보고된 것은 1953년도 피부·연조직 감염이다[14]. 1980년대까지 MAB와 M. chelonae 균종을 분리하지 않고 M. chelonae/abscessus complex로 명명하다가 1990년대 초부터 MAB를 독립된 균종으로 구분하였다. 이후 분자미생물 검사의 발달로 인하여 최근까지도 MAB의 명명법이 변하고 있다.

2006년에는 MAB로부터 M. massilienseM. bolletii를 분리하여 각각 독립된 균종으로 구분하였다[15]. 2011년에는 MAB를 M. abscessus subspecies abscessusM. abscessus subspecies bolletii의 2개의 아종(subspecies)으로 분류하면서, 기 존의 M. massilienseM. bolletii 2개의 균종을 M. abscessus subspecies bolletii로 하나의 아종으로 통합하였다[16]. 그 이후 전장 유전체 분석(whole-genome sequencing)이 도입되면서, 2013년에는 M. abscessus subspecies massilienseM. abscessus subspecies bolletii로부터 다시 독립된 아종으로 분류되었다[17] (Fig. 2).

현재는 MAB를 M. abscessus subspecies abscessus, M. abscessus subspecies massiliense 그리고 M. abscessus subspecies bolletii의 3가지의 아종으로 분류하는 방법이 가장 널리 받아들여지고 있다[18,19]. 본고에서는 MAB를 위와 같은 3가지 아종으로 분류하고, 그중 임상적으로 가장 중요한 M. abscessus subspecies abscessus, M. abscessus subspecies massiliense에 의한 폐질환에서 대해서 알아보고자 한다. 국내 많은 병원에서 균종을 아종으로 분류하지 않고 보고하고 있는 현실적인 면과 독자들의 편의를 고려하여, 이하에서는 M. abscessus subspecies abscessusM. abscessus로, M. abscessus subspecies massilienseM. massiliense로, 그리고 M. abscessus subspecies bolletiiM. bolletii로 표기하였다. 3가지 아종을 통합하여 지칭할 때는 MAB 약자를 사용하였다.

M. abscessusM. massiliense 구분의 중요성

MAB의 3가지 아종의 분포는 국가와 지역에 따라 다르다[20]. 국내와 중국, 대만, 일본의 보고에 따르면 동아시아 국가에서 MAB 폐질환을 일으키는 대부분의 아종은 M. abscessusM. massiliense로 특히 국내에서는 2가지 아종이 거의 비슷한 비율로 발생하며, M. bolletii의 비율은 1-2% 내외로 그 비율이 매우 낮다[20,21]. MAB 폐질환은 아종에 따라서 효과적인 약제와 치료 반응이 차이가 나기 때문에, 치료 전 정확한 아종을 확인하는 것이 매우 중요하다. 특히 M. massiliense는 macrolide 항생제에 좋은 치료 반응을 보이는 반면, 대부분의 M. abscessus는 macrolide에 대해서 유도내성을 보이기 때문에 치료 반응이 좋지 못하기 때문이다[22-24]. 그리고 국내 대부분 병원의 임상미생물 검사실에서 사용하는 검사법으로는 M. bolletiiM. abscessus를 구분하지 못하는데, M. bolletii는 모두 macrolide 유도내성이 있어서 최소한 M. abscessusM. massiliense의 구분은 국내에서 특히 중요하다고 할 수 있다.

Macrolide 유도내성은 약제 감수성 검사에서 3일째에는 macrolide에 대한 최소 억제 농도(minimal inhibitory concentration, MIC)가 낮아 감수성으로 보이지만, 14일까지 기간을 연장하면 MIC가 상승하여 macrolide 내성을 보이는 것으로 erythromycin resistance methylase (erm) 유전자 중 erm (41) 유전자가 관여한다. 이러한 erm (41) 유전자는 대부분의 M. abscessus에서는 활성화되어 있어서 macrolide 유도내성을 보이는 반면, 모든 M. massilienseerm (41) 유전자의 염기서열 일부에 소실(deletion)이 있기 때문에 macrolide 유도내성을 보이지 않는다[22-25]. 한편 약 10-20%의 M. abscessuserm (41) 유전자의 T→C 점돌연변이(C28 sequevar)에 의해서 erm (41) 유전자가 비활성화되기 때문에 M. massiliense와 마찬가지로 유도내성을 보이지 않는다[26,27] (Table 1).

Macrolide susceptibility of M. abscessus

2011년도 미국임상검사표준연구원(Clinical and Laboratory Standards Institute)에서는 MAB에 대해서 3일째와 14일째에 macrolide 항생제의 MIC를 측정하여 macrolide 유도내성 여부를 보고하도록 권고하였다[28]. 따라서 환자의 원인균이 M. massiliense로 동정된 경우와 M. abscessus에서 14일째 약제 감수성 검사 결과에서 macrolide 감수성으로 확인된 경우에는 macrolide에 대한 유도내성이 없어, macrolide 항생제에 좋은 치료 반응이 예상되므로 반드시 macrolide를 치료 약제에 포함해야 한다[24,27,29].

과거 MAB 폐질환의 치료 결과를 보고한 연구들에서는 정확한 아종의 구분이나 erm (41) 유전자에 대한 분석을 시행하지 않았기 때문에, 각 연구마다 치료 반응이 다양하게 보고되었다. 예를 들어 미국에서 시행된 69명의 MAB 폐질환 환자를 분석한 연구에서는 아종 구분이나 erm (41) 유전자를 분석하지 않았는데, 장기간의 치료에도 불구하고 48%의 환자들만 균음전에 성공하였다[30]. 반면 국내 65명의 MAB 폐질환 환자의 치료 성적을 보고한 연구에서는 12개월 균음전률이 58%로 상대적으로 높았고[31], 국내 다른 연구에서는 41명의 MAB 폐질환 중 균음전 성공률이 81%까지 높게 보고되기도 하였다[32]. 이후 MAB의 아종 및 erm (41) 유전자에 대한 분석을 포함한 국내 연구에서는 M. massiliense 폐질환의 치료 성공률이 82-96%로 높은 반면 M. abscessus 폐질환의 치료 성공률이 25-51%로 낮음을 보여, MAB 아종 구분의 중요성을 보여주었다[24,29,33-35]. 또한 국외 문헌을 포함한 메타분석에서도 M. massiliense 폐질환의 균음전률은 57-79%로 높은 반면 M. abscessus 폐질환의 균음전률은 33-41%로 차이를 보였다[9-11]. 따라서 MAB 폐질환 치료 전 정확한 아종의 구분과 macrolide 유도내성 여부 혹은 erm (41) 유전자의 평가는 매우 중요하다.

항생제 치료

MAB 폐질환의 치료는 장기간의 병합 항생제 치료가 권고되며, 2007년 미국흉부학회/미국감염학회, 2016년 미국/유럽낭포성섬유증학회 그리고 2017년 영국흉부학회에서 MAB 폐질환의 진료 지침을 발표하였다[1,36,37]. 하지만 MAB는 대부분의 항결핵약제와 많은 항생제에 내성을 보이고, 권고되는 항생제의 대부분이 무작위 비교 연구가 아닌 관찰 연구를 통해서 효과를 보인 것이기 때문에 이상적인 약제 조합에 대한 근거는 아직 부족하다. 따라서 MAB 폐질환 치료 약제는 개별 환자에서 정확한 아종의 확인, macrolide 유도내성 유무 혹은 erm (41) 유전자의 활성도 여부, 그리고 다른 약제에 대한 감수성 검사 결과를 바탕으로 선택해야 한다. 2011년도 미국임상검사표준연구원은 MAB 치료 전에 반드시 약제 감수성 평가에 포함되어야 하는 항생제로 amikacin, cefoxitin, ciprofloxacin, clarithromycin, doxycycline (혹은 minocycline), imipenem, linezolid, moxifloxacin, trimethoprim-sulfamethoxazole, tobramycin 등을 제 시하였는데, 2018년 개정된 지침에서는 tigecycline과 clofazimine에 대한 검사도 고려하도록 하였다[28,38]. 따라서 본고에서는 우선 MAB 치료에 사용되는 주요 항생제에 대해서 언급하고, 이어서 진료현장에서 실제 적용 가능한 약제의 조합에 대해서 알아보고자 한다.

Macrolide (azithromycin, clarithromycin)

M. massiliense와 일부 macrolide 유도내성이 없는 M. abscessus는 macrolide 항생제에 감수성이므로, 이 경우에 macrolide는 반드시 치료 약제 구성에 포함되어야 한다. 한편 80-90%의 M. abscessus는 macrolide에 대해서 유도내성을 보이는데, 이러한 경우에서 macrolide의 효과에 대해서는 아직 근거가 부족하다. 하지만 실험 연구와 최근 국내외 14개의 문헌을 분석한 메타 연구에서는 M. abscessus 폐질환에서 azithromycin 사용이 효과가 있을 가능성을 보여주었으며 [11,25], macrolide 항생제는 만성 기도 질환에 대하여 항염증 효과가 있다[39,40]. 따라서 현재 미국/유럽낭포성섬유증학회와 영국흉부학회에서는 macrolide 유도내성을 가진 M. abscessus 폐질환에서도 macrolide 사용을 권하고 있다[36,37].

Macrolide 항생제 사용에 있어서 유의해야 하는 다른 한 가지는 획득내성 유무이다. Macrolide는 세균의 단백질 합성에 관여하는 리보솜 대단위체(ribosomal large subunit)의 23S ribosomal ribonucleic acid (rRNA) 펩티드에 결합하여, 펩티드기 전달효소의 활성도를 억제하게 되어 정균 작용을 나타낸다. 하지만 세균이 장기간 macrolide에 노출되게 되면, 23S rRNA의 펩티드기 전달효소 영역을 코딩하는 유전자(rrl)의 점돌연변이가 발생하여 획득내성을 유발할 수 있다. Macrolide 획득내성은 주로 부적절한 병합요법이나 macrolide 단독 치료를 하게 되는 경우 발생하며 erm (41) 유전자의 활성도 유무와 무관하게 발생한다. Macrolide 획득내성이 발생한 경우는 치료 반응이 매우 좋지 못하며, 최근 보고에 의하면 장기간 항생제 치료받은 13명 중 1명(8%)만이 수술 후에 균음전이 가능하였다[41].

Amikacin

Amikacin은 MAB 폐질환 치료에 있어서 가장 중요한 정주용 항생제로, 여러 진료 지침에서 amikacin 사용을 권고한다[1,36,37]. 국내 보고에 따르면 M. abscessusM. massiliense 임상균주의 약 70-100%가 amikacin 감수성으로 보고되었다[42-44]. 메타분석 연구에서도 M. abscessus 폐질환에서 정주용 amikacin이 치료 성공과 관련됨을 보여주었다[11]. 최근에는 장기간 정주용 amikacin 사용과 관련된 부작용을 줄이고, 폐흡수를 통한 치료 효과를 증대시키는 목적으로 amikacin 흡입 치료가 권고되고 있다[36,37]. Amikacin 흡입 치료의 효과를 보고한 최근 연구들에서는 주로 기존의 항생제 치료에 반응하지 않은 MAB 폐질환 환자를 대상으로 흡입 amikacin을 추가하였고 13-67%까지 다양한 균음전 성공률을 보고하였다[45-47].

최근에는 기존의 amikacin에 liposome을 조합하여 amikacin의 전신 부작용을 줄이고 폐 침투 효과를 증대시킬 목적으로 amikacin liposome inhalation suspension (ALIS)이 개발되었다. ALIS를 이용하여 난치성 NTM 폐질환을 대상으로 무작위 전향적 연구가 시행되었는데, ALIS가 추가된 15명의 MAB 환자 중에서 3명이 균음전에 성공하였다[48]. 최근에는 ALIS가 난치성 M. avium complex 폐질환에서 효과가 증명되어 미국식품의약국의 승인을 받고 미국에서는 사용이 가능하게 되었는데[49,50], MAB 폐질환에서는 임상 연구가 부족하고 아직까지는 미국에서도 승인이 되지 않았다.

Imipenem or cefoxitin

Imipenem 혹은 cefoxitin은 MAB 폐질환에 권고되는 정주용 β-lactam 항생제이다. 국내 조사에 따르면 imipenem에 대해서는 M. abscessusM. massiliense의 24% 그리고 cefoxitin에 대해서는 7-13% 정도만이 내성으로 보고되었다[42-44]. 최근 메타분석 연구에서는 MAB 폐질환 치료에서 imipenem의 사용이 치료 성공과 관련되어 있었고[11], MAB 폐질환 치료 성적을 보고한 여러 국내외 연구에서도 imipenem이 사용되었다[24,29,31,32,35]. Cefoxitin은 장기간 사용할 경우 호중구감소증, 혈소판감소증 그리고 간독성 등이 발생할 수 있어 주의가 필요하다[31].

Tigecycline

Tigecycline은 minocycline 유도체로 단백합성을 방해하여 정균작용을 보이는 약제로써 MAB, M. chelonae, M. fortuitum등의 신속 성장균에 대한 약제감수성 검사에서 낮은 MIC를 보인다[51,52]. 52명의 난치성 MAB 감염증 환자에 대해서 tigecycline을 포함한 약제를 사용한 연구에서 30명의 MAB 폐질환환자 중 43%에서 임상적 호전을 보였다[53]. Tigecycline은 오심, 구토 등의 위장장애를 흔히 일으키기 때문에 tigecycline 투여 전 ondansetron과 같은 항구토제의 사용이 권고된다[37]. 하지만 MAB 폐질환을 대상으로 tigecycline을 사용한 임상 연구 결과는 많지 않고, 아직까지 확실한 내성 판정 기준이 정립되지 않았다[54].

Clofazimine

Clofazimine은 지용성 riminophenazine 계열 항생제로 나병 치료제이면서 최근 다제내성결핵 치료에 사용되고 있는 약제이다[55]. 최근에는 clofazimine이 MAB 폐질환 치료에 효과가 있음이 보고되었다[56,57]. 국내 42명의 M. abscessus 폐질환 환자에서 clofazimine을 포함한 약제 치료를 받은 연구에서는 전체 환자 중 81%에서 증상의 호전, 31%에서 방사선학적 호전 그리고 24%에서 균음전을 보였다[56]. 또한 실험실 연구에서 MAB에 대하여 clofazimine과 amikacin의 상승효과(synergism)가 증명되었다[58,59]. 하지만 여전히 clofazimine을 이용한 임상 연구 결과는 많지 않고, MAB에 대한 적절한 내성 판정 기준이 정립되지 않았다[54].

Linezolid

Linezolid는 oxazolidinone 계열 항생제로써 다제내성결핵 치료에 효과가 입증된 약물이다[55]. 국내 연구에서는 M. abscessusM. massiliense의 62-97%가 linezolid에 감수성을 보인다[42-44]. 하지만 M. abscessusM. massiliense는 결핵균과 비교하여 linezolid에 대한 MIC가 높고, MAB 폐질환 환자를 대상으로 linezolid의 치료 효과를 보고한 연구는 현재까지 없다. 최근 MAB 감염증을 포함한 102명의 NTM 감염증 환자를 대상으로 linezolid를 사용한 결과가 보고되었는데, 약 45%의 환자에서 말초신경병증, 골수기능 저하 등의 부작용이 발생하였다[60]. 따라서 아직 MAB 폐질환 치료에서 linezolid의 적절한 용량과 치료 기간 및 그 효과는 불확실한 상태이다.

치료의 실제 적용

MAB 폐질환의 치료는 정확한 아종, macrolide 유도내성 여부, 다른 약제에 대한 감수성 결과를 기반으로 하여, 효과가 증명되거나 예상되는 약제를 병합하여 사용하여야 한다. MAB 폐질환의 치료에 대해서 1997년 미국흉부학회 진료 지침에서는 초기 2-4주 그리고 2007년 미국흉부학회/미국감염학회에서는 초기 2-4개월간 macrolide 항생제와 함께 amikacin과 imipenem (혹은 cefoxitin)을 사용할 것을 권고하였다[1,61]. 최근 2016년 미국/유럽낭포성섬유증학회와 2017년 도 영국흉부학회의 진료 지침에서는 전체 치료 기간을, 병합 정주 항생제 및 macrolide를 사용하는 초기 치료기(initial phase)와 amikacin 흡입과 경구용 항생제를 사용하는 유지 치료기(continuation phase)로 구분하였다[36,37].

초기 치료기에 대해서, 미국/유럽낭포성섬유증학회에서는 macrolide와 정주용 amikacin과 함께 tigecycline 혹은 imipenem (혹은 cefoxitin)을 병합한 2-3개의 정주용 항생제를 3-12주간 사용하도록 하였다. 그리고 macrolide 유도내성을 보이는 M. abscessus나 획득내성을 보이는 아종에 대해서는 초기 치료기의 기간을 연장하는 것을 권하였다[36]. 영국흉 부학회는 초기 치료기에는 경구 macrolide와 함께 amikacin, tigecycline 그리고 imipenem (혹은 cefoxitin)의 2-3개의 정주용 항생제를 병합하여 최소 1개월 이상 사용하도록 권하는데, macrolide 유도내성 혹은 획득내성을 보이는 경우에는 환자의 약물 순응도를 고려하여 3-6개월까지 초기 치료기를 연장하는 것이 적절할 수 있다고 하였다[37]. 그리고 macrolide 획득내성인 경우는 macrolide 투여를 권하지 않으나, macrolide 유도내성을 보이는 M. abscessus에서는 macrolide 항생제 사용을 권고하였다[37]. 하지만 수개월간의 장기간의 정주용 항생제 치료는 치료 성적을 높일 수 있지만, 비용과 부작용을 고려할 때 실제 환자 진료에 적용하기 힘든 면이 있다. 한편 M. massiliense와 같이 erm (41) 유전자가 비활성화된 경우와 일부 macrolide 유도내성이 없는 M. abscessus는 macrolide에 좋은 치료 반응을 기대해 볼 수 있다. 이러한 경우, 최근 일부 연구에서는 초기 치료기를 2-4주로 단축하더라도 좋은 치료 성적을 보고하였고, 이에 대해서는 좀 더 많은 연구가 필요하다[27,29].

이후 유지 치료기에는 amikacin을 흡입 치료로 변경하고, macrolide를 계속 유지하면서 이 외에 감수성이 있거나 효과가 예상되는 경구용 항생제를 병합한다. 미국/유럽낭포성섬유증학회에서는 amikacin 흡입 및 경구 macrolide와 함께 2-3개의 경구 항생제를 사용하도록 권하는데, clofazimine, linezolid, moxifloxacin 그리고 minocycline 등을 고려할 수 있다[36]. 영국흉부학회에서는 macrolide 획득내성 없이, 감수성이거나 유도내성인 경우는 amikacin 흡입치료와 macrolide를 유지하고, 이외 1-3개의 경구 항생제를 추가하도록 권한다. 이때 약제 감수성 결과에 따라서 clofazimine, linezolid, moxifloxacin, minocycline, co-trimoxazole 등을 고려할 수 있다[37]. Macrolide 이외의 경구용 항생제 선택시 고려해야 할 점이 몇 가지 있다. Clofazimine은 이전 보고에서 MAB에 낮은 MIC를 보여 효과가 있을 것으로 기대되지만[58,62], 현재까지 확실한 내성 판정 기준이 없다[38]. 또한 국내 보고에 의하면 linezolid는 M. abscessus의 62%, M. massiliense의 72%가 감수성인 반면, moxifloxacin에 대해서는 M. abscessus의 2-7%, M. massiliense의 5-9%만이 감수성이다[42,43]. 전체 유지 치료 기간은 균음전에 성공한 이후 최소 12개월 이상 균음전 상태가 유지되도록 사용하도록 권고한다. 위의 여러 진료 지침에서 권고한 치료 내용과 저자들의 의견을 정리하면 표 2와 같다.

장기간 치료에도 불구하고 균음전에 실패한 난치성 환자들에서는, 수개월 이상의 항생제 치료 이후에 수술적 절제를 고려할 수 있다. 수술적 절제는 폐병변이 국소적으로 있어서 해부학적으로 절제가 가능하며, 환자의 심폐기능이 수술을 견딜 수 있는 경우에 고려할 수 있다[30,63,64]. 수술은 항생제 치료 후 균음전에 실패한 환자에서 완치를 기대해 볼 수있는 방법이다. 하지만 수술 후 높게는 20%의 환자들에서 기관지 흉막루 등과 같은 합병증이 발생할 수 있기 때문에 신중히 결정해야 한다[64].

결 론

NTM 폐질환은 국내외에서 매우 빠른 속도로 증가하고 있으며, 국내에서는 MAB가 두 번째로 흔한 원인균이다. MAB 중 M. abscessusM. massiliense가 폐질환을 일으키는 대표적인 아종으로 장기간의 병합 항생제 치료가 필요하다. MAB 폐질환 치료는 정확한 아종의 확인, macrolide 유도내성 여부, 다른 약제의 감수성 결과를 고려하여 효과가 증명되거나 예상되는 약제를 사용한다. 현재 권고되는 치료는 정주용 항생제를 병합한 초기 치료기와 이후의 amikacin 흡입치료와 함께 경구용 항생제를 이용한 유지 치료기로 구분된다. 전체 치료 기간은 균음전에 성공한 이후에 최소 12개월 이상 균음전 상태가 유지되도록 하는 것이다. 하지만 현재까지 전체적인 MAB 폐질환의 치료 결과는 만족스럽지 못하며, 특히 M. abscessus 폐질환 치료 성적은 매우 낮다. 따라서 MAB 폐질환의 치료 성적 향상을 위해서는 앞으로 더 많은 임상연구가 필요한 실정이다.
