Trans Abstract

Acute kidney injury (AKI), characterized by an acute decline in renal function, commonly develops in hospitalized patients and is associated with increased morbidity and mortality. Recent studies have identified new therapeutic strategies for its management. In this review, we will discuss the definition, etiology, and general treatment of AKI.

서 론

급성신손상(acute kidney injury, AKI)은 입원 환자에게 매우 흔하며 환자의 이환율과 사망률을 현저하게 증가시킨다. 또한 AKI는 만성 콩팥병(chronic kidney disease, CKD)과 말기신부전(end stage renal disease, ESRD)의 발생 위험도를 증가시킨다[1]. AKI는 전체 입원 환자의 5-10% 정도에서 발생하며, 중환자실 환자의 경우 60%까지도 발생하는 것으로 보고되고 있다[2]. 뿐만 아니라 AKI의 발생률은 증가하고 있으며 이로 인한 ESRD 발생 또한 증가하고 있다[3,4].

AKI는 원인이 매우 다양하기 때문에 하나의 질병으로 접근하기 어렵다. 어떤 치료는 AKI 환자에게 공통적으로 적용할 수 있으나 많은 경우 질병에 특이하게 접근해야 한다. 따라서 AKI가 발생하였다면 먼저 AKI의 원인을 파악하려는 노력이 필수적이다. 이러한 원인 질병의 다양성 때문에 임상 연구에서 공통적인 효과를 나타내는 AKI의 치료 방법이 대두되기 어려운 실정이다. 그러나 최근 새로운 시각을 갖게 해줄 여러 가지 임상 연구들이 보고되고 있다. 본고에서는 AKI의 정의와 원인에 대하여 알아보고 일반적인 치료에 대하여 기술하고자 한다.

본 론

AKI의 정의

전통적으로 급성신부전(acute renal failure, ARF)이라는 개념은 심한 핍뇨 및 무뇨 혹은 투석을 필요로 하는 정도의 심한 신기능 저하에 초점을 맞추었다. 그러나 경한 신기능의 손상 및 그로 인하여 발생하는 소변량의 변화와 전해질 이상 등이 단기 및 장기 예후에 중요한 영향을 미친다는 것이 보고되면서 경한 신기능 손상부터 투석을 필요로 하는 ARF 단계까지 포함하여 2004년 Acute Dialysis Quality Initiative에서 RIFLE 진단기준을 발표하였다[5]. RIFLE 분류는 AKI를 혈청 크레아티닌과 소변량의 변화에 따라 각각 ‘Risk’, ‘Injury’, ‘Failure’의 3단계로 나누고, 신손상의 결과에 따라 ‘Loss’와 ‘End-Stage Kidney Disease’의 범주를 포함시켰다. 소변량과 혈청 크레아티닌 농도가 같은 단계로 일치하지 않는다면, 둘 중 더 높은 단계로 AKI를 정의한다. 2007년에는 Acute Kidney Injury Network에서 RIFLE 분류의 민감도를 향상시킬 목적으로 ‘Risk’의 정의에 48시간 이내에 0.3 mg/dL 이상의 크레아티닌 증가 항목을 추가하였다[6]. 2012년에는 Kidney Disease: Improving Global Outcome (KDIGO) 그룹에서 이전의 정의들을 통합하여 AKI의 정의를 발표하였다(Table 1) [1].

신기능을 평가하는 방법으로 사구체 여과율(glomerular filtration rate, GFR)이 gold standard이며 임상 상황에서는 GFR을 혈청 크레아티닌을 이용하여 간접적으로 평가하게 된다. 그러나 AKI와 같이 혈청 크레아티닌이 안정적이지 않고 변화하는 상황에서 현재 GFR을 평가하는 식(Cockcroft-Gault, modification of diet in renal disease [MDRD], and CKD-EPI)이 신기능을 적절하게 평가하는지에 대해서 연구가 부족하다. 따라서 AKI시 단계를 평가하는 데 GFR보다 혈청 크레아티닌을 사용하나 혈청 크레아티닌 또한 제한점이 있다. 혈청 크레아티닌은 신손상이 발생 후 일정 시간이 지난 뒤 상승되기 때문에 현재 신기능을 반영하지 못한다. 또한 급성 및 만성 질환으로 인하여 근육 감소가 있는 경우, 체액 증가나 수액을 투여하는 경우 혈청 크레아티닌 농도가 감소되어 실제 신기능보다 과대평가 될 수 있다. 이를 보완하기 위하여 근육량과 관련이 없는 cystatin C를 사용할 수 있으나 안정된 상태의 동역학(kinetics) 및 분포 용적(volume of distribution)에 대한 연구가 부족하다는 단점이 있다[7]. 혈청 크레아티닌보다 초기에 세뇨관의 손상을 반영할 수 있는 표지자(biomarker)로 neutrophil gelatinase associated lipocalin (NGAL), kidney injury molecule 1 (KIM-1), interleukin 18 (IL-18), liver-type fatty acid binding protein (L-FABP) 등이 연구되고 있다[8]. 또한 최근 미국 Food and Drug Administration 승인을 받은 cell cycle arrest marker인 tissue injury metalloproteinase 2 (TIMP-2) and insulin-like growth factor binding protein 7 (IGFBP-7)이 중환자에서 중등도 이상의 AKI를 유의하게 예측한다고 보고되고 있다[9].

AKI의 원인

AKI가 발생하면 그 원인에 대하여 조사해야 하며(Table 2), 주의 깊은 병력 청취, 신체 진찰 및 검사 소견 등을 종합하여 판단해야 한다. 체액 감소 및 저혈압으로 인하여 바소프레신 분비가 자극되면 원위 세뇨관 및 집합관에서 물이 재흡수되며 속 속질(inner medulla)의 농축 기전으로 인하여 소변의 삼투압은 증가하고 소변량은 감소하게 된다. 세뇨관 손상이 발생하면 신장의 농축능이 감소하여 소변의 삼투압은 감소하고 소변량은 정상화된다. 따라서 소변량이 감소하고 고질소 혈증(azotemia)이 발생하였으나 소변의 삼투압이 높고 fractional excretion of sodium and urea가 낮다면 세뇨관의 기능이 보존되어 있음을 뜻한다. 그러나 사구체신염과 같이 세뇨관 기능이 보존되어 있는 경우 혹은 패혈증과 같이 특징적인 소변 검사를 보이지 않는 경우가 있으므로 소변 삼투압이 증가되어 있는 핍뇨를 모두 pre-renal AKI로 분류하지 않아야 한다. 심한 핍뇨나 무뇨 발생시 요관 폐쇄나 완전한 동맥 또는 정맥 폐쇄를 의심해 볼 수 있다[1].

Causes of acute kidney injury

또한 새롭게 현저한 단백뇨가 발생하였거나 혈뇨가 발생한 경우, 활동성 요 침전물을 보이는 경우, 혹은 원인이 불명확한 경우 신성 원인을 감별하기 위하여 신조직검사를 시행할 수 있다[10]. 급성 간질성 신염(acute interstitial nephritis)의 경우 발열, 호산구 증가증, 발진 등의 전신증상이 동반될 수 있으나 22-36%에 불과하며 신증후군 범위 이하의 단백뇨, 혈뇨, 농뇨가 빈번하게 동반된다. 원인으로는 양성자 펌프 억제제, 항생제 등의 약제가 75% 이상을 차지하고 감염 및 전신 질환 등도 유발 가능하다. 명확한 진단을 위해서는 신조직 검사가 필수적이고 치료로는 원인 약제를 중단하거나 스테로이드를 고려할 수 있다[11].

AKI의 치료

AKI 발생 위험이 높거나 AKI가 발생한 환자는 혈청 크레아티닌과 소변량을 주의 깊게 관찰해야 한다. 또한 생체 징후 및 용적 상태를 판단하여 정맥 주사, 이뇨제 및 승압제 등을 투여해야 한다.

Aminoglycoside, vancomycin, nonsteroidal anti-inflammatory drugs (NSAIDS), angiotensin-converting enzyme inhibitors/angiotensin receptor blockers 등 AKI 원인이 되는 약제 및 악화 시킬 수 있는 약제를 리뷰하여 변경하거나 중단해야 한다. 또한 GFR이 감소됨에 따라 축적될 수 있는 약제를 피하거나 조정해야 한다(Table 3) [12]. 또한 iodinated radiocontrast의 사용을 피해야 한다. 여기서는 신대체 요법 이외의 일반적인 AKI의 치료에 대하여 설명하고자 한다.

정맥 내 수액 요법

저혈압은 허혈성 AKI를 발생시킬 수 있으나 적절한 volume replacement 없이 승압제를 투여하는 경우 신혈류를 감소시킬 수 있다. 또한 저혈압 상태이지만 용적이 과다된 환자에게 정맥 내 수액을 투여하는 경우 오히려 환자의 상태를 악화시킬 수 있다. 대규모 임상 연구에서 패혈증 환자에게 정맥 내 수액, 승압제, 수혈 등의 early goal directed therapy를 하는 것이 사망이나 신대체 요법을 줄이는 데 효과가 없는 것이 발표되었다[13]. 또한 대규모 코호트 연구에서 중환자실 환자에서 투석을 시작하는 시점에 용적 과다가 있는 환자가 사망의 위험도가 2.07배 의미 있게 높다는 것이 보고되었다[14]. 따라서 AKI 환자에서 용적 상태를 평가하여 용적이 저하된 경우 fluid resuscitation을 시도하고 용적 반응성을 평가해야 한다. 또한 용적 과다 징후가 있다면 수액을 줄이거나 중단하는 것이 필요하다.

Fluid resuscitation이 필요하다면 수액의 종류를 선택해야 한다. Saline versus albumin fluid evaluation (SAFE) trial에서 중환자실 환자에서 4% albumin을 투여하는 것이 사망이나 신기능 호전에 효과가 없었으나 필요한 총 수액량이 의미 있게 줄어든다는 것이 보고되었다(2.2 L vs. 3.1 L) [15]. 따라서 간경화 환자가 패혈증 쇼크가 있을 때와 같이 많은 양의 volume resuscitation이 예상되는 특정 상황에서 albumin을 투여해 볼 수 있겠다.

여러 연구에서 hydroxyethyl starche (HES)가 근위 세뇨관의 공포화(vacuolization) 및 부종을 일으켜 신독성이 있다는 것이 보고되었다. 7,000명의 중환자실 환자에서 saline 혹은 6% HES를 투여한 경우 HES군에서 신대체 요법을 할 위험도가 의미 있게 높았다(7.0% vs. 5.8%; p= 0.04) [16]. 따라서 저혈량 쇼크 환자에서 교질 용액(colloids)을 투여하는 것이 긍정적인 효과가 있을 것이라는 임상적인 관심이 있으나 현재까지 뒷받침할 만한 연구가 없다.

따라서 수액 투여가 필요한 환자에게 교질 용액보다 정질 (crystalloid) 용액을 선택하게 된다(Table 4). 등장성 수액인 0.9% saline은 plasma보다 chloride 농도가 높다. 0.9% saline을 투여하는 환자는 hyperchloremic metabolic acidosis가 발생할 위험이 있다. 동물 및 인간을 대상으로 한 연구에서 0.9% saline은 balanced salt solution에 비하여 신혈관 수축을 일으켜 신피질 관류 및 GFR을 감소시키는 것이 보고되었다[17,18]. 중환자에서 다른 종류의 정질 용액을 비교하는 임상 연구는 상충되는 결과를 보인다. 0.9% saline vs. plasma-lyte 148 (PL-148) for ICU fluid therapy (SPLIT) trial은 중환자실 환자 2,278명을 대상으로 saline과 plasma-lyte를 투여받은 환자를 비교하였고 AKI 발생이 의미 있게 다르지 않았다[19]. 그러나 이 연구의 환자군은 동반 질환이 적었고 평균 투여받은 수액이 2 L 미만이라는 것에서 제한점이 있다. Isotonic Solutions and Major Adverse Renal Events Trial (SMART) 연구는 15,802명의 환자를 0.9% saline과 buffered crystalloid solution을 비교하였으며 30일 이내 major adverse kidney event (renal replacement therapy, death, and/or final serum creatinine ≥ 200% of the baseline value)가 의미 있게 감소하였다(14.3% vs. 15.4%; p= 0.04) [20]. 응급실에 내원한 13,347명의 환자를 같은 방법으로 비교한 결과에서도 30일 이내 major adverse kidney event가 의미 있게 감소하였다[21]. 최근의 증거들로 보아 fluid resuscitation시 AKI 위험도가 있는 환자에서 buffered solution을 사용하는 것이 더 추천되며, 0.9% saline은 체액 감소가 있거나 hypochloremia가 있을 때 더 선호된다.

적절한 혈압 유지

Sepsis and mean arterial pressure (SEPSISPAM) trial에서 패혈증 쇼크가 있는 환자를 대상으로 low mean arterial pressure (MAP) target (65-70 mmHg)과 high MAP target (80-85 mmHg)을 비교한 결과, 사망률에는 차이가 없었으나 만성 고혈압이 있는 환자에서는 high MAP target군에서 AKI와 신대체 요법이 감소하였다. 그러나 high MAP군에서 심방세동이 의미 있게 증가하였다[22]. 따라서 blood pressure의 target은 동반 질환에 따라 결정될 수 있겠다.

이뇨제

Furosemide 등의 루프 이뇨제가 핍뇨가 있는 AKI 환자에게 임상적으로 흔히 사용된다. 루프 이뇨제는 이론적으로 Na+/K+/2Cl-를 억제하여 산소가 적은 속질에 metabolic demand를 줄인다고 알려져 있으나 여러 임상 연구에서 AKI시이뇨제의 의미 있는 효과를 보이지 못하였다. 따라서 KDIGO 그룹에서는 용적 과다가 있는 경우를 제외하고는 AKI를 치료하는 데 이뇨제를 사용하지 않는 것을 추천하고 있다[1]. Furosemide challenge에 소변량이 증가하는 경우는 세뇨관 기능이 적절한 것을 의미하며 이 경우 신장 관련 예후가 좋다는 것이 보고되고 있으나[23] 이에 대해서는 좀 더 대규모 연구가 필요하다.

Contrast induced AKI

조영제 유발 급성신손상(contrast induced AKI, CI-AKI)은 AKI의 특이 형태로 주로 정맥 내 iodinated contrast 사용 후 수 일 이내에 혈청 크레아티닌이 소폭 증가로 나타난다. 대부분 자연적으로 회복되나 단기 및 장기 사망률 및 CKD 진행의 위험도를 증가시키는 것이 보고되었다.

조영제는 세뇨관 세포의 극성(polarity)을 감소시켜 Na+/K+-ATPase를 basolateral side에서 luminal side로 재분포시킨다. 그 결과 이온 수송에 이상이 생기며 원위 세뇨관으로 Na+의 이동이 증가하며 tubuloglomerular feedback에 의하여 신혈관이 수축되어 세뇨관 손상이 발생한다. 세뇨관 손상의 결과 상피세포가 기저막에서 떨어져 나가며 세뇨관 폐쇄를 일으켜 세뇨관 내 압력이 증가하며 GFR이 감소를 일으킨다. 또한 조영제는 신혈관 수축, 혈액 내 점성을 증가시킨다[24].

CI-AKI의 위험인자는 CKD이며 GFR이 감소하거나 단백뇨가 증가할수록 CI-AKI의 위험도는 증가한다. 또한 유효 용적이 적거나 NSAIDS 사용, 조영제 양, 동맥 내 시술, 여러 번 조영제에 노출된 경우, 고삼투압 조영제를 사용한 경우가 CI-AKI의 위험인자로 알려져 있다[25].

CI-AKI의 치료는 예방에 중점을 두고 있다. 등장성 수액을 정맥 주사하는 것이 CI-AKI의 위험도를 의미 있게 낮추는 것이 보고되었기 때문에 위험인자가 있는 환자에서 시행 되어야 한다. 조영제 노출 전후로 1 mL/kg/h의 등장성 수액을 12시간 동안 주거나 조영제 노출 전 1시간 동안 3 mL/kg/h, 노출 후 4-6시간 동안 1.5 mL/kg/h를 투여할 수 있다[26]. 소규모 연구에서는 등장성 sodium bicarbonate solution이 이득이 있었으나 대규모 무작위 연구에서 고위험 환자에서 혈관 조영술을 하기 전 bicarbonate 혹은 N-acetylcysteine의 투여가 사망이나 신대체 요법, GFR 50% 감소 등의 예후에 효과를 보이지 못하였다[27]. 따라서 CI-AKI의 예방 요법은 등장성 수액 치료가 중점이 되어야 할 것이다.

결 론

AKI는 특히 중증 환자에서 흔하게 발생하며 환자의 단기 혹은 장기 예후에 직접적인 영향을 주는 중요한 질환이다. 그러나 AKI의 원인 및 심각도가 매우 다양하므로 임상 연구에서 단일한 치료가 효과를 나타내기 어려운 상황이다. 따라서 AKI 발생시 일관된 치료로 접근하기보다는 원인을 파악하고 그것을 교정하려는 노력이 필요하며 체액 상태를 지속적으로 관찰하여 적절한 수액 및 이뇨제를 투여해야 한다.
