Trans Abstract

The World Health Organization (WHO) Classification of Tumors of Haematopoietic and Lymphoid Tissues was recently published in a revised fourth edition. The categories of myeloproliferative neoplasms (MPNs) have not significantly changed since the 2008 fourth edition of the classification; however, newly discovered mutations including CALR and CSF3R and improved characterizations and standardizations of morphological features of some entities, particularly BCR-ABL1-negative MPNs, have impacted the diagnostic criteria of disease entities, increasing the reliability and reproducibility of diagnoses. The 2017 revised edition attempts to incorporate new clinical, prognostic, morphologic, and genetic data that have emerged since the last edition. This article reviews the major changes in the classification and their rationale for MPN classification within the revised 2017 WHO system.

서 론

골수증식 종양(myeloproliferative neoplasms, MPN)은 하나 이상의 골수세포(과립구계, 적혈구계, 거대핵세포)가 과도하게 증식하는 클론성 조혈모세포 혈액 질환으로, BCR-ABL1 양성 만성 골수백혈병(chronic myeloid leukemia), 만성 호중구백혈병(chronic neutrophilic leukemia), 진성적혈구증가증(polycythemia vera), 일차골수섬유증(primary myelofibrosis), 본태혈소판증가증(essential thrombocythemia), 미범주만성호산구백혈병(chronic eosinophilic leukemia, not otherwise specified), 미분류 골수증식 종양(MPN, unclassifiable)을 포함한 총 7개의 세부 질환이 포함되어 있다[1]. 초기에는 특별한 증상이 없고(insidious onset) 조혈기능 증가로 인하여 말초혈액 혈구와 골수세포 충실도의 증가를 보이지만, 단계적으로 진행하여 질환 말기에는 골수섬유화, 비효과조혈(ineffective hematopoiesis) 또는 급성모세포기(acute blast phase)로 진행하여 골수부전(marrow failure)에 이르게 된다[1].

골수증식 종양의 진단을 위해서 세계보건기구(World Health Organization, WHO)에서 발간하는 조혈모세포의 악성 질환과 림프구 질환에 관한 진단기준을 주로 이용하고 있으며, 2008년 WHO 4판 이후 9년 만인 2017년 새 개정판이 발표되었다[1]. 2017년 WHO 진단기준은 이전 4판의 분류 체계에서 크게 벗어나지 않으면서 2008년 이후 새롭게 밝혀진 임상 데이터 및 골수 검체의 형태학적 소견 그리고 분자진단 표지자 등이 기존 내용에 추가되었기 때문에 개정 4판(revised fourth edition) 버전으로 출간되었다. 새롭게 포함된 주요 변화 내용을 요약하면 다음과 같다. 만성 골수백혈병의 BCR-ABL1 융합유전자와 진성적혈구증가증의 JAK2 유전자, 일차골수섬유증과 본태혈소판증가증의 MPL 유전자 외에 최근 CALRCSF3R 유전자 등의 새로운 분자진단 표지자가 골수증식 종양의 진단뿐 아니라 질환의 병태생리를 이해하고 예후를 추정하는데 유용한 것으로 검증되어 진단기준에 새롭게 포함되었다[2,3]. 통상 ‘필라델피아 염색체 음성’인 전형적인 골수증식 종양(philadelphia chromosome-negative classical MPN)으로 불리는 세 질환(진성적혈구증가증, 본태혈소판증가증, 일차골수섬유증)은 최근 질환별로 골수 조직의 형태학적 소견 기준이 정립이 되었는데 이를 진단 시 활용하면 질환 감별뿐 아니라 반응성 골수세포 증가를 동반할 수 있는 양성 질환을 배제하는데 유용하여 2017년 WHO 개정 진단기준에서는 주 진단기준(major criteria)으로 포함되어 그 중요성이 강조되었다[4-8]. WHO 진단기준의 기본 원칙은 질환을 분류할 때 형태학적(morphologic) 소견뿐 아니라 혈액학적(hematologic), 세포유전학(cytogenetic), 분자유전학(molecular genetic) 소견 등 수집 가능한 모든 데이터를 통합(integrated)하여 진단에 이용하는 것이다. 골수증식 종양을 대상으로 임상적-병리학적(clinicopathological) 특성을 분석한 최근 연구 결과들에서도 이러한 통합적인 접근 방식이 진단의 정확성과 환자의 예후 예측 가능성을 높이는 것으로 검증되어 2008년 이후 새롭게 밝혀진 여러 정보가 이번 개정 진단기준에도 반영이 되어있다[6-12]. 마지막으로, 2008년 WHO 진단기준에서 골수증식 종양의 세부 질환으로 포함되어 있던 비만세포증(mastocytosis)은 2017년 새로운 진단기준부터는 고유의 임상 및 병리 소견을 가지는 독립적인 질환 범주로 분리되었다[1].

최근 건강보험심사평가원의 자료를 이용하여 골수증식 종양의 국내 역학조사를 시행한 연구 결과에 따르면 각 질환의 발생률은 본태성혈소판증가증, 진성적혈구증가증, 일차골수섬유증 순서로 인구 10만 명당 각각 평균 2.426명, 1.162명, 0.425명으로 보고되었다[13]. 국내 골수증식 종양의 유병률은 지난 10년간 약 3.8배 증가하였고[13,14] 유전자 변이를 표적으로 한 약제가 개발되는 등 골수증식 종양에 대한 관심 또한 높아지고 있으므로 이에 본 종설에서는 2017년 WHO 개정 4판의 진단기준의 주요 변경 사항에 대하여 질환별로 소개하고자 한다.

질환별 WHO 개정 4판 진단기준 소개

BCR-ABL1 양성 만성 골수백혈병(chronic myeloid leukemia)

만성 골수백혈병은 골수세포 중 특히 과립구계 세포의 증식을 특징으로 하는 조혈모세포 혈액 질환으로 특징적인 말초혈액 소견(여러 성숙 단계의 백혈구 증가, 호염기구와 호산구 증가, 혈소판은 정상이거나 혹은 증가)을 보이면서 염색체 또는 분자 진단 검사를 이용하여 t(9;22)(q34.1;q11.2) 또는 BCR-ABL1 융합유전자를 확인하는 경우 진단이 가능하다[1]. 치료하지 않으면 처음에는 증상이 없는 만성기(chronic phase)에 이어서 가속기(accelerated phase), 모세포기(blast phase)로 진행이 되지만, 표적항암제인 티로신키나아제억제제(tyrosine-kinase inhibitor)가 도입되면서 가속기 또는 모세포기로 진행하는 환자의 비율이 과거에 비하여 낮아졌다[1].

2017년 WHO 개정 4판에서는 만성 골수백혈병의 가속기 진단을 위하여 이전에 통용되어 오던 여러 임상적, 검사실적 소견들을 정리하여 표 1과 같이 보편화된 진단기준으로 정립하였다. 다음의 여덟 가지 혈액학적 또는 세포 유전학적 기준들, 곧 1) 치료에 반응하지 않는 백혈구 수 증가(> 10 × 109/L), 2) 치료에 반응하지 않는 비장비대(splenomegaly) 악화, 3) 치료에 반응하지 않는 지속적인 혈소판 수 증가(> 1,000 × 109/L), 4) 치료에 반응하지 않는 지속적인 혈소판 수 감소(< 100 × 109/L), 5) 호염기구가 말초혈액 백혈구의 20% 이상, 6) 모세포가 말초혈액 백혈구 또는 골수 유핵세포의 10-19%로 증가, 7) 진단 시 필라델피아 염색체 이외에 추가적인 클론성 염색체 이상(secondary Philadelphia chromosome, trisomy 8, isochromosome 17q, trisomy 19 또는 complex karyotype 또는 3q26.2 부위 이상)이 발견되는 경우, 8) 치료 중 어떠한 클론성 염색체 이상이라도 새롭게 발생하는 경우 중에서 하나 이상 해당되는 경우 가속기로 정의한다. 이 외에도 골수 검체를 관찰하였을때 형태학적 소견상 거대핵세포가 현저히 증식하여 군집(cluster)이나 판(sheet)을 이루고 레티큘린이나 콜라겐 섬유화가 동반되는 경우 만성 골수백혈병의 질환 가속기를 추정하는 소견(presumptive evidence)으로 판단할 수 있다(Table 1).

그러나 연구에 따르면 혈액학적 또는 세포 유전학적 소견만으로는 가속기 환자들의 다양한 질환 경과나 예후를 정확히 판단할 수 없기 때문에 만성 골수백혈병 치료에 대한 반응성(response-to-therapy)과 연관된 기준을 추가하는 것이 유용하다고 밝히고 있다[15-17]. 따라서, 2017년 WHO 개정 4판에서는 티로신키나아제억제제에 대한 반응성과 관련한 세 가지 잠정적인 기준들(provisional criteria)을 다음과 같이 새롭게 추가하였다. 먼저, 1) 티로신키나아제억제제 초치료에 완전 혈액학적 반응(complete hematologic response)에 도달하지 않은 경우, 2) 두 가지 티로신키나아제억제제의 순차(sequential) 치료에도 불구하고 어떠한 치료 반응(혈액학적, 세포 유전학적, 분자유전학적)에도 도달하지 않은 경우, 또는 3) 티로신키나아제억제제 치료 중 BCR-ABL1 융합유전자에 두 가지 이상 돌연변이가 발생한 경우 중 하나 이상 해당되는 경우에는 만성기(chronic phase) 환자라 할지라도 기능면에서는 장기적인 예후가 나쁘고 무진행 생존율(progression-free survival)이 짧은 가속기 환자일 가능성이 크다(Table 1). 모세포기(blast phase)의 진단은 말초혈액 또는 골수 검체에 20% 이상 모세포가 관찰되거나 또는 모세포의 골수외침범(extramedullary accumulation)이 관찰되는 경우에 진단할 수 있으며, 이는 기존의 2008년 WHO 진단기준과 동일하다.

만성 호중구백혈병(chronic neutrophilic leukemia)

만성 호중구백혈병은 말초혈액에 지속적인 호중구 증가와 골수세포 충실도 증가, 간비장종대를 특징으로 하는 드문 골수증식 종양으로, BCR-ABL1 융합 유전자나 반응성 호중구증가증(reactive neutrophilia)을 일으킬 수 있는 동반 질환이 배제되어야 한다. 표 2에 정리된 2017년 WHO 개정 진단기준에 따르면 만성 호중구백혈병은 1) 말초혈액 검사상 백혈구 수가 25 × 109/L 이상 증가하고, 분엽호중구(segmented neutrophil) 및 대상호중구(banded neutrophil)가 백혈구 수의 80% 이상, 미성숙 과립구(전골수구, 골수구, 후골수구)가 백혈구의 10% 미만, 골수모세포(myeloblast)가 거의 발견되지 않으며, 단구(monocyte)의 수는 < 1 × 109/L개 미만이고 과립구형성이상(dysgranulopoiesis)이 없어야 한다. 2) 골수생검 소견에서는 세포 충실도가 증가하고 호중구 비율 및 수의 증가를 보이지만 호중구의 성숙 단계는 정상이고, 골수 모세포가 전체 유핵세포의 5% 미만이어야 한다. 3) 앞서 기술된 1), 2)와 유사한 혈액학적, 형태학적 소견을 보일 수 있는 다른 골수증식 종양인 BCR-ABL1 양성 만성 골수백혈병, 진성적혈구증가증, 본태혈소판증가증, 또는 일차골수섬유증이 배제되어야 하며, 4) PDGFRA, PDGFRB, FGFR1PCM1-JAK2 유전자 재배열이 음성이어야 한다. 마지막으로 5) CSF3R 유전자의 T618I 돌연변이나 또 다른 CSF3R 유전자 변이가 검출되는 경우 또는 3개월 이상 지속되는 만성적인 호중구증가증이 있지만 반응성 증가의 원인이 없는 경우 진단할 수 있다.

새롭게 진단기준으로 포함된 분자진단 표지자인 CSF3R은 colony stimulating factor 3 수용체 단백을 만드는 유전자로 과립구의 성장과 분화에 중요한 역할을 담당한다[18,19]. 기존에 CSF3R 유전자 변이는 선천성 호중구감소증(severe congenital neutropenia) 환자에서 검출되는 것으로도 잘 알려져 있는데, 변이의 타입이 대부분 무의미(nonsense) 또는 틀이동(frameshift) 변이로 단백질의 미성숙절단(premature truncation)을 일으키는 기능소실 돌연변이인 경우가 대부분이다[20], 이에 비하여, 만성 호중구백혈병 환자에서 검출되는 CSF3R 유전자 변이는 엑손 14번 위치의 extracellular 도메인 위치에서 아미노산이 바뀌는 과오변이(missense) 형태의 기능획득 돌연변이로서 JAK-STAT pathway의 지속적인 활성화를 유도하여 과립구계 세포의 증식을 일으킨다. 특히 CSF3R 유전자의 618번째 코돈인 트레오닌(threonine)에서 이소류신(isoleucine)으로 바뀌게 되는 CSF3R T618I 돌연변이는 연구에 따라 다소 차이가 있으나 만성 호중구백혈병 환자의 40-83%에서 검출된다고 보고되었다[18,19]. CSF3R 유전자 변이와 함께 SETBP1 또는 ASXL1 유전자 변이가 동시에 검출이 되는 경우가 있는데[18,21-23], 특히 ASXL1 유전자 변이와 동반되는 경우 예후가 좋지 않은 것으로 알려져 있다[24].

진성적혈구증가증(polycythemia vera)

정상적인 적혈구 생성 조절 기전과는 무관하게 적혈구 생성이 증가되는 특징을 갖는 골수증식 종양으로 거의 모든 환자에서 JAK2 V617F 돌연변이(> 95%) 또는 이와 기능적으로 유사한 JAK2 엑손 12번 돌연변이(< 5%)가 발견되며, 적혈구계뿐만 아니라 과립구계, 거대핵세포계도 함께 증식하는 범혈구증가증(panmyelosis)이 특징적이다[1]. 2017년 WHO 개정 4판에 따른 진성적혈구증가증의 진단기준은 표 3에 제시된 바와 같다. 2008년 진단기준과 비교하여 주요 차이점으로는 1) 혈색소 진단기준이 낮아지고(남자의 경우 18.5 g/dL에서 16.5 g/dL, 여자의 경우 16.5 g/dL에서 16.0 g/dL로 변경), 적혈구용적률(hematocrit) 기준이 추가되었으며(남자의 경우 > 49%, 여자의 경우 > 48%), 2) 골수 조직의 형태학적 소견이 기존에 부 진단기준(minor criteria)에서 주 진단기준(major criteria)으로 상향 조정되었다. 또한, 3) 과거 부 진단기준에 포함되어 있던 내인성 적혈구 콜로니형성(erythroid colony formation in vitro)은 실제 의료현장에서 거의 활용되고 있지 않고 검사법이 표준화되지 않은 등의 이유로 새로운 진단기준에서는 삭제되었다[1,3].

실제 진성적혈구증가증 환자임에도 불구하고 빈혈을 동반하거나 적혈구증가 정도가 경미한 단계(initial, pre-polycythemic phase)의 환자에서 혈색소 수치가 WHO 진단기준에 미치지 못하여 과소 진단되는(underdiagnosed) 경우를 통상 “masked” 진성적혈구증가증이라고 한다[9,25-27]. 또한 심한 혈소판 증가 소견을 동반한 진성적혈구증가증 환자에서 JAK2 돌연변이 양성 본태성혈소판증가증으로 잘못 진단되는 경우도 종종 있어 질환을 정확하게 감별할 수 있는 최적의 혈색소 및 적혈구용적률의 진단기준을 마련하기 위한 연구가 진행되었다[27,28]. 그 결과, 혈색소 기준을 남자 > 16.5 g/dL, 여자 > 16.0 g/dL, 적혈구용적률 기준을 남자 > 49%, 여자 > 48%로 적용하였을 때 진성적혈구증가증과 본태혈소판증가증을 정확하게 진단하는 비율은 남자 95%, 여자 93%로 높은 빈도로 확인되어 WHO의 새로운 진단기준으로 포함되었다[3,27,28].

흔히 필라델피아 염색체 음성 골수증식 종양으로 불리는 진성적혈구증가증, 본태혈소판증가증, 일차골수섬유증 질환의 골수조직 소견에 대한 다양한 임상 연구들을 이뤄졌으며 이를 바탕으로 각 질환별 형태학적 소견이 표준화되었다[29,30]. 진성적혈구증가증은 골수조직의 세포 충실도가 증가되어 있고 적혈구생성(erythropoiesis)은 증가하여 큰 섬(islets) 또는 판(sheets)을 이루는 형태를 보인다.

과립구생성(granulopoiesis)은 증가되어 있지만 형태학적으로 정상이며, 거대핵세포의 생성은 증가되어 있는데 밀집된 모양보다는 느슨한 형태의 군집을 형성하는 경우가 흔하며, 다형태성(pleomorphism)이 있어 크기가 다양하다(difference in size) (Table 3). 골수조직의 형태학적 소견은 골수증식 종양 간의 감별진단뿐 아니라 반응성 적혈구 증가를 동반할 수 있는 양성 질환을 배제하는데 유용하다고 입증되어 WHO 개정 진단 기준에서는 주 진단기준으로 중요성이 강조되었다[3,4].

일차골수섬유증(primary myelofibrosis)

일차골수섬유증은 골수의 거대핵세포와 과립구계의 뚜렷한 증식과 함께 골수외조혈(extramedullary hematopoiesis)을 동반한 심한 골수섬유화를 특징으로 하는 골수증식 종양이다[1]. 환자의 약 30%는 진단 시 무증상 또는 건강검진에서 비장비대 소견이나 혈액 검사에서 빈혈, 백혈구 증가, 혈소판 증가가 우연히 발견되어 진단되는 경우가 있다. 주요 증상은 피로감, 호흡곤란, 체중감소, 야간발한, 미열, 악액질(cachexia) 등을 보이며, 골수 조혈에 의한 비장비대(90%)와 간비대(50%) 동반이 흔하다.

골수섬유화 정도는 질환이 진행되면서 초기 섬유증전단계(prefibrotic/early stage)에서 섬유화 단계(fibrotic stage)로 점차 심해지는데, 진단 당시 환자들은 이미 섬유화 단계인 경우가 대부분이다. 2017년 WHO 개정 진단기준에 따른 일차골수섬유증의 진단기준은 표 4에 제시된 바와 같다. 섬유화 단계의 골수 조직은 grade 2-3 정도의 심한 레티쿨린 또는 콜라겐이 침착된 섬유화 소견이 관찰되고 비정상적인 형태의 비정형 거대핵세포가 큰 집단(clusters or sheets)을 이루는 특징을 보이며, 골경화(osteosclerosis)도 동반될 수 있다[1]. 가장 흔히 골수외조혈이 일어나는 장기는 비장으로 주로 비장 적수가 팽창되고 신생혈관생성이 증가하며, 간에서도 골수외조혈을 볼 수 있다. 특징적인 혈액학적 소견으로는 말초혈액에 눈물방울모양의 적혈구를 동반한 대소부동변형적혈구증가증(anisopoikilocytosis)과 함께 미성숙골수전구세포 및 백적모세포증(leukoerythroblastosis)이 관찰되고 혈청 lactate dehydrogenase가 증가한다[1].

이에 비하여, 초기 섬유증 전단계에서는 혈소판 증가 이외에 다른 특별한 이상 소견이 동반되지 않는 경우가 많다. 특히 본태혈소판증가증에서 관찰될 수 있는 혈소판 수의 증가 정도와 유사하기 때문에 혈액학적 소견만으로는 두 질환을 감별하기가 어렵다[1]. 기존 연구에 의하면, 본태혈소판증가증으로 진단받은 438명의 환자들을 WHO 기준으로 재분류하였을 때 162명의 실제 본태혈소판증가증 이외에 나머지는 일차골수섬유증의 전섬유화 단계이거나(184명) 초기 섬유화(137명) 단계였다는 보고도 있다[31]. 하지만, 일차골수섬유증은 본태혈소판증가증에 비하여 섬유화 단계로 진행할 가능성이 높고 생존율이 낮으며 진단에 따른 치료 전략이 달라 감별진단이 중요하기 때문에[32,33], 골수 검사를 시행하여 형태학적 특징 비교를 통한 감별진단을 하는 것이 매우 중요하다(Table 5) [1,29]. 일차골수섬유증 초기 섬유증 전단계는 본태혈소판증가증에 비하여 세포 충실도가 높고, 주로 증식된 세포는 호중구와 비정형 거대핵세포이다. 비정형 거대핵세포는 초기 섬유증 전단계에서도 뚜렷하게 관찰되어 본태혈소판증가증과 중요한 감별기준이 되며, 심한 비정상 형태를 취하며 골수 혈관동과 뼈잔기둥 주위에 집단적으로 분포하고 grade 0-1정도의 섬유화를 나타낸다(Table 4). 이에 비하여, 본태혈소판증가증은 특징적인 과분엽화된 핵과 풍부한 세포질을 가진 모양의 거대핵세포가 증가하고 호중구 및 적혈구 생성 증가나 좌방이동(left shift)이 없다.

JAK2 V617F 변이는 일차골수섬유증 환자의 50-60%에서 나타나고, 기능적으로 유사한 CALR 유전자 변이가 30%, MPL 유전자 변이가 8%에서 검출된다[2,34]. JAK2, CALR, MPL 세 유전자 변이가 모두 발견되지 않는 경우(triple-negative 또는 JAK2/CALR/MPL-wild-type)는 전체 환자의 약 12%에서 확인되는데 그렇지 않은 환자에 비하여 예후가 좋지 않은 것으로 알려져 있다[2,35].

본태혈소판증가증(essential thrombocythemia)

일차적으로 거대핵세포계가 증식하는 골수증식 종양으로 말초혈액에서 지속적인 450 × 109/L 이상의 혈소판증가증과 함께 골수에서 크고 성숙한 거대핵세포의 증가 소견이 관찰된다. 본태혈소판증가증 환자들은 대부분 긴 무증상기를 보이나 일부에서는 혈전증이나 출혈 경향을 나타낼 수 있다[1].

말초혈액에 증가된 혈소판은 크기가 다양하고, 드물게 위족(pseudopod) 또는 무과립혈소판과 같은 이상한 형태로 관찰될 수 있다. 골수조직 검사에서 세포 충실도는 정상 또는 중등도로 증가되어 있고, 특징적인 형태학적 소견으로 크기가 크고 세포질의 양이 풍부하며 핵은 깊게 패이고 과분엽화되어 마치 사슴뿔 모양처럼 보이는 성숙한 거대핵세포가 증식된 소견이 관찰된다[1].

분자유전학적 이상으로 JAK2 V617F 돌연변이가 환자의 50-60%에서 검출되며, 기능적으로 유사한 CALR 유전자 변이가 30%, MPL 유전자 돌연변이가 3%에서 검출된다[2,34]. 본태혈소판증가증의 2017년 WHO 개정 진단기준은 표 6에 제시된 바와 같다.

골수증식 종양의 2017년 WHO 개정 진단기준은 가장 잘 알려진 분자유전학적 이상인 BCR-ABL1 융합유전자, JAK2 V617F 돌연변이 이외에도 MPL 유전자, CALR 유전자, CSF3R 유전자 돌연변이 등의 새로운 분자진단 지표를 포함함으로써 질환의 병태생리를 이해하고 진단의 정확성을 높이는데 기여하였다. 이와 더불어 질환별로 골수조직의 형태학적 소견을 정립하고 표준화함으로써 임상적 또는 혈액학적 소견이 서로 유사한 질환들을 감별하고 판독자 사이에 진단 일치율도 높일 수 있게 되었다. 골수 생검은 침습적인 검사이기 때문에 과거에는 주로 임상양상을 통한 감별진단과 혈액학적 소견 등의 비특이적인 방법으로 골수증식 종양을 진단하는 경우가 많았다. 그러나 2017년 WHO 개정 진단기준에 따라 임상적, 혈액학적, 유전학적 소견 등 다양한 정보를 통합하여 진단에 이용하고 골수 검사를 시행하여 정밀한 형태학적 평가를 시행하는 것이 골수증식 종양의 정확한 진단뿐 아니라 질환 예후의 추정에 있어서도 매우 중요할 것으로 생각된다.

Defining criteria for the accelerated phase (AP) of chronic myeloid leukemia (CML)

CML-AP is defined by the presence of ≥ 1 of the following hematological/cytogenetic criteria or provisional criteria concerning response to tyrosine kinase inhibitor (TKI) therapy
- Hematological/cytogenetic criteriaa
- Persistent or increasing high white blood cell count (> 10 × 109/L), unresponsive to therapy
