Abstract

목적

노화에 따라 약물의 분포와 약동학적 반응에 변화가 생기며 사용하는 약제의 수도 늘어나게 되면서 노인에서 약제에 관련한 여러 문제점이 야기될 위험성이 높다. 타 연령대에 비하여 노인에서의 약물유해반응 특성을 확인하고자 하였다.

방법

2010년 2월부터 2013년 9월까지 서울대학교병원 지역약물감시센터로 보고된 모든 약물유해반응 사례들을 대상으로 원인 약물의 성분명 및 ATC 상위 코드에 따른 보고 빈도, WHO‐ART 기관계별 분류에 따른 유해사례 분포, 중증도 등을 분석하였다.

결과

18세 이상 환자에서 총 15,541개의 사례가 보고되었다. ATC 코드에 따라 원인 약물을 분석해본 결과 신경계 약물, 항종양제 및 면역조절제, 전신 항감염제가 4,696건(30.2%), 3,490건(22.5%), 3,185건(20.5%)으로 가장 많이 보고되었다. 60세 이상 군에서는 호흡기계 약물 363건(6.1%), 심혈관계 약물 183건(3.1%)으로 다른 연령군에 비해 비교적 높은 비중을 차지하였다. 유해사례를 WHO‐ART 기관계별 분류 코드에 따라 살펴볼 때 소화기 질환이 3,961건(25.5%), 피부 및 부속기 질환이 3,427건(24.1%)으로 가장 빈번하게 보고되었다. 60세 이상 군에서는 다른 연령군에 비해 정신 질환, 심혈관계 질환, 혈액 질환, 요로계 질환의 비중이 높은 경향을 보였다. 보고된 약물들의 중증도를 분류해보았을 때 경증은 10,025건(64.5%), 중등증은 4,703건(30.3%), 중증은 775건(5.0%)을 나타냈다. 60세 이상 군에서는 중증으로 분류된 사례가 358건(6.1%)으로 다른 연령군에 비해 상대적으로 높은 비율을 나타냈다. 여성에 비해 남성에서 상대적으로 중증도의 비중이 높았다.

결론

노인에서는 ATC 분류 코드에 따른 약물군 중 호흡기계 약물과 심혈관계 약물에 의한 약물유해반응이 다른 연령대에 비해 비교적 빈번하였다. 약물유해반응에 의한 증상은 정신 질환, 심혈관계 질환, 혈액 질환, 요로계 질환의 비중이 다른 연령대에 비해 높은 비중을 차지하였다. 약물유해반응에 더욱 취약한 노인에 있어서 약물유해반응의 특성을 이해하고 예방 및 대처에 대한 더 많은 노력이 필요하다.

Background/Aims

Several factors contribute to the greater propensity for adverse drug reactions (ADRs) in the elderly, including the use of multiple drugs and pharmacokinetic and pharmacodynamic alterations due to aging. We evaluated the characteristics of ADRs in elderly versus younger adults.

Methods

ADRs were collected from a spontaneous reporting system at Seoul National University Hospital from February 2010 to September 2013. We analyzed causative drugs, clinical manifestations, and the severity of ADRs.

Results

In total, 15,541 ADRs were reported in patients 18 years of age or older. Common causative drug categories included nervous system, anti-neoplastics, and anti-infectives. The prevalence of ADRs due to respiratory drugs and cardiovascular drugs was higher in the elderly group (≥ 60 years) than in other groups. The most common clinical types were gastrointestinal and skin and appendage issues. The elderly group had a tendency to show a higher proportion of psychiatric, cardiovascular, hematological, and genitourinary symptoms. The proportions of severe ADRs were higher in the elderly groups and in male patients.

Conclusions

Elderly patients were susceptible to ADRs related to respiratory and cardiovascular drugs. Psychiatric, cardiovascular, hematological, and genitourinary disorders account for a higher proportion of ADR symptoms in the elderly than in other age groups. Further efforts to understand, manage, and prevent ADRs in the elderly are required.

서 론

질병의 진단과 치료에 있어 약물 요법은 핵심적인 역할을 하며, 약물의 사용은 지속적으로 증가하는 추세이다. 이에 따라 이러한 의약품의 사용에 의해 발생할 수 있는 약물유해반응(adverse drug reaction, ADR)의 중요성에 대한 관심이 높아지고 있다. 약물유해반응은 의약품 등을 정상적으로 투여 및 사용하였을 때에 발생한 해롭고 의도하지 아니한 반응을 말한다[1]. 약물유해반응은 환자를 사망에 이르게 하거나 입원기간을 연장시키고[2], 입원비를 증가시키기도 한다[3]. 모든 입원환자의 10-20%에서 약물유해반응이 발생한다는 보고가 있으며[4], 입원 치료를 받는 환자들의 원인의 3-6% 정도를 약물유해반응이 차지하고 있다는 연구결과가 보고된 바 있으며, 특히 노인에서는 그 빈도가 3-24%로 증가한다[5]. 병용 약제의 수, 신기능 저하, 불량한 영양상태 등이 약물유해반응 발생의 위험 인자로 알려져 있다[6,7].

약물유해반응 감시를 위해 국내에서는 1985년 ‘의약품 등 안전성 정보처리 규정’을 제정하였으며 1988년부터는 자발적 신고를 위한 약물부작용모니터링기관을 지정하였다. 2000년부터는 전국 15개 대학병원과 종합병원을 대상으로 원내 부작용 모니터링 체계를 구축한 후 유해사례 신고 건수가 증가하기 시작하였다[8]. 약물유해반응 보고체계로 데이터베이스가 구축되고 이를 통해 특정 약물에 의한 유해반응의 실마리정보(signal)를 찾고, 이를 데이터마이닝 기법을 이용해서 환자에게 보다 안전한 의약품 사용의 기초가 되도록 노력하고 있다. 또한 약물유해반응과 관련하여 그 평가지표를 재평가하거나 단일 기관에서의 약물유해반응 자료 분석, 의약품 부작용 심각도 인식조사 등 다양한 접근 방법으로 약물유해반응이 연구되고 있다[9].

약물 치료는 노인들의 만성 질환 치료, 통증 감소, 삶의 질 개선에 있어 핵심적인 수단이다[10]. 노화에 따른 약물의 분포와 약동학적 반응의 차이가 임상적으로도 유의한 차이를 나타내고 있으며, 노인 인구에서 사용하는 약제의 수가 늘어나게 되면서 약제에 관련한 여러 문제점이 야기될 위험성이 높다[11]. 약물로 인해서 생길 수 있는 반응은 매우 비특이적이며, 어떠한 증상과 징후도 모두 발현될 수 있어 간과되기 쉬우며, 특히 노인의 경우 노화에 따른 변화 및 기능 저하, 기저 질환과 명확하게 구분이 되지 않아 인구에서의 약물유해반응은 다른 연령군에 비해 저평가되는 경향이 있다. 향후 평균 수명의 증가와 더불어 노인 인구의 비율이 커지고 그에 따른 약제의 사용도 더욱 증가할 추세이기 때문에 노인에서의 약물유해반응에 연관된 자료들을 체계적으로 분석하는 것이 중요하다. 그러나, 지금까지는 노인들의 약제 사용 및 그에 따른 유해반응의 현황 및 특징에 대한 연구가 활발하게 이루어지지 않았다.

따라서 본 연구에서는 약물유해반응 자발보고 데이터베이스를 이용하여 노인에서의 약물유해반응 양상을 확인하고자 하였다. 타 연령대에 대비하여 노인 연령대에서 주요 원인약제, 발현양상, 중증도에 차이가 있는지 비교 분석하고자 하였다.

대상 및 방법

분석 대상 자료

본 연구는 2010년 2월부터 2013년 9월까지 서울대학교병원 약물유해반응감시센터로 보고된 모든 약물유해반응 사례들을 대상으로 하였다. 18세 미만 환자는 분석에서 제외하였으며 60세 이상의 환자군, 40세 이상 60세 미만의 환자군, 18세 이상 40세 미만의 환자군으로 구분하였다. 노인을 정의하는데 있어 생물학적인 나이 이외에 생물학적, 사회학적, 심리학적, 기능적 측면 등이 다양하게 고려될 수 있으나 본 연구에서는 분석 가능한 자료를 기반으로 생물학적인 나이에 따라 구분하였다. 노인 연령 구분도 나라마다 다양하나 본 연구에서는 Brody가 제시하였던 구분에 따라 60세 이상을 노인으로 정의하였다[12].

분석 방법

원내 약물유해반응감시센터에 신고받은 약물유해반응 사례들에는 작성일자, 보고자 정보, 환자 정보, 투여의약품 정보(의심의약품 및 병용의약품), 약물유해사례 정보, 약물유해반응 경과 및 조치에 대한 정보가 포함되어 있다.

보고된 자료는 의약품명의 경우 영문 성분명, anatomical therapeutic chemical (ATC) 분류체계 코드로, 유해사례의 경우 World Health Organization Adverse Reactions Terminology (WHO-ART) 092 버전으로 입력되었다[13,14]. 또한 보고된 사례들은 WHO The Uppsala Monitoring Centre 인과관계 평가기준에 따라 분류되었다[15].

한 건의 약물유해반응 보고서에 2가지 이상의 약물성분이 포함된 경우, 각각의 약물성분에 대해서 독립적으로 분석하였다. 또한 한 건의 약물유해반응 보고서에 두 가지 이상의 기관계별 분류(system-organ classes, SOC) 코드를 유해사례로 보고하였다면(예: urticaria – SOC code 100, dyspnea – SOC code 1100), 각각을 하나씩 처리하여 ADR 보고 빈도를 분석하였다. 예를 들어 병용약물 3개, 유해사례 SOC code 2개라면, 자료는 병용약물 수 (3) × 유해사례 수 (2)로 총 6개의 사례로 취급하였다.

각 연령군별로 유해사례별 보고분율비(proportional reporting ratio)도 산출하였다[16]. 보고분율비는 약물부작용 실마리 정보를 분석하는데 주로 사용되는 기법으로, 본 연구에서는 특정 연령군에서의 보고건 중 특정 유해사례가 차지하는 분율을 다른 모든 연령군에서의 보고건 중 그 특정 유해사례가 차지하는 분율로 나눈 값으로 산출하였다.

보고된 약물유해반응의 중증도는 경증, 중등증, 중증의 3가지로 분류하였다. 이는 Hartwig 등이 제안한 분류법[17]과 식약청의 관련 규정에 따라 보편적으로 사용되는 분류이다. 경증의 정의는 다른 치료제나 해독제(antidote) 등이 필요하지 않았고 입원기간 연장이 발생하지 않은 경우, 중등증은 특정 치료제나 해독제가 필요했거나 입원기간이 1일 이상 연장된 경우를 말하며, 중증의 경우는 영구적인 손상으로 인해 생명을 위협할 가능성이 있거나 집중 치료(intensive medical care)가 필요한 경우를 말한다.

대상 사례들에 대해 보고자 유형, 증상 발생 당시 환자의 나이 및 성별 분포를 조사하였다. 의심약물 성분명 및 ATC 상위 코드에 따른 보고 빈도와 투약 경로가 함께 조사되었다. 보고 빈도가 가장 높았던 ATC 상위 코드 약물에 대하여, 하위 코드에 따른 빈도를 추가로 조사하였다.

결 과

총 자발 보고 건수 및 보고 유형

자료 수집 기간 동안 18세 이상의 환자만을 대상으로 하였을 때 총 13,889건의 유해사례가 보고되었고, 의심약물과 유해사례 유형에 따라 총 15,541개의 조합이 생성되었다. 전체적으로 남성이 6,296건, 여성이 9,245건으로 1:1.47의 비율을 보였다. 60세 이상 군은 5,906건으로 전체의 38%를 차지하였으며, 40-59세 군은 5,668건(36.5%), 18-39세 군은 3,967건(25.5%)이었다(Table 1).

ATC 코드로 분류한 계열별 약물유해반응 의심약물

신고된 ADR 원인 약제를 ATC 코드별로 분류하였다(Table 2). 대분류로 보았을 때 가장 빈번한 빈도를 보인 계열은 신경계(nervous system) 약물로 전체 사례 중 총 4,696건(30.2%)이었으며 세 연령군 모두에서 가장 많았다. 다음으로 항종양제 및 면역조절제가 3,490건(22.5%)으로 많았다. 세 번째로는 전신 항감염제 계열 약물이 3,185건으로 20.5%를 차지하였다. 그 외에 비뇨기계 및 성호르몬 관련 계열 약물이 731건(4.7%), 소화관 및 대사 계열 약물이 692건(4.5%), 호흡기계 약물이 645건(4.2%)으로 보고되었다.

원인 약물의 계열은 대체로 연령군별로 뚜렷한 차이를 보이지 않았으나, 연령이 높은 군일수록 통계적으로 유의하게 호흡기계 약물(ATC code R), 심혈관계 약물(ATC code C)이 차지하는 비중이 높았다(Fig. 1).

성분명으로 분류한 약물유해반응 원인 약물

성분명을 기준으로 원인 약물로 보고된 약물들을 살펴보았다(Table 3). 전체 사례 중에서 가장 빈번하게 보고된 원인약물은 fentanyl로 1,788건(11.5%)이 보고되었다. 이 밖에 morphine 985건(6.3%), oxaliplatin 799건(5.1%), ritodrine 593건(3.8%), irinotecan 496건(3.0%) 등이 뒤를 이었다.

60세 이상 군에서는 18-39세 군에서 원인 약물의 많은 빈도를 차지한 ritodrine, magnesium sulfate, magnesium oxide 등의 약물은 많이 보고되지 않았으며, 주로 항종양제와 전신항감염제가 주된 원인 약물을 차지하였다.

WHO-ART 기관계별 분류 코드에 따른 유해사례 분포

유해사례를 WHO-ART 기관계별 분류 코드에 따라 그 분포를 조사하였다 (Table 4). 전체적으로 소화기 증상이 3,961건(25.5%), 피부 및 부속기 증상이 3,427건(24.1%)으로 가장 빈번히 보고되었다. 중추신경계 및 말초신경계 증상은 2,327건(15.0%), 전신 증상은 1,816건(11.7%)으로 비교적 높은 비중을 차지하였다.

60세 이상 군에서는 다른 연령 군에 정신 증상, 심혈관계 증상, 혈액 증상, 요로계 증상의 비중이 통계적으로 유의하게 높은 경향을 보였고, 이는 보고분율비 값을 통해서도 확인할 수 있었다(Table 5).

중증도에 따른 유해사례 분포

보고된 약물들의 중증도를 분류해보았을 때 경증은 10,025건(64.5%), 중등증은 4,703건(30.3%), 중증은 775건(5.0%)이었다(Tables 6-9). 60세 이상 군에서는 중증으로 분류된 사례가 358건(6.1%)으로 다른 연령군에 비해 상대적으로 비율이 높았으며, 높은 연령군일수록 중증도의 비중이 커지는 경향은 통계적으로도 유의하였다(p< 0.001). 전체 연령군에서 성별에 따라 분석하였을 경우 여성에 비해 남성에서 상대적으로 중증도의 비중이 높았으며 통계적 유의성이 확인되었다(p< 0.001). 세 연령군에 대해서 각각 분석하였을 때도 세 연령군 모두에서 남성에서 중증도의 비중이 높았고 통계적으로도 유의한 결과를 보였다(결과 제시하지 않음). 중증으로 분류되었던 775건의 사례들에 국한하여 분석하였을때 전신적 증상이 265건(34.2%)으로 가장 많이 나타난 유해사례였으며 피부 및 부속기 질환이 154건(19.9%), 간 및 담도계 질환이 76건(9.8%), 일반적 심혈관계 질환 62건(8.0%), 호흡기계 질환 51건(6.6%) 등으로 나타났다. 중증으로 분류된 사례들의 약제의 ATC 코드를 살펴보았을 때 전신 항감염제가 221건(28.5%)으로 가장 많은 빈도를 보였으며 항종양제 및 면역조절제와 신경계 약물도 각각 185건(23.9%)과 148건(19.1%)으로 비교적 높은 빈도를 나타냈다.

고 찰

본 연구에서는 단일기관 원내 약물유해반응감시센터를 통해 약물유해반응 사례를 분석하고 연령대별 약물유해반응의 특징을 살펴보았다. 15,541개의 유해사례 조합을 분석하였고, 그중 5,906건(38%)이 60세 이상 환자에서 발생한 사례였다. 분석을 통해 60세 이상 환자군에서 다른 연령군에 비해 호흡기계 약물과 순환기계 약물에 의한 유해반응이 차지하는 비율이 높았으며 유해사례 증상은 정신 질환, 심혈관계 질환, 혈액 질환, 요로계 질환의 비중이 상대적으로 높음을 확인하였다. 유해사례의 최대강도가 중증으로 분류된 사례 역시 60세 이상 환자군에서 다른 연령군에 비해 비교적 높은 비율을 나타냈다.

본 연구에서 60세 이상 환자들에서 약물유해반응의 원인으로 의심되는 약물은 신경계약물, 항종양제 및 면역조절제, 전신 항감염제가 각각 30.2%, 22.5%, 20.5%로 대부분을 차지하였으며, 다른 연령군에서와 비슷한 분포 양상을 보였다. 이러한 결과들은 항생제, 항혈전제, 항고혈압제, 혈당강하제, 진통소염제, 항암제가 입원 노인 환자의 전체 약물유해반응 중 70%를 차지하였다는 기존의 연구 결과들과 다소 차이를 보인다[18]. 이는 성분명으로 분류된 원인 약물의 결과에서 볼 때 신경계 약물로 분류된 fentanyl과 morphine이 각각 전체의 11.5%와 6.3%로 적지 않은 비중을 차지하면서 발생한 결과로 파악된다. 호흡기계 약물과 순환기계 약물의 경우 6.1%와 3.1%로 전체 유해사례에서 차지하는 절대적인 비율이 높지는 않았지만, 다른 연령군의 환자들에 비해 상대적으로 높은 빈도로 관찰되었고, 높은 연령군일 경우 이들 유해사례가 차지하는 비율이 점차 높아지는 경향을 보였다. 따라서 노인 환자에서 호흡기계 및 순환기계 약물을 사용할 경우 유해반응 발생에 대해 보다 주의 깊은 관찰을 할 필요가 있겠다. 국내 한 대학병원에서 입원환자를 대상으로 노인 환자군과 비노인 환자군의 약물유해반응 특성을 분석한 연구가 있었으며, 순환기계 약물에 의한 약물유해반응의 빈도가 높음이 확인된 바 있다[19]. 최근에 캐나다에서 65세 이상 노인 입원환자를 대상으로 시행된 대규모 후향적 코호트 연구에서도 심혈관계 약물이 가장 흔한 약물군으로 분류된 바 있으며[20], 이탈리아에서 노인 입원환자를 대상으로 시행한 대규모 연구에서도 비스테로이드 항염증제 이외에 순환기계 약물들이 약물유해반응의 주요 원인이었다[5].

약물유해반응의 가장 흔한 임상 양상은 소화기계 질환, 피부 및 부속기 질환이 가장 흔한 형태로 발현되었으며, 중추신경계 및 말초신경계 질환, 전신 증상 역시 비교적 높은 비중을 차지하였다. 60세 이상 군에서는 다른 연령군에 비해 정신 질환, 심혈관계 질환, 혈액 질환, 요로계 질환의 비중이 높은 경향을 보였다. 스위스에서 이루어진 한 연구에서도 70세 이상의 노인에서 피부이상 반응보다는 신경정신학적, 혈액학적 이상반응이 상대적으로 더 높음을 보고한 바 있다[21].

일반적으로 젊은 사람들보다는 노인이, 남성보다는 여성이 약물유해반응에 취약한 것으로 알려져 있다[22]. 본 연구에서는 60세 이상 군에서는 중증으로 분류된 사례가 358건(6.1%)으로 다른 연령군에 비해 비교적 높은 비율을 나타냈으며, 여성에 비해 오히려 남성에서 상대적으로 중증도의 비중이 높았다. 연령이 약물유해반응을 결정하는 독립적 인자는 아니지만, 기저 질환, 병용약물 수 등이 노인에서의 중증도를 높이는데 기여할 것으로 추정된다. 성별에 따른 약물유해반응의 차이에 대해 살펴보면 나이, 복용약의 개수와 상관없이 여성에게 약물유해반응이 더 많이 발생한다고 한다는 보고가 있다[23]. 반면 2002년 프랑스에서 시행된 연구 결과에 따르면, 노인에서의 심각한 약물유해반응이 남성에서 더 흔하게 보고되었고 이는 본 연구의 결과와도 일치하는 바이다[24]. 본 연구에서는 중증에 해당하는 사례에서의 WHO-ART 증상 분포를 살펴보았을 때 높은 비중을 차지하였던 피부 및 부속기 질환과 간 및 담도계 질환이 여성에 비해 남성에서 상대적으로 더 많이 나타난 것도 이 결과와 연관된 것으로 추정된다.

본 연구는 단일기관 자료를 분석하여 노인에서의 약물유해반응의 원인 약물로 빈번하게 보고되는 약물을 분석해 보았고, 주요 발현양상, 중증도 등에 대해 살펴보았다. 기저 질환, 병용 약제, 낮은 인지 기능 등으로 인해 약물유해반응에 더욱 취약한 노인 환자에 있어서 약물유해반응 예방 및 대처에 대한 더 많은 관심과 주의가 필요하며, 이에 대한 지속적인 연구가 수반되어야 할 것이다.
