Trans Abstract

The greatest achievement in the treatment of obesity and diabetes would be the development of bariatric/metabolic surgery. At the beginning, bariatric surgeries were developed to simply reduce body weight in morbidly obese subjects. Before long, it was discovered that diabetes and other metabolic complications of obesity could be placed in remission. The remission rate of diabetes after bariatric surgery is strikingly high and, in the case of Roux-en-Y gastric bypass surgery, diabetes remission commonly occurs immediately after the surgery, when significant weight loss does not take place. Therefore, the concept of bariatric surgery has evolved into metabolic surgery. Physiologic changes in gastrointestinal endocrine system following the anatomical changes made by bariatric/metabolic surgery are regarded as the major mechanisms of weight loss and diabetes remission. In this regard, the foregut and hindgut hypotheses were suggested as the mechanisms associated with diabetes remission. With the advent of sleeve gastrectomy, which does not bypass the foregut (duodenum and proximal jejunum) but increases the secretion of glucagon-like peptide-1, the foregut hypothesis is currently under attack. However, a single mechanism is not enough to explain the metabolic effect of bariatric/metabolic surgery. Further studies are warranted to elucidate the mechanisms of metabolic improvements after bariatric/metabolic surgery. (Korean J Med 2013;84:629-639)

서 론

비만의 치료는 매우 어렵고 비만한 당뇨병의 치료 또한 매우 어렵다. 식이요법과 운동요법으로 조절이 되는 경우가 있기는 하지만 이러한 방법으로 감량된 체중을 유지하는 것은 매우 어려움이 잘 알려져 있으며, 약물 요법의 경우 생활습관개선 치료에 추가하였을 때 위약 대비 고작 3-5% 더 체중이 감소할 뿐이다[1]. 미국의 경우 비만 환자의 10%가 당뇨병을 가진 반면 당뇨병 환자의 80%가 비만하다[2]. 2012년 대한당뇨병학회에서 발간한 자료에 의하면 국내의 경우는 체질량지수 23.0-24.9 kg/m2를 과체중, 25 kg/m2 이상을 비만으로 정의하였을 때, 제2형 당뇨병 환자의 약 50%가 비만이며 약 25%가 과체중에 해당한다. 비만한 경우 당뇨병이 잘 발생하는 기전은 혈중 유리지방산의 증가, 지방세포에서의 비정상적인 아디포카인(adipokine) 분비, 이소성 지방 축적(ectopic fat accumulation), 상기 열거한 요인과 관련된 인슐린 저항성 증가, 그리고 췌장 베타세포의 지방독성(lipotoxicity)에 따른 기능 장애 등으로 설명할 수 있다[3,4]. 그러면 체중을 줄이면 당뇨병이 생기지 않을 것인가? 미국에서 시행된 당뇨병 예방연구(Diabetes Prevention Program)에서는 내당능장애를 보이는 체질량지수 약 34 kg/m2인 사람을 대상으로 평균 2.8년(1.8-4.6년)간 생활습관교정요법을 시행하였을 때, 첫 1년간 약 6.5 kg의 체중 감소가 있었으나 시간이 지나면서 체중은 다시 증가하여 4년 추적한 환자의 경우 시작 시점 대비 약 4 kg의 체중 감소가 있었지만, 58%에서 당뇨병으로의 진행을 억제하는 효과를 나타내었다[5]. 또한 과체중인 제2형 당뇨병 환자를 대상으로 4년간 철저한 생활습관교정을 시행한 Look AHEAD 연구에서는 평균 7.9%의 체질량지수 감소를 보였고 1년째 11.5%에서 정상 내당능 혹은 전당뇨병(prediabetes)로 회귀함을 보였으며 4년째에는 7.3%에서 정상 내당능 혹은 전당뇨병 소견을 보였다[6]. 이처럼 내과적 영역에서 다양한 노력이 있었지만 아주 만족스러운 결과를 얻지 못하였으나, 그 동안 외과적 영역에서는 실로 믿기 어려울 정도로 괄목할 결과를 보여 주었다. 1995년에 미국의 외과의사 Pories는 “누가 생각이라도 해 봤겠는가? 수술이 제2형 당뇨병의 가장 효과적인 치료가 될 것이라는 것을(Who would have thought it? An operation proves to be the most effective therapy for adult-onset diabetes mellitus)”이라는 다소 선정적인 제목의 논문을 외과학연보에 게재하였다[7]. 당시 루와이 위우회술(Roux-en-Y gastric bypass)을 시행받은 고도비만 환자 중에서 제2형 당뇨병 환자 146명 중 121명(82.9%) 그리고 152명의 내당능장애 환자 중 150 (98.7%)명이 정상 혈당과 정상 당화혈색소를 유지하는 놀라운 결과를 보였다[7]. 비만한 제2형 당뇨병 환자를 대상으로 통상적인 내과적 치료와 대표적인 비만 수술인 루와이 위우회술 혹은 담췌우회술(biliopancreatic diversion)을 시행하였을 때 2년 후 내과적 치료군에서는 아무도 당뇨병의 관해(공복혈장포도당 < 100 mg/dL이면서 동시에 HbA1c < 6.5%)에 이르지 못하였으나 루와이 위우회술의 경우 75%에서 담췌우회술의 경우 95%에서 당뇨병 관해에 도달함이 보고된 바 있다. 바야흐로 체중조절을 주목적으로 한 비만수술(bariatric surgery)의 개념이 대사수술(metabolic surgery)이라는 개념까지 진화하였다.

비만/대사 수술의 효과는 단지 체중과 혈당 조절에서 끝나지 않는다. 스웨덴에서 비만 환자를 대상으로 이루어진 SOS (Swedish Obesity Study)에서 비만수술(위밴드술 [gastric banding], 수직밴드위성형술[vertical banded gastroplasty] 및 루와이 위우회술)을 시행한 결과 수술 후 2년째에 현저한 체중 감소와 함께 많은 사람들이 당뇨병, 고혈압, 이상지혈증, 고콜레스테롤혈증, 고요산혈증 등으로부터 회복되었으며 10년 후까지 상당수에서 회복된 상태를 유지함을 관찰할 수 있었다[8]. 이와 더불어 15년 추적관찰 연구에서 당뇨병 발병이 83%에서 예방되었다[9]. 생리학적 관점에서 볼 때, 비만/대사수술 후에 제2형 당뇨병 환자의 인슐린 감수성이 크게 증가하며 베타세포의 기능 역시 의미 있는 증가를 보임이 일반적으로 나타난다(Fig. 1). 또한 비만대사수술의 결과 앞서 언급한 바와 같이 심혈관 위험인자가 줄어들고[8], 이에 수반하여 심혈관 사건 발생과 심혈관 사망이 줄어들고[10], 암 발생이 줄어들고[11], 연령, 성별, 위험인자 보정 사망률의 감소는 29%에 달했으며[12], 삶의 질 또한 크게 개선되었다[13]. 즉 비만과 관련된 중대한 내과적 합병증이 모든 면에서 놀라운 수준으로 개선됨을 알 수 있다. 이하 본 종설에서는 비만/대사 수술이 어떠한 기전으로 체중 감소와 포도당대사 개선 효과를 보이는지에 대하여 가장 대표적인 비만/대사 수술법인 루와이 위우회술을 중심으로 위장관 내분비 시스템에 바탕을 두고 논하고자 한다.

위장관 내분비 시스템 개관

위장관 내분비 시스템은 뇌하수체, 갑상선, 부신, 성선처럼 내분비세포가 하나의 장기를 이루고 있지 않고 위장관 전장에 걸쳐 분포하며 위장관 상피세포의 약 1-2%를 차지하고 있다. 이 중에서 포도당 대사와 연관하여 glucose-dependent insulinotropic polypeptide (GIP)를 분비하는 K 세포[14]와 glucagon-like peptide-1 (GLP-1), GLP-2, peptide YY (PYY), oxyntomodulin을 분비하는 L 세포[15]가 잘 알려져 있다. 비만/대사 수술 후에 cholecystokinin, serotonin 및 vasoactive intestinal peptide 등의 분비는 일반적으로 변동이 없는 것으로 알려져 있으며[16,17] 위장관 호르몬은 아니지만 식욕조절에 핵심적인 역할을 수행하는 leptin 및 insulin은 수술 후에 그 농도가 유의하게 감소하기 때문에[18,19] 특히 위우회수술 후에 관찰되는 식욕 감소에는 기여하지 않을 것으로 추정된다. 현재 수술 후의 대사 개선에서 가장 주목을 끌고 있는 것은 L-세포에서 분비되는 GLP-1, PYY, 그리고 oxyntomodulin 등의 호르몬이다.

루와이 위우회술의 구성 요소

현재 사용하고 있는 루와이 위우회술은 1969년에 Mason등[20]이 개발한 수술법을 기반으로 일부 변형한 것이다. 당시 Mason은 위절제술 후에 작은 용적의 잔존 위를 가진 환자들이 상당한 체중 감소를 경험하는 것을 보고 이 수술을 개발하였다고 한다. 그림 2A에 나타낸 것처럼 루와이 위우회술은 전체 위 용적의 약 5%에 해당하는 작은 용적의 근위부 위 파우치(pouch)를 만들고 나머지 위는 음식물이 지나가지 않도록 분리한다. 위 파우치는 근위부 공장과 루와이 형태로 문합한다(gastrojejunal anastomosis in a Roux-en-Y configuration). 위 파우치에서부터 담즙과 췌장의 소화효소가 흘러나오는 길과 만나는 곳까지를 루 가지(Roux limb) 혹은 영양 가지(alimentary limb)라고 부른다. 소화효소와 담즙은 십이지장을 거쳐 약 15-20 cm의 근위부 공장을 지나는데 이 부위를 담췌 가지(biliopancreatic limb)라고 부르며 이후 영양 가지와 만나고 이 결합 부위에서부터 비로소 소화가 시작된다. 결국 루와이 위우회술로 인해 야기되는 해부학적 특성은 세 가지로 요약할 수 있는데 첫째, 위용적의 감소 둘째, 95%의 위와 십이지장 및 상부공장의 우회 셋째, 음식물이 하부 소장에 빠른 속도로 도달함이다(Fig. 2A). 위우회술이 제2형 당뇨병의 관해를 가져오는 기전은 아직 명확치 않으나 (1) 위용적 감소와 식욕 감소에 의한 음식 섭취량 감소, (2) 95%의 위와 십이지장 및 상부공장을 우회한 효과에 기인한다는 전장 가설(foregut hypothesis), (3) 음식물이 하부 소장에 빠른 속도로 도발함으로써 촉발되는 위장관 호르몬 분비에 의한다는 후장 가설(hindgut hypothesis) 등이 거론되고 있으며 지금부터 상세히 논하겠다.

루와이 위우회술이 제2형 당뇨병의 관해를 유도하는 기전

식사량 감소와 체중 감소

루와이 위우회술은 35-40%의 체중 감소를 유도하는데 과다 체중 감소(excess body weight loss)는 약 50-80%에 달한다[21]. 위우회술이 이처럼 현저한 체중 감소를 일으키는 주요 기전은 위용적 감소로 인한 음식 섭취량 감소와 현저한 식욕 감퇴이다. 그러나 식욕이 감소하지만 음식의 맛을 달리 인식하는 것은 아니다[22]. 또한 영양소 흡수 장애 및 덤핑 증후군 증상을 경험함에 따른 단순당을 많이 함유한 음식을 기피하는 음성 조건화 반응(negative conditioning response) 등이 체중 감소에 기여할 것으로 생각해 볼 수 있으나, 덤핑 증후군의 중증도가 체중 감량 효과와 좋은 상관관계를 보이고 있지 않으며, 또한 알부민 혹은 프리알부민(prealbumin) 그리고 분변지방(fecal fat) 검사 결과를 보면 유의미한 흡수장애는 보이지 않는다[21].

Pories 등[7]이 1995년에 보고한 바와 같이 루와이 위우회술 시행 후의 당뇨병 호전은 수술 직후에 바로 나타나는 것이 흔히 관찰되는데, 한 보고에 따르면 89%의 환자가 루와이 위우회 수술 후 입원 기간 중 당뇨병의 관해가 관찰되었다[23]. 이러한 사실은 매우 충격적인데, 일반적으로 외과 병동에서 다양한 종류의 위장관 수술을 받은 직후의 환자들은 각종 스트레스 호르몬과 염증성 사이토카인 등의 영향으로 혈당 조절이 악화됨을 경험한다[21]. 흥미로운 점은, 루와이 위우회 수술 후 1주일 동안의 체중 감량이 크지 않다는 점을 고려한다면, 체중 감소와 직접적 관련성이 없음을 알 수 있다. 더욱 재미있는 사실은 위우회술이나 위밴드술의 경우 모두 수술 초기에 식사량의 급격한 감소가 있지만 위우회술의 경우에서만 수술 직후에 급격한 혈당의 호전이 나타난다[7,24]. 따라서 루와이 위우회술 직후에 보이는 제2형 당뇨병의 급격한 호전은 체중과 무관한 기전에 의함을 알 수 있다. 한편 일부 혈당 조절이 비교적 양호한 당뇨병 환자를 포함한 고도비만 환자군을 대상으로 한 소규모 연구에서 수술을 하지 않고 열량 제한만 시행한 군과 루와이 위우회술을 시행한 군을 비교한 결과, 루와이 위우회술 직후의 급격한 인슐린 감수성 개선에는 열량 제한이 필요조건이라고 보고된 바 있다[25]. 또한 고도비만을 동반한 10명의 제2형 당뇨병 환자를 대상으로 수술 전 10일간 열량 제한을 하고 6주 이상 간격을 두고 같은 환자에게 루와이 위우회술을 시행하여 열량 제한과 수술이 혈당에 미치는 영향이 비슷함을 보임으로써, 수술 직후 혈당 개선에 있어서 열량 제한이 중요함을 보인 바 있다[26]. 그러나 이들 연구에서는 수술을 하지 않은 대조군을 이용하였기 때문에 수술의 스트레스가 없는 상황에서의 열량 제한 효과를 관찰하였다는 점을 고려하여 해석할 필요가 있다.

당뇨병이 없는 고도비만 환자에서 위우회술 후의 말초에서의 인슐린 감수성 개선은 체중 감소가 상당량 일어나야 관찰되며 체중의 감소된 정도와 밀접한 관계가 있다고 보고된 바 있다[27]. 체중 감소의 중요성을 뒷받침하는 또 다른 소견으로, 당뇨병을 진단받은 지 2년 미만의 비교적 경한 당뇨병을 가진 환자에서 위밴드술의 혈당 조절에 미치는 영향을 본 결과 당뇨병의 관해가 체중 감소와 좋은 상관관계를 보임이 알려진 바 있다[24]. 당뇨병이 없는 비만 환자를 대상으로 루와이 위우회술과 위밴드술을 시행하여 20% 체중 감소가 이루어진 시점에 인슐린 감수성, 베타세포 기능 등을 분석한 결과, 두 그룹 간 차이가 없었으며 지방조직의 유전자 발현에도 차이가 없었음이 보고된 바 있는데[28], 이는 결국 당뇨병이 없는 고도 비만 환자에서는 포도당 대사 호전에 있어서 체중 감소의 역할이 매우 중요하다는 것을 시사하는 소견이다. 그러나 당뇨병이 있는 상황에서는 수술 전 혈당 조절 정도의 차이, 약제의 효과, 포도당 독성, 베타세포의 양적 질적 변화(일부 비가역적인 부분이 있음) 등등의 영향으로 인해 포도당 대사에 미치는 영향이 다를 수 있을 것이므로 주의해서 해석해야 할 것이다. 종합하면 최근의 한 리뷰에서 자세히 소개된 바와 같이[29], 당뇨병을 동반한 고도비만에서 루와이 위우회수술 직후 체중 감소가 얼마 일어나지 않은 시기와 그 이후 현저한 체중 감소가 일어나는 시기에 포도당 대사가 호전되는 기전이 서로 다르며, 각각 체중과는 독립적인 기전과 체중 감소에 의존적인 기전으로 나누어 볼 수 있겠다.

그렐린

그렐린(ghrelin)은 위저부(fundus) 및 십이지장에서 분비되는 호르몬으로, 위장관에서 분비되는 호르몬 중에서 유일하게 식욕을 촉진시키고 식사 시간에 임박하여 공복감을 증가시키고 식사행위를 개시하는데 기여한다. 루와이 위우회술은 위저부와 십이지장을 음식물이 통과하는 길에서 제외시키기 때문에 이 부위에서 생성되는 그렐린의 분비가 감소하게 되고 따라서 식욕이 감소할 것이라고 보고된 바 있다[30]. 그러나 그렐린이 공복 상태에서 증가하는 호르몬이라는 점을 고려할 때, 식사량이 줄고 특히 위저부가 음식물이 지나가는 경로에서 분리된 상황이라면, 오히려 분비가 더욱 증가해야 옳을 것이나 반대로 감소하는 것은 이해하기가 어렵다. 이러한 역설적인 결과와 유사하게, 만성적인 자극에 의한 호르몬의 박동성 분비 억제가 다른 내분비 시스템에 알려져 있는데, 성선자극호르몬분비호르몬이나 성장호르몬 분비자극호르몬을 지속적으로 주입하면 이들의 작용점이 되는 성선자극호르몬이나 성장호르몬의 박동성 분비가 소실된다[21]. 또한 위저부를 제외시키지 않는 위밴드술이나 담췌 우회술의 경우 장기간 관찰하였을 때 그렐린의 분비에는 변동이 없거나 증가함이 보고된 바 있다[21,31]. 그러나 이와 상반되는 결과들도 다수 보고된 바 있어서 과연 루와이 위우회술 후의 식욕 감소가 그렐린 분비 감소와 관련이 있을지 의문이 있는데, Cummings 등은 이러한 불일치를 보이는 결과는 위저부를 얼마나 철저히 분리시키는지 그리고 미주신경을 어떻게 처리하는지 등의 수술적 기법의 차이에 기인할 것이라고 추정하고 있다[21,31]. 루와이 위우회술과 위소매절제술을 1년간 전향적으로 비교한 연구에서 루와이 위우회술의 경우 그렐린이 수술 직후 감소하였다가 1년 후 수술 전 수준으로 회복하나 위소매 절제술의 경우에는 지속적으로 그렐린이 감소한 결과를 보인 바 있으며[32], 이는 그렐린의 감소가 위소매 절제술의 체중 감소의 하나의 기전임을 시사한다. 그러나 흥미롭게도 최근 그렐린이 결핍된 마우스에서 위소매 절제술을 시행한 결과 식욕, 체중 및 포도당대사에 미치는 영향이 정상 마우스와 차이가 없음이 밝혀져[33] 그렐린이 위우회술 후의 식욕 감소와 포도당 대사 개선에 영향을 미칠 가능성은 적거나 혹은 주된 기전은 아닌 것으로 생각한다. 또한 그렐린 분비를 억제하는 신호가 발생하는 곳이 위나 십이지장이 아니라는 보고[34]는 더더욱 그렐린 분비의 변화가 식욕의 변화를 야기하지 않을 것이라는 점을 지지한다.

전장가설(foregut hypothesis)

전장가설은 당뇨병에서 십이지장과 상부 공장이 기능 이상을 보이면서 이 부위에서 분비되는 소위 항인크레틴(antiincretin)에 의하여 포도당대사 이상을 일으키며 이 부위를 우회함으로써 대사적 개선을 이룰 수 있다는 매우 흥미로운 이론이다[35]. 그러면 위우회술에서 십이지장과 상부 공장을 우회한 효과를 어떻게 검증할 수 있을까? 그림 2B에서 처럼 위용적을 줄이지 않으면서 십이지장공장우회술(duodenal-jejunal bypass, DJB)을 시도해 볼 수 있다[36]. 그렇지만 이러한 방법은 역시 소화 흡수되지 않은 음식물이 원위부 소장으로 빨리 도달하게 되는 성질을 그대로 지닌다. 전장가설을 증명하기 위해 Rubino 등[36,37]은 비만하지 않은 제2형 당뇨병 모델인 Goto-Kakizaki 쥐를 이용하였다. 비만하지 않은 당뇨병 모델을 이용한 것은 체중 감소에 따른 효과를 배제하고 순전히 대사 조절 효과만 보기 위함이었다. 우선 그림 3A와 같이 위공장문합술(gastrojejunostomy)를 시행하였을 때 음식물은 유문을 통과하여 십이지장으로도 흘러가고 동시에 문합 부위를 통해 공장으로도 직접 흘러가지만, 이 경우 포도당 대사의 개선은 없었다[37]. 그러나 그림 3B와 같이 위공장문합술을 시행한 후에 유문부와 십이지장을 분리해 줌으로써 십이지장과 상부 공장을 우회하였을 때 포도당 대사의 개선이 관찰되었다[37]. 또한 십이지장공장우회술을 시행한 결과 포도당대사의 개선이 관찰되었으나 분리된 십이지장을 다시 위에 연결하게 되면 포도당대사 개선은 관찰되지 않았다[37]. 이러한 사실은 비만하지 않은 제2형 당뇨병 모델에서 십이지장과 상부공장을 우회함으로써 포도당대사의 개선을 가져올 수 있으며, 십이지장과 상부공장에서 유래하는 알려지지 않은 인자에 의해 매개될 것으로 추정하였다[35]. 그러나 십이지장공장우회술을 시행한 쥐에서 십이지장과 위를 다시 문합한 실험의 경우 수술의 사망률이 높았으므로 수술에 따른 스트레스에 의해 혈당이 상승하였을 가능성이 있고 또한 겨우 두 마리에서 관찰된 결과이기 때문에 해석에 주의를 요한다[37]. 또한 초기에 보고된 바와 달리 다른 연구에서는 Goto-Kakizaki 쥐에서 수술 후 3개월까지는 혈당 조절에 유의한 차이를 보이지 않은 바 있다[38].

이러한 전장 가설을 지지하는 매우 흥미로운 증례보고가 있는데, 제2형 당뇨병이 있는 51세 남자 비만 환자가 위우회술을 받은 후 위공장문합(gastro-jejunostomy) 부위의 누출(leakage)이 의심되어 경피적 위튜브(percutaneous gastric tube)를 설치하여 이를 통해 3주간 경장영양을 시행하고 이후 2주간 경구로 음식을 섭취하였다. 5주째에 각각의 경로로 음식물을 투여하고 혈당과 호르몬 변화를 관찰한 바, 경피적 위튜브를 통해 음식을 섭취할 때보다 경구로 음식을 섭취하여 변경된 해부학적 구조를 통해 위와 십이지장을 우회하고 음식물이 장으로 전달될 때에 GLP-1분비가 더욱 높고 인슐린 분비가 증가되며 이에 따라 혈당이 낮게 유지됨이 보고된 바 있다[39]. 그러나 이러한 실험을 하기 전 2주간은 경구로 음식을 섭취하였고 위와 십이지장은 우회되어 음식물의 소화흡수에서 제외된 상태였기 때문에 이 부위의 상피세포 위축과 내분비세포의 변화가 나타났을 가능성을 배제할 수 없다.

아직 미흡한 부분은 많으나, 이러한 전장가설에 힘입어 내시경을 이용하여 영양소가 투과되지 않는 튜브형태의 구조물을 십이지장 구부(bulb)에 고정하여 십이지장과 상부공장에서 음식물의 소화흡수를 막는 “내강 소매(endoluminal sleeve)”가 개발된 바 있으며, 동물 실험과 임상 시험에서 체중 감소와 혈당 개선이 보고된 바 있다[40-42]. 그러나 사람에서 실험적으로 십이지장공장우회술을 시행하였을 때(대부분이 대조군이 없이 시행되었거나[43-45] 제대로 무작위배정이 되지 않았음[46]) 그 효과가 만족스럽지 못하였다.

전장가설과 관련하여 주의할 점으로, 십이지장공장우회술 자체가 전장가설을 설명하는 것처럼 생각하는 경우가 많은데, 사실 이 경우에도 원위부 소장으로 소화흡수되지 않은 영양소가 빨리 도달할 수 있도록 하는 특성이 있음을 간과해서는 안 된다. 즉, 후장가설(hindgut hypothesis)로부터 자유롭지 못하다는 것이다. 실제로 Goto-Kakizaki 쥐에서 십이지장공장우회술을 시행한 후 GLP-1이 증가하였으며 GLP-1 수용체에 대한 길항제인 exendin9-39를 투여하면 포도당대사의 개선이 소실됨이 알려진 바, 대표적인 후장호르몬인 GLP-1이 십이지장공장우회술의 포도당 대사 개선 기전에서 핵심적 역할을 담당함을 시사한다[47]. 따라서 전장가설을 시험하는 연구 결과를 해석할 때 그 실험 결과가 정말 전장가설로 해석할 수 있는 것인지, 아니면 후장가설의 요소로도 설명할 수 있는지를 주의 깊게 살펴야 할 것이다. 또한 흥미롭게도 최근 한 연구에 의해서 십이지장공장우회술의 한 기전으로 공장의 영양소 센서를 통해 장-뇌-간 축(gut-brain-liver axis)를 거쳐 혈당을 낮출 수 있음이 보고된 바 있다[48]. 이러한 결과는 십이지장공장우회술의 작용 기전이 매우 복합적임을 시사한다. 그러나 최근 전장가설이 매우 강력한 반박에 부딪히고 있다. 즉, 위소매 절제술의 경우 십이지장과 상부공장과 위장과의 해부학적 관계가 정상으로 유지됨에도 불구하고 위우회술과 유사한 수준의 체중 감량과 혈당의 호전을 보인다는 사실 때문이다[49-51]. 따라서 위소매 절제술의 놀라운 당뇨병 관해 효과를 통해 현재 전장가설은 위기에 봉착해 있다[32].

후장가설(hindgut hypothesis)

위우회술을 시행하면 소화 흡수되지 않은 영양소가 원위부 소장에 빨리 도달하게 된다. 원위부 소장은 GLP-1, PYY, oxyntomodulin을 생산하는 L-세포가 풍부하게 위치하는 곳이다[15]. GLP-1이 식욕과 인슐린 분비에 중요한 작용을 하고 PYY가 식욕 조절에 중요한 작용을 하는 점을 감안하면 이러한 후장효과(hindgut effect)가 위우회술의 대사 개선의 주요 기전일 가능성이 있다. 원위부 소장으로 소화 흡수되지 않은 음식물이 지나가게 되면 우리 몸은 위배출 속도를 늦추고 위장관 운동 속도를 떨어뜨리고자 노력한다. 이것을 회장브레이크(ileal brake)라고 부르는데 GLP-1이 주된 작용을 나타낸다[52]. 이와 함께 식욕을 감소시키는 호르몬인 PYY가 동시에 많이 분비되는 것도 일맥상통하는 현상이라고 볼 수 있다. 실제로 제2형 당뇨병을 동반한 비만 환자에서 위우회술 1개월 후에 L-세포에서 분비되는 세 가지 호르몬인 GLP-1, PYY, oxyntomodulin이 경구포도당부하 후에 모두 증가하였으며, 식이요법으로 비슷한 정도의 감량을 이룬 환자에서는 이러한 소견이 관찰되지 않았다[53]. 또한 루와이 위우회술 그룹에서 위밴드 수술을 받은 그룹에 비하여 포도당 흡수 속도가 빠르고 GLP-1 상승이 빠르며 초기 인슐린 분비가 급속히 일어난다[28]. 흥미롭게도 위우회술을 받고 제2형 당뇨병의 관해가 온 환자에게 GLP-1수용체 길항제인 exendin9-39을 투여해도 당대사의 악화가 뚜렷이 나타나지 않는다는 사실은 GLP-1 이외에도 다른 요소가 혈당 개선에 기여함을 시사한다[54].

루와이 위우회술에서 GLP-1이 증가하는 것과 마찬가지로 담췌우회술에서도 원위부 소장으로 소화 흡수되지 않은 영양소가 많이 도달하여 GLP-1이 증가한다[55]. 그러나 제한적 술식(restrictive procedure)인 위밴드술에서는 GLP-1의 증가가 관찰되지 않는다[56]. 하지만 놀랍게도, 제한적 술식으로 애당초 개발되었던 위소매 절제술의 경우 루와이 위우회술에 견줄만한 GLP-1상승이 일어나며[50,57], 그 기전으로는 위산분비 역할을 맡은 부위를 대부분 잘라냄으로 인해 위산 분비가 감소하고 이에 대한 피드백으로 가스트린유리펩티드(gastrin- releasing peptide)가 분비되며 이것이 자극이 되어 GLP-1이 분비된다는 소위 위장가설(gastric hypothesis)이 있고[58] 또 위소매 절제술 이후 위배출 시간이 빨라지기 때문일 것이라는 가설이 있다[59]. 아직 이것은 가설의 수준에 머무르고 있기 때문에 향후 메커니즘에 대한 연구가 필요하다.

루와이 위우회술은 매우 복합적인 기전에 의해 체중 감소와 대사 개선을 일으키기 때문에 후장효과를 증명하기는 쉽지 않다. 회장전치술(ileal transposition)은 이러한 목적으로 개발된 실험적 술식으로 원위부 회장을 상부 공장으로 혈관과 신경이 연결된 장간막과 함께 위치를 옮기는 것이다[60-62] (Fig. 4). 따라서 기타 위장관의 해부학적 구조에 전혀 영향을 전혀 주지 않으면서 원위부 회장이 근위부로 옮겨져서 소화 흡수되지 않은 영양소가 다량으로 이 부위와 접촉하게 된다[60-62]. 동물 실험 결과를 종합하면 체중 변화는 없거나 약간 줄어들며, 정상 쥐에서는 혈당 변화가 없으나 당뇨병 쥐 모델에서는 혈당이 개선되며, 전반적으로 인슐린 감수성이 증가한다[60-65]. 이러한 변화는 GLP-1 분비 및 PYY 분비의 증가와 연관되어 있다[61-65]. 동물실험에서 회장 전치술 후의 체중 감소는 뚜렷치 않으며[60] 특히 비만한 쥐 모델에서 시행한 수술의 경우 체중에 유의한 변화가 관찰되지 않았다[63,64]. 사람에서는 회장 전치술과 위소매 절제술을 병행하여 87%에서 당뇨병 관해와 함께 23%의 체중 감소 효과가 나타난 바 있다[66]. 그러나 사람에서 각각의 수술을 따로 했을 때와 비교한 연구는 없으며, 또한 장기간의 체중 조절에 대한 연구도 현재로서는 없는 실정이다. 결국 후장가설이 중요하게 작용할 것으로 생각되나 그 단독만으로는 루와이 위우회술에서 나타나는 체중 감소와 다양한 대사개선 효과를 설명할 수는 없다.

결 론

본 종설에서 살펴본 것처럼 비만/대사 수술을 통해 체중 감소가 일어나고 대사 이상이 개선되는 것은 매우 다양한 기전의 복합적인 상호작용에 의해서 일어난다. 각각의 기전에 대해서는 좀더 연구가 필요하며, 이러한 기전을 밝힘으로써 수술을 하지 않고 내과적 치료만으로 비슷한 효과를 얻을 수 있는 시대가 곧 다가 올 것으로 믿어 의심치 않는다. 그러나 수술과는 달리 평생 약을 복용해야 할 가능성이 있기 때문에 일회성의 치료로 식행동까지 바꾸어 놓는 비만/대사수술은 여전히 유용한 치료 수단으로 남을 것으로 전망된다. 무수한 미해결 과제가 산재해 있지만, 대한민국의 의사로서 가장 궁금한 점은 “과연 우리나라 환자들에게도 비만대사 수술이 유용할 것인가?”라는 질문일 것이다. 최근 한 연구에서 22명의 체질량지수 32 kg/m2 정도인 비만한 제2형 당뇨병 환자를 대상으로 루와이 위우회술을 시행한 결과 16명(73%)에서 수술 후 1년째 검사에서 당뇨병의 관해가 확인되었으며(공복혈당 < 126 mg/dL이면서 HbA1c < 6.5%를 기준으로 함), 수술 전 내장지방이 많은 경우 관해가 잘 일어나지 않았고 관해가 일어난 환자들은 수술 후 1년째 인슐린 분비능에는 차이가 없었으나 인슐린 감수성이 우수한 특징을 보였다[67]. 전국적으로 이루어진 연구에서 2008년부터 2011년까지 비만/대사 수술(27.6%가 위밴드술, 28.0%가 루와이 위우회술, 44.4%가 위소매 절제술)을 받은 261명의 환자와 생활습관교정과 약물 치료 등의 내과적 치료를 받은 224명을 후향적으로 분석한 결과, 치료 후 18개월 경과 시점에 치료 시작 시점 대비 수술군에서 22.6% 대조군에서 6.7%의 체중 감소가 있었고, 당뇨병, 고혈압, 이상지혈증으로부터 각각 57%, 47%, 84%에서 회복이 되어 대조군의 10%, 20%, 24% 보다 유의하게 우수한 성적을 보임을 알 수 있었다[68]. 체중 감소는 큰 차이는 없었으나 루와이 위우회술에서 수술전 체중 대비 26.6%의 감소가 있었고, 위밴드술에서 20.8%, 위소매 절제술에서 22.3%의 체중 감소를 보였다[68]. 수술군에서 1명이 30일 이내에 흡인성 폐렴으로 사망하여 수술 관련 사망률은 0.38%이었으며, 위소매 절제술의 경우 수술 직후 및 그 이후의 합병증이 가장 낮았다[68]. 그러나 수술군이 대조군에 비해 유의하게 체질량지수가 높았고(39.0 ± 6.2 kg/m2 vs. 34.3 ± 3.8 kg/m2) 당뇨병 유병률도 수술군이 대조군에 비해 유의하게 높은(39.1% vs. 12.9%) 등, 두 그룹 간의 차이가 있었기 때문에 정확한 비교가 이루어졌다고 볼 수 없다. 그럼에도 불구하고 수술군에서 현저한 체중 감소와 대사 합병증의 호전이 있었으므로 국내 비만 환자에게도 비만/대사수술은 중요한 치료 방법의 하나이며 어쩌면 가장 우수한 치료법이라고 할 수 있을 것이다. 이러한 점에서 대사 합병증을 동반한 비만 환자의 경우 비만/대사수술이 하루 빨리 건강보험의 혜택을 받아야 할 것이다. 앞으로 여러 진료과의 의사들이 협력하여 비만대사수술을 받는 환자를 관리해야 할 것이며, 또한 이 분야의 미해결 과제를 풀기 위해 함께 노력해야 할 것이다.

The Roux-en-Y gastric bypass surgery is characterized by (1) reduced functional gastric volume, (2) bypassing 95% of the stomach, whole duodenum, and the proximal jejunum, and (3) expedited delivery of undigested nutrientsto the distal small intestine. The duodenal-jejunal bypass is different from the Roux-en-Y gastric bypass only by the absence of reduced functional gastric volume.
