Abstract

목적:

우울증은 COPD 환자들에게 흔하게 동반되는 질환이다. 하지만 현재 COPD 환자들에게 우울증의 선별 검사의 효과와 선별 검사를 무슨 도구로 시행해야 하는지 알려진 바가 전혀 없는 실정이다. 이에 체계적 문헌고찰을 통해 COPD 환자의 우울증 선별 검사 효과와 도구에 대해서 알아 보고자 하였다.

방법:

COPD 환자에서 우울증 선별 검사 효과와 우울증 진단 도구로 일차 진료의가 선별 검사 목적으로 사용할 수 있는 것이 어느 것인지 알아보기 위해 “COPD 환자에게 우울증 선별 검사를 하는 것이 필요한가?” 와 “우울증 진단 도구 중, 일선 진료의의 선별 검사 목적으로 권장할 만한 것은 무엇인가?” 라는 두 가지 핵심 질문을 선정하고 이에 대한 답을 구하고자 하였다. 문헌 검색을 위한 데이터베이스로는 국내의 경우 국내의학학술지초록검색(kmbase.medric.or.kr), 학술연구정보서비스(www.riss.kr), 코리아메드(www.koreamed.org)를 이용하였고 국외의 경우는 Medline, Embase, Cochrane Library를 이용하였다.

결과:

COPD가 있을 경우 우울증의 발생 또한 높아진다. 우울증이 동반이 될 경우 COPD 환자들이 삶의 질에 나쁜 영향을 받았다. 우울증 선별 검사 도구 중 한글화되어 있는 도구는 8가지가 있으며 각 도구는 비슷한 정도의 신뢰도와 타당도를 가진다.

결론:

COPD 환자에서 우울증의 선별 검사는 필요하며 우울증 선별을 위해서 한글화된 8가지 도구 사용이 모두 가능하다. 그중에서 문항수가 적고 환자의 자가 기입이 가능한 도구가 현재의 진료 현실에서는 적합할 것으로 생각된다.

Background/Aims:

Chronic obstructive pulmonary disease (COPD) often coexists with co-morbid conditions that significantlyaffect the prognosis. Although depression frequently coexists with COPD, little about how to screen for depression in COPDpatients is known. We report the effectiveness of depression screening in patients with COPD and of the instruments used for thispurpose.

Methods:

We performed a systematic literature review of the domestic (Research Information Sharing Service, KoreaMed, andKmbase) and international (Medline, Embase, Cochrane library) databases using “COPD AND depression AND screening tools”as key words.

Results:

No study directly evaluated the effectiveness of screening for depression in COPD patients. However, depression waspresent more frequently in patients with COPD. COPD patients with depression were also more likely to be in poor health. A totalof eight screening instruments for depression were translated into Korean. All had similar reliabilities and internal consistencies.

Conclusions:

Screening of COPD patients for depression is necessary because individuals with this condition are more likely tosuffer from depression, which has a negative impact on health. The choice of depression screening instruments may need to bebased on considerations of simplicity. (Korean J Med 2012;83:468-475)

서 론

우울증은 많은 만성 질환과 관련이 있다. 종합 병원 내과계 환자들의 10-20%에서 우울증이 동반되어 있는 것으로 알려져 있다[1]. 실제로 당뇨병 환자의 11% [2], 보존적 치료를 받는 암 환자들의 15% [3], 심근 경색 환자들의 20% [4], 파킨슨병 환자의 17% [5]가 우울증에 이환되어 있다.

우울증은 질환 자체가 가지고 있는 불쾌한 감정 상태와는 별개로 동반 질환의 예후를 나쁘게 한다. 우울증은 인해 동반질환에 대한 환자들의 순응도가 떨어뜨리며[6] 환자들의 삶의 질에 영향을 준다. 또한 우울증은 심부전[7], 후천성 면역 결핍증(HIV/AIDS) [8], 암[9], 신부전[10], 심근 경색[11], 뇌졸중[12]과 같은 질환의 사망률을 높이는 것으로 알려져 있다. 이처럼 우울증은 많은 만성 질환에서 환자들에게 흔하고 중요한 문제로 만성 질환에 동반된 우울증의 치료에 대한 중요성은 계속 강조되어 오고 있다.

만성폐쇄성폐질환(chronic obstructive pulmonary disease, COPD)은 우리나라에서 2001년도에 45세 이상 성인에서 17.2%, 2008년도에는 40세 이상의 성인의 13.4%에서 COPD에 이환되어 있는 것으로 조사될 정도로 흔한 질환이다[13]. COPD는 심근 경색과 협심증, 골다공증, 골절, 당뇨, 빈혈, 수면장애 등 다른 질환이 잘 동반되며, 우울증도 그중의 하나로 알려져 있다[14]. 하지만, 현재 COPD 환자들에게 우울증의 선별 검사의 효과와 선별 검사를 무슨 도구로 시행해야 하는지 알려진 바가 전혀 없는 실정이다. 이에 저자들은 COPD 환자에서 우울증 선별 검사 효과와 우울증 진단 도구로 일차 진료의가 선별 검사 목적으로 사용할 수 있는 것이 어느것인지 알아보기 위해 “COPD 환자에게 우울증 선별 검사를 하는 것이 필요한가?”와 “우울증 진단 도구 중, 일선 진료의의 선별 검사 목적으로 권장할 만한 것은 무엇인가?”라는 두 가지 핵심 질문을 선정하고 이에 대한 답을 구하고자 하였다.

방법

저자들은 위의 두 가지 핵심 질문에 대한 답을 얻기 위해 체계적인 문헌고찰을 수행하였다. 체계적인 문헌고찰을 위해 먼저 선정된 핵심 질문에 대한 핵심 용어를 선정한 후 선정된 핵심 용어를 바탕으로 데이터베이스에서 문헌 검색을 시행하였다. 검색된 문헌은 연구자들이 초록을 확인한 후 최종 문헌을 선택하였다

핵심 용어의 선정

문헌 검색에 사용된 핵심 용어는 국문의 경우 ‘만성폐쇄성폐질환 AND 우울증 AND 선별 검사’ 영문의 경우는 “COPD AND depression AND screening tools”였으며 유사어로 ‘chronic obstructive pulmonary disease, emphysema, chronic bronchitis, chronic obstructive lung disease, obstructive lung disease, obstructive pulmonary disease, obstructive lung diseases, obstructive pulmonary diseases, dysthymua, depressive disorder, depressive symptoms, depressive mood, diagnosis, psychometric scale, psychometric questionnaire, detection, identification’을 사용하였다.

문헌 검색

첫 번째 핵심 질문을 위한 문헌 검색을 위해 사용한 데이터베이스로는 국내의 경우 코리아메드(www.koreamed.org), 학술연구정보서비스(www.riss.kr), 국내의학학술지초록검색(kmbase.medric.or.kr)를 이용하였고 국외의 경우는 Medline, Embase, Cochrane Library를 이용하였다. 검색하고자 하는 문헌의 종류는 메타 분석, 대조 임상 시험, 비대조 임상 시험, 관찰 연구 모두 포함하고 사용된 언어는 영어와 한국어로 제한하였다. 두 번째 핵심 질문인 COPD 환자에게 적절한 선별 검사 도구와 관련하여서는 국제적으로 신체 질환이 있는 환자들에게 우울증 선별 검사로 권장되는 우울증 선별 도구 [15] 중 한국어로 번역되어 검증이 완료된 선별 도구에 한해서만 연구를 진행하여 국내 문헌만을 대상으로 검색을 수행하였다.

선택된 문헌의 질 평가

본 연구에서 포함된 연구에 대한 문헌의 질 평가는 무작위 대조군 연구의 경우의 질 평가는 QUADAS tool, 비무작위 대조군 연구의 질 평가는 Newcastle-Ottawa Scale 을 이용하여 수행하였다. 또한 문헌 검색 결과 얻어진 결과에 대한 근거 수준 평가는 개별 연구의 연구디자인에 따른 문헌의 질평가 결과와 연구 결과의 일관성, 연구 결과의 직접성, 효과의 크기, 연구 결과의 정확성, 출판 오류, 측정된 효과의 양반응, 교란변수의 가능성 등을 고려한 GRADE 그룹(Grading of Recommendations Assessment, Development and Evaluation [GRADE] Working Group)에서 제시하는 방법론에 따라 평가하였다.

결과

COPD 환자에게 우울증 선별 검사를 하는 것이 필요한가?

첫 번째 핵심 질문에 대해서는 총 250개의 문헌이 검색되었으며 이 중 COPD 환자에서 우울증 선별 검사의 효과에 대한 관찰 연구 2편[16,17], COPD 환자의 우울증 유병률에 대한 메타 분석 1편[18], 대규모 역학 연구 3편[19-21]과 국내 연구 4편[22-25]을 선택하였다(Table 1).

Prevalence of depression in COPD patients

COPD 환자들을 대상으로 우울증 선별 검사의 유용성을 조사한 무작위 대조 연구는 검색이 되지 않았지만 COPD 환자들을 대상으로 한 우울증 선별 검사에 대한 관찰 연구 결과를 보면 우울증 선별 검사를 통해 COPD 환자들의 11.2% [16], 12.9% [17]에서 주요 우울증(major depression)이 진단되었으며 메타 연구 결과에서는 COPD 환자의 우울증 유병률은 24.6% (95% CI: 20.0-28.6%)로 조사되어 대조군의 유병률 11.7% (95% CI: 9.0-15.1%)보다 통계적으로 의미 있게 높았다[18]. 또한 COPD 환자의 우울증 위험도(Odds Ratio, OR)도 2.81에서 4.26 [18-20]까지 조사가 되어 COPD 환자들이 우울증에 이환될 위험이 높을 뿐만 아니라 COPD 환자들은 일반인들보다 심리적인 고통을 더 많이 겪고 있는 것으로 조사되었다[21]. 우울증 선별 검사에 대한 관찰 연구와 메타 분석 사이의 유병률 차이에 대해서는 추가적인 연구가 필요할 것이다.

국내 연구 결과들은 외국의 연구보다는 우울증의 유병률이 높게 조사가 되었다. 국내 COPD 환자들은 17% [22]에서 55%까지 우울증이 동반되어 있으며[23-25] 이는 일반인의 우울증 유병률보다 높은 것이다[48]. COPD 환자들이 우울증의 위험도는 2.7로 조사되었다[25].

국내 연구에서는 COPD 환자들의 삶의 질에 대해서도 조사가 된 연구가 두 가지 있었으며[23,24] 우울증 점수와 삶의 질을 나타내는 St. Goerge Respiratory Questionnaire (SGRQ), COPD Assessment test (CAT) 사이에 유의한 상관관계가 관찰되었다.

우울증 진단 도구 중, 일선 진료의의 선별 검사 목적으로 권장할 만한 것은 무엇인가?

두 번째 질문에 대해서는 국내 데이터베이스에서 총 11,582개 문헌이 검색되었으며 중복된 문헌을 제외하고 우울증 진단을 위해 타당도 검증을 시행한 문헌을 추출하여 총 22개의 문헌이 선택되었다[26-47]. 검사 도구 별로는 Hamilton Depression Rating Scale (HDRS) 1편[26], Montgomery-Asberg Depression Rating Scale (MADRS) 1편[27], Center for Epidemiological Studies-Depression Scale (CES-D) 4편[28-31], Zung’s Self-Rating Depression Scale (SRS) 3편[32-34], Hospital Anxiety and Depression Scale (HADS) 1편[35], Beck Depression Inventory (BDI) 5편[36-40], Geriatric Depression Scale (GDS) 5편[41-45], Patient Health Questionnaire (PHQ) 2편[46,47]이었다. 이 중 환자의 자가 기입이 가능한 도구는 여섯 가지이다[28-47].

Hamilton Depression Rating Scale (HDRS)

17개의 항목으로 구성된 관찰자 평가 척도로 103명의 주요 우울증 환자를 대상으로한 연구에서 높은 내적 일치도(chronbach α = 0.76)을 보였고 MADRS 와 높은 상관관계를 보였고(r = 0.84, p < 0.001) BDI와도 의미 있는 상관관계를 보였다(r = 0.58, p < 0.001) [26].

Montgomery-Asberg Depression Rating Scale (MADRS)

10개의 항목으로 구성된 관찰자 평가 척도로 107명의 주요 우울증 환자를 대상으로 한 연구에서 높은 내적 일치도(chronbach α = 0.79)를 보였고 HDRS와 높은 상관관계를 보였고(r = 0.82, p < 0.001) 자가 보고 척도인 BDI와도 의미 있는 상관관계를 보였다(r = 0.47, p < 0.001) [27].

Center for Epidemiological Studies-Depression Scale (CES-D)

20개의 항목으로 구성된 자가보고형 평가 척도로 일반인을 대상으로 하여 내적 일치도를 평가한 네 가지 연구에서 모두 높은 내적 일치도를 보였다(0.84, 0.89, 0.91, 0.91) [28-31]. 우울증 환자에서의 내적 일치도도 0.89로 높게 측정되었다[30]. 우울증 진단을 위한 절단점(cut-off point)은 외국의 경우 일반적으로는 16점 이상을 우울증 선별을 위한 값으로 사용하나 우울증, 정신분열증, 알코올의존 환자들의 CED-D를 비교한 초기 연구에서는 16점 이상인 환자의 비율이 우울증에서는 100%, 정신 분열증에서는 61%, 알코올의존증에서는 52%로 조사되어 16점보다는 높은 값의 필요성이 제기되었고[28] 이후 연구에서는 24점을 절단점으로 사용할 경우 민감도 94%, 특이도 100%로 조사되었으며[29] 또 다른 연구에서는 정신과 환자들을 대상으로 사용할 경우에는 25점, 일반인을 대상으로 사용할 경우에는 21점을 절단점으로 제시하였다[30].

Zung’s Self-Rating Depression Scale (SDS)

20개의 문항으로 구성된 자가 보고형 도구로 초기 연구에서 우울증 환자의 증상 평가에 도움이 되었고[32] 일반인과 우울증 환자 모두에게서 높은 내적 일치도를 보인다(일반인 0.84, 우울증 환자 0.77) [33,34]. 또한 BDI와도 높은 상관계수를 보였다(r = 0.72, p = 0.0001) [34].

Hospital Anxiety and Depression Scale (HADS)

14개의 문항으로 구성된 자가 보고형 도구로 정상인, 우울 장애 환자, 불안 장애 환자를 대상으로 한 연구에서 내적 일치도는 0.86, BDI와의 상관계수는 0.80이었다. 절단점을 8점으로 할 경우 민감도는 89.2% 특이도는 82.5%였다[35].

Beck Depression Inventory (BDI)

21개의 항목으로 구성된 자가 보고가 가능한 도구로 여러 집단을 대상으로 한 연구에서 모두 높은 내적 일치도를 보이고 있다[36-40]. 다만 연구 대상마다 절단점 값이 차이가 있어 일반인을 대상으로 한 경우에는 21점[36], 다른 연구에서는 남자 23점, 여자 24점[37]이 제시되었고 일차 진료의를 찾아 오는 만성 질환 환자에서는 14점[39]이 제시되었다. 정신과 환자들의 경우에도 남자 23점, 여자 24점을 제시한 연구[39]와 22점을 제시한 연구[40]가 있다.

Geriatric Depression Scale (GDS)

30개의 문항으로 구성된 자가 보고형 도구로 노인 환자들을 대상으로 사용이 된다. GDS는 정상인과 노인 환자에서 모두 높은 내적 일치도를 보이며[41,44,45] 다른 우울증 선별 도구인 BDI, SDS, CES-D, HRDS 등과도 높은 상관관계를 보인다[44,45]. 하지만 연구마다 다양한 절단점을 보인다(Table 2).

Patient Health Questionnaire-9 (PHQ-9)

9개의 문항으로 구성된 자가보고형 도구로 정상인, 만성 질환 환자, 정신과 외래 환자들 모두에서 높은 내적 일치도를 보이며 절단점은 9점, 9.5점으로 제시되고 있다[46,47].

고 찰

본 연구에서 COPD 환자의 우울증 유병률은 11.2% [16]에서 55% [25]까지 다양하였지만 COPD 환자들은 우울증의 위험성이 높은 것으로 조사되었다[18-20,25]. 이러한 유병률의 차이는 선택된 문헌의 연구 방법의 차이에 기인한 것으로 생각되지만 보다 정확한 유병률을 구하기 위한 노력이 필요할 것이다.

COPD 환자들에서 우울증이 잘 동반되는 이유로는 환자들의 나이, 폐기능, 연령과 같은 임상 지표들보다는 COPD 자체의 증상이나 COPD로 인한 삶의 질 저하 등이 우울증 발생에 더욱 기여하는 것으로 알려져 있다[18,49]. 또한 우울증은 COPD 환자들의 삶의 질에 나쁜 영향을 끼쳐[23,24] COPD환자들은 일반인들보다 심리적인 고통을 더 많이 겪고 있다 [21]. 실제로 2010년 국내에서 시행된 실태 조사 결과를 보면 우울증이 동반이 될 경우 COPD Assessment Test (CAT)로 측정한 삶의 질 평가에서 97.9%의 환자들이 CAT 점수가 10점 이상으로 삶의 질에 나쁜 영향을 받는 것으로 조사되었다 [14,24].

따라서 COPD 환자들에게서 우울증은 중요한 문제이며 우울증의 조기 발견과 치료를 위한 노력이 필요하다. 일반인들에게 일률적인 우울증 선별 검사가 우울증의 진단, 치료, 예후에 큰 영향을 끼치지는 않지만[50], 영국의 National Institute for Health and Clinical Excellence (NICE) 지침에서 만성 질환이 동반된 환자들에게 우울증 치료를 강조하고 있고[15], 미국의 U.S. Preventive Services Task Force (USPSTF)에서도 우울증 발견 후 진단, 치료, 추적관찰이 가능할 경우에는 일반 성인에서도 우울증 선별 검사를 권장하고 있으므로[51] COPD환자들에게 우울증 선별 검사를 시행하는 것이 필요할 것이다.

COPD와 같은 만성 질환 환자들에게서 정확한 우울증의 진단은 중요한 문제이다. 하지만 신경정신과 의사들이 아닌 의사들이 우울증을 진단하는 것은 쉽지 않다. 일차 진료의들을 대상으로 한 연구에서 만성 질환이 동반된 환자들에게서 우울증을 진단할 수 있는 확률은 23%로 만성 질환이 없을 경우의 94%와 큰 차이를 보인다[52]. 따라서 우울증 발견을 위한 적절한 선별 검사의 사용이 필요하다. 영국의 NICE 지침에서는 13가지의 선별 검사 도구를 제시하고 있으며 각 도구는 모두 비슷한 진단적 정확도를 보이고 있으며[15] 어떤 한 도구가 다른 도구보다 우월하다는 연구는 없다[50].

본 연구에서는 우울증 선별 검사 도구 중 한글로 번역이 되어 있는 8개 도구의 신뢰도, 타당도를 조사하였다. 한글화된 각각의 도구들은 10-30개의 문항으로 이루어져 있었으며 모두 높은 내적 일치도를 보이고 있다. 자가 기입이 가능한 6개의 도구들은 모두 비슷한 정도의 민감도와 특이도를 보이고 있다(Table 2). 한글화된 도구들은 한 가지 도구를 여러 연구자가 서로 다르게 번역하거나 연구에 따라서는 우울증 진단의 황금률(Golden Standard)이 불명확한 경우가 있기는 하나 일선 진료 현장에서 사용하는 데는 무리가 없을 것으로 판단된다. 국내와 같이 바쁜 외래 진료 환경을 고려하면 환자의 자가 기입이 가능하고 문항 수가 상대적으로 적은 선별 도구의 사용이 유리 할 것으로 생각된다.

결론적으로 COPD 환자에서 우울증은 흔한 동반 질환이고 COPD가 있을 경우 우울증의 발생 또한 높아진다. 우울증이 동반이 될 경우 COPD 환자의 삶의 질은 더욱 나쁘게 되므로 COPD 환자에게 우울증 선별 검사가 필요하다.

우울증 선별 검사 도구 중 한글화되어 있는 도구는 8가지가 있으며 각각의 도구는 비슷한 정도의 신뢰도와 타당도를 가진다. 우울증의 선별을 위해서 한글화된 8가지 도구도 사용이 모두 가능하나 문항 수가 적고 환자의 자가 기입이 가능한 도구가 현재의 진료 현실에서는 적합할 것으로 생각된다.
