Abstract

저자들은 반복적인 다량의 혈변을 주소로 내원한 29세 여자 환자에서 출혈의 원인이 되었던 멕켈 게실 내 이소성 췌장을 경험하였기에 문헌고찰과 함께 보고하는 바이다.

Trans Abstract

Heterotopic pancreas is a tissue histologically similar to normal pancreatic tissue found in a location other than the usual place, and having no anatomic or vascular connection with the pancreas itself. Heterotopic pancreas is usually asymptomatic and found incidentally. However, it can cause problems such as abdominal pain, melena, anemia, and severe bleeding. We report a case of a 29-year-old woman with obscure gastrointestinal bleeding caused by heterotopic pancreas located in Meckel’s diverticulum. Gastroduodenoscopy and colonoscopy showed no bleeding focus. By capsule endoscopy and double balloon enteroscopy, a bleeding focus was suspected in the ileum. The patient received a laparoscopic resection of the Meckel’s diverticulum. Pathologic examination revealed heterotopic pancreatic tissue within the muscular layer of the diverticulum. The patient recovered well without further bleeding. (Korean J Med 2011;81:492-495)

서 론

이소성 췌장은 췌장 조직의 일부가 췌장과의 해부학적, 혈관적 연결 관계 없이 원래의 해부학적 위치가 아닌 위, 십이지장, 공장, 멕켈 게실, 회장 등 다른 곳에서 발생된 것을 의미한다[1]. 대다수 이소성 췌장은 무증상이나 간혹 심한 복통, 흑색변, 빈혈, 구역, 구토 또는 체중 감소 등과 같은 증상을 동반하기도 한다[2,3]. 이소성 췌장과 연관되어 발생하는 위장관 출혈은 대부분 상부위장관에서 보고되어 있으며 이소성 췌장 조직이 점막하 혈관 구조를 파괴하거나, 점막내 염증을 일으켜 궤양을 형성하여 출혈을 유발하는 것으로 설명되고 있다[4].

멕켈 게실은 젊은 성인에서의 원인불명 위장관 출혈의 주요 원인 중 하나로 알려져 있으며, 약 14%에서 게실 내에 이소성 조직이 발견되는 것으로 보고되어 있다[5]. 출혈을 동반한 멕켈 게실에서 발견되는 대부분의 이소성 조직은 위 점막조직이며, 이소성 췌장 조직이 동반된 경우는 거의 보고된 바가 없다[6]. 저자들은 반복적인 다량의 혈변을 주소로 내원한 29세 여자 환자에서 출혈의 원인이 되었던 멕켈 게실 내 이소성 췌장을 경험하였기에 문헌고찰과 함께 보고하는 바이다.

증 례

환 자: 29세 여자

주 소: 혈변

현병력: 환자는 내원 2년 전 혈변, 어지러움증, 호흡곤란을 주소로 인근 병원에 내원하였으며 당시 측정된 혈색소치는 3.0 mg/dL였다. 해당 병원에서 위장관 출혈에 대하여 검사를 시행받았으나 명확한 출혈 원인은 밝히지 못하였다. 이후에도 간헐적으로 혈변이 있었으나 추가적인 검사는 시행 받지 않았고 경구용 철분제제를 복용하고 수혈을 받으면서 지내왔다. 환자는 경구용 철분제제 외의 다른 약제를 복용하지는 않았다. 환자는 내원 2달 전부터 다시 선홍색의 혈변을 보기 시작하였고 내원 3일 전에는 다량의 혈변과 함께 심한 어지럼증이 동반되어 본원에 내원하였다.

과거력: 특이사항 없었다.

가족력: 특이사항 없었다.

신체 검사: 활력증후는 혈압 110/60 mmHg, 맥박수 150회, 호흡수 분당 20회이었으며, 체온은 36℃이었다. 환자는 급성병색을 보였으며 결막은 창백하였다. 복부촉진상 압통은 없었고 종괴는 만져지지 않았다. 직장 수지 검사에서 혈변이 관찰되었다.

검사 소견: 말초혈액 검사에서 백혈구 11,170/mm3, 혈색소 5.7 mg/dL, 적혈구 용적률 17.9%, 혈소판 232,000/mm3이었으며, 프로트롬빈시간은 14.6초였다. 혈청 생화학 검사에서 AST/ALT 18/20 IU/L, 총 단백 5.2 g/dL, 알부민 3.5 g/dL, alkaline phosphatase 37 IU/L, 총 빌리루빈 0.3 mg/dL, BUN 11 mg/dL, creatinine 0.6 mg/dL이었다. 혈청 철 16 µg/dL, 총철분결합능(TIBC) 258 µg/dL, 혈청 ferritin 26 ng/mL으로 철결핍성 빈혈에 합당한 소견이었다.

출혈의 원인 병소를 찾기 위해 시행한 상부위장관 내시경 검사에서 위전정부에 다수의 미란이 관찰되었으나, 출혈을 일으킬만한 병소는 발견할 수 없었다. 대장내시경 검사에서도 직장에서 말단 회장 부분까지 부분적인 혈괴가 관찰되었으나 출혈의 원인으로 의심되는 병소는 관찰되지 않았다. 이후 시행한 복부 전산화 단층 촬영과 캡슐 내시경에서도 출혈의 원인으로 의심되는 병소는 찾을 수 없었다. 환자의 연령에서 원인불명 위장관 출혈의 주요 원인 중 하나가 멕켈 게실이므로 Tc 99m pertechnetate 스캔 검사를 시행하였으나 이 검사에서도 명확한 이상 소견은 관찰되지 않았다(Fig. 1). 환자는 입원 중에도 간헐적으로 혈변이 지속되었으나 상기 검사에서 출혈 병소를 발견하지 못하여 이중 풍선 소장 내시경을 시행하였다. 경구 접근으로 원위부 공장으로 생각되는 부위까지 관찰하였으나 출혈 병소는 관찰되지 않았다. 그러나 경항문 접근으로 시행한 이중 풍선 소장 내시경에서는 회맹장판으로부터 근위부 80 cm 부위에 직경 3 cm 정도의 게실이 관찰되었으며, 게실 내부에 백색의 삼출물로 덮여 있는 궤양이 관찰되었다. 관찰 당시 현성 출혈은 없었으며 게실 내에 노출 혈관, 혈괴 부착 등의 소견은 관찰되지 않았다(Fig. 2).

수술 및 병리 소견: 멕켈 게실에 의한 위장관 출혈의 가능성이 높을 것으로 판단하고, 입원 10일째 복강경하 게실 절제술을 시행하였다. 수술 중 회맹장판으로부터 근위부 70 cm 부위의 장간막 반대 부위에 3 cm 크기의 배꼽과 연결 부분이 없는 게실이 관찰되었으며 이 병소를 성공적으로 절제하였다(Fig. 3). 현미경적 소견에서 멕켈 게실을 이루는 장 점막층에 약간의 염증 소견이 관찰되었고, 조직학적으로 출혈의 증거는 없었다(Fig. 4A). 멕켈 게실의 고유근층 내부에는 췌선 및 췌관으로 이루어진 이소성 췌장조직이 관찰되었으며 이소성 위점막 조직은 관찰되지 않았다(Fig. 4B). 환자는 수술 후 6일째 퇴원하였으며, 수술 후 6개월이 경과한 현재 더 이상의 출혈 증상 없이 외래 추적관찰 중이다.

고 찰

이소성 췌장은 상복부 수술 시 0.2%에서 위절제술 후 검체의 0.9% 정도에서 우연히 발견된다. 이소성 췌장은 대부분 무증상이나[7] 악성 변화[8], 낭성 변성[9] 및 비정상적인 호르몬 분비를 유발할 수 있고 드물게 장폐색, 출혈 등의 증상을 유발할 수 있다.

이소성 췌장과 연관되어 발생하는 위장관 출혈은 대부분 상부위장관에서 보고되어 있으며 이소성 췌장 조직이 점막하 혈관 구조를 파괴하거나, 점막내 염증을 일으켜 궤양을 형성하여 출혈을 유발하는 것으로 설명되고 있다[4]. 그러나 본 증례와 같이 하부위장관 출혈을 일으킨 사례는 보고된 바가 없다.

멕켈 게실에는 이소성 조직이 종종 발견된다. 가장 흔히 발견되는 이소성 조직은 위점막이며(8%), 드물게 췌장(3%)이나 carcinoid (2%)를 포함할 수 있다[5].

40세 이하의 젊은 성인에서 위장관 출혈 발생 시, 상하부 위장관 내시경 검사에서 출혈 병소를 찾을 수 없을 때에 가장 흔한 원인으로 멕켈 게실, Dieulafoy 병변, 소장의 종양 등이 있다[10]. 출혈 병소가 애매모호한 상태에서 시행하는 멕켈 스캔의 경우, 감수성이 65%이며[11], 캡슐 내시경은 60-70%이다[12]. 따라서 멕켈 스캔 또는 캡슐 내시경에서도 음성이 나올 수 있으므로, 위의 증례와 같이 지속적인 출혈이 있을 경우는 추가적인 적극적 검사가 필요하다.

이 증례에서 멕켈 게실 내에서 발견된 이소성 췌장조직이 직접적인 출혈의 원인이었는지 여부는 내시경 소견과 병리 소견에서 현성 출혈 소견이 관찰되지 않아 단정적으로 결론지을 수는 없다. 그러나 이중 풍선 소장 내시경 시행 전에 시행한 여러 검사에서 명확한 출혈 병소를 발견하지 못하였고 멕켈 게실 내부에 궤양이 관찰되었으며 반복적인 혈변이 있었던 환자가 게실 절제 후 상당 기간 동안의 추적관찰에서 더 이상의 출혈 증상이 없었다는 점 등을 고려할 때 임상적으로 멕켈 게실 내의 이소성 췌장이 출혈 병소였을 가능성이 높을 것으로 판단된다.
