총담관에서 발생한 외과적 클립 결석

71세 여자가 개인의원에서 건강검진 목적으로 시행한 복부초음파검사에서 총담관 결석이 발견되어 입원하였다. 환자는 6년 전 급성 결석성 담낭염 진단하에 복강경담낭절제술을 시행 받았다. 입원 당시 활력징후는 정상이었고, 혈액검사에서는 AST 73 IU/L, ALT 63 IU/L, r-GT 483 IU/L, 알칼리 포스파타아제 780 IU/L이었으나, 특별한 증상을 호소하지는 않았다. 복부 X-선 검사에서 우상복부에 2개의 외과적 클립이 관찰되고 2번째 요추 우측에서 또 다른 한 개의 클립이 관찰되었고, 복부 컴퓨터단층촬영에서 경도의 총담관 확장과 총담관 내에 금속성 인공물이 관찰되었다(Fig. 1). 내시경 역행담도조영술에서 총담관의 확장이 관찰되고, 총담관 내 중심부에 외과적 클립으로 생각되는 방사선비투과 물질을 포함한 타원형의 음영결손이 관찰되었다. 내시경괄약근절개술을 시행한 후 바스켓과 회수용 풍선 카테터를 이용하여 결석을 제거하였다(Fig. 2). 제거된 결석은 갈색석이었다.

외과적 클립에 의한 결석은 매우 드문 질환으로 1978년 Walker 등[1]에 의해 처음 보고된 이래 매우 적은 예가 보고되었다. 외과적 클립에 의한 총담관 결석은 복강경 담낭절제술의 후기 합병증으로 담낭관 밑동의 외과적 클립이 총담관 내강 내로 이동하여 결석 형성의 핵(nidus)으로 작용하는 것으로 알려져 있다[2]. 치료는 일반적인 총담관결석의 치료와 동일하여 내시경을 이용한 유두괄약근절개술 후 바스켓이나 풍선 카테터를 이용한 결석 제거가 유용하다. 최근에는 이러한 외과적 클립에 의한 총담관 결석을 예방하기 위하여 체내 흡수가 되는 클립을 사용하기도 하고, 초음파진동에 의한 마찰열을 이용한 harmonic scalpel을 사용하여 클립 없이 수술하기도 한다[3,4].
