Trans Abstract

Pancreatic endocrine tumors (PET) are rare neoplasms of the pancreas accounting for less than 5% of all primary pancreatic malignancies. Insulinomas, gastrinomas, glucagonomas and somatostatinomas is included in PET. PETs are usually classified into functioning and non-functioning tumors and presents with a range of benignity or malignancy. It is very important to accurate diagnose the PET location and to predict the benignity or malignancy of PET in terms of the treatment strategy, because PET have higher respectability, better response to chemotherapy and better prognosis compared to that of pancreatic adenocarcinoma. The utility and reliability of different imaging modalities depends on the characteristics of PETs, specifically their size. Functioning PET tend to be small (less than 2 cm), well circumscribed, homogeneous, and usually shown as strong enhancement on contrast enhanced CT or MR imaging. Non-functioning PET tend to be larger (4~10 cm), heterogeneous, and may contain the cystic areas of degeneration and necrosis. In this article, we present the various imaging findings of PET according to recent WHO classification. (Korean J Med 2011;80:378-385)

서 론

췌장의 내분비종양(pancreas endocrine tumor, PET)은 췌장종양 전체의 약 2% 전후를 차지하며 PET는 혈액 속에 호르몬을 과다하게 분비하는 기능성(functioning)과 거의 호르몬을 생산하지 않는 비기능성(non-functioning) 종양으로 분류할 수 있다[1]. 이 두 종류의 종양은 현미경적으로 서로 잘 감별되지 않으며, 종양자체에서 분비하는 활성 신경내분비 호르몬과 연관된 임상 증상의 발현여부를 바탕으로 분류한다[2].

기능성 종양으로 분비되는 호르몬에 따라 서로 다른 증상이 나타난다. 기능성 종양의 약 60%는 혈당을 조절하는 호르몬인 insulin을 과다하게 생산하는 인슐린종이 gastrin을 과잉 생산하여 과산증에 의한 난치성 소화궤양을 일으키는 가스트린종이 약 20%이며, 나머지는 기타 호르몬을 과잉 생산하는 종양이 차지한다. 비기능성 종양은 모든 PET의 70~90%를 차지하며, 영상 검사법의 발전에 따라 우연히 발견되는 비기능성 종양이 최근에 증가하고 있다[3].

PET는 islet tumor 또는 neuroendocrine tumor라 과거에 불려졌으나, WHO에서 조직학적 기준과 기능성 유무를 중심으로 재분류하였다(Table 1) [4]. WHO 분류에 의하면 양성인 well-differentiated endocrine tumor, 저등도의 악성 종양인 well-differentiated endocrine carcinoma, 그리고 악성인 poorly-differentiated endocrine carcinoma로 구분한다. Well-differentiated endocrine tumor는 육안적으로 2 cm를 기준으로 benign과 uncertain behavior로 구분한다. 영상 소견에서 양성과 악성은 전이된 종양이 보이지 않으면 감별하기 어렵다. 가장 흔한 PET인 인슐린종의 5~10%만 악성인 반면에 다른 내분비 종양은 50~90%가 악성이다[5].

PET의 진단은 치료에 직접 영향을 미치며 췌장암과 달리 수술로써 치료가 가능한 경우가 많고 항암화학치료에 반응을 잘하며 예후가 좋기 때문에 매우 중요하다. 이 글에서는 PET의 영상의학 및 인터벤션 진단법의 장단점과 WHO분류에 따른 영상소견을 기술하고자 한다.

영상의학 진단법

영상의학 진단법으로 복부 초음파가 가장 널리 간편하게 이용된다. 초음파에서 PET는 많은 경우 분명한 경계를 갖는 저에코 병변으로 관찰된다. 그러나 복부 초음파 검사에서 PET의 발견할 수 있는 예민도는 50% 정도로 낮고 특히 병변이 작을 때와 위장관에 가스가 차 있을 때 진단의 제한 점이 있다[6].

내시경 초음파(endoscopic ultrasound, EUS)는 고해상능의 초음파로 복부 초음파의 단점을 극복하고 1 cm 이하의 작은 병변도 발견할 수 있을 뿐 아니라 병변의 발견 시 조직학적 검사까지도 동시에 진행할 수 있는 장점이 있다. 췌장의 두부나 체부의 작은 PET를 발견하는데 유용하나 미부의 종양을 발견하는데 제한 점이 있다. Anderson 등에 따르면 췌장의 PET를 발견할 수 있는 EUS의 예민도는 약 93% 정도로 우수하고 대부분의 병변은 분명한 경계를 갖는 균일한 저에코 병변으로 관찰되며 췌장의 두부에 호발한다[7]. 그러나 EUS의 예민도는 시술자의 수기능력 및 병변의 위치에 따라 달라 질 수 있다. 최근의 일부 보고에 따르면 EUS에서 악성의 비기능성 종양이 양성에 비해 종양 내부에 불규칙한 중심성 고에코를 보이는 경향이 있어 이 경우 악성의 감별에 있어 도움을 될 수도 있다[8].

복강경을 이용한 초음파 검사(intraoperative ultrasound, IOUS)는 일부 병원에서 시행되고 있는 검사법으로 인슐린종의 진단 예민도는 80% 정도이고 CT와 같이 검사 시 예민도는 93%로 향상된다[9]. IOUS는 가장 침습적 검사로 PET의 위치를 결정하는데 유용한 검사로 PET가 췌장 내에 있을 때 약 85%에서 발견이 가능하나, 췌장 밖에 위치할 때 IOUS의 예민도는 50% 정도로 낮아진다[10].

CT는 가장 널리 이용되는 PET의 수술 전 이용되는 기본 검사이다. 2000년도 초부터 소개된 multidetector CT는 과거의 고식적 CT에 비하여 작은 검사시간의 단축과 더불어 보다 정밀한 조영증강 영상을 획득할 수 있고 해상도의 증가 때문에 PET를 진단하는데 보다 우수한 진단 도구가 되었다. 역동적 조영증강 검사로 작은 병변의 발견에 도움이 되어 인슐린종의 약 80%를 발견할 수 있으나 가스트린종의 경우는 단지 40% 정도를 발견한다[11,12]. MRI는 최근에 CT와 더불어 많이 이용되고 있는 검사법으로 PET의 발견율은 CT와 유사하다[13].

PET를 진단하는 중재적 시술로 percutaneous transhepatic portal venous sampling (PTPVS)와 arterial stimulation with venous sampling (ASVS)이 있다. 이 검사법들은 CT 또는 MRI 검사에서 PET가 의심되는 환자에서 종양을 발견할 수 없을 때 시행하는 검사법으로 수술 전에 종양의 발견과 위치를 결정하는 가장 예민한 검사법이다[14]. PTPVS 검사법은 간을 통해서 문맥에 카테타를 위치시키고 비장 정맥, 상장간정맥 및 간문맥에서 호르몬 샘플링을 한다. ASVS는 카테타를 대퇴동맥을 통하여 secretagogues를 췌장에 혈액을 공급하는 비장동맥, 상간동맥 및 위십이지장 동맥에 각각 주입 후 간정맥에서 호르몬 샘플링 한다. 인슐린종의 경우 calcium glucagonate은 인슐린의 분비를 자극하는 약제로 주입 후 인슐린 양이 2배 이상 증가하면 PET로 진단할 수 있다. 이 진단법으로 인슐린종의 80% 정도는 진단할 수 있으나 췌장의 바깥쪽에 많이 발생하는 가스트린종는 약 41% 정도만 진단할 수 있다[15].

Somatostatin receptor scintigraphy (SRS)는 수술 전 PET의 발견뿐 아니라 악성인 경우 주위 림프절, 골 및 간 전이 정도를 평가하는데 유용한 검사이다[16]. SRS의 원리는 somatostatin의 analogue인 111In-DTPA-D-Phel 1 octreotide를 주입 시 somatostatin receptor가 있는 종양에 축척 되어 PET의 위치와 전이를 평가할 수 있다. SRS는 carcinoid 경우 88%, 가스트린종 경우 77%, 비기능성 PET의 경우 83%, 그리고 glucagonoma의 경우 100%에서 진단이 가능하다[17-19]. 그러나 병변의 크기와 주변장기의 침범 유무에 대한 정보가 없고, 인슐린종의 경우 50%정도에서만 somatostatin receptor를 포함하고 있어 진단율은 50~60% 정도로 낮다는 제한점이 있다[20].

18F-FDG PET-CT검사법은 18F-FDG가 PET종양에 섭취가 잘 되지 않기 때문에 진단하는데 크게 도움이 되지 않는다. 그러나 예외적으로 poorly-differentiated endocrine carcinoma의 경우 PET의 진단과 전이 정도를 평가하는데 유용할 수 있다. 그러므로 poorly-differentiated endocrine carcinoma가 의심되는 경우 18F-FDG PET-CT가 필요할 수 있다. 최근에 일부 병원에서 6-L-18F-fluorodihydroxyphenylalanine (18F-DOPA)라는 PET의 진단에 특이적인 새로운 radiotracer를 이용하여 PET 진단에 이용한다고 보고하였다[21]. 이와 같은 새로운 radiotracer를 이용한 검사는 일부에서 시도되고 있으나 더 많은 연구가 필요하다.

PET의 영상소견

Well-differentiated endocrine tumors (benign behavior)

대부분의 PET가 이 분류에 속하며 양성은 크기는 2 cm 이하이다. 기능성과 비기능성으로 나눌 수 있으며 기능성 종양의 60%는 인슐린종이다.

인슐린종의 대부분은 단발성이나 약 8% 정도가 다발성으로 발생할 수 있다. 인슐린종는 경계가 좋은 종양으로 초음파상 경계가 좋은 균일한 저에코의 병변으로 보이며 조영증강 CT에서 동맥기와 문맥기에 균일한 또는 과녁모양의 강한 조영증강을 보인다(Fig. 1) [22]. 종양의 크기가 작은 경우 췌장 모양의 변화 없이 vascular blush 양상으로 보일 수 있다. 대부분의 작은 종양의 경우 동맥기 CT에서 강한 조영 증강을 보이기 때문에 작은 종양을 발견하기 위해서는 적절한 조영 증강 영상을 획득하는 것이 중요하다. MRI에서는 췌장실질조직의 조직강도에 비해 T1강조영상에서 저 신호강도, T2강조영상에서 고 신호강도의 분명한 경계를 갖는 종양으로 관찰되며 조영증강 MRI에서 CT와 유사하게 동맥기와 문맥기에 강한 조영증강을 보인다(Fig. 2).

비기능성 PET 역시 혈관성이 풍부한 종양으로 영상 소견은 기능성 종양과 유사하며, 영상의학 소견 단독으로 기능성종양과 구분이 불가능하므로 여러 임상소견과 종합하여 감별 진단해야 된다.

신장세포암과 같은 과혈관성 종양으로 인한 췌장의 전이암의 경우 PET와 유사한 영상을 보일 수 있으나 이 경우 다발성으로 발생하는 경우가 많고 다른 장기에도 전이를 동반하는 경우가 흔하며 원발성 종양이 동반되는 점에서 PET와 감별된다(Fig. 3).

Well-differentiated endocrine tumors (uncertain behavior)

기능성과 비기능성으로 구분 할 수 있으며 크기는 2 cm 이상이다. 기능성으로 glucagonoma와 somatostatinoma 등이 흔하다 이들은 영상 소견으로 기능성 인슐린종과는 달리 내부의 낭성 변화, 괴사 및 출혈 등이 보일 수 있다.

비기능성 종양은 크기가 크고, 경계가 좋은 종양으로 중등도에서 고등도로 조영증강되고 낭성 변화와 석회화를 보일 수 있으나 주변 혈관의 침범은 없다(Fig. 4) [22]. 일반적으로 췌관의 폐색을 일으키지 않으나 드물게 췌관의 확장을 동반하는 경우도 있다. 젊은 여성에서 크기가 크고 경계가 좋으며 내부에 낭성 변화나 출혈을 동반한 비기능성 종양이 발생한 경우 solid pseudopapillary tumor (SPT) 감별이 힘들 수 있다. 그러나 SPT의 경우 PET에 비해 종양이 좀 더 불균질하며 약한 조영증강을 보이는 경향이 감별에 도움이 된다[23].

Well-differentiated endocrine carcinoma

기능성 종양으로 가스트린종이 가장 흔하다. Gastrinoma의 임상 증상은 높은 gastrin 수치, 산의 과분비 및 Zollinger-Ellison 증후군(산의 과분비, 설사, 재발되는 소화성 궤양) 등이 생긴다. 발생 위치는 췌장 두부가 흔하나 인슐린종과 달리 췌장 외에 위치해 발생하거나 다발성 병변을 보이는 경향이 있다[22]. 췌장 외에 생기는 가스트린종의 90%는 ‘gastrinoma triangle (상부 경계는 총간관과 담낭관이 만나는 부위, 하부 경계는 십이지장의 두 번째와 세 번째 부위, 내측 경계는 췌장의 경부와 체부가 나뉘는 부위)’의 십이지장과 위벽, 림프절 등에서 발생한다(Fig. 5). 조영 증강 동맥기 CT에서 비기능성 PET와 마찬가지로 조영증강이 잘되는 종괴로 관찰되며, 소화성 궤양과 위벽 비후를 시사하는 위벽두께의 증가가 동반되는 소견이 진단에 도움이 된다[22]. MRI에서 다른 내분비 종양과 같이 T1강조영상에서 저 신호강도를 T2강조영상에서 고 신호강도를 보이고, 조영 증강 영상에서는 인슐린종에 비해 약한 조영증강을 보인다. 가스트린종의 경우 악성 병변이 많아 70~80%에서 진단 당시 간이나 림프절로의 전이가 발견된다[24].

비기능성 종양은 크기가 3~24 cm 이상으로 크며 약 90%가 악성이다. 악성을 시사하는 소견은 괴사, 주변 장기 침범 및 결절형 석회화 등이다(Fig. 6). 악성 췌장선암과 유사한 영상소견을 보일 수 있으므로 수술 전 선암과 감별이 필요하다. 비기능성 암은 석회화가 흔한 반면, 혈관침범과 췌관의 폐쇄가 없으며, desmoplastic reaction이 없다는 점이 선암과 감별점이다[24].

Poorly-differentiated endocrine carcinoma

고등도의 암으로 조직학적으로 폐의 소세포암과 유사하다. 췌장 종괴로 관찰되며 주변 림프절 증대 및 간 전이가 관찰된다. 그러나 췌관의 폐쇄가 없는 점에서 선암과 차이가 있으나 림프종이나 전이암과 감별이 어려울 수 있다[22]. 간 전이가 의심되는 경우 작은 병변을 진단하는데 조영증강 CT에 비해 MRI가 유리하다(Fig. 7). 따라서 간 전이에 따른 수술 가능여부를 판단하기 위해서는 MRI 검사를 고려해야 한다.

결 론

PET의 영상 소견은 기능성과 악성도 유무에 따라 다양하다. 기능성이며 양성인 PET는 크기가 2 cm 이하로 경계가 좋으며 균질하게 조영증강이 잘된다. 반대로 비기능성이며 악성도가 증가할수록 종괴의 크기가 4~10 cm로 크며 불균질하게 조영증강되며 내부에 낭성 괴사, 석회화 등의 이차적인 변화가 동반될 수 있다. PET를 발견하는데 초음파, CT와 MRI 등이 가장 널리 이용되고 있다. 그러나 수술 전 발견하지 못하는 경우 내시경 초음파나 침습적인 인터벤션 기법 등이 도움이 될 수 있다.
