Abstract

본 증례는 간경화 환자에서 non-O1 V. cholerae에 의한 고름근육염이 발생한 국내 첫 예이다. 이러한 기저 질환이 있는 환자에서 고름근육염의 원인균으로 non-O1 V. cholerae도 고려하여야 한다.

Trans Abstract

A 63-year-old man with underlying liver cirrhosis was admitted with painful swelling of the right thigh. We identified a non-O1 Vibrio cholerae strain in blood cultures and multiple pyomyositis in the lower limbs. Non-O1 V. cholerae strains have caused several well-studied food-borne outbreaks of gastroenteritis and have been responsible for sporadic cases of otitis media, skin and soft tissue infection, and bacteremia. Skin and soft tissue infection due to non-O1 V. cholerae is rare and is commonly associated with the presence of chronic underlying disease, such as liver cirrhosis, diabetes mellitus, an immunocompromised state, or a hematological malignancy. We report the first case of pyomyositis caused by non-O1 V. cholerae in Korea. Physicians should consider non-O1 V. cholerae strains as a pathogen that can cause pyomyositis. 서 론

고름근육염은 골격근의 급성 세균 감염으로 고름이 근육 내에서 생성되어 인접한 피부, 연조직, 뼈에 이차적인 감염을 일으키지 않고 축적되어 있는 상태이며 이는 감염의 선행부위 없이 발생하고 가장 흔한 원인균은 Staphylococcus aureus로 알려져 있다[1,2]. 고름근육염이 발생할 수 있는 위험요인으로 당뇨, 알코올성 간질환, 스테로이드 치료, 면역억제 상태, HIV 감염이 있으며 하지의 큰 근육들에 주로 침범한다[2-4]. Non-O1 Vibrio cholerae에 의한 피부 및 연조직감염 중 고름근육염의 예가 국외에서는 1예, 국내에서는 보고된 바가 없다[5]. 저자들은 간경화 및 림프종 환자에서 발생한 non-O1 V. cholerae에 의한 고름근육염 1예를 경험하여 문헌고찰과 함께 보고한다.

증 례

환 자: 63세, 남자

주 소: 우측 넓적다리 통증

현병력: 최근 해산물 섭취, 오염된 해수의 접촉 및 특별한 여행력이 없었던 자로 내원 한달 전부터 하루 3~4회 물설사가 지속되며 근육통이 있던 중 내원 하루 전 계단에서 넘어지면서 우측 넓적다리를 수상하였고, 내원 당일 우측 넓적다리의 통증이 심하고 움직일 수 없어 응급실 경유하여 입원하였다.

과거력: 수년 전 알코올에 의한 간경화 진단받았으나 치료받지 않고 지냈으며 당뇨나 암을 진단받지는 않았다.

거주지: 인천광역시 서구

사회력: 5회/주, 소주 1병씩, 40년, 40갑년 흡연

직업력: 청소용역

가족력: 특이사항 없음

이학적 소견: 신장은 172 cm, 몸무게는 75 kg이었다. 활력징후는 혈압 140/80 mmHg, 맥박수 분당 84회, 호흡수 분당 20회, 체온 36.8℃였고, 급성 병색을 보였으며 의식은 명료하였다. 지속적인 물설사는 있으나 복통을 호소하지는 않았고, 진찰소견에서 복부는 물렁, 납작하였고, 장음은 정상이며 우측 하복부의 경한 압통이 있었으나 반동압통은 없었다. 우측 넓적다리의 심한 통증을 호소하였고, 피부의 손상이나 색의 변화, 수포 등은 관찰할 수 없었으나 열감, 56 cm 둘레의 종창 및 압통이 있었다. 이외 다른 진찰 소견에서는 특이사항 없었다.

검사실 소견: 말초혈액 검사에서 혈색소 14.9 g/dL, 적혈구 용적 42.8%, 백혈구 19,300/mm3 (호중구 33.9%, 림프구 54.6%, 단핵구 11.2%), 혈소판 25,000/mm3이었다. C 반응성단백 15.02 mg/dL, 적혈구침강속도 17 mm/hr이었다. 혈청 생화학 검사에서 총 단백 5.7 g/dL, 알부민 2.8 g/dL, 총 빌리루빈 4.4 mg/dL, 직접형빌리루빈 1.96 mg/dL, alkaline phosphatase 63 IU/L, gamma-glutamyl transpeptidase 372 U/L, aspartate aminotransferase 165 IU/L, alanine aminotransferase 84 IU/L, BUN/creatinine 29.0/1.3 mg/dL이었다. 혈액응고 검사에서 PT (INR) 1.86, aPTT는 44.7 sec이었다. 소변 검사에서는 특이사항 없었다. 입원 3일째 말초혈액 배양검사에서 non-O1 V. cholerae가 확인되었고, 대변 배양검사에서 의미있는 균 검출은 없었다.

방사선학적 소견: 내원 후 시행한 넓적다리 전산화단층촬영에서 주변의 조영이 증강되는 다초점 병변이 우측 넓적다리 모음근(adductor muscle)에서 9 × 4.5 cm으로 가장 컸으며, 그 외 우측 정강앞근(tibialis anterior muscle), 우측 가자미근(soleus muscle), 좌측 긴엄지굽힘근(flexor hallucis longus muscle)에서 관찰되었다(Fig. 1). 복부 전산화단층촬영에서 작고 쭈그러든 간과 비장비대, 다발성 대동맥주위, 창자간막 림프절 비대, 미만성 결장벽의 비후 소견을 보였다.

치료 및 경과:입원 후 정맥 수액요법을 유지하면서 경험적으로 cefazolin (2 g q 8hr) 정맥 주사 치료를 시작, 입원 3일째부터 말초혈액 배양검사에서 non-O1 V. cholerae가 확인되어 ciprofloxacin (200 mg q 12hr) 정맥 주사로 바꿔 총 25일간 유지하였다. 물설사는 호전되었으나 우측 넓적다리 통증, 종창과 발열이 지속되어 입원 10일째 가장 큰 크기(9 × 4.5 cm)를 보이는 우측 넓적다리 모음근의 고름집에 대해 절개배액술을 시행하였다. 배액된 고름은 혈성, 장액성으로 절개배액술 이후 우측 넓적다리 통증과 종창이 호전되었고, 발열은 사라졌다. 절개배액술 당시 시행한 조직 배양검사에서 균 검출은 없었다. 이후 추적 말초혈액 검사에서 지속적인 백혈구 증가증, 빈혈과 혈소판감소증으로 말초혈액바른표본과 골수생검을 시행하였고, marginal zone B cell lymphoma를 진단하고 항암화학요법 후 입원 39일째 걸어서 퇴원하였다.

고 찰

Vibrionaceae는 그람음성 막대기균으로 V. cholerae, V. parahaemolyticus, V. vulnificus, V. alginolyticus가 대표적인 사람 병원체이며 V. choleraeV. parahaemolyticus는 위창자염을 통한 급성 설사 질환을 일으키고, V. vulnificus는 상처감염과 패혈증, V. alginolyticus는 피부나 귀의 얕은 상처감염의 원인균으로 알려져 있다[6,7]. 이 중 V. cholerae는 과거 범세계적 유행으로 수많은 사망자를 발생시켰고, O1, O139 V. cholerae가 대표적인 혈청군이다[8].

Non-O1 V. cholerae는 O1 항혈청에 응집되지 않는 V. cholerae를 의미하며 주로 위창자염을 일으키나 드물게 패혈증, 중이염, 피부 및 연조직감염, 복막염, 폐렴, 뇌염 등의 장외 감염을 일으키고 침습적인 non-O1 V. cholera 감염일 경우 치명률이 높은 것으로 알려져 있다[9,10]. 이러한 장외 감염을 일으킬 수 있는 숙주 요인으로는 간경화를 포함한 만성 간질환이 있으며 그 외 당뇨, 암, 기타 면역억제 상태 등이 있다[11,12]. 또한 풍토지역에서 해산물 섭취, 오염된 해수의 접촉이 위험요인으로 알려져 있지만 Lin 등의 연구에서는 28명의 non-O1 V. cholerae에 의한 세균혈증 환자에서 9명의 환자에서만 해산물 섭취나 해수의 접촉의 기왕력이 있다고 보고하였다[13].

Vibrio species에 의한 피부 및 연조직감염은 물집 형태의 피부병변이나 국소의 연조직염에서부터 2차적인 패혈증을 동반한 심한 괴사성 연조직감염까지 다양하게 나타난다[12]. Non-O1 V. cholerae에 의한 피부 및 연조직감염이 국내에서는 물집 형태의 피부병변을 포함한 연조직염이 2예, 괴사성근막염이 1예 보고되었다[14-16]. 전세계적으로 Couzigou 등[5]이 간경화 및 HIV 감염 환자에서 튀니지 여행 후 non-O1 V. cholera에 의한 하지의 고름근육염 1예를 보고하였고, 국내에서는 고름근육염으로 보고된 바 없다. 동물 실험에서 S. aureus를 정맥 주사 후 근육에 외상을 가했을 때 고름근육염이 발생하였고, 이러한 locus minoris resistentiae가 고름근육염의 발병기전으로 생각된다[17].

본 증례는 알코올에 의한 간경화를 진단받은 환자에서 최근 해산물 섭취, 오염된 해수의 접촉 및 특별한 여행력이 없던 상태로 non-O1 V. cholerae에 의한 세균혈증이 확인되었고, 다발성 고름근육염의 형태로 발현되었다. 환자는 ciprofloxacin의 사용과 고름근육염의 절개배액술로 호전되었다. 또한 내원 하루 전 계단에서의 외상력이 고름근육염의 발생, 증상호전 후에 진단된 림프종이 non-O1 V. cholerae에 의한 장외감염의 발생에 영향을 미쳤을 것으로 생각된다.
