Trans Abstract

We reviewed the pathophysiology of gout in this review article. Gout occures as a response to monosodium urate crystal(MSU) that is present in joints, bones and soft tissue. Hyperuricemia in gout is mainly due to the decreased renal clearance of uric acid and is considered to be the cause of MSU formation. However, hyperuricemia alone does not explain the development of gout. Acute gout attack is due to the response to MSU, which is derived by innate immunity. Tophi are deposits of MSU and cell debris surrounded by granulomatous inflammation. (Korean J Med 2011;80:251-254)

서 론

통풍(gout)은 고요산혈증에 의해 생성된 요산염결정(monosodium urate crystal, MSU)으로 인해 발생하는 관절염이다. 20세기 중반 편광현미경이 개발되면서 통풍 환자의 관절액에서 요산염결정을 확인할 수 있게 되었고, 고요산혈증과 요산염결정이 질병의 원인임이 밝혀졌다[1]. 최근 고요산혈증의 기전에 대한 이해가 높아지고[2,3] 우수한 치료효과를 보이는 약제들[4]이 개발되어 통풍의 진단과 치료에서 많은 발전이 있었다.

통풍은 과거에는 육식위주의 식생활을 하는 서양에 흔하였으나 최근 우리나라도 식생활이 서구화되고 비만이 증가함에 따라 통풍의 발생률이 늘어나고 있는 추세이며 통풍은 오늘날 국내에서도 쉽게 접할 수 있는 질병이 되었다. 이러한 관점에서 통풍의 원인, 진단, 치료 그리고 예방에 대해 충분히 숙지하는 것이 도움이 되겠다. 본 종설에서는 통풍의 병태 생리에 대한 최신 지견을 정리해 보고자 한다.

고요산혈증

요산(uric acid)은 핵산(nucleic acid)의 기본 구성 단위 중 하나인 퓨린 뉴클레오티드(purine nucleotide)의 대사과정에서 발생하는 화합물이다. 퓨린 대사과정에서 ribose 5-phosphate는 대사 과정을 거쳐 IMP (inosine monophosphate)로 전환되고 IMP는 다시 AMP (adenine monophosphate), GMP (guanine monophosphate)로 대사되고 이들은 다시 DNA (deoxyribonucleic acid), RNA (ribonucleic acid)의 기본 구성 단위인 adenine, guanine으로 합성된다. 이러한 퓨린 대사 과정에서 hypoxanthine, xanthine이 합성되는데 이들은 xanthine oxidase에 의해 hypoxanthine은 xanthine으로, xanthine은 다시 uric acid로 전환된다(Fig. 1). 요산은 이렇게 인체 내에서 직접 만들어지기도 하고(de novo synthesis) 음식물을 통해 섭취한 퓨린으로부터 생성될 수도 있다.

대부분의 요산은 신장 및 신장외 경로(extrarenal route)를 통해서 배설되며 생체 내에서 분해되는 비율은 2% 미만으로 미미하다. 신장외 경로로는 타액, 위액, 이자액, 소장, 대장 등이 있는데 전체 요산 배설량의 3분의 1정도를 차지한다.

고요산혈증은 요산의 배설이 감소하거나, 요산이 과다하게 생성되거나 혹은 퓨린을 과도하게 섭취할 경우에 발생하는데 고요산혈증의 85~90%는 신장에서의 요산 배설 감소가 원인이다. 인체는 항상성을 유지하기 위해 요산의 하루 생산량과 배설량이 같도록 하려 하는데 고요산혈증 환자는 요산 배설의 능률(efficiency)이 떨어져 있어 효과적인 요산 배설과 항상성 유지를 위해서는 정상인 보다 높은 요산 농도가 필요하게 된다[5]. 결과적으로 혈중 요산 농도가 정상인보다 2~3 mg/dL 높은 상태에서 요산 생성-배설의 항상성이 유지되게 된다. 이러한 요산의 신장 배설 감소를 일으키는 원인들은 매우 다양하다(Table 1).

신장 배설능 감소를 제외한 나머지 10~15%의 고요산 혈증은 요산의 과다 생성이 원인인데 이들은 대부분 퓨린 대사 과정의 유전적 결함, ATP 대사 과정의 문제, 세포 전환 속도가 증가하게 되는 질환 등이 원인이다.

요산염 결정의 생성

정상 체온의 혈액에서 요산의 포화 농도는 대략 7 mg/dL인데 그 이상의 농도에서 요산은 과포화(supersaturation) 상태로 체액 내에 존재하게 된다. 오랜 기간 혈중 요산 농도가 높게 유지되어 과포화 상태가 지속될 경우 요산염 결정이 만들어져 관절과 관절 주위조직에 침착하게 되고 통풍이 발생하게 된다. 요산의 포화 농도는 37℃에서 7 mg/dL이지만 온도가 내려갈 경우 포화농도는 급격히 떨어지게 된다. 통풍 발작이 중심부에서 먼 부위인 엄지발가락, 발목 등에서 호발하는 것은 이들 관절 내부의 온도가 중심부 체온보다 낮고 그에 따라 요산의 포화농도가 떨어지기 때문으로 설명이 가능하다. 그러나 대부분의 고요산혈증 환자는 통풍이 발생하지 않기 때문에 고요산혈증만으로 통풍의 발병을 설명하기는 힘들다[6]. 요산염 결정 발생에 대해서 많은 연구가 진행되었지만 아직 정확한 기전은 밝혀지지 않았다. 요산의 핵화(nucleation)와 그에 따른 요산염 결정의 성장에 있어 고요산 혈증의 정도가 중요한 것으로 알려져 있다.

급성 통풍 발작

급성 통풍 발작은 요산염결정에 대한 신체의 면역 반응에 의해 일어나는데 주로 선천 면역계(innate immunity)가 중요한 역할을 한다. 관절내 요산염결정은 보체계를 활성화시켜 C5a를 생성하게 하고 이는 백혈구들을 화학주성(chemotaxis)으로 끌어당기고 membrane attack complex를 만들게 된다[7]. 또한 요산염 결정은 Toll-like receptor를 통하여 대식세포(macrophage), 활액막 세포(synovial lining cell) 및 중성구(neutrophil)를 활성화시킬 뿐만 아니라 NFκB와 같은 전사인자(transcription factor)를 만들어내어 급성 통풍 발작의 발생에 있어 중심적인 역할을 한다[8,9]. 이러한 요산염결정에 대한 염증반응으로 인해 급성 통풍 발작이 일어난 관절 부위엔 부종, 발적, 압통이 발생하게 된다.

급성 통풍 발작은 특별한 치료 없이도 7~10일 내에 저절로 소실되게 되는데 이에 대한 설명은 다양하다. 급성 염증 반응과정에서 발생한 단백질들이 요산염 결정을 감싸게 되어 백혈구의 반응성을 감소시키게 되는데 apolipoprotein B 100 혹은 apolipoprotein E가 요산염 결정을 감싸면 백혈구의 반응성이 떨어지게 된다[10]. 또한 adrenocorticotropic hormone(ACTH)와 같은 melanocortin이 급성 발작의 소실에 기여하기도 하며[11] 염증 반응의 주요한 조절인자인 peroxisome proliferator activated receptor-γ (PPAR-γ)가 기여한다는 보고도 있다[12].

통풍 결절

급성 통풍 발작이 반복적으로 나타나는 만성 통풍 환자에서 관찰할 수 있는 통풍 결절(tophus)은 조직학적으로 요산염 결정과 조직 파편을 둘러싸고 있는 육아종이다. 통풍 결절은 견고한 결체조직에 의해 둘러싸여져 있는데 결절 내부엔 다수의 대식세포들이 존재하며 표면에 성숙한 분화단계를 보이는 표지자들을 표현하고 있으며 많은 세포들이 세포자멸사(apoptosis)하고 있다. 반면, 주변부의 대식세포들은 최근의 세포이동을 나타내는 표지자들을 가지고 있는데 이로 미뤄볼 때 통풍 결절은 대식세포들이 지속적으로 모이고, 염증반응이 활성화되고, 대식세포가 성숙하고 사멸하는 역동적인 과정의 부산물임을 알 수 있다[13].

통풍 결절은 연골의 파괴와 골미란(bone erosion)에 관여한다. 통풍 결절 내의 대식세포들은 matrix metalloproteinase-2(MMP-2)와 MMP-9을 분비하는데 이들은 제4형, 제5형 콜라젠(collagen), 엘라스틴(elastin)과 젤라틴(gelatin)을 분해할 수 있다[14]. 이외에도 결절의 버팀질 세포(stromal cell), 연골세포, 활액막세포 역시 MMP를 분비할 수 있는데 이들 세포들이 분비하는 MMP에 의해 통풍 결절 주변 조직들이 파괴된다[15].

결론

통풍은 요산염 결정에 대한 면역 반응으로 일어나는 관절염으로 고요산혈증이 가장 중요한 요산염결정 발생의 위험 인자이며 원인으로 지목되고 있다. 그러나 단순히 고요산혈증만으로 통풍의 발생을 설명할 수는 없기 때문에 발병의 원인과 병태 생리에 대한 추가적인 연구가 필요하다.
