Trans Abstract

Colorectal cancer (CRC) is 2nd to 3rd common cancer and the annual incidence of CRC is increasing in Korea. The liver is the most frequent metastatic site of colorectal cancer and approximately one forth of patients presents with liver metastasis at initial diagnosis. Without treatment, patients with colorectal liver metastasis (CRLM) have a poor prognosis; however, long-term survival benefits and even cure have been reported in a portion of patients undergoing surgical resection of liver metastasis. In addition, advances in chemotherapeutic agents, imaging, and surgical techniques can increase the number of patients who are eligible for curative resection. In the past 10 years, combination chemotherapy with 5-fluorouracil (5-FU)/leucovorin (LV) and oxaliplatin or irinotecan was becoming the standard chemotherapy of treatment for metastatic CRC including CRLM. Furthermore, the combined use of targeted agents, such as cetuximab and bevacizumab, plus standard chemotherapeutics revealed more improvements in response rates and survival. Even in patients with resectable CRLM, perioperative chemotherapy with surgical resection could improve in progression free survival. However, the curable portion of patients with CRLM was still less than 20~30%, more detail management based on multidisciplinary team approaches should be needed. (Korean J Med 79:125-132, 2010)

서 론

최근 우리나라에서 대장암의 발생률은 점점 증가하는 추세로 2006년, 2007년 국가 암 발생 통계보고를 보면 대장암의 발생빈도는 2~3위를 차지하였고, 전체 암환자의 12.7%를 차지한다1). 대장암은 내시경 검사 등 조기발견을 위한 선별검사가 가능함에도 불구하고 아직 대부분의 경우 진행성 또는 전이성 상태로 진단된다. 간은 대장암에서 전이가 가장 호발하는 부위로 간 전이의 발생은 처음 진단 시 약 25%에서 동반되고 병의 자연경과 중 전체 대장암의 50%에서 간 전이가 발생한다2,3). 간 전이는 진행된 정도를 병기로 나타낼 때 4기 상태로 적극적인 치료를 하지 않는다면 기대여명이 4개월 정도로 매우 나쁘며 예전에는 완치를 기대할 수 없이 고식적 항암제치료에 의존해 왔다. 하지만 최근 항암제의 발전과 수술방법의 향상으로 절제 가능한 간 전이의 경우에는 완치를 기대할 수도 있어 전이 상태와 치료방법에 따라 매우 다른 경과를 기대할 수 있다2,3).

대장암의 간 전이 상태는 치료방법을 고려하여 세 가지 유형으로 나누어 볼 수 있는데, 첫 번째는 수술로 완전절제가 가능한 경우이고 두 번째는 어떤 경우에도 수술적 절제가 어려운 경우, 세 번째는 진단 시는 수술적 절제가 어려우나 항암제치료 후 절제가 가능해지는 경우이다(그림 1).

두 번째의 경우에는 간 전이의 치료방법으로 고식적인 항암제치료가 유용할 것이며 첫 번째의 경우에도 수술 전후로 항암제치료를 고려해 볼 수 있을 것이나 우선 수술적 완전절제가 중요한 치료방법이다. 세 번째의 경우가 항암제 치료를 먼저 시행하여 수술적 절제가 가능하도록 간 전이 정도를 호전시킨 후 수술하는 방법의 치료가 필요한데 이 때에는 특히 어떤 항암제치료가 유용할 것인지, 또 어느 정도의 기간으로 항암제를 사용할 것인지 등에 있어 여러 방법이 시도되고 있다.

수술적 절제가 ‘불가능한’ 간 전이 상태란?

대장암의 간 전이를 수술할 경우 생존율의 호전과 완치가능성이 알려지면서 우선 간 전이가 동반된 환자에서 간 전이의 절제 가능성 여부는 완치를 위한 치료 결정에 매우 중요한 인자가 되었다. 하지만 절제 가능한 간 전이를 정의하는 데 있어서는 매우 다양한 분류가 시도되고 있다4).

과거에는 절제 불가능한 간 전이 상태를 주로 간 전이 병변의 크기가 크거나(5 cm 이상), 위치상 수술이 어려운 부위에 있는 경우, 여러 개의 전이가 있는 경우(3~4개 이상), 간이외의 다른 장기에 전이가 동반된 경우, 고령 등으로 정의하였다4,5). 하지만 최근에는 이러한 절제 불가능에 대한 결정이 병원 간 또는 수술하는 외과의에 따라 다르다. 특히 경험 많은 외과의가 수술 할 경우 여러 개의 전이가 있더라도 부분적 절제로 남아 있는 간이 30% 이상 유지된다면 어떤 위치에 몇 개의 전이가 있던지 대부분 수술이 가능해진다. 또한 타 장기의 전이도 절제 가능한 부분이냐에 따라 간 절제와 동시 시행이 가능하므로 ‘절제가 불가능한’ 간 전이의 조건이 되지 않는다4,5). 나이 또한 수술이 불가능한 간 전이의 조건이 되지는 않는다. 하지만 역시 70세 이상 고령에서 수술 후 사망률이 4.5%로 70세 이하의 1.5%보다 높았다6).

초기에 절제가 불가능한 간 전이의 치료

전이성 대장암에서 항암제치료의 근간이 되는 약제는 5-fluorouracil (5-FU)로 지난 40여 년간 널리 사용되어 왔으며 1990년대에 들어서 oxaliplatin, irinotecan과 같은 약제가 개발되어 5-FU, leucovorin (LV)과 병용치료 시 반응률과 생존율의 향상이 입증되어 현재 표준항암화학요법으로 자리잡게 되었다7). 절제가 불가능한 간 전이를 동반한 대장암 환자에서도 5-FU/LV 지속정주를 근간으로 하여 oxaliplatin 또는 irinotecan을 같이 투여하는 FOLFOX, FOLFIRI 요법이 표준치료로 사용되고 있으며 여기에 oxaliplatin과 irinotecan을 5-FU/LV과 동시에 3제 요법으로 사용하는 FOLFOXIRI 요법이 시도될 수 있으며 최근에 개발된 표적치료제를 이용하여 EGFR (epidermal growth factor receptor)를 표적으로 하는 cetuximab 또는 VEGF(vascular endothelial growth factor)를 표적으로 하는 bevacizumab의 병용요법이 시도되고 있다. 초기에 절제 불가능한 간 전이가 동반된 환자에서 이러한 고식적 항암제 치료 후 약 10~30%의 환자가 종양의 축소로 수술 가능한 상태로 호전될 수 있음이 지난 10여 년간 많은 임상연구에서 보고된 바있다2,3). 이렇게 항암화학요법과 수술의 병용치료로 절제가 가능한 환자의 5년 생존율은 약 30~50%에 달하며 현재 임상에서 항암제치료로 간 전이를 가능한 줄인 상태에서 절제수술을 진행하는 치료가 흔히 시도되고 있다4).

그러면 과연 어떤 항암제를 사용하는 것이 가장 효과적일 것인가에 대해 의문이 생기게 되는데, 간 전이를 동반한 대장암환자에서 시행되는 ‘선행항암화학요법’의 가장 이상적인 조건은 우선 간 전이를 효과적으로 줄여 주면서 미세전이까지 제거하여 수술 후 재발률을 낮춰 줄 수 있는 약제라 하겠다. 수술 시 완전 절제율(R0 resection)은 항암제의 반응정도와 비례하는 경향을 보인다8). 전이성 대장암의 표준요법제인 FOLFOX와 FOLFIRI는 대규모의 후향적, 전향적 연구에서 12.5~38%의 간 전이 절제율과 30~50%의 5년 생존율을 보였다(표 1)9-14).

Oxaliplatin과 irinotecan을 5-FU/LV과 동시에 투여하는 3제항암화학요법(FOLFOXIRI)의 경우 FOLFIRI의 2제요법과 비교하였을 때 완전 절제율의 증가가 2개의 3상 임상연구에서 관찰되었다. 먼저 시행된 Hellenic Oncology Research Group의 연구에서는 두 치료군 간에 전체생존기간이나 진행까지의 기간, 반응률에서는 통계적으로 유의한 차이가 없었으나 완전절제율은 FOLFOXIRI 치료군에서 10%로 FOLFIRI군의 4%에 비해 높았다(p=0.08)15). 또 다른 3상 연구에서는 FOLFOXIRI군의 반응률이 60%로 FOLFIRI군의 34%에 비해 높았고 (p<0.0001) 전체 환자에서 완전절제율은 15%와 6% (p=0.033), 간 전이만 있는 환자에서 36%와 12% (p=0.017)로 FOLFOXIRI군에서 높았으며 무진행 생존기간(progression free survival, PFS)과 전체생존기간(overall survival, OS)도 FOLFOXIRI 군에서 높았다(표 1)16).

최근에는 대장암에 효과적인 cetuximab 또는 bevacizumab 등의 표적치료제를 5-FU/LV와 oxaliplatin 또는 irintecan과 병용하는 3제요법에서 항암치료 효과의 증가와 생존기간 연장이 보고되었으며 이러한 치료 효과에 더하여 완전 절제율의 증가도 기대되었다17-19). Cetuximab의 병용효과를 연구한 대규모 3상 임상시험(CRYSTAL과 OPUS trial)에서 cetuximab을 추가한 군에서 수술적 절제율과 완전절제율이 더 높았다(표 2)20-23). 특히 cetuximab을 포함하는 병용치료는 KRAS 유전자의 돌연변이가 없는 환자에서 더 높은 반응률과 생존기간의 향상을 보여 KRAS 돌연변이가 보이는 경우 cetuximab의 병용이 별다른 차이를 보이지 않아 현재로는 KRAS 돌연변이를 동반한 환자에서 cetuximab의 사용은 추천되지 않는다. 또한 최근 발표된 CELIM 연구에서는 소규모이긴 하나 절제 불가능한 대장암 간 전이 환자에서 cetuximab과 FOLFOX, FOLFIRI의 병용이 높은 반응률과 완전절제율을 보여 간 전이를 동반한 대장암환자에서 절제 가능성을 높일 수 있으리라 기대된다24). Bevacizumab의 경우에도 NO16966 3상 연구에서 FOLFOX/XELOX에 bevacizumab의 추가가 무진행 생존 기간의 연장을 보이며 완전절제의 가능성도 약간 상승시켰으나 반응률의 차이는 없었다25).

이상의 결과를 볼 때 아직 어떤 항암화학요법이 가장 효율적인지에 대해서는 결론을 내리기 어렵다. 또한 이러한 임상시험에서 완전절제율이 생존기간의 연장으로 연결될 수 있을지에 대해서도 추가적인 연구결과들을 기다려 보아야 할 것이다.

절제 가능한 간 전이를 동반한 대장암의 치료

대장암이 간으로 전이된 환자의 약 15~20% 정도는 진단 시 수술적 절제가 가능하며 50% 정도의 5년 생존율을 기대할 수 있다2,3). 하지만 많은 수의 환자가 수술 후 재발되어 일부의 환자에서만 완치를 기대할 수 있다. 이러한 수술 후 치료성적의 향상을 위해 최근에는 선행항암화학요법(neoadjuvant chemotherapy)이 시행되고 있는데 이러한 선행항암화학요법에서 기대할 수 있는 장점은 완전절제의 가능성을 증가시키고, 간 절제 범위를 줄일 수 있으며, 수술 후 회복을 빠르게 하고, 미세전이를 치료하여 재발률을 줄이고, 수술 전에 항암제의 반응 정도를 미리 파악하여 비효율적인 치료를 줄임으로써 무병 생존기간을 연장시키고자 한다. 이러한 선행항암화학요법의 효과를 확인하기 위해 대규모 3상 임상연구(EORTC40983 study)가 시행되었다26). 이 연구는 간 전이 개수가 4개 이하인 절제 가능한 간 전이를 동반한 364명의 환자를 수술만 시행하거나 수술 전 후로 6회의 FOLFOX4 항암제치료를 받는 군으로 무작위 배정하여 무진행 생존기간(progression free survival)을 포함하여 치료성적을 비교하였는데, 3년 무진행 생존율은 항암제치료를 수술 전후로 받은 경우가 전체 환자 대상 시 7.3% [HR 0.79, p=0.058]의 증가를 실제 치료받은 환자 대상 시 8.1% [HR 0.77, p=0.041], 수술까지 시행한 환자 대상 시 9.2% [HR 0.73, p=0.025]의 증가를 보였다(그림 2). 수술 후 부작용으로는 회복 가능한 합병증이 수술 전후 항암제 치료군에서 높았으나(25% vs. 16%, p=0.04) 수술관련 사망자는 수술만 받은 경우 2명, 수술 전후 항암제 치료군에서 1명으로 차이가 없었다. 이러한 선행항암화학요법의 단점으로는 수술이 지연되고 수술 후 회복 가능한 합병증은 증가될 수 있으며 선행항암화학요법을 오래 지속할 경우 항암제에 의한 간 손상으로 수술 합병증이 증가될 우려가 있다. 또한 작은 간 전이 병변의 경우 수술 전항암제치료반응이 좋을 경우 간 전이부위의 완전소실로 적절한 수술적 절제가 오히려 불가능해지는 경우가 있을 수 있다.

최근에는 기존 2제 병용항암화학요법에 표적치료제를 같이 사용하는 선행항암화학요법의 효과에 대한 연구가 시도되고 있다27).

최근 간 전이에 대한 치료방법 중 하나로 대두되는 것은 고주파 절제술(radiofrequency ablation)이다. 아직 간 절제술을 대신할 표준치료로 적용되지는 못하지만 수술과 병용하여 다른 동반질환 등으로 수술이 불가능한 환자에서 간 전이 부위를 완전히 제거하는 데에 일조하고 있다28).

간 절제 수술 후 보조항암화학요법

대장암의 수술 후 보조항암화학요법의 효과는 대장암 3기의 환자에서 재발 가능성을 낮추고 생존연장의 효과가 있음이 이미 여러 연구에서 입증된 바 있다29,30). 이러한 배경을 근거로 간 전이가 동반된 대장암 4기에서도 원발 대장암과 간 전이가 완전 절제된 경우 수술 후 보조항암화학요법이 도움이 될 것인지에 대해서도 소수의 임상연구가 진행되었다. FFCD ACHBTH AURC 9002 임상연구에서는 173명의 환자를 완전절제수술(R0) 후 경과관찰군과 R0 수술 후 6개월 간의 5-FU/LV 급속정주 치료군으로 무작위 배정하여 5년 무병생존기간을 비교하였는데, 각각 27%와 34%로 단일변수분석에서는 통계적으로 유의한 차이가 없었으나 Cox 다변량분석에서 수술 후 항암제투여를 받은 경우가 무병생존기간 연장의 효과를 보였으며 전체생존기간도 연장되는 경향을 보였다31). EORTC/NCIC CTG/GIVIO 임상연구32)도 앞의 임상시험과 유사한 경향을 보였으며 두 연구를 통합하여 분석한 결과에서도 통계적 유의성이 뚜렷하지는 않았지만 무진행 생존기간과 전체생존기간이 연장되는 경향을 보였고, 다변량분석에서 수술 후 항암제치료가 유의한 예후인자로 나타났다33). 이러한 결과는 다른 후향적 연구에서도 수술 후 항암제치료가 생존기간의 연장을 보여 유사하였다34,35). 또한 완전 절제수술(R0) 후 5-FU/LV의 지속정주와 FOLFIRI 치료를 무작위 배정한 연구에서는 FOLFIRI군과 5-FU/LV 지속정주군에서 각각 중앙 무병생존기간 24.7개월과 21.6개월, 1년 생존율 77%와 51%, 2년 생존율 63%와 46%의 결과를 보였다36). 현재 간 전이를 동반한 대장암에서 완전절제 후 보조항암화학요법은 표준치료로 권고되고 있으며 치료에 사용되는 약제는 3기 대장암의 보조항암화학요법과 같이 사용할 수 있겠다.

항암제에 의한 간 손상

수술 전 항암제치료를 사용할 경우 항암제에 의한 간 손상 또한 고려되어야 할 중요한 문제 중 하나이다. 우선 항암제 사용 후 간에 대한 조직학적 소견의 변화를 알아보면 oxaliplatin의 경우에는 주로 혈관성병변이 동반되는데 sinusoid 병변의 경우는 수술 시 동반 합병증이나 수술사망률의 증가가 없으나 심한 혈관성 병변의 경우(출혈성 중심엽 괴사, 재생성 결절성 과증식)에는 수술 시 출혈위험이 높고 수혈이 필요하게 된다37,38).

또 대장암에서 흔히 사용되는 항암제 중 하나인 irinotecan의 경우에는 지방 간염(steatohepatitis)이 동반되며 수술 후 간기능 저하로 인해 수술 후 90일 이내 사망률 증가와 연관될 것으로 생각된다34). 하지만 이러한 결과는 다른 연구에서는 관찰되지 않아 항암제치료의 기간과 연관성을 고려해 보아야 한다38,39).

수술 전 사용되는 항암제 중 표적치료제 중 하나인 bevacizumab은 신생혈관생성에 관여하는 VEGF를 표적으로 하는 혈관생성억제제로 수술 후 간 재생에 영향을 미칠 수 있다. 하지만 최근의 연구에서 최소 수술 5주 전에 사용을 중단할 경우 수술 후 합병증에 영향을 주지 않는 것으로 알려졌다40).

대장암 간 전이 환자에서 치료 방향의 결정

우선 간 전이를 동반한 대장암의 치료를 결정할 때 가장 중요한 것은 다학제간 협력이다. 이는 앞에서 기술한 바와 같이 간 전이를 동반한 대장암 환자에서 수술적 절제방법의 고려, 항암제 치료의 시기, 사용할 약제의 종류 등을 논의함에 있어서 대장암 전문 외과의, 간 전문 외과의, 종양내과 전문의, 영상의학 전문의, 방사선 종양학 전문의 등으로 구성된 팀접근이 가장 바람직한 치료 결정을 내릴 수 있기 때문이다.

간 전이를 동반한 대장암의 치료 시 대표적인 나쁜 예후인자는 여러 개의 간 전이가 관찰되는 경우, 직경 5 cm 초과 간 전이, 동시에 간 전이가 확인된 경우, 원발암의 림프절전이, 종양표지자가 높은 경우 등으로41) 수술적으로 절제가 가능한 간 전이상태라 하더라도 불량한 예후인자를 1개 이상 가지고 있다면 먼저 선행항암화학요법을 시행하도록 권고되고 있다. 이는 EORTC-EPOC 임상연구의 PFS그래프 결과를 자세히 보면 생존율의 차이가 주로 첫 두 달에 나타나서 이후 평행하게 진행되는 것을 볼 때 수술 전 선행항암화학요법이 조기재발을 줄여 주는 효과가 있을 것으로 추측된다26).

단, 대부분의 전문가들은 2 cm 이하의 단독 간 전이는 바로 수술적 절제를 하도록 권유하는데, 이는 수술 전 선행항암화학요법이 자칫 완전 관해로 인한 종양의 수술적 절제를 어렵게 할 가능성을 고려한 것으로 실제 임상에서 이러한 환자는 10% 미만을 차지한다42).

또 한 가지 선행항암화학요법 시행 시 동반되는 문제점은 항암제치료 후 악화소견을 보여 2차 구제요법을 시도하게 되는 경우이다. 이러한 경우도 실제 임상에서는 10~15% 정도로 드물며 최근에는 KRAS와 같은 치료효과 예측인자의 개발이 진행되고 있으므로 사전에 환자에게 맞는 항암제 선택이 좀 더 용이할 수 있다. 또한 2차 구제요법에서 효과를 보이는 환자도 추후 간 절제를 시도할 수 있다43).

결 론

간 전이를 동반한 대장암 환자에서 일부는 간 절제로 완치를 시도할 수 있으며 20~30%에서 재발 없이 장기생존을 기대할 수 있으나 대부분의 환자가 수술이 불가능하거나 수술 후 재발된다. 최근 표적치료제를 포함한 항암화학요법의 발전과 수술 방법의 향상으로 완치를 시도하려는 노력이 지속되고 있으며 현재까지의 연구결과를 종합해 본다면 우선 절제 가능한 대장암 간 전이의 경우에도 수술 전후의 항암화학요법이 추천되며 초기에는 절제가 어려운 간 전이의 경우에도 반응률이 높은 항암화학요법을 시도하여 종양의 크기 감소가 뚜렷하다면 수술적 절제를 고려해야 할 것이다. 하지만 수술 전 항암제 투여 기간 등은 수술 후 합병증이나 간 손상 등과의 연관성에 대해 고려해야 하겠고, 처음 항암제치료를 결정할 때 특히 cetuximab의 사용을 계획한다면 KRAS 돌연변이와 같은 종양의 생물학적 예측인자에 대한 고려가 있어야 한다. 간 전이를 포함한 종양의 완전 절제 후에도 재발율을 줄이기 위한 수술 후 보조항암화학요법을 고려해야 할 것이다. 앞으로도 표적치료제를 포함한 보다 효율적이고 이상적인 항암화학요법과 수술 방법에 대한 지속적인 연구가 필요하며 실제 임상에서 간 전이를 동반한 대장암 환자의 치료를 결정할 때 무엇보다 중요한 것은 여러 전문의의 다학제적 협력일 것이다.
