대장 점막을 침범한 피부 병변이 없는 Blastic Plasmacytoid Dendritic Cell Neoplasm 1예

요약

BPDCN은 드문 혈액종양으로 80%에서 피부 병변을 동반한다. 피부 병변을 동반하지 않는 BPDCN은 이전 문헌 고찰에 따르면 폐침범, 연부 조직 종괴 등으로 내원하여 경피적 조직 검사로 진단된 경우가 대부분이다. 저자들은 피부 병변을 동반하지 않은 BPDCN을 대장 내시경을 통한 조직 검사와 해당 임파선의 조직 검사를 통하여 진단한 증례를 보고한다.

Abstract

Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a relatively rare hematological malignancy, and most cases present with characteristic skin lesions. The skin lesions appear as nodules, plaques, and bruise-like macules with redness or ulcerations. BPDCN without skin lesions is rare. Cases with hepatosplenomegaly, pulmonary involvement, multiple lymphadenopathies, renal masses, sinonasal sinus, and subcutaneous masses have also been reported. Here, we report the first case of ileocecal valve involvement of BPDCN without skin lesions.

서 론

Blastic plasmacytoid dendritic cell neoplasm (BPDCN)은 형질세포양 수지상세포에서 기원하는 드문 혈액종양으로 유병률은 혈액암의 0.44%, 피부 임파종의 0.7%로 알려져 있다. 특징적인 피부 병변, 다발성 림프절 비대과 골수 침범이 특징이며 피부 병변은 BPDCN 환자의 80%에서 결절(nodule), 판(plaque), 멍을 동반한 반(bruise like macule)으로 나타난다[1,2]. 피부 병변을 동반하지 않는 BPDCN은 20% 정도로 알려져 있으며 다발성 림프절 비대, 폐침윤, 연부 조직 종괴 등으로 내원하여 침범 장기의 조직 검사로 진단된다. 저자들은 복통으로 내원하여 대장 내시경과 경피적 림프절 조직 검사를 통하여 진단된 피부 병변을 동반하지 않은 BPDCN의 증례를 보고한다.

증 례

81세 여자가 2주 전부터 시작된 우하복부 통증으로 내원하였다. 복통의 양상은 지속적인 둔통의 양상이었다. 과거력은 없었으며 내원시 활력 징후는 혈압 130/80 mmHg, 맥박수 85회/분, 호흡수 20회/분, 체온 36.5도로 정상 소견이었고 신체 검사에서 우하복부의 압통이 관찰되었다. 혈액 검사 상은 LDH 927 IU/L (reference range: 130-270)와 CRP 2.21 mg/dL의 상승 외 다른 이상 소견은 없었다. 복부 전산화단층촬영에서 회맹판의 현저한 지방 침착과 말단 회장부의 점막 비대가 관찰되었다(Fig. 1A and 1B). 다수의 림프절 비대가 장간막, 후 복막, 오른 창자(right colic), 대동정맥(aortocaval) 부근에 관찰되었고 일부는 림프절 내에 괴사가 관찰되었다(Fig. 1C). 흉부 전산화단층촬영에 좌측 쇄골상부 림프절, 기관옆 림프절의 비대(Fig. 2)가 관찰되었다. 다발성 림프절 비대 소견으로 림프종 의심 하에 좌측 쇄골상부 림프절 생검을 하였고 회맹판 주변부의 이상 소견 평가를 위하여 대장 내시경을 시행하였다. 말단 회장부에서 점막의 결절성 변화(nodular change)가 관찰되었고(Fig. 3A) 비대한 회맹판과 회맹판 표면에 4 mm 크기의 발적을 띄는 점막 병변이 관찰되었다(Fig. 3B). 회맹판은 생검 겸자로 눌렀을 때 딱딱한 양상이었다. 회맹판 표면의 발적을 띄는 점막 병변과 말단 회장부에서 생검을 하였다. 림프절 생검에서 불규칙한 핵막과 염색질을 가진 중간 크기의 비전형적인 림프구가 관찰되었다(Fig. 4A and 4B). 면역조직화학염색 결과 CD4 양성(Fig. 4C), CD56이 국소적으로 양성 소견으로(Fig. 4D) BPDCN으로 진단하였다. 회맹판에서 시행한 생검에서도 고유판과 점막근층에 비전형 림프구의 증식이 관찰되었다(Fig. 4E). 고배율에서 고유판에 임파절 생검과 동일하게 중간 크기의 비전형적인 임파구가 관찰되어(Fig. 4F) 림프절 생검과 동일하게 BPDCN의 침범에 의한 것으로 확인되었다. 말초혈액 도말 검사 결과 이상 소견은 관찰되지 않았고 골수 검사를 고려하였으나 환자가 거부하여 시행하지 못하였다. 이후 환자는 요양병원으로 전원하였다.

고 찰

BPDCN은 1994년 Adachi 등이 natural killer cell leukemia로 보고 후 2008년 World Health Organization classification에 따라 BPDCN으로 분류되었다[3]. BPDCN은 모든 연령에서 발생 가능하나 60대 남성에 흔하며[1] 주로 피부 병변을 주소로 병원을 방문하여 진단된다. 피부 병변을 동반하지 않은 BPDCN의 경우는 20% 정도로 보통 병이 진행하며 피부 병변이 나타난다[4]. BPDCN의 진단은 침범 조직에서 면역조직화학염색에서 CD4, CD56 양성인 암세포를 확인하는 것이다. 전체 생존기간은 8.7개월이며[1] 무진행 생존기간은 11.2개월이었다[5]. 아직 표준 치료는 없으며 acute lymphoid leukemia/lymphoma, acute myeloid leukemia, myelodysplastic syndrome의 치료를 관해, 유지 치료로 사용할 수 있다[1]. 완전 관해율은 보고에 따라 27-92%이나 완전 관해되더라도 24개월 이내 재발하였다[5]. BPDCN의 피부 병변은 전산화단층촬영에서 병변 주변부의 염증과 원형 또는 난원형의 균일한(homogenous) 연부조직 음영으로 관찰된다[6]. 피부 병변이 없는 BPDCN은 다발성 림프절 비대, 간비종대, 폐침범, 신종괴, 피하 종괴로 내원하여 시행한 조직 검사에서 진단된다(Table 1). 본 증례는 전산화단층촬영에서 흉강, 복강내의 괴사를 포함한 다발성 임파선 비대와 회맹판 비대가 관찰되어 림프종과 결핵성 임파선염을 생각하였다. 그러나 대장 내시경상 결핵성 장염의 소견은 관찰되지 않았고 회맹판의 점막 병변과 말단 회장의 과립성 변화가 관찰되었다. 말단 회장의 조직 검사에서 항산염색과 결핵 중합연쇄반응(TB PCR)을 시행하였으나 결과는 음성이었다. 회맹판의 점막 병변은 좌측 쇄골상 임파선의 조직검사와 동일하게 점막층에서 중간 크기의 비전형적인 림프구가 관찰되어 BPDCN으로 진단할 수 있었다. 회맹판의 비대는 종양세포의 침윤에 의한 것으로 생각되었으나 내시경 조직검사에서 점막층만 포함되어 있어 점막하층 이하의 종양세포 침윤은 확인할 수 없었다. BPDCN의 위장관 침범은 Saito 등[7]이 처음 보고하였다. 피부 병변을 동반한 BPDCN 증례로 흑색변 평가를 위하여 시행한 대장 내시경에서 아프타 궤양이 관찰되었다. 아프타 궤양의 조직 검사 결과 BPDCN이 진단된 증례로 추적 내시경에서도 다발성 미란이 관찰되었다. 본 증례는 피부 병변이 없는 BPDCN 환자의 대장 점막 침범의 첫 보고이다. BPDCN은 드문 혈액종양으로 위장관의 침범 빈도나 내시경 소견에 대해서는 알려진 바 없다. 본 증례도 내시경에서 발적조의 점막 병변으로 BPDCN만의 특징적인 내시경 소견은 아니었다. 제한적이지만 BPDCN 환자에서 내시경을 시행하는 경우 비특이적인 병변이라도 생검 검사를 고려하여야 한다.
