말기 심부전의 비약물적 치료

Abstract

During last 2 decades, advance in the mechanical circulatory support has altered the management of end-stage heart failure and cardiogenic shock. This includes percutaneous mechanical support devices such as extracorporeal membrane oxygenator (ECMO) and implantable left ventricular assist devices (LVAD). Percutaneous devices may provide prompt circulatory support and save lives of the patients with cardiogenic shock. But for a prolonged support, long-term devices such as implantable LVAD are mandatory. LVAD is useful for both bridge-to-transplantation strategy and destination therapy for the patients with end-stage heart failure. Newer LVAD using continuous flow pump has several advantages over those with pulsatile flow pump, which includes smaller size and durability and showed better outcome in recent studies. In this manuscript, brief reviews of percutaneous and implantable devices for the management of advanced heart failure will be provided. (Korean J Med 2012;82:658-666)

서 론

인구의 고령화 및 급성 심장 질환 후의 생존 확률 증가로 인하여 심부전의 발생이 증가하여, 이로 인한 사회경제적 손실은 지속적으로 증가 되고 있다. 최근 신경 호르몬계 활성이 심부전의 발생 및 진행과정의 병태생리에 중요하다는 것이 알려지고, 이를 조절하는 약물치료가 만성 심부전 환자의 재입원 및 사망률을 감소시킬 수 있음이 밝혀짐에 따라, 만성 심부전 환자의 치료성적은 지속적으로 향상되었다[1-10]. 급성으로 악화된 심부전 환자들을 대상으로 한 여러 가지 약물에 대한 임상연구들도 시행되었으나, 만성 심부전에서와는 달리 장기적인 예후를 호전시키지는 못하였다[11-14]. 따라서, 불응성 혹은 말기 심부전 환자들에게는 강심제의 지속적 정맥투여나 기계적 순환 보조 장치를 사용하여 심실기능을 보조해 주는 치료가 필요하며, 궁극적으로는 심장이식이나 임종 환자에 대한 호스피스 치료가 시행되어야 한다. 지난 10여년간 심실 보조 장치(ventricular assist device, VAD)가 불응성 심부전 환자의 치료에 도움이 된다는 것이 확인되어 말기 심부전 환자의 치료 방침에도 커다란 변화가 있었다. 본 종설에서는 약물치료에 반응하지 않는 불응성 심부전 혹은 급성 심부전의 치료에 사용되고 있는 기계적 순환 보조 장치들에 대하여 기술하고자 한다.

본 론

진행성 심부전 환자의 치료 전략

현재 미국 심장학회의 심부전 치료 지침의 분류에 따르면 약물 치료에 잘 반응하지 않는 불응성 심부전 혹은 말기 심부전 환자는 D 단계로, 강심제의 지속적 정맥 투여나 기계적 순환 보조 장치를 사용하여 심실 기능을 보조해 주어야하며 궁극적인 호전을 위해서는 심장이식이 필요하다[15]. 일반적인 경우 심장이식 대상자보다 공여자의 수가 부족하므로, 이식 대기 시간이 길어져 강심제의 지속적인 투여만으로는 적절한 공여자가 나타날 때까지 환자 상태를 유지할 수 없는 경우가 많다. 이러한 경우 심장이식을 위한 가교 치료(bridge to transplantation)의 목적으로 기계적 순환 보조 장치를 사용할 수 있다[16]. 최근에는 심장이식의 대상이 되지 않는 말기 심부전 환자에게 기계적 순환 보조 장치를 사용하면 약물 치료보다 생존율이 현저하게 호전된다는 것이 알려져, 말기 심부전 환자의 궁극적인 치료(destination therapy, DT)로서 기계적 순환 보조 장치가 사용되고 있다. 또한 심근경색 후 심인성 쇽이나 전격성 심근염과 같은 급성 심부전 환자에게 기계적 순환 보조 장치가 혈역학적으로 불안정한 시기에 심근이 회복될 때까지(bridge to recovery) 일시적으로 도움을 줄 수 있고, 악화된 심실 기능에 의한 다장기 손상등으로 그 시점에는 심장이식 대상자가 되지 못하더라도 환자 상태가 심장이식을 받을 수 있는 상태로 호전될 때까지 순환 보조 장치를 가교 치료(bridge to candidacy)로 사용하는 경우도 있다[17]. 또한 갑자기 발생한 심인성 쇽으로 혈역학적 상태가 매우 불안정하거나 심부전이 급성 악화되어 강심제의 정맥 투여로도 활력 증후를 유지할 수 없는 경우에는 경피적 체외 심폐 보조 장치(percutaneous cardiopulmonary support device, PCPS)와 같은 경피적 순환 보조 장치(percutaneous mechanical support device)를 사용하여, 장기적으로 사용할 수 있는 삽입형 심실 보조 장치를 시술하기 전까지(bridge to bridge) 혹은 이후의 장기 치료 결정 시점까지 환자상태를 유지하기 위한 가교 치료(bridge to decision)의 목적으로 순환 보조 장치를 사용할 수 있다[16]. 이러한 경피적 순환 보조 장치들은 비교적 간단하게 중환자실 침상에서도 응급으로 시행할 수 있어 급성기에 환자의 생명을 유지시킬 수 있는 중요한 수단으로 사용되지만, 사지 허혈 및 출혈 등의 합병증이 흔하고 장기적으로 사용할 수 없어, 장기간의 심실 보조를 위하여는 보다 안정적인 삽입형 심실 보조 장치를 사용하여야 한다.

대동맥내 풍선 펌프(intra-aortic balloon pump, IABP)

1968년 Kantrowitz 등이 2 심인성 쇽 환자 2명에게 IABP를 시행하여 전신 혈압과 중심정맥압 및 소변량이 호전됨을 보고하였다[18]. IABP는 이완기에 하행 대동맥내에 위치한 풍선을 확장시켜 관상동맥 혈류를 증가시키고, 수축기에 풍선을 감압시켜 후부하를 감소시키는 기전으로 좌심실을 보조하여 심박출량을 약 0.5 L/min 가량 증가시킬 수 있다. 이후 심인성 쇽을 동반한 심근경색 환자에게 혈전용해술 시행시에 IABP를 같이 시행하면 병원내 사망을 줄일 수 있다고 알려져 심인성 쇽 환자의 치료의 중요 수단으로 사용되어 왔다[19]. 이후 23,180명의 심근경색 환자를 대상으로 한 후향적 연구에서 심인성 쇽 환자 중 7,268명에게 IABP가 시행되었고, 혈전용해술을 받은 경우 IABP를 사용한 환자들의 병원내 사망률은 49%로 그렇지 않은 환자들의 67%에 비하여 유의하게 낮았다[20]. 하지만 일차적 관상동맥 중재시술을 받은 경우에는 IABP의 시행이 병원내 사망률에 큰 영향을 주지 못하였다. 최근 시행된 전향적 연구에서도 심인성 쇽을 동반한 급성 심근경색으로 관상동맥 중재시술을 시행한 경우 IABP가 혈역학적 지표들을 호전시키지 못하였으며, 이후 시행된 메타분석에서도 사망률이나 좌심실 구혈률에 차이가 없었고, 오히려 뇌졸중이나 출혈 등의 합병증만 증가하였다고 보고되어, 심인성 쇽 환자에게 IABP가 유용하다는 증거를 제시하지는 못하였다[21,22]. 즉, 실제 임상에서는 급성 심근경색에 의한 심인성 쇽에서 IABP가 혈역학적 상태를 호전시키는 경우가 종종 있지만, IABP가 심인성 쇽 환자에서 관상동맥 중재시술에 추가적으로 사망률 및 이환율을 감소시킨다는 증거는 충분하지 못하다고 할 수 있다.
IABP는 관상동맥 협착을 동반하지 않은 심부전 환자에서도 전신 혈압 즉 후부하를 감소시켜 좌심실 심근에 가해지는 장력을 낮추고 산소 요구량과 좌심실 확장기말압을 감소시키며, 이에 따라 심박출량도 증가시켜 심부전의 급성 악화에 도움이 될 수 있다[23]. 이외에도 심실 중격 파열이나 유두근 손상에 의한 심인성 쇽, 약물 치료에 반응하지 않는 심실 부정맥 및 불안정성 협심증 등에도 도움이 된다고 알려져 있다[20,24,25].
관상동맥 우회수술을 시행받는 환자들중 지속되는 허혈 증상 및 비대상성 심부전으로 인한 혈역학적 이상이 동반되어 응급 수술을 시행하는 경우에는수술 전 IABP가 수술에 의한 사망률을 감소시키는 것으로 알려졌다[26,27]. 하지만 지속되는 심근 허혈이나, 심부전의 동반 없이 단지 심실 기능이 저하된 이유만으로 수술 전 IABP를 일괄적으로 시행하는 것은 바람직하지 않다

체외 순환 펌프를 이용한 순환 보조 장치(Mechanical circulatory support with extracorporeal pumps)

IABP가 심인성 쇽 환자에게 어느 정도의 심실 기능을 보조할 수는 있으나 심실 기능을 전적으로 대치할 수 없어 환자 고유의 심실 기능에 전적으로 의존하는 것과는 달리 심실 보조 장치는 심실 수축이 없어도 전신 혈액 순환에 필요한 심박출량을 제공할 수 있어 심인성 쇽 환자의 치료에 더욱 효과적일 수 있다. 1990년 공기 압축식 체외 펌프를 사용하는 Abiomed BVS System 5000 (Abiomed, Inc, Danvers, MA)이 개발되어 좌심방에서 혈액을 흡인하여 흉부 대동맥으로 밀어주는 수술적 심실 보조 장치가 시술되어 말기 심부전 환자의 약 2/3가 심실 기능의 호전이나 심장이식까지 가교 치료 이후 생존할 수 있었다고 보고되었다[28,29]. 이후 CentriMag VAD (Levitronix, Waltham, MA) 등의 원심성 펌프를 이용한 연속파형 펌프가 개심술 후의 심인성 쇽에 일시적인 순환 보조 장치로 사용되었다[30]. 하지만, 이와 같은 수술적으로 사입하는 체외 순환 보조 장치들은 개흉술 및 이를 위한 전신 마취가 필요하여 혈역학적으로 불안정한 심인성 쇽 환자에게 응급으로 시행하기에는 제한이 많아, 급성 악화된 심인성 쇽의 응급 치료에는 경피적으로 시술하는 경피적 순환 보조 장치가 우선적으로 시행된다. 현재 전 세계적으로 사용 가능한 경피적 순환 보조 장치에는 TandemHeart, Impella Recover 2.5 및 경피적 막성 체외순환 보조 장치(extracorporeal membrane oxygenator, ECMO) 등이 있으나 국내에는 ECMO를 사용하는 경피적 심폐 보조 장치(percutaneous cardiopulmonary support device, PCPS)만이 사용되고 있다(Table 1).

ECMO는 1960년대부터 개발되어 호흡부전의 치료에 사용되었으나, 최근 ECMO 관련 기술이 급속도로 발전하여 급성 심부전 및 심인성 쇽 환자에서 그 사용이 증가하였다. 즉, ECMO 장비가 소형화되어 이동이 좋으며, 삽입하기까지 준비시간이 매우 단축되어 응급 상황에서 시술이 가능해졌고, 정맥-동맥 간 체외순환(venous-arterial ECMO) 뿐만 아니라 정맥-정맥 간 체외순환(venous-venous ECMO)의 방식으로도 사용되어 급성 호흡 부전이 동반된 환자에서도 사용이 가능하다. ECMO의 가장 큰 장점은 뒤에 설명할 TandemHeart와 달리 심방 중격을 천자할 필요가 없어 침상에서도 간단히 대퇴정맥과 대퇴동맥을 통하여 경피적으로 시술할 수 있다는 것이다. 심장 수술 후에 발생한 심인성 쇽이나 급성 전격성 심근염과 같은 가역적 심부전 환자에게 심실 기능이 회복될 때까지 가교치료(bridge to recovery)로 사용할 수 있고 급성 심근경색에 동반된 심인성 쇽에서 일차적 관상동맥 중재 시술에 혈역학적 보조 장치로 사용하여 생존율을 호전시켰다는 보고가 있다[31-34]. 최근에는 더욱 소형화되어 이동성이 향상된 장비들이 개발되어, 2차 병원에 내원한 38명의 심인성 쇽 환자에게 ECMO를 시행한 후 3차 병원으로 이송하여 그 중 21명이 성공적으로 회복되거나 심장이식을 받을 수 있었다는 보고가 있었다[35-37]. ECMO를 이용한 심폐 보조 장치의 최대 단점은 좌심실의 기능 저하로 인한 확장기말압의 증가를 효과적으로 해결할 수가 없어 폐부종 등이 지속될 수 있다는 점이다. 이를 위하여 추가적인 심방 중격 천자를 시행하여 좌심방압을 낮출 수 있었다는 보고가 있으나, 추가 시술에 따른 합병증이 발생할 수 있어, 장기적인 순환 보조를 위해서는 좌심실 보조 장치로 변경하는 것이 필요하다[38]. 또한 대퇴정맥 및 대퇴동맥을 통하여 도관이 이루어지므로 하지 혈관에 심각한 허혈을 초래할 수 있고, 막성 산소화기(membrane oxygenator)가 일정 시간 이상 사용하면 혈장 누출로 인하여 산소 교환율이 저하되어 빈번하게 교체해야 하는 단점 등으로 장기간 사용할 수 없다[39]. 따라서 ECMO를 사용한 경피적 심폐 보조 장치는 일시적인 심실 보조를 위해 응급상황에서 단기적으로 사용할 수는 있으나 장기간의 심실 보조를 위해서는 적합하지 않다.

경피적 좌심실 보조 장치(percutaneous left ventricular assist devices, percutaneous LVAD)

경피적 좌심실 보조 장치는 현재 국내에서는 사용되고 있지는 않으나 전 세계적으로는 장기적인 심실 보조 장치를 시술하기 전까지 일시적으로 사용되고 있다. 여기에는 TandemHeart(CardiacAssist, Inc, Pittsburgh, PA)와 Impella Recover 2.5(Abiomed, Inc) 등의 경피적 좌심실 보조 장치가 있다. TandemHeart는 대퇴정맥-우심방 경로를 통하여 심방 중격 천자를 시행하여 좌심방의 혈류를 흡인하여 체외 원심성 펌프로 대퇴동맥으로 혈액을 밀어주어 3.5-4.5 L/min의 심박출량을 보조할 수 있다. 초기 보고에서는 심인성 쇽 환자에게 IABP에 비하여 월등한 혈역학적인 개선 효과를 보일 수 있으나 시술에 인한 합병증은 비슷하다고 알려졌다[40]. 하지만 30일 사망률은 IABP와 큰 차이 없고 출혈이나 하지 허혈 등의 합병증은 오히려 증가했다는 보고도 있어 각각의 환자 상태에 따른 치료 방침 결정이 필요하다[41]. 이외에도 급성 심부전 환자에서 심장이식, 치료 결정, 혹은 심실기능 회복까지의 일시적 가교 치료로 TandemHeart를 사용 할 수 있으며, 급성 이식심장 거부 반응과 같이 심한 혈역학적 이상이 동반되는 경우에도 일시적으로 사용될 수 있다[42-45].
Impella Recover 2.5는 2.5 L/min 가량의 심박출량을 보조할 수 있어 부분적인 순환 보조가 가능하다[17]. 급성 심근경색 후에 발생한 심인성 쇽 환자에게 IABP에 추가적으로 사용하여 30분 후 혈역학적 지표들을 호전시킬 수는 있었으나, 용혈 현상이 증가하였고 30일 생존율을 증가시키지는 못하였다[46]. 하지만 TandemHeart 및 IABP 등과 함께 사용하여 심한 심실 기능 저하에 일시적으로 도움이 된다는 보고들이 있어, 급성 심부전의 효과적인 일시적 치료로 사용할 수는 있겠다[47,48].

삽입형 심실 보조 장치

삽입형 심실 보조 장치는 1984년 Novacor 심실 보조 장치를 시술 받은 환자가 성공적으로 심장이식을 받을 수 있었다는 보고 이후, 1990년대 중반에는 여러 가지 삽입형 심실 보조 장치들이 심장이식까지의 가교 치료로 미국 식약청의 허가를 받아 사용되었다[16,49]. 이후 삽입형 심실 보조 장치의 사용에 획기적인 계기를 마련한 대표적인 연구인 REMATCH 연구가 2001년에 발표되었다[50]. 즉, 심장이식의 대상이 되지 않는 말기 심부전 환자들에게 궁극적인 치료(destination therapy)로 박동형 펌프(pulsatile flow pump)를 사용하는 Heart-Mate XVE가 약물 치료보다 생존율을 호전시킨다는 것이 확인되어, 이후 좌심실 보조 장치가 말기 심부전 치료의 중요수단으로 자리잡는 계기가 되었다. 하지만 박동형 펌프의 내구성이 좋지 않아 대부분의 경우 2년 이내에 펌프 이상이 발생하여, 1년 생존율이 50%, 2년 생존율이 25% 이하의 기대에 못미치는 성적으 인하여 좌심실 보조 장치가 말기 심부전의 보편적인 치료 수단이 사용되지는 못하였다. 이후 연속파형 펌프(continuous flow pump)인 HeartMate II가 개발되어 심장이식 대기 환자에게 성공적으로 사용되었고, 2009년에는 전향적 무작위 연구인 HeartMate II DT 연구에서 말기 심부전 환자의 궁극적 치료에 HeartMate II가 박동형 심실 보조 장치인 HeartMate XVE보다 장기 생존율을 호전시킨다는것이 알려졌다[51,52]. 이러한 HeartMate II의 월등한 성적은 연속파형 펌프가 박동형 펌프에 비하여 구조가 간단하고 내구성이 좋은 점에 기인한다. 이후 심장이식의 대상이 되지 못하는 말기 심부전 환자에게 연속파형 좌심실 보조 장치를 사용하는 빈도가 10배 이상 증가하는 등 좌심실 보조 장치는 말기 심부전 치료의 효과적인 치료법으로 받아들여 지고 있다[53,54]. 이후로도 INTERMACS 등록 사업에서도 연속파형 심실 보조 장치의 1년 및 2년 생존율이 각각 68%, 55%로 박동형 좌심실 보조 장치(1년 생존율 55%, 2년 생존율 24%)에 비하여 월등한 성적을 보인다는 것이 확인되었다[55]. 한 가지 특이할 만한 점은 REMATCH 연구 이후 박동형 심실보조 장치의 성적은 HeartMate II DT 연구에서도 1년 생존율이 50-60% 정도로 10년 가량 축적된 임상 경험에도 불구하고 의미있는 성적의 향상이 없었으나, 연속파형 심실 보조 장치는 초기 연구에서 1년 생존율이 68%였던 것에 비하여, 후속 연구에서는 임상경험이 축적됨에 따라 1년 생존율이 74-85%까지 증가되었으며, 이러한 성적 향상에는 대상 환자의 선택, 심장 외과의의 수술 경험 및 수술 후 관리의 개선 등이 중요하게 작용한 것으로 생각된다[56-58]. 이외에도 연속파형 좌심실 보조 장치는 크기가 작아 체구가 작은 여성이나 청소년에게도 시술하기 쉽고, 소음이 크지 않아 환자 및 환자 가족에게도 편리하며, 현재 개발되어 임상 시험 중인 장치 들은 심낭내에 설치할 수 있을 정도로 소형화되었다(Table 2) [59,60].
초기 연구에서는 심장이식을 위한 가교 치료로 심실 보조 장치를 시행 받은 환자들이 그렇지 않은 환자들에 비하여 심장이식 후 성적이 좋지 않다는 보고들이 있었으나, 연속파형 심실 보조 장치가 사용된 이후로는 가교 치료로 시술 받은 경우의 심장이식 성적이 이식 후 30일 생존율이 97%, 1년 생존율이 87%로 가교 치료를 받지 경우와 차이가 없었다[61,62].
심실 보조 장치가 내구성이 향상되고 소형화되어 기기 작동 이상과 같은 문제들은 상당 부분 해결되었으나, 출혈, 뇌졸중 혹은 감염 등의 합병증에 대한 위험은 아직도 해결되어야 할 문제로 남아 있다. 연속파형 심실 보조 장치를 사용하고 심장이식을 기다리는 환자 100명당 매년 8명에서 장애를 동반하는 뇌졸중이 생기는 것으로 보고되어 박동형 심실 보조 장치의 성적과(11 stroke events per 100 person-year, p = 0.38) 차이가 없었고, 심장이식의 대상이 되지 않는 환자를 대상으로 한 연구에서도 매년 11%의 뇌졸중이 발생하여 박동형 심실 보조 장치(12%, p = 0.56)와 차이가 없었다[51,57].
심실 보조 장치와 관련된 감염은 아직도 드물지 않으며, 주로 보조 장치 삽입부와 경피 연결 부위에 발생하는 것으로 알려져 있다[63,64]. 다행히도 펌프내의 혈류에 직접 노출되는 부위의 감염으로 인한 감염성 심내막염은 드물다고 알려져 있다. 일단 감염이 발생하면 치료가 매우 어려워 장치의 제거 및 조속한 심장이식이 필요하며, 심장이식을 받을 수 없는 경우 치명적인 경우가 많다.
연속파형 심실 보조 장치는 심한 혈관내 용혈을 일으키지는 않지만, 위장관 혹은 대뇌 혈관의 동정맥 기형(arteriovenous malformation)의 발생과 관련이 있고, 후천성 von Willebrand 인자 결핍에 의한 혈소판 기능 이상이 비교적 흔하게 동반될 수 있으며, 항응고 요법과 관련하여 위장관이나 비강 등의 점막출혈 혹은 뇌출혈 등과 관련이 있다[65-68].

향후 전망

1999년 이후 심장이식 대기 환자의 30% 이상이 삽입형 심실 보조 장치의 도움을 받았고, 특히 응급도가 높은 UNOS (United Network for Organ Sharing status) IA 등급 환자들의 경우에는 75%가 심실 보조 장치를 시행 받았다[69]. 이는 말기 심부전으로 심실 보조 장치를 시술 받고 퇴원이 가능하며, 심장이식을 받을 때까지 일상 생활에 지장이 없을 정도로 삶의 질이 향상된다는 것을 의미한다[70]. 최근에는 심부전 치료 성적이 향상되어 비교적 응급도가 낮은 UNOS II 등급 환자들의 1년 생존율이 81%, 2년 생존율이 74% 정도로 향상되어 순환 보조 장치를 시행한 환자의 예후와 비슷해 졌으나, 이러한 환자들에게도 심실 보조 장치가 활동 능력을 높여 삶의 질을 향상시킬 수 있을 것으로 기대되고 있다.[16] 이차성 폐동맥 고혈압으로 심장이식의 상대적인 금기에 해당하는 환자에게도 심실 보조 장치를 시행하여 폐동맥압을 낮추어 성공적으로 심장이식을 시행하였다는 보고도 있어, 심장이식의 대상자가 되기 위한 심실 보조 장치의 역할(bridge to candidacy)도 기대되고 있다[71].
연속파형 심실 보조 장치의 성적이 향상됨에 따라 말기 심부전 환자에게 주요 장기 부전이나 이와 관련된 합병증이 발생하기 전에 심실 보조 장치를 시행하는 것이 필요하다는 의견이 일반적으로 받아들여지고 있다. 이러한 견해를 뒷받침하기 위해 강심제의 정맥주입이 필요하지 않은 진행된 심부전 환자들을 대상으로 소형 순환 보조 장치인 HeartWare HVAD를 심낭내에 삽입하는 치료의 유용성을 확인하기 위한 연구인 REVIVE-IT (Randomized Evaluation of VAD intervention before Inotropic Therapy) 연구가 시행되고 있고, 이와 비슷한 대상 환자들에게 HeartMate II를 사용한 궁극적 치료를 일반적인 약물 치료와 비교하는 전향적 연구인 ROADMAP (Risk Assessment and Comparative Effectiveness of LVAD and Medical Management in Ambulatory Heart Failure Patients) 연구도 시행될 예정으로, 말기 심부전으로 진행하지 않은 환자에게 심실 보조 장치가 과연 유용한가에 대한 관심이 높아지고 있다[16,72].

결 론

기계적 순환 보조 장치가 최근 10여년간 말기 혹은 급성 심부전 환자의 치료에 도움이 된다는 것이 확인되어, 말기 심부전의 치료 방침에 많은 변화가 진행되고 있다. 국내에서는 경피적 순환 보조 장치인 IABP와 ECMO를 사용한 PCPS를 시행할 수 있어 심인성 쇽이나 급성 심부전 환자의 일시적인 치료에 도움을 받고 있지만, 장기적으로 사용할 수 있는 삽입형 심실 보조 장치에 대한 경험은 아직 전무한 실정이다. 향후 국내에서도 삽입형 심실 보조 장치에 대한 임상시험이 시행될 예정으로 조만간 국내에 도입되어 말기 심부전 환자의 치료에 효과적으로 사용될 수 있는 중요한 치료 수단이 될 것으로 기대된다.
