류마티스 질환의 생물학적 치료제

Abstract

During the past three decades the treatment in rheumatology was revolutionized by the introduction of disease-modifying anti-rheumatic drugs. Advances in our understanding of the pathogenesis of rheumatic diseases have led to the discovery of critical pathogenetic mechanisms of inflammation and autoimmunity and the invention of new target-specific biologic agents. The development of biologic agents led to a new era in treatment of various rheumatic diseases. Treatments of rheumatic diseases such as rheumatoid arthritis (RA) had been very difficult in former days until conventional DMARDs was developed. Although synthetic DMARDs made it a lot easier to control rheumatic diseases, still, there were some difficulties on controlling diseases. The biologic agents inhibit specific pro-inflammatory cytokines as a targeted therapy to prevent inflammation and destruction of affected joint and/or tissues. Three TNF blocking agents, etanercept, infliximab and adalimumab, are the most well recognized and widely used biologic agents in RA, ankylosing spondylitis (AS), and some other inflammatory autoimmune diseases. Furthermore, an inhibitor of IL-1 (anakinra), a B-cell depleting drug (rituximab) and an inhibitor of T-cell costimulation (abatacept) are developed in turn asnovel biologic agents which are believed to be effective in number of rheumatic diseases. Recently, an inhibitor of IL-6 (tocilizumab) and 2 other TNF inhibitors (golimumab and certolizumab) are developed. Here, the brief descriptions of recently used biologic agents and their efficacies and safeties are discussed. (Korean J Med 2012;82:549-561)

류마티스 질환은 자가면역에 의한 여러 장기의 염증으로 심각한 장애나 수명단축을 초래하는 난치병 혹은 불치병으로 인식되고 있다. 그렇지만 최근에는 기초 의학의 발전, 다양한 항류마티스 약제의 개발, 생물학적 제제의 도입, 류마티스 질환의 조기 진단, 치료 방법의 변화 등으로 이러한 인식이 바뀌게 되었다. 류마티스 질환의 정확한 원인은 잘 알려져 있지 않지만, 유전적 소인을 갖고 있는 개체에서 면역관용의 이상으로 자가반응세포가 나타나며 이로 인해 T, B 림프구의 활성화, 자가항체의 생성, 보체의 활성화, 면역복합체의 침착과 표적 장기에 염증세포의 침윤 등의 다양한 기전에 의해 자가면역반응이 일어난다. 면역체계의 이상은 류마티스 질환마다 다르지만, 공통적으로 중요하게 관련된 이상은 치료의 중요한 표적이 될 수 있다. 스테로이드와 같은 광범위 면역억제제나, 항염증제제는 임상적으로 유용하지만, 중대한 부작용을 일으켜 장기간 사용하는 데 문제가 된다. 이러한 제제는 대개 경험적인 효과에 의존해 사용되며, 면역체계에 미치는 중요한 기전에 대해서는 잘 알려져 있지 않다. 따라서 염증 과정의 핵심적인 역할을 하는 요소를 선택적으로 억제하는 것이 보다 효율적이고 원하는 치료방법이 될 수 있다.
최근에는 면역 기전의 규명과 단일클론항체(monoclonal antibodies, mAb)와 fusion proteins 등과 같은 표적 생물학 치료제의 개발로 다양한 생물학적 제제를 새로운 치료로 이용하게 되었다. 이러한 생물학 제제는 주로 세포 표면에 존재하거나 세포에서 분비된 물질을 표적으로 하며, 가장 중요한 표적은 류마티스 질환에서 염증과 침범된 조직의 파괴에 관여하는 염증성 사이토카인이다. 1998-1999년에 TNF 길항제인 infliximab과 etanercept가 처음으로 미국 식약청에서 승인을 받은 후에 TNF 길항제인 adalimumab, golimumab, certolizumab, IL-1 길항제인 anakinra, T 세포의 기능을 억제시키는 abatacept, B 세포의 CD20에 작용하여 B 세포를 선택적으로 차단시키는 rituximab과 IL-6 길항제인 tocilizumab 등이 개발되어 사용되고 있다[1-3]. 이러한 제제들은 류마티스 관절염, 혈청음성 척추병증(seronegative spondyloarthritis)과 기타 염증성 질환을 호전시키고 진행을 억제할 수 있어 치료에 획기적인 변화를 가져오게 하였다(Table 1). 이외에도 다양한 표적물질을 이용한 새로운 생물학제제에 대한 연구가 지속되고 있어 향후 류마티스 질환의 치료에 많은 영향을 줄 수 있을 것으로 기대된다.
그렇지만, 많은 연구 보고에서 기대와 달리 효과가 적은 경우가 많이 있었고, 고전적인 항류마티스제제와 비교하여 매우 고가이며, 심각한 감염, 결핵의 활성화 등의 부작용을 발생시킬 수 있는 문제점을 갖고 있다. 또한, 대부분의 최소 한 가지 이상의 고전적인 항류마티스제제에 실패한 류마티스 질환에서 생물학적 제제의 사용을 권장하고 있으나, 치료의 효율성을 감소시킬 수 있어 생물학제제를 이용한 효율적인 치료를 위한 적절한 전략에 대한 연구가 필요하다. 여기에서는 류마티스 질환에서 다양한 생물학제제의 사용에 대하여 살펴보고자 한다.

본 론

종양괴사인자(tumor necrosis factor, TNF) 길항제

Tumor necrosis factor-α (TNF-α)는 염증성 사이토카인으로 숙주의 방어, 염증, 세포자멸사(apoptosis), 림프구의 자극, 뼈의 대사, T-B 림프구 상호 반응, 림프기관의 발생, 면역세포 기능의 활성화 등에 관여한다. 특히, 류마티스 관절염의 염증 반응에 가장 결정적인 역할을 하며, 비가역적인 관절의 손상과 골, 연골의 파괴에 관련이 있다. TNF-α transgenic mice에서 류마티스 관절염과 유사한 소견이 보이고[5], TNF-α 수용체를 이용한 TNFR2-Fc 와 mAb치료에 호전을 보인 결과를 근거로 개발되었다. TNF 길항제는 류마티스 관절염 이외에도 연소성 관절염, 강직성척추염, 건선 관절염 등에 사용이 승인되었다.

Etanercept

Etanercept (Enbrel™)는 TNF receptor p75 Fc fusion protein (TNFR:Fc)으로 사람의 면역글로불린 G1 (IgG1)의 Fc fragment와 TNFR2 수용체의 extracellular domain이 합해진 가용성 재조합 물질이며, 가용성 TNF와 lymphotoxin-α (LT-α)와 결합하여 TNF-α의 생물학적인 기능을 억제한다.
Etanercept 단독 또는 methotrexate (MTX)와의 병합요법을 하는 경우 초기 류마티스 관절염과 오래 지속되어온 류마티스 관절염에서 증상의 완화와 안정성이 보고되었으며[5-7], 2년 이상의 연장 시험[8]과 5년 연장 시험에서도 염증과 임상 증상의 개선이 유지되었고 진행의 억제와 안전성이 유지되었다[9]. 최근에는 adalimumab, infliximab과의 비교 연구에서 세 종류의 TNF 길항제가 치료에 대한 반응, 관해율 및 진행의 억제 효과는 유사하였으나, etanercept가 다른 제제에 비해서 순응도가 높았다[10].
강직척추염에서도 etanercept는 질병의 활성도와 삶의 질을 유의하게 개선시켰으며[11,12], 4년 이상 지속적으로 사용한 경우에도 유효한 효과가 지속되었고[13], 비용 효과면에서도 긍정적인 결과를 보였다[14]. 이러한 질병의 활성도와 삶의 질의 개선에도 불구하고 척추의 강직을 포함한 진행에 미치는 영향에 대해서는 부정적이며, 2년간 etanercept와 infliximab을 사용한 후 방사선학적인 진행에 효과가 없는 것으로 보고되었다[15,16]. 이러한 결과는 TNF 길항제의 효과가 TNF에 의한 염증 및 관절의 파괴에 미치는 영향에는 효과가 있지만 척추의 강직에는 영향이 없는 것으로 설명되어 이에 대한 추가적인 연구가 필요하고, 또한 척추의 강직을 일으키는 기전을 차단할 수 있는 새로운 제제의 개발의 필요성이 요구되고 있다. Etanercept는 다관절 침범 연소성 관절염[17], 건선, 성인에서 활동성 건선 관절염[18]에도 효과가 있어 승인되어 사용되고 있다.
Etanercept는 1998년 미국 식약청에서 승인되었으며, 2004년에는 우리나라에서도 승인이 되어 사용되기 시작하였다. 류마티스 관절염에서 TNF 길항제의 사용은 3개월 미만의 조기 류마티스 관절염 환자라도 질병의 활성도가 매우 높고, 나쁜 예후를 예측할 수 있는 인자(많은 수의 관절염, 고역가의 류마티스 인자, 항-CCP 항체 양성, 류마티스 관절염과 관련된 유전자를 갖거나, 조기에 관절의 파괴 소견 등)가 있는 경우에 금기사항이 없으면 MTX와 병합요법이 권장되며, MTX의 치료에도 중등도 이상의 활성도를 보이며 나쁜 예후인자가 있는 경우에도 권해진다. 또한, MTX나 다른 항류마티스제제와 병합요법에 실패한 경우에도 사용이 권장된다 [19]. 우리나라에서는 두 종류 이상의 고전적 항류마티스제제를 6개월 이상 사용한 후에도 지속적으로 관절염이 심하게 보이는 경우에 사용이 인정이 되고 있다.
강직척추염에서는 고전적인 치료에도 지속적으로 질병의 활성도가 높게 유지되는 환자에서 사용이 권장되며[20], 우리나라에서는 3개월 이상의 치료에도 지속적으로 활성도가 높게 유지되는 경우에 사용이 인정되고 있다. 피하주사로 일주일에 1번 50 mg 혹은 일주일에 2회 25 mg 투여할 수 있으나, 고용량을 1회에 투여 시에 효과는 비슷하나 부작용이 더 많다는 보고가 있다. 단독요법이나 MTX와 병용투여가 가능하며, 부작용으로는 주사부위에 발생하는 과민반응이 가장 많았으며 감염, 소화기계 부작용 등이 높은 빈도로 나타났다[9,10,21].

Infliximab

Infliximab (Remicade™)은 mouse anti-TNF mAb의 항원 결합 부위와 사람의 IgG1의 constant region으로 구성된 키메라의 단일클론항체(chimeric mAb)로, 가용성, 세포막을 통과하는(transmembrane) TNF-α와 결합하여 TNF의 작용을 억제한다.
조기 류마티스 관절염에서 MTX와 infliximab의 병합요법이 MTX 단독요법보다 유의하게 증상을 개선시켰고, 관절 손상의 진행을 감소시키는 것보다 더 효과적이었다[22,23]. 이러한 효과는 2년 이상의 연장 연구에서도 지속되었으며, 고전적 항류마티스제제에 적절히 반응하지 않는 조기 류마티스 관절염에는 항류마티스제제와 infliximab의 병합요법도 보다 효과적이었다[24,25]. 강직척추염에 대한 연구에서는 infliximab이 질병의 활성도와 삶의 질을 개선하는 데 유의한 효과를 보였으며[26,27], 최근 5년 이상 장기간 치료에서도 대부분 환자에서 유의한 효과를 보였고, 투여 받은 환자의 50% 이상에서 효과와 안전성이 만족스러워 지속적으로 투여하고 있는 것으로 보고되었지만[28], 척추 강직의 진행을 억제하는 데는 부정적인 견해가 있다[16].
Infliximab은 1999년에 미국 식약청에서 승인되었으며, 우리나라에서는 2007년에 승인되어 사용되고 있다. 우리나라에서는 류마티스 관절염에서는 두 종류 이상의 고전적 항류마티스제제를 6개월 이상 사용한 후에도 관절염이 심하게 지속되는 경우에 사용이 인정이 되며, 강직척추염에는 3개월 이상의 고전적 치료에도 지속적으로 활성도가 높게 유지되는 경우에 사용이 인정된다.
류마티스 관절염 환자에서는 3 mg/kg를 0, 2, 6주에 정맥투여하고 이후에는 8주마다 투여하며, 강직척추염에서는 5 mg/kg의 용량을 같은 방법으로 투여한다. 강직척추염에 대해서는 질병의 조절 정도에 따라 6주마다 투여하는 방법을 사용할 수도 있다. 단독 사용 시에 약 40% 정도에서 항체형성이 나타나는 것으로 보고되고 있어 이러한 면역반응을 줄이기 위해 MTX와의 병용투여가 권장된다. 이러한 면역반응은 고용량보다는 저용량 투여 시 증가되는 것으로 알려져 있어 저용량을 사용하는 류마티스 관절염에서 MTX와의 병용투여가 권장된다[19]. 부작용으로는 감염, 주사부위의 과민반응이 많으며 신경계 증상 및 소화기계 증상 등이 나타날 수 있다[1,24-27].

Adalimumab

Adalimumab (Humira™)은 infliximab과 달리 전체가 사람에 존재하는 형태로 제조하여 면역성을 낮춘 재조합 IgG1 anti-TNF-α 단일클론항체이다. 조기 류마티스 관절염 환자에서 2년간 MTX와 병용요법을 한 경우에 MTX나 adalimumab 단독요법보다 관절염의 증상과 관절의 파괴의 억제에 효과가 더 있었으며[29], 5년간 연장 연구에서도 효과가 지속되는 것으로 보고되었다[30]. 또한, adalimumab 단독요법이나 다양한 항류마티스약제와의 병합요법에 대한 연구에서도 관절염에 대한 효과와 안정성이 보고되었으며[31], 최근에는 adalimumab이 관절염에 미치는 효과와 관계없이 관절의 파괴를 억제하는 효과가 있는 것으로 보고되었다[32]. 또한 장기간 adalimumab과 항류마티스제제를 병용하였을 때 효과가 지속되고 관절파괴의 진행을 억제하며 안전성이 유지됨이 보고가 있었다[31].
강직척추염에서는 신체기능, 운동, 피로감과 삶의 질에 유의한 효과를 보였고, 이러한 효과는 2년 후에도 82.0%에서 지속적으로 유지되는 것이 보고되었으며[33,34], 홍채염의 재발을 줄이는 것으로 보고되었다[35]. 2002년 말에 미국 식약청에서 승인을 받았으며, 2006년에는 우리나라에서도 승인되어 사용되고 있다. Etanercept, infliximab과 더불어 가장 흔히 사용되는 생물학 제제이며, 고전적인 치료에도 지속적으로 질병의 활성도가 높게 유지되는 환자에서 사용이 권장되고 있다[20]. 우리나라에서는 다른 TNF 길항제와 같은사용 기준을 만족하는 경우에 사용할 수 있다.
2주 간격으로 40 mg을 피하주사하며, 소수의 환자에서는 조절이 잘되지 않는 경우 1주 간격으로 40 mg 투여하는 식으로 증량하여 사용하는 것을 고려해 볼 수도 있다. 가장 흔한 부작용으로는 감염이며, 주사부위 반응, 복통 등이 나타날 수 있고 비호지킨 림프종이나 피부암의 악성종양이 다소 많이 발생함이 보고된 바 있다[1,31,33,34].

Golimumab

Golimumab (Simponi™)은 TNF에 대한 항체 전체가 사람에 존재하는 형태로 재조합된 단일클론항체로 면역력을 줄였으며, 가용성이고 세포막을 통하는 형태의 TNF의 역할을 차단한다. Golimumab은 MTX를 사용하지 않은 류마티스 관절염 환자에서 MTX 단독요법에 비하여 유의한 효과를 보였으며[36], MTX에 반응이 부적절한 활동성 류마티스 관절염 환자에서도 MTX와 위약을 사용한 군에 비해 MTX와 golimumab의 병합치료 시 유의하게 좋은 효과를 보여 20% 환자는 16주에 관해를 이루었고, 대부분의 평가에서 16주에 최고의 효과를 보였으며, 이러한 효과는 지속적으로 유지되었다[37,38]. 또한, 이러한 효과는 이전에 다른 TNF 길항제를 사용한 환자에서도 유의한 효과를 보였다[39]. Golimumab의 효과를 다른 TNF 길항제와 직접 비교한 연구는 없지만, 기존 연구에 대한 간접 비교에서 효과에 유의한 차이가 없었다[40].
강직척추염에서도 다른 TNF 길항제와 유사한 효과와 안전성을 보였으며[41-43], 건선 관절염에서는 관절염, 피부소견, 신체기능, 삶의 질과 더불어 피부와 조갑의 증상이 개선되었다[42]. 2009년 미국 식약청에서부터 류마티스 관절염, 강직척추염, 건선관절염에서 사용하도록 승인하였다. 이와 같이 golimumab은 중등도 혹은 중증의 류마티스 관절염, 강직척추염, 활성도가 높은 건선 관절염 환자에 대하여 단독 혹은 MTX를 비롯한 다른 고전적인 항류마티스제제와 병합요법이 권장되고 있다. 한 달에 한 번 50 mg을 피하투여하며, 다른 TNF 길항제와 같이 감염이 가장 흔한 부작용이고, 주사부위 반응, 두통 등이 흔히 발생하며, 일부 연구에서 천식의 급성 악화가 보고되어 천식 환자에서 사용 시 각별한 주의가 필요하다[1,44].

Certolizumab pegol

Certolizumab pegol (Cimzia™)은 TNF-α에 대한 PEGylated humanised Fab' 단일클론항체이며, pegylation을 통하여 mAb의 반감기를 늘리고 chimeric Fc 부위를 제거하여 보체-매개와 항체-의존에 의한 세포독성을 줄여 부작용을 줄이고 작용시간을 빠르고 길게 하고자 하였다. Certolizumab pegol 단독치료가 조기에 류마티스 관절염의 활성도를 줄이며[45], MTX와의 병합요법은 MTX 단독요법에 비해 6개월부터 관절염의 방사선학적 진행을 더 억제하는 것으로 보고되었다[46]. 최근에는 건선에 대한 phase II 연구에서 유효한 효과가 보고되었으며 추가적인 연구가 진행 중이다. 2009년 미국 식약청에서 중등도 이상의 류마티스 관절염에서 단독, 또는 병합요법으로 사용을 승인하였다. 현재 이 약제는 중등도 혹은 중증의 류마티스 관절염 환자에서 고전적인 항류마티스제제나 다른 TNF 길항제의 치료에 실패하였을 경우에 단독으로 혹은 MTX와 병용으로 사용이 권장된다. 피하주사하며 첫 달에는 2주 간격으로 400 mg을 투여하고 이후에는 2주 간격으로 200 mg을 투여한다. 다른 TNF 길항제와 비교하여 특이할 만한 부작용은 관찰되지 않았으며 부작용으로는 감염이 가장 많았다[44,46].

류마티스 질환에서 TNF 길항제의 적절한 사용

최근에는 류마티스 관절염, 강직척추염과 연소성 관절염의 치료에 많은 경험과 연구 결과 적절한 치료를 위한 치료 전략의 변화로, 조기 진단과 더불어 조기에 적절하고 충분한 치료를 위한 권고가 발표되었다[19,20]. 류마티스 관절염은 조기에 항류마티스제제나 생물학제제를 사용하여 적극적으로 치료하는 것을 권장하고 있으며, 특히 TNF 길항제는 MTX나 다른 두 가지 이상의 고전적 항류마티스제제의 사용에도 지속적으로 질병의 높은 활성도를 보이거나 높은 위험도를 가지고 있는 경우에 권장된다. 높은 위험도로는 1) 많은 수의 관절이 침범된 경우 2) 높은 류마티스 인자 혹은 항 CCP 항체를 가지고 있는 경우 3) 기능적 장애가 동반되었거나 영상학적으로 미란이 관찰되는 경우 4) 지속적으로 높은 ESR, CRP가 관찰되는 경우 5) 류마티스 관절염과 관련된 유전자를 가지는 등이다.
TNF 길항제를 사용 후에 적절한 반응을 얻지 못하는 경우에는 다른 TNF 길항제로 교환 것이 권장되고 있다[47]. Etanercept에 효과가 없는 경우에 infliximab이나 adalimumab을 효과적으로 사용할 수 있었으며, infliximab이 부작용으로 사용하지 못한 환자에서 etanercept을 사용할 수 있었다[48]. TNF 길항제를 지속적으로 유지하지 못하는 원인에 상관없이 다른 TNF 길항제가 효과적으로 사용될 수 있었다[49]. 그렇지만, 한 종류의 TNF 길항제에 효과가 없는 경우에는 다른 TNF 길항제에 효과가 적을 것으로 예측할 수 있어 이런 환자에서는 다음에 언급하는 다른 기전에 의해 작용하는 rituximab, abatacept이나 tocilizumab의 사용이 권장된다.
TNF 길항제는 사용 중에 다양한 원인에 의해 지속적으로 투여하지 못하게 된다. 한 연구에 의하면 1년 후에 21-35%의 환자가 중지하게 되었으며[49], 그 원인으로는 반응 부족, 반응 소실, 부작용 순이었다[48,50]. TNF 길항제의 안정성에 대한 문제가 시판 후에 평가되었으며, 감염, 악성 종양, demyelinating disorders, 재생불량성 빈혈, 울혈성 심부전과 혈관염 등에 대한 관심이 있었다. 주사 부위 반응이 가장 흔한 부작용이었으며(20-40%) [5], infliximab은 hypersensitivity reaction의 빈도는 적지만 발생하면 보다 심하였고, 세균 및 기회 감염의 빈도가 증가하였다. 또한 치료 시작 수개월 내에는 잠복결핵의 활성화에 따른 결핵이 발생하므로 치료 시작 이전에 잠복결핵에 대한 검사와 적절한 처치가 권해지고 있다. 아직까지 임상적인 중요성에 대해서는 확실하지 않으나 TNF 길항제 치료 중에는 항핵항체나 항dsDNA 항체가 나타날 수 있는 것으로 보고되었으며, MTX와 병용투여 시 빈도가 낮았다. TNF 길항제를 이용한 류마티스 관절염 환자의 임상시험에서 악성 종양이나 림프종이 보고되었으나, 류마티스 관절염 환자가 림프증식 질환이나 비호지킨 림프종의 위험도가 높은 것을 고려하면 이에 대한 추가적인 연구가 필요하다.
TNF 길항제를 사용 시 상대적으로 금기가 되는 경우는 전신 홍반성 루푸스 및 lupus overlap syndrome, 다발성 경화증, 시신경염, 탈수초성 질환, 심각한 감염, 만성 감염성 질환, 잠재 혹은 활동성 결핵, 만성 간염, 울혈성 심부전, 임신 등이 있으며 이러한 경우에서는 약제 사용에 많은 주의가 필요하다[1,19]. 또한 B형 간염을 가지고 있는 환자나 HBcAb 양성인 환자에서는 라미부딘과 같은 항 바이러스 제제를 동시투여하도록 하고 있다[1,19].

다른 류마티스 질환에서의 TNF 길항제

유사한 면역 기전을 보이는 다양한 류마티스 질환에서 생물학 제제의 효과가 보고되고 있지만, 류마티스 관절염이나 강직척추염과 달리 체계적인 연구가 적은 편이어서 아직까지 사용에 대해서는 한계를 갖고 있다. 단기간의 TNF 길항제의 사용이 루푸스 신염에서 효과를 보였다는 보고가 있으며[51,52], 일차성 쇼그렌 증후군에서는 무작위 임상시험에서 관절염과 같은 증상에는 효과가 있었지만 건조 증상에는 효과를 보이지 않은 결과를 보였다[53]. 염증성 근질환에서는 다양한 관찰연구에서 TNF 길항제의 효과가 충분하지 않고 오히려 간질성 폐렴이나 감염 등의 증가가 보고되었다[54]. 현재는 전신 홍반성 루푸스와 쇼그렌 증후군, 특발성 근염에서는 TNF 길항제의 사용은 권고되지 않는다. Anti- neutrophil cytoplasmic antibody (ANCA)-연관성 혈관염에서 etanercept가 효과는 없고 단지 고형 종양의 발생률만을 증가시켰다는 보고가 있었다. 전신 경화증 환자에서는 etanecept와 infliximab의 사용에 대한 관찰 연구가 있으며, 미란성 관절염이 동반된 경우에는 효과가 있는 것으로 보고하지만 아직 충분한 보고가 없으며[55], 거대세포 혈관염, 타카야수 혈관염에서 TNF 길항제의 효과에 대한 보고가 있으나[56] 추가적인 연구가 필요한 실정이다. TNF 길항제는 류마티스성 다발성 근육통, 성인형 스틸씨 병, 재발성 다발 연골염 등에서 다양한 치료에 반응하지 않을 경우에도 사용된 보고가 있다. 최근에는 TNF 길항제의 관절강 내 주입에 대한 연구도 진행되고 있으나 아직 코르티코스테로이드와 비교하여 더 좋은 효과를 나타내지는 못하고 있다[57].

Anakinra

Interleukin-1 (IL-1)은 1) T 세포와 B 세포를 활성화 시키고, 2) IL-6, TNF-α, IL-8과 같은 염증성시토카인을 증가시키며, 3) 염증성 매개체인 NO (nitric oxide), COX-2 (cyclooxygenase-2), PAF (platelet activating factor)의 분비를 촉진하여 활막세포의 증식을 촉진시키고, 4) 파골세포의 작용을 활성화하여 골흡수와 연골의 파괴를 증가시켜 류마티스 관절염 등의 류마티스 질환의 병인에 결정적인 역할을 한다. Anakinra (Kineret™)는 재조합 human IL-1Ra로 IL-1을 억제하는 작용을 하며, 항류마티스제제로 적절하게 조절되지 않는 류마티스 관절염 환자에서 임상적인 호전과 방사선학적 진행을 억제하는 효과가 보고되었으며[58], 2001년 미국 식약청이 류마티스 관절염에서 사용하도록 승인하였다. 작용기전이 다른 etanercept와의 병합요법은 부작용이 증가하고 중대한 감염의 증가로 권장되고 있지 않으며[8], TNF 길항제보다 우월한 장점이 없어 사용이 제한적인 편이다.
전신 홍반성 루푸스 환자에서는 anakinra가 미란성 관절염에 부분적 효과를 보이는 것으로 보고되었으나[8], 추가적으로 진행되는 연구는 없다. 이외에도 특발성 근염, 재발성 다발 연골염, 통풍, 가성통풍, 강직척추염 환자에서도 사용된 보고가 있으나 아직 논란의 여지가 있어 이들에 대해서는 추가적인 연구가 필요하다[57]. 쇼그렌 증후군에서 IL-1R antagonist polymorphism이 발견되어 효과가 있을 것으로 제시되었지만, 이에 대한 연구 결과가 부족하다. 용량은 100 mg을 피하로 매일 투여하며, 가장 흔한 부작용은 주사부위 반응과 감염이 증가하는 것이지만 결핵의 증가에 대한 보고는 없다.

Abatacept

B, T 림프구의 활성화에는 세포의 표면에 존재하는 수용체를 통한 항원-특이 신호와 항원제시세포에 존재하는 CD40, CD28과 같은 항원-비특이 ‘costimulatory’ 신호가 필요하다. 정상적인 면역반응 동안 활성화된 T 림프구는 cytotoxic T cell-associated antigen 4 (CTLA4)을 발현하며, 이는 CD86나 CD28과 반응한다. 따라서 이를 통한 CD86:CD28 반응의 억제는 염증과 조직의 파괴를 억제할 수 있다. 이를 근거로 CD28 costimulation antagonist인 CTLA4 Ig (abatacept; Orencia™)가 개발되었다. CTLA4 Ig는 human CTLA4의 extracellular domain과 human IgG1의 Fc domain의 fragment로 구성된 가용성 fusion protein이다.
Abatacept은 TNF 길항제 치료나 MTX에 적절한 반응을 보이지 않는 류마티스 관절염 환자에서 유효한 효과를 보였으며[60], 2년 이상의 연장 임상 연구에서 방사선학적인 관절 파괴의 진행을 억제하는 효과와 안정성이 보고되었다[61]. Abatacept는 고전적 항류마티스제제나 TNF 길항제에 부적절한 반응을 보이는 류마티스 관절염의 치료제로 2005년 미국 식약청의 승인을 받았으며, 우리나라에서는 2010년 승인을 받아 2011년부터 TNF 길항제에 부적절한 반응을 보이는 경우에 사용되고 있다. 그러나 Abatacept가 T 림프구의 활성화를 억제하므로 조기 류마티스 관절염이나 생물학 제제를 사용하지 않았던 환자에서 유의한 효과가 있어[62], 향후 MTX에 반응을 보이지 않는 조기 류마티스 관절염에 사용될 수 있을 것으로 기대된다.
강직척추염에서는 TNF 길항제에 실패한 환자에서 유의한 효과를 얻지 못했으며[63], CTLA4 haplotype/phenotype이 쇼그렌 증후군에서 관련이 있어 효과가 있을 것으로 추측되지만 이에 대한 연구가 없는 실정이다.
Abatacept는 단독 혹은 항류마티스제제와 병용투여가 가능하며 4주 간격으로 10 mg/kg의 용량을 정맥투여한다. 부작용으로는 감염이 가장 많았으며 특히 이전에 생물학적 제제를 사용한 뒤에 abatacept로 교체 투여하였거나 TNF 길항제와 함께 투여하였을 때 심각한 감염의 위험성이 증가하는 것으로 알려있으며, etanercept와의 병용투여는 감염과 같은 심각한 부작용 빈도를 증가시켜 사용을 권장하지 않는다[64]. 결핵과의 관련성은 동물 실험에서 TNF 길항제와 비교하였을 때 위험성이 낮게 관찰되었지만, 임상에서 투약 시에는 결핵에 대한 검사가 투약 전에 반드시 진행되어야 하며, 필요 시 적절한 처치가 필요하다.

B-cell depleting drug

B 림프구는 전신 홍반성 루푸스, 류마티스 관절염, 쇼그렌 증후군과 같은 류마티스 질환의 병인에 중요한 역할을 한다. B 림프구는 면역글로불린을 분비하는 형질세포의 전구세포이며, 효과적인 항원제시 세포로서 다양한 사이토카인의 생성을 통하여 다른 면역세포의 기능을 조절한다. 많은 자가면역 질환에서는 B cell hyperactivity를 보이며, 정상인과 달리 많은 양의 면역글로불린을 생성한다. 따라서 CD20나 CD22B 등의 B 림프구에 특이한 항원을 이용하여 B 림프구를 제거하는 방법이 류마티스 질환의 새로운 치료로 대두되고 있다.

Rituximab

Rituximab (Rituxan™, Mabthera™)은 B 림프구-특이 항원인 CD20에 대한 chimeric mouse-human mAb로 보체-매개, 항체-의존적 세포독성에 의해 apoptosis 유도와 세포 성장의 억제를 통해 선택적으로 B 림프구를 제거한다. 류마티스 관절염에서의 B 세포는 T 세포의 활성화와 염증성 사이토카인의 분비, 류마티스 인자 및 항CCP 항체 등의 생성에 관여한다.
Rituximab 단독 또는 MTX와의 병합요법이 류마티스 관절염에 효과적이고 안전하여 TNF 길항제에 실패한 중등도 이상의 류마티스 관절염의 치료제로 미국 식약청에서 승인하였다. Rituximab은 2년 이상의 연구에서 효과가 지속되고 관절의 파괴를 억제하였으며[65], TNF 길항제에 반응하지 않는 환자에서 다른 TNF 길항제로 교체하는 것보다 rituximab을 사용하는 것이 보다 더 효과적이어서[66] TNF 길항제에반응하지 않는 경우에 바로 사용할 수 있다. 강직 척추염과 건선 관절염 환자에서 TNF 길항제를 사용하지 않은 환자에서 유의한 효과를 보였지만, TNF 길항제에 실패한 환자에서는 유의한 효과를 얻지 못했다[67]. 따라서 TNF 길항제를 사용할 수 없는 환자에서 사용될 수 있을 것으로 보이지만 추가적인 연구가 필요하다.
류마티스 관절염 외에 다른 결제조직 질환에 대해서도 rituximab의 효과에 대한 연구가 진행되고 있다. 전신 홍반성 루푸스에서 rituximab이 효과가 있다는 관찰연구가 있었고, phase I/II 임상 연구에서 항체를 감소시키고 B 세포를 효과적으로 제거하였지만, phase III 연구에서는 항-dsDNA 항체를 낮추고 C3/C4를 높였으나 임상소견의 호전이 없어 원칙적으로 사용이 권장되지 않는다[68]. ANCA와 관련된 혈관염에서는 질병의 관해에 rituximab이 싸이클로포스파마이드에 비교하여 열등하지 않았으며[69], 2011년 스테로이드와의 병합요법이 미국 식약청의 승인을 얻게 되었다. 특히 심각한 폐출혈이나 신질환이 동반되는 경우에 대해서는 싸이클로포스파마이드에 실패한 경우에 사용하도록 권장하고 있다. B 림프구는 쇼그렌 증후군의 병인에 중요한 역할을 하므로 치료의 표적이 될 수 있고 현재까지 소규모의 임상연구에서 효과가 있어 샘외(extra-grandular) 증상에 대해서만 고려해 볼 수 있으나 추가적인 연구가 필요하다. 염증성 근질환에서는 근력과 피부 및 간질성 폐렴이 호전된 보고가 있어 추가적인 연구가 진행되고 있으며, 전신성 경화증 환자에서는 피부증상에는 적응증이 되지 않으나 간질성 폐질환이 동반된 경우에 사용할 수 있다. Rituximab은 현재 거의 대부분의 질환에서 2nd line 치료로 권장되고 있으며 명확한 효과를 나타내는 질환은 류마티스 관절염이다. 다른 결체조직 질환에서는 아직 논란이 있으나 효과가 있을 것으로 생각되는 질환들에 대해서는 추후 더 많은 연구가 진행되어야 할 것으로 보인다[57].
Rituximab은 정맥으로 투여하며 2주 간격으로 2회 투여하는 것을 한 번의 cycle로 한다. 과민반응의 예방을 위하여 스테로이드의 전처치가 권장되며, 발생 가능한 쇼크 등에 대비하여 심폐소생술 장비를 함께 비치하는 것이 바람직하다. 부작용으로는 감염이 가장 많으며, transfusion reactions, 저감마글로불린혈증, rituximab-specific human anti-chimeric antibodies (HACA) 생성, sickness-like disorder 등이 있다. 결핵의 위험을 증가시킨다는 보고는 없으나 투여 전에 결핵에 대한 검사를 시행하는 것이 권고된다. 또한 B형 간염 환자나 HBcAb 양성인 환자에서는 항 바이러스 제제를 동시 투여하도록 하고 있다[1].

Epratuzumab

CD22는 B 림프구의 활성화를 조절하고 T 림프구와 반응하는 물질로 B 림프구의 발생과 생존에 필수적이다. Epratuzumab은 B 림프구의 표면에 특이하게 존재하는 CD22에 대한 mAb이다. 중등도의 전신 홍반성 루푸스 환자를 대상으로 시행한 phase IIb 연구에서 임상적인 호전이 보고되어[70] 현재는 3상 연구가 루푸스 신염 환자를 대상으로 진행 중에 있다. 또한 16명의 일차성 쇼그렌 증후군 환자에서 epratuzumab을 이용한 open-label, phase I/II 임상 연구에서 효과를 보였으나[71] 추가 연구가 필요하다. Infusion reaction과 human antihuman/epratuzumab antibodies (HAHA)가 보고되었다.

Belimumab

B-lymphocyte stimulator인 BLyS (BAFF, TALL-1, THANK,TNFSF20)는 TNF superfamily에 해당하는 사이토카인으로 대식세포나 다른 항원제시세포에 발현하여 B 림프구의 분화,증식과 면역글로불린의 생성에 결정적인 역할을 한다. 일부연구에서 혈청 BLyS치가 전신 홍반성 루푸스 환자에서 증가해 있으며, 질병의 활성도, 항-dsDNA 항체와 Ig치와 관계를 보였으며, 쇼그렌 증후군 환자에서도 혈청과 침샘에서BLyS가 증가해 있고 혈청 자가항체 역가와 관련이 있는 등병인에 관련이 있어 새로운 치료의 표적으로 받아들여지고있다. BLyS에 대한 mAb인 belimumab (LymphoStat-B™)이 전신 홍반성 루푸스에서 혈청학적 검사를 호전시킨 보고가 있어[72], 새로운 치료제로서의 가능성을 제시받고 있다.

Tocilizumab

Interleukin-6 (IL-6)는 다양한 생물학적 기능을 갖는 사이토카인으로 면역반응, 염증, 급성기 반응과 hematopoiesis에 중요한 역할을 한다. 가장 중요한 생물학적인 역할은 B 림프구의 성숙을 자극하는 것이며, IL-6의 영향하에서 B 림프구는 성숙된 형질세포로 분화하여 면역글로불린을 분비하는 세포가 된다. IL-6는 또한 IL-2 및 IL-2 수용체를 유도하여 T 림프구의 성장과 분화에 관여한다. IL-6는 류마티스 관절염, 연소성 관절염, 쇼그렌 증후군, 전신성 홍반성 루푸스와 같은 류마티스 질환의 병인에 관련이 있다. Tocilizumab (Actemra™)은 IL-6 receptor에 대한 재조합 mAb로 IL-6를 억제한다.
류마티스 관절염에서 tocilizumab 단독 또는 MTX와의 병합요법이 MTX 단독요법에 비해 유의한 효과를 보였으며[73,74], 고전적 항류마티스제제와 TNF 길항제에 실패한 류마티스 관절염에서도 위약에 비해 신체 기능, 피로감 등에 유의한 효과를 보였으며[75], 3년간의 연장 연구에서는 방사선학적인 진행이 억제되었다[76]. 한 가지 이상의 항류마티스제제나 TNF 길항제에 실패하거나 TNF 길항제를 사용할 수 없는 중등도 이상의 류마티스 관절염의 치료제로 2010년 미국 식약청의 승인을 받았다. 연소성 관절염과 강직성 척추염에서 유효한 효과가 보고되었으며, 전신 홍반성 루푸스에 대한 phase I 연구에서 안전성이 보고되어[77] 추가적인 연구가 진행 중이다.
권장되는 용량은 매 4주마다 4 mg/kg을 정맥투여 한다. 안전성 연구에서 감염이 가장 흔하며, 심각한 부작용으로는 위장관계의 천공의 위험이 높아진다는 보고가 있어 이 약제를 사용 시에 새롭게 복통 등을 호소하는 경우에 주의를 기울여야 한다. 이외에도 호중구감소증, 혈소판 감소증, 간효소 수치의 증가, 혈중 지질의 증가 등이 나타날 수 있는 것으로 되어 있어 4-8주 간격의 혈액검사 관찰이 필요하다[76,77].

다른 생물학 제제

다양한 사이토카인과 세포내 신호전달물질이 류마티스 질환의 병인에 관련되는 것으로 알려지면서 이를 표적으로 하는 다양한 생물학제제에 대한 연구가 진행되고 있다.

Alefacet

항원제시세포의 LFA-3와 T 림프구의 CD2 간의 반응을 억제하는 fully human fusion protein으로 T 림프구의 활성화를 억제한다. 건선에서의 사용이 미국에서 승인되었으며, 건선 관절염에서 효과가 있는 것으로 phase II 연구에서 보고되어[78] 추가적인 연구가 진행 중이다.

Ustekinumab

Interleukin 12/13 (IL-12/13)의 p40 subunit에 대한 mAb로 TH17와 TH1 pathway를 억제한다. 미국 식약청에서 건선에서 사용할 수 있도록 승인하였으며, 건선 관절염에서 효과가 있는 것으로 phase II 연구에서 보고되었으며[79], 추가적인 연구 결과가 기대된다.

Tofacitinib

Janus kinase 3와 같은 kinases는 interleukin의 신호 전달에 중요한 역할을 담당하는 세포내 물질이다. 이 효소를 억제할 수 있으며 구강으로 복용 가능한 Janus kinase 억제제인 tofacitinib (CP-690550)이 개발되었으며, MTX에 부적절한 반응을 보이는 류마티스 관절염 환자에서 MTX와의 병합요법이 유의한 효과와 안전성을 보였다[80]. 항류마티스제제에 불충분한 반응을 보인 환자를 대상으로 한 phase IIb 연구에서도 유의한 효과와 안전성을 보여[81] 추가적인 연구가 진행되고 있다.

결 론

최근 류마티스 질환의 병인에 대한 이해의 증가와 기술의 발전으로 특정 병인의 기전에 근거를 둔 생물학적 제제가 류마티스 질환의 치료에 도입되었고, 고전적인 항류마티스제제에 효과가 없는 환자들에게 사용할 경우에 우수한 효과를 보이고 있다. 최근에는 다양한 특정 기전을 표적으로 하는 생물학제제가 개발되어 기존의 제제에 비해 보다 우수한 제제가 사용될 수 있기를 기대하게 한다. 그렇지만, 이러한 생물학적 제제들은 탁월한 효과에도 불구하고 기존의 항류마티스제제와 비교하여 비싼 가격과 다양한 부작용을 가지고 있으며 예측하지 못하는 반응이 있어나는 경우도 있기에 사용에 주의를 기울여야 하겠다. 또한 최근에 새로이 개발된 생물학적 제제들은 아직 관찰 기간이 짧으며 많은 질환에서 논란의 여지가 많아 추후 더 많은 연구가 필요할 것으로 생각된다.
