간경변성 복수 환자에서 Vaptan계열 약제의 유용성

Abstract

Ascites is the most common complication of liver cirrhosis, but its pharmacological management is unsatisfactory in some patients because of a lack of response to treatment with conventional diuretics. Patients with cirrhosis and ascites generally have increased non-osmotic secretion of vasopressin which participates in the pathogenesis of fluid retention. Vaptans are a new family of orally active drugs that increase urine volume by antagonizing specifically the vasopressin receptors in the principal cells of the collecting ducts that have been shown to correct dilutional hyponatremia effectively. They also seem to be promising in the management of ascites by reducing the increased extracellular fluid volume in conditions associated with water retention including liver cirrhosis. However, there is a paucity of information on whether vaptans might have beneficial effect in enhancing ascites reduction in patients with cirrhosis. This review addresses the pharmacological actions of vaptans, their clinical applications, and future potential roles in managing patients with liver cirrhosis and ascites. 서 론

복수는 간경변증의 가장 흔한 합병증으로, 대상성 간경변증 환자의 경우 10년 이내에 약 60%의 환자에서 발생한다[1]. 발생기전은 간경변으로 간내 혈관 저항(intrahepatic vascular resistance)이 증가하여 문맥압이 항진되면, 전신 또는 내장 동맥혈관 내피가 신장(stretching)하게 되어, nitric oxide와 같은 강력한 혈관확장 물질 생성을 촉진한다[2-4]. 증가된 혈관확장 물질은 내장혈관 확장(splanchnic vessel vasodilatation)을 유발하고, 내장 혈관의 혈액량 증가를 초래하지만, 상대적으로 전신 유효 혈류량(effective circulating volume)은 감소하게 된다[5-7]. 이에 대한 보상기전으로 레닌-안지오텐신-알도스테론 체계(rennin angiotensin aldosterone system, RAAS), 아르기닌-바소프레신(arginine-vasopressin, AVP), 교감신경계 등이 활성화되고, 이들은 신장에서 나트륨 저류와 수분 재흡수를 촉진하여, 임상적으로 복수, 하지 부종 및 저나트륨혈증(hyponatremia)을 유발한다(Fig. 1).
복수 치료는 염분 섭취 제한과 함께 알도스테론 길항제인 spironolactone, 루프이뇨제인 furosemide 등의 이뇨제 투여를 원칙으로 한다[8]. 하지만 최대 용량의 이뇨제 복용에도 불구하고, 조절되지 않는 난치성 복수(refractory ascites)가 발생하는 경우, 치료 목적으로 대량복수천자(large volume paracentesis, LVP)를 시행한다[9-11]. 그럼에도 불구하고 상당수 환자는 복수가 바로 재발하여, 복수 천자를 자주 시행 받아야만 하는데, 반복적 복수 천자는 감염의 위험 증가, 단백질 소실로 인한 영양 결핍, 삶의 질 저하 등을 초래할 수 있다[12]. 이런 경우 간내문맥전신 단락술(transjugular intrahepatic portosystemic shunt, TIPS), 간이식 등을 고려하지만[13,14], TIPS의 경우 간성혼수, 신기능 장애 등의 부작용으로 난치성 복수 환자의 약 절반 정도에서만 시행 가능하며[15], 근본적 치료가 가능한 간이식술도 공여자 부족, 비용 등의 이유로 매우 제한된 환자에서만 시행하고 있다.
최근 신장의 집합관(collecting tubule)에서 이루어지는 수분 재흡수 과정을 차단함으로써, 나트륨, 칼륨 등 전해질에는 영향을 미치지 않고 소변으로 오로지 수분(electrolyte-free water)만을 배출하도록 유도하는 vaptan계열 약제가 개발되었다[16,17]. 대표적 약제로 conivaptan과 tolvaptan이 있으며[18,19], 이들 약제는 심부전, 항이뇨호르몬 분비이상 증후군(syndrome of inappropriate antidiuretic hormone secretion, SIADH) 및 간경변증 환자에서 저나트륨혈증의 치료제로 미국과 유럽 등에서 승인 받아 이들 지역에서는 현재 임상에 사용하고 있다. 본 종설에서는 현재까지 발표된 문헌들을 토대로 vaptan계열 약제의 작용 기전과 저나트륨혈증에 대한 임상 효과 및 새로운 간경변성 복수 치료제로써의 가능성에 대해 고찰해 보고자 한다.

본 론

Vaptan계열 약제의 작용

항이뇨호르몬(antidiuretic hormone, ADH)으로도 불리는 AVP는 전신 체액 조절의 항상성(homeostasis) 유지에 중요한 역할을 담당한다. 이와 결합하는 AVP 수용체(AVP receptor, VR)는 세 종류의 아형(subtype)이 있는데, 이 중 V1a 수용체(V1aR)는 혈관 평활근 세포(smooth muscle cells), 혈소판, 간세포 등에 분포하여 혈관 수축, 혈소판 응집, 글리코겐 분해 등에 관여한다. V1b 수용체(또는 V3 수용체라고도 함)는 뇌하수체 전엽, 뇌, 췌장 등에 분포하여 부신피질자극 호르몬(adenocorticotropic hormone, ACTH) 방출, 스트레스 적응 및 인슐린 분비 등과 관련이 있다[20,21]. V2 수용체는 주로 신장에 분포하여 수분의 재흡수에 관여를 한다(Table 1).
AVP에 의한 수분 재흡수 과정은 AVP가 신장의 집합관에 분포되어 있는 V2 수용체와 결합하면 G-protein을 경유하는 adenylyl cyclase가 활성화되고, cAMP의 생성을 유발하여 세포내 cAMP 농도가 증가한다. 증가된 cAMP는 protein kinase A (PKA)를 경유하여 수분 통로인 aquaporin-2 (AQP2)를 인산화(phosporylation)하며, 인산화된 AQP-2를 함유한 소포(AQP2 containing vesicle)가 자유막에 삽입하여 수분 통로가 열림으로써 수분 재흡수 과정을 촉진한다(Fig. 2) [22]. 따라서 V2 수용체가 차단되면, 수분 재흡수도 차단되며, 결과적으로 수분 성분만 소변으로 배설하는 aquaresis 효과가 나타나는데, 이러한 작용을 하는 V2 수용체 길항제(antagonist)를 aquaretics라고도 한다[16].
V2 수용체 길항제 중 peptide 계열 약제는 이미 1960년대 개발되었으나, 반감기가 짧고, 장기간 투약시 오히려 부분적 작용제(partial agonist) 역할을 하며, 생체이용률(bioavailability)이 매우 낮아 경구용으로는 사용할 수 없다는 단점이 있어서 실제로 임상적 유용성은 매우 적었다[23]. 하지만 현재 이용되는 V2 수용체 길항제는 non-peptide 계열로 ‘vaptan계열 약제’로 명명하기도 하며, 장기간 투약이 가능하고, 경구용으로 개발되어 있어 임상석 유용성이 높다[19,24,25]. 이들 약제는 현재 마이너스 수분 균형(negative water balance)를 통해 저나트륨혈증을 개선하기 위한 목적으로 사용할 수 있지만, 체액량 조절 자체를 목적으로 사용하는 것은 공인되지 않다. 탈수 등 저혈량성 저나트륨혈증(hypovolemic hyponatremia) 환자에서 사용하면 안되며, cytochrome P450 3A에 의해 약물이 대사되므로, 강력한 cytochrome P450 3A 억제제인 ketoconazole, clarithromycin, itraconazole 등과 병용 투여는 금기이다[26]. 가능성은 극히 희박하지만 심실부정맥을 유발할 수 있으므로 투여기간 중 심전도의 QT간격 변화를 주기적으로 관찰하여야 한다. 갈증, 구강건조증, 기립성 저혈압, 피로감 등의 부작용이 발생할 수 있으며, 빈맥, 고나트륨혈증, 크레아티닌 상승 등으로 약제 투여가 중단되는 경우도 있다[27]. 혈액응고인자에 영향을 주어 출혈을 유발할 수 있다는 보고가 있으므로 출혈 소인이 있는 간경변증 환자에서는 이 점에 대한 각별한 주의가 필요하다[28,29].

저나트륨혈증 치료에 관한 임상연구

현재까지 임상연구가 진행된 vaptan계열 약제로는 V2 수용체를 길항하는 tolvaptan, lixivaptan, satavaptan, mozavaptan 등이 있으며, V1a 수용체와 V2 수용체를 동시에 길항하는 conivaptan이 있다(Table 2). 이 중 conivaptan은 미국 식약청(US Food and Drug Administration, US FDA)으로부터 2005년, 2007년에 각각 정상혈량성 저나트륨혈증(euvolemic hyponatremia)과 과혈량성 나트륨혈증(hypervolemic hyponatremia)의 치료제로 공인받았다. Tolvaptan은 2008년에 미국 식약청으로부터 저나트륨혈증 치료제로 공인받았으며, 2009년에는 유럽의약안정청(European Medicines Agency, EMEA)으로부터 SIADH 환자의 저나트륨혈증 치료제로 공인받았다[26]. Mozavaptan의 경우 2006년 일본에서 SIADH 환자의 치료제로 공인받았다[30]. 그 외 lixivaptan, satavaptan 등에 대한 임상연구가 진행되고 있다.
공인된 약제를 중심으로 살펴보면 conivaptan의 경우 경구용, 주사용 제제가 모두 있으나, 미국식약청에서 허가된 기준은 저나트륨혈증 치료 목적으로 혈청나트륨 모니터링이 가능한 입원 환자에서 4일 이내의 주사제를 투약하는 것으로 제한하고 있다. 부종 등 체액 조절 목적으로는 승인되어 있지 않다. 심부전 환자와 SIADH 환자를 대상으로 한 다기관 임상연구에서 주사제 conivaptan을 20 mg 부하용량(loading dose) 투여한 후 4일간 매일 40 내지 80 mg으로 정주하면 대략 6에서 9 mEq/L 정도의 혈청 나트륨 상승 효과가 있었다(Fig. 3A) [31]. 단기간(4일 내지 5일) 사용한 경구용 제제에서도 효과적으로 저나트륨혈증이 교정되었다[18]. 하지만 심부전 환자에서 심박출량 증가, 활동력 증가, 삶의 질 향상 등의 임상 지표에 대한 개선 효과는 뚜렷하지 않았다[18,32]. Tolvaptan의 경우 저나트륨혈증 치료 효과를 평가하기 위한 대규모 다기간 임상연구가 진행되었다. 이 중 15 mg 부하용량으로 시작하여 매일 30 내지 60 mg 용량으로 1개월 동안 투여한 연구(SALT-1/SALT-2연구)에서는 심각한 부작용 없이 효과적으로 저나트륨혈증이 교정되었다[19]. 심부전 환자를 대상으로 1년 이상 장기 투여한 연구 결과(EVEREST 연구)에서도 저나트륨혈증이 효과적으로 교정되었고(Fig. 3B), 호흡곤란, 부종, 체중 변화 등의 임상 지표도 어느 정도 개선 효과를 보였다[33,34]. 하지만 EVEREST 연구에서 일차평가지표(primary endpoint)로 판단하고자 했던 사망률 감소, 입원기간 단축 등은 통계적으로 유의한 효과를 보이지 못했다. 일본에서 공인된 mozavaptan의 경우, SIADH 환자를 대상으로 7일간 30 mg 단일 용량으로 투약한 임상연구에서 저나트륨혈증이 효과적으로 개선되었음을 보고하였고, 투약 기간 동안 약제로 인한 심각한 부작용은 발생하지 않았다[30].

간경변성 복수 치료에 관한 임상연구

간경변증 환자를 대상으로 한 vaptan계열 약제의 임상연구를 분석해 보면(Table 3), tolvaptan과 lixivaptan은 주로 복수 조절 목적보다는 저나트륨혈증 개선을 일차 평가지표로 연구가 진행되었다. Tolvaptan의 대규모 임상연구 중 하나인 SALT연구에서는 63명의 간경변증 환자가 포함되어 있다. 이들에 대한 하위 분석(subgroup’s analysis)이 최근 발표되었는데, 복수 조절에 대해서는 평가 지표에 포함되어 있지 않아 이에 대해서는 파악할 수 없다. 다만 이뇨제와 함께 병용 투여하여도 신기능에 미치는 영향은 적은 것으로 보고하였다[35]. 최근 일본에서 소규모(18명) 비대상성 간경변성 복수 환자를 대상으로 진행된 다기관 임상연구에서는 15 내지 60 mg의 tolvaptan을 3일간 투여한 결과 효과적으로 복부 둘레, 하지 부종과 체중이 감소하였다[36]. 이런 효과는 처음 부하용량에서부터 나타나기 시작했으며, tolvaptan 용량을 증량할수록 더 효과적이었다.
Lixivaptan은 60명의 간경변증 환자를 대상으로 시행한 다기관, 위약대조 임상연구에서 7일간 100 또는 200 mg 용량으로 투여한 결과 저나트륨혈증 개선과 체중 감소의 효과를 보였다. 이 연구에서 lixivaptan의 용량을 증량함에 따라 체중 감소 효과도 뚜렷했지만, 동시에 갈증 등의 부작용 발생 빈도도 더 증가하였다. 또한 위약대조군에 비해 통계적 유의성은 없었지만, 일부 환자에서 신기능 장애, 위장관 출혈 등이 발생했다[37]. 25명의 간경변증 환자를 대상으로 시행한 또 다른 연구 결과에서도 저나트륨혈증 개선에는 도움이 되었지만, 일부 환자에서 탈수, 자발성 세균성 복막염, 상부위장관 출혈이 발생하였고, 2명의 환자에서 간성 혼수가 발생하였으며, 이 연구의 평가지표는 아니지만 복수가 감소하는 환자는 없다고 보고하였다[38]. Lixivaptan의 경우 2000년대 초반에는 주로 간경변증, SIADH 환자를 대상으로 연구가 진행되었지만, 최근에는 THE BALANCE 연구라는 명칭으로 심부전 환자를 대상으로 대규모 임상연구가 진행 중이며[39], 간경변증 환자를 대상으로 한 추가 연구는 진행되지 않고 있다.
Conivaptan은 V1a 수용체도 함께 길항하는데, 이런 경우 내장혈관이 확장되어 문맥압이 증가하고 혈소판 응집을 저해할 수 있기 때문에 문맥압 항진증과 정맥류 출혈 등의 가능성이 있는 간경변증 환자를 대상으로 사용하기에는 부적절하다고 평가받고 있다[40]. Mozavaptan의 경우 같은 제조사(오츠카 제약회사)에서 생산하는 tolvaptan에 비해 역가(potency)가 떨어지는 것으로 자체 평가되고 있으며, 따라서 대규모 임상연구가 진행되기는 어려울 것으로 판단된다.
현재까지 저나트륨혈증 동반 유무와 상관없이 간경변성 복수를 치료 목적으로 임상연구가 진행된 vaptan계열 약제는 satavaptan이 유일하다[25,41-43]. HypoCAT trial로 명명된 다기관, 이중맹검, 무작위배정, 위약대조 임상연구에서는 저나트륨혈증이 동반된 간경변성 복수 환자 110명을 대상으로 하였으며, 모든 환자에서 spironolactone 100 mg을 투약하였고, 대상군에게는 추가적으로 satavaptan 5, 12.5, 25 mg을 14일간 투약하였다[25]. 연구 결과 저나트륨혈증 개선뿐 아니라, 체중 감소 및 복부 둘레가 감소하였고, 갈증을 제외한 부작용은 위약대조군에 비해 차이를 보이지 않았다. 이 연구와 비슷한 시기에 동일한 책임연구자들이 주도한 다기관 이상(phase II) 임상연구에서는 저나트륨혈증 유무와 무관하게 간경변성 복수가 있는 173명의 환자를 대상으로 satavaptan 5, 12, 5, 25 mg을 12주 동안 투약하였고, 대상 환자군 및 위약대조군 모두에게 spironolactone 100 mg을 투약하였다[42]. 연구 결과 일차평가지표인 일주일간 복수 증가량은 위약대조군에 비해 통계적으로 유의한 성적을 보이지 못했지만, 이차평가지표인 12주간의 복수천자 횟수는 satavaptan 투약군에서 복수천자 횟수가 통계적으로 유의하게 적었다. 전체 부작용 발생 빈도는 satavaptan 투약군과 위약대조군이 차이를 보이지 않았지만, 기립성 저혈압, 간성혼수, 근육 경련(muscle cramp), 고칼륨혈증, 갈증 등의 부작용 발생 빈도는 satavaptan 투약군에서 높은 경향을 보였다. 이후에도 같은 책임연구자들은 1,200명의 대규모 환자를 대상으로 satavaptan 5 mg, 10 mg을 장기간(52주) 투약하는 다기관, 위약대조 삼상(phase III) 연구를 진행하였다[41]. 대상 환자는 간경변성 복수 환자로, 대상 환자의 평균 혈청나트륨 농도가 137 mmol/L 정도로 저나트륨혈증 환자는 많지 않았다. 연구는 복수 양상에 따라 세 군으로 나누어 연구가 진행되었는데, 첫 번째 군은 이뇨제(평균 이뇨제 복용량; spironolactone 100 mg, furosemide 40 mg 병용 투여)는 복용하지만 대량복수천자가 필요 없는 환자를 대상으로 하였으며, 두 번째 군은 이뇨제를 복용하면서 대량복수천자가 필요한 환자를 대상으로 하였다. 세 번째 군은 대량복수천자가 필요한 환자에서 satavaptan 이외의 이뇨제를 12주간 투약하지 않는 상태로 연구가 진행되었다. 일차평가지표는 첫 번째 군의 경우 첫 12주 동안 복수의 악화 여부에 따라 평가하였고, 두 번째 군과 세 번째 군에서는 복수의 재발 여부, 즉 약물 투여 직전 복수 천자 후, 12주 동안 복수천자 횟수로 평가하였다. 연구 결과 각 군에서 모두 satavaptan 투약군이 위약대조군에 비해 통계적으로 유의한 치료성적을 보이지 못했다(Fig. 4A). 다만 첫 번째 군에서 이차평가지표 중 하나인 복수천자량은 satavaptan 투약군이 위약대조군에 비해 유의하게 줄었으며, 두 번째, 세 번째 군에서 약물 투약 후 첫 번째 복수천자가 필요한 시점이 satavaptan 투여군이 위약대조군에 비해 유의하게 늦게 발생하였다. 하지만 결과적으로 일차평가지표 평가에서 효과적인 치료성적을 보이지 못함으로써, satavaptan 투여가 복수 조절에 있어 임상적으로 효과적이지 않다고 결론지었다. 더욱이 결정적으로 두 번째 군에서 satavaptan 투여군이 위약대조군에 비해 52주 이내 사망률이 통계적으로 유의하게 높은 결과를 보여 주었다(Fig. 4B). 이와 같은 연구 결과로 인해 Sanofi-Aventis 회사가 Aquilda라는 상품명으로 satavaptan을저나트륨혈증 치료 이외에도 간경변증 환자의 복수 치료 등 좀 더 폭 넓은 적응증으로 시판하고자 했던 야심찬 계획은 차질이 불가피해졌으며, 결국 유럽의약안정청에 제출된 판매승인 신청도 철회된 상태이다[28].

향후 연구 과제

현재까지 임상 결과 중 간경변성 복수 환자에서 vaptan계열 약제가 저나트륨혈증의 치료 이외에 효과적인 복수 조절, 생존률 향상 등 뚜렷한 임상 지표의 개선을 보여주지는 못했다. 따라서 아직까지 저나트륨혈증의 개선이 목적이 아닌, 복수나 부종 등 체액량 조절을 위한 치료제로 vaptan계열 약제의 적응증을 넓히기는 그 근거가 부족하다. 향후 복수 조절의 치료 등을 목적으로 약제의 유용성을 넓히기 위해서는 간경변성 복수 환자의 임상적 특성을 고려한 단계적 접근과 연구가 필요하다.
안정성 면에서 보면, 간경변성 복수 환자의 경우 이뇨제를 복용하는 경우가 대부분이기 때문에 vaptan계열 약제의 추가 투여, 또는 변경 투여가 치료의 상승 효과(synergistic effect)내지는 부가 효과(additional effect)를 가져올지, 아니면 오히려 탈수, 신기능 저하 등 부작용 발생 빈도를 높일지에 대한 평가가 진행되어야 한다. 현재까지 진행되었던 임상연구 중 장기간 약물을 투약한 임상연구는 상당수의 환자에서 부작용으로 인한 중도 탈락을 보이는데, 이들 부작용의 약제 관련성 여부와 중증 정도에 대한 면밀한 검토가 진행되어야 한다. 특히 정맥류 출혈 등 출혈 소인이 높은 간경변증 환자에서 vaptan계열 약제가 출혈에 미치는 영향에 대한 추가적인 정보 수집이 필요하다. 최근 미국식약청의 검토 내용 중에서 tolvaptan이 비타민 K 의존성 혈액응고인자와 혈소판의 기능에 영향을 미칠 수 있다고 논의되었다[28]. 간경변성 복수 환자의 상당수는 Child-Pugh class C 환자에 해당하며 이들 환자에게 vaptan계열 약제의 장기간 투약이 안전한지에 대한 고찰도 필요하다.
유용성 면에서 보면, 간경변성 복수의 발생 기전은 AVP 호르몬 이외에 복합적 요인이 작용하므로, AVP 수용체 길항제 투여로 임상 효과 개선을 보일 수 있는 환자군을 선별하는 과정이 필요하고, 치료 효과를 최대화하면서 장기간 투약하여도 부작용을 최소화 할 수 있는 적정 용량에 대한 분석이 선행되어야 한다. 간경변증 환자에서 저나트륨혈증은 간성뇌증을 유발하고, 간이식에 부정적 영향을 미칠 수 있으며, 생존률 저하와 관련된 것으로 보고하고 있다. 따라서 약제에 의한 인위적인 혈청 나트륨 개선과 복수 조절이 장기적으로 생존률에 어떠한 영향을 미치며, 임상 증상 개선 등 삶의 질 향상에 어떤 기여를 할 수 있는지에 대해서도 접근되어야 한다. 마지막으로 vaptan계열 약제의 장기간 투약시 비용-효과(cost effectiveness)면에서 충분한 이득이 있는지에 대한 평가도 이뤄져야 할 것이다.

결 론

간경변증 환자에서 복수와 저나트륨혈증 발생의 병태생리학적 과정은 다양한 요소가 작용하지만, 이들 과정 중 AVP의 증가가 복수, 저나트륨혈증 발생 모두에 있어 중요한 요인으로 추정되고 있다. 하지만 AVP 수용체 길항제인 vaptan계열 약제의 임상연구 결과에서 저나트륨혈증 치료제로는 뚜렷한 임상 효과를 보이는 반면, 간경변성 복수 치료제로써의 연구 결과는 매우 부족하거나, 일부 결과는 미흡한 치료성적을 보여주고 있다. 그럼에도 불구하고 상당수 간경변성 복수 환자는 현재의 표준 치료에 한계가 있으며, vaptan계열 약제 등 새로운 치료제의 도움이 필요한 상황이다. 아직까지 어느 시점에서 어떤 환자를 대상으로, 어느 정도의 적정 용량으로, 얼마만큼의 기간 동안 투약하고 치료해야 할지에 대한 자료가 매우 부족한 실정이며, 향후 이에 대한 추가적인 임상연구가 기대된다.
