심방세동의 기전

Abstract

Atrial fibrillation (AF) remains the most common adult rhythm disorder, and it associated with a substantial rate of morbidity and economic burden. AF involves a wide spectrum of arrhythmias from lone AF to paroxysmal to chronic AF. It is likely that AF comprises a spectrum of disease with no single mechanism adequate enough to comprehensively explain AF and its variability. Mechanism of fibrillation is explained by multiple wavelets and focal activation theories. Electrical, contractile and mechanical remodeling is involved in AF progression. Atrial remodeling may also increase in atrial fibrosis which can slow conduction velocity and can shorten the refractory period in atria with long-standing AF. Mechanical remodeling manifests as decreased atrial contractility and increased atrial compliance which leads to a stretch of the atrial myocardium. Abnormal intracellular calcium dynamics is observed in AF. Modulating factors such as genetic factors, age, obesity, sleep apnea, inflammation, autonomic factors and atrial and pulmonary vein stretch only partially account for the increase in AF. It is still unclear whether initiation of AF activates direct inflammatory effects or whether the presence of a pre-existing systemic inflammatory state promotes further persistence of AF. Although significant progress in understanding the mechanism of this arrhythmia has been accomplished, the pathophysiology of AF is complex and likely has many possible mechanisms which may be interrelated. 서 론

심방세동(atrial fibrillation)은 임상적으로 가장 흔한 부정맥으로 국내의 정확한 통계는 없지만, 미국의 경우 2.3백만 명 이상의 환자가 있는 것으로 알려져 있다. 심방세동은 다른 동반 질환이 없이 발생하는 경우(lone AF)부터 만성적으로 발생한 경우까지 다양한 스펙트럼을 보인다. 울혈성심부전, 판막질환, 뇌졸중, 좌심방확장, 고혈압, 고령이 심방세동 발생의 중요한 독립적 인자로 알려져 있다. 최근에는 비만, 수면 무호흡증, 장기간의 음주 등이 심방세동의 증가와 연관됨이 밝혀졌다. 심방세동의 기전에 대하여는 지금까지 많은 연구들이 이루어져 왔지만 아직까지는 정확한 기전은 밝혀지지 않았고, 한 가지 기전이 아니라 다양한 기전이 연관되어 있을 것으로 생각한다.

본 론

세동을 설명하는 두 가지 대표적 이론

심방세동 기전은 “multiple wavelets”와 “focal activation”의 이론에 의하여 설명이 되고 있다. “Multiple wavelets” 이론은 다수의 무질서하게 움직이는(wandering) 회귀성 소파형들이 서로 부딪히고, 자발적으로 종료되고, 새롭게 만들어지면서 심방세동을 만든다는 이론이다[1]. “Focal activation” 이론은 국소적으로 빠르게 박동을 형성하는 부위(rotor)가 있으며, 주변 조직으로 세동 전도(fibrillatory conduction)가 되어서 발생한다는 이론이다(Fig. 1). Rotor의 발생기전은 다시 “ectopic focus” 혹은 “single reentry” 기전으로 세분화 할 수 있다.
이러한 이론들은 심방세동의 치료 효과를 설명하는 데도 중요하다. “Multiple wavelets” 이론의 경우 불응기를 증가시키는 부정맥 약제와 “Maze” 수술의 효과를 설명하는 데 도움이 된다. 이에 반하여 “focal activation” 이론은 도관(catheter)을 이용한 국소적 치료의 효과를 설명할 수 있다. “Focal activation”의 기전 중 “ectopic focus”는 “automaticity”를 억제하는 약물, 그리고 “single reentry”인 경우 불응기를 연장하고, 회귀를 억제하는 약물의 치료 효과를 설명 가능하다. “Multiple wavelets” 이론은 지금까지 심방세동을 설명하는 가장 중요한 이론이었다. 하지만 심방세동의 발생과 지속에 폐정맥(pulmonary vein)의 중요함이 밝혀지고, 폐정맥을 포함하는 도자절제술이 좋은 치료 효과를 보이면서[2] 최근에는 “focal activation”이 일부 심방세동을 설명하는 중요한 기전으로 제시되고 있다.
동물 실험에도 심방세동이 유발된 동물에서 일차적 박동 형성부(rotor)가 관찰되었고, 이 부위는 “ectopic focus” 혹은 “single reentry”를 보였다[3,4]. 또한 심방세동을 지속하는데 좌심방에 dominant rotor가 존재하고, 우심방은 수동적으로 활성화됨이 관찰되었다[3,4]. 흥미롭게도 dominant rotor 부위는 박동이 매우 규칙적이었으며, 주변부로는 complex fragmented atrial electrogram (CFAE)이 관찰되었다[5]. 하지만 아직까지 rotors의 존재 유무와 위치는 논란의 여지가 있다. 최근에는 rotor의 위치가 고정되지 않고, 움직인다는 drifting rotor [6] 개념이 제시되었다.

심방세동에서 발생하는 재구도화(remodeling)

심방세동이 오래되면 심방세동이 쉽게 발생하고, 유지될 수 있게 재구도화되는 “atrial fibrillation begets atrial fibrillation” 현상이 발생한다[7]. 심방세동이나 빠른 심방빈맥이 있으면 각각의 활동전위 때마다 inward L-type Ca2+ current (ICaL)을 통하여 칼슘이 세포 내로 들어가게 되어 세포의 칼슘이 증가하게 된다[8]. 증가된 세포내 칼슘은 세포 생존에 치명적이므로 칼슘을 감소시키기 위한 적응 기전으로 ICaL가 억제되고, IK1IKAch가 증가하여, 활동전위기간(APD)이 짧아지게 된다(Fig. 2) [9]. 이와 같은 현상을 전기적 재구도화(electrical remodeling)라고 하며, 심방세동이 지속되고, 쉽게 재발되는 조건을 만들게 된다[10].
심방세동은 또한 심방의 수축력 재구도화(contractile remodeling)를 유발한다. 전기적 재구도화에 의한 L-type Ca2+ channels의 저하는 수축력 재구도화의 일차적 원인이라고 생각한다. 이로 인하여 심방의 수축력은 감소하고, compliance는 증가하게 되며, 근육 세포들이 늘어나게 된다. 또한 심방 섬유화를 증가시키고, connexin의 숫자 감소 및 위치가 외측화(lateralization)되어서, 조직 간의 이질성을 증가시키는 구조적 재구도화를 유발한다. 이러한 현상은 다시 악순환을 이루어서 전도 속도를 늦추고, 심방의 불응기에도 영향을 주어서 심방세동을 더욱 오래 지속시킬 수 있는 섬유화 등의 전기-해부학적 기질을 만들 수 있다(Fig. 3) [11]. 전기적 리모델링 및 수축력 저하는 심방세동이 종료되면 가역적으로 호전이 되는데 구조적 재구도화는 완전하게 호전되지는 않는 것으로 알려져 있다[11].

세포내 칼슘 대사 변화와 심방세동

세포내 칼슘 대사의 변화는 심방세동의 기전과 연관됨이 최근에 밝혀졌다. Honjo 등[12]은 소량의 ryanodine을 이용하여 sarcoplasmic reticulum (SR)의 자발적인 칼슘 분비를 증가시키면 폐정맥에서 빠른 빈맥이 나옴을 보고하였다. 또한 지속적인 심방세동 환자에서 정상인에 비하여 심방 세포의 Ca2+ spark 및 wave가 증가함이 밝혀졌다[13]. 이러한 현상의 원인으로는 SR ryanodine receptor type 2 [14] 혹은 phospholamban의 과인산화[15]가 제시되고 있다. 이와 같이 증가된 SR 칼슘 분비는 심방세동이 재발하는 기전으로 제시되는 late-phase 3 EAD와도 밀접한 연관이 있다[16]. 흥미롭게도 심방조직은 이와 같이 SR 칼슘의 분비가 증가된 상태를 보였지만, 동결절은 오히려 칼슘의 분비가 저하되는 소견을 보였다[17].

심방세동이 발생을 조절하는 인자

심방세동의 발생을 조절하는 인자로는 교정가능 혹은 불가능한 인자로 분류할 수 있다. 교정 불가능한 인자로는 유전적 요인, 고령 등을 들 수 있다. 교정 가능한 인자로는 염증, 내분비계 이상, 심방 및 폐정맥의 stretch, 자율신경계 이상 등이 있다(Fig. 4).

Pulmonary veins and atrial function

폐정맥은 trigger로 작용하여 심방세동을 발생시킬 수 있으며[2], 회귀를 조장하고, 심방세동을 지속시키는 심방의 재구도화를 유발시킬 수 있다[18]. 폐정맥에서는 동결절과 유사하게 P cells, transitional cells, Purkinje cells 등이 관찰되었다[19]. 또한 심방의 압력이 증가하고, 늘어나면 폐정맥에서 부정맥의 유발됨이 관찰되었다[20]. 이와 같은 현상은 아직까지 논란이 있으나 stretch-receptor의 항진과 연관되며, stretch- receptor를 억제하는 “tarantula peptide”는 심방세동 발생을 억제함이 보고되었다[21]. 또한 폐정맥으로는 여러 층으로 구성된 좌심방 심근 섬유들은 다양한 배열로 분포되어 복잡한 3차원 섬유 배열을 보여서, 회귀가 조장되는 것으로 알려져 있다[22].

자율신경계

심방세포의 전기생리학적 성상은 부교감 및 교감신경의 영향에 따라서 다르게 조절된다. 예를 들어 미주신경의 영향은 활동전위기간과 불응기를 줄여서 심방의 이질성을 유발하고, 회귀 기전을 조장한다. 교감신경은 비정상적인 자동능 및 triggered activity를 조장한다. 미주신경성(vagally mediated) 심방세동은 주로 젊은 남성, 기저 질환이 없는 사람에서 발생하고, 심방조동도 같이 보이는 경우가 흔하다. 운동선수에서 발견되는 심방세동의 경우도 항진된 미주신경의 톤에 영향을 받는 것으로 알려져 있다. 최근 연구에 의하면 운동 선수의 심방세동도 전극도자절제술이 좋은 결과를 보였다[23]. 이에 반하여 교감신경성 심방세동은 주로 심장 질환을 가진 경우에 발생하며, vagal withdrawal와 연관된다. Framingham Heart Study에 따르면 heart rate variability로 측정된 자율신경계의 이상이 심방세동과 연관됨을 보여 주었다[24].

염증

염증을 반영하는 C-reactive protein (CRP)는 심방세동의 초기 24시간 이내에 증가하고[25], 지속성 심방세동에서 발작성 심방세동보다 높은 수준을 보였다[26]. 또한 오래된 심방세동에서는 염증세포 침윤, 섬유화, 세포 괴사 등이 관찰되었다[27]. 이러한 점은 염증이 심방세동과 연관됨을 시사한다. 하지만 심방세동이 염증을 활성화시키는지, 아니면 기존에 있던 전신의 염증 상태가 심방세동을 조장하는지는 아직은 불분명하다.
관동맥 및 심장 염증에 의한 심방세동의 발생 기전은 그림 5(Fig. 5)와 같다. 수술을 하는 환자에서 corticosteroid 투여로 도움을 심방세동을 억제하였다는 보고가 있으며, 최근에는 심방세동 도관 절제술 후 steroid를 투여하여 심방세동을 억제하였다는 보고가 있다[28]. HMG-CoA reductase 억제제의 다양한 효과도 심방세동의 발생에 영향을 주는지 연구가 되고 있다. 섬유화 및 염증은 심방의 해부학적 전기생리학적 기질의 변화를 초래하는 것으로 알려져 있다. Statin 등을 이용한 치료는 심방세동의 염증 및 섬유화를 조절하여 도움이 될 수 있을 것으로 예측되고 있다.

내분비계 이상

갑상선 호르몬의 농도 변화는 심방세동의 시작과 억제에 영향을 준다[29]. Renin-angiotensin system (RAS) 또한 심방세동과 연관되어 있는데, RAS를 억제하는 captopril 및 candesartan은 심방빈맥을 감소시킨다. RAS 억제제는 섬유화를 억제하여서, “structural remodeling”을 예방하는 것과 연관되어 있다[30]. Framingham Heart Study에 따르면 당뇨병이 있으면 심방세동의 빈도가 남성에서 1.4배, 여성에서 1.6배로 증가되었다[31]. 당뇨병이 심방세동의 발생을 증가시키는 기전도 심방의 섬유화와 연관된 것으로 추정되고 있다.

결 론

다른 상심성빈맥에 비하여 심방세동의 기전은 매우 복잡하다. 그 이유는 심방세동이 다양한 스펙트럼을 가진 질환이기 때문이다. 심방세동은 다양한 질병 및 조건과 연관되어 발생한다. 지금까지 많은 동물 실험 및 실험 모델에서 밝혀진 기전의 경우도 실제 사람에서 심방세동의 발생과 지속됨을 설명하는데 부족한 점이 많다. 결론적으로 한 개의 기전으로 심방세동의 다양성을 설명하기는 부족하며, 여러 가지 기전이 심방세동의 발생과 지속에 연관되어 있음을 알아야 한다. 따라서 심방세동의 예방 및 치료에 있어서도 이러한 다양성을 인정하는 접근 방법이 필요하다고 할 수 있다.
