결핵성 흉막염의 진단과 치료

Abstract

Tuberculous (TB) pleuritis is the second most common form of extrapulmonary tuberculosis. Because the yield of pleural fluid mycobacterial culture is as low as 20% and the pleural biopsy is rather invasive, the measurement of adenosine deaminase (ADA) has been a cornerstone of the diagnosis of TB pleuritis. If the ADA level of pleural fluid is higher than 70 IU/L, the diagnosis of TB pleuritis can be made safely. The treatment is based on a standard short course anti-TB treatment starting with isoniazid, rifampicin, ethambutol, and pyrazinamide. Although systemic steroids and drainage of pleural fluid have been tried to reduce the residual pleural thickening, the results are contradicting. (Korean J Med 2011;81:150-153)

역학

결핵은 인류를 가장 끈질기게 괴롭혀온 전염병이다. 세계보건기구의 발표에 따르면 2008년 한 해 동안 세계적으로 모두 937만 명의 결핵 신환자가 발생하여 발생률은 인구 십만 명당 139명에 이르며, 유병률은 인구 10만 명 당 164명으로 약 1,110만 명의 세계인이 결핵을 앓고 있는 것으로 추산되고 있다[1].
폐 이외의 기관을 침범하는 폐외결핵은 전체 결핵의 약 9.7-46%를 차지하는데[2-4], 몇 지역에서는 폐외결핵이 차지하는 비율이 점차 증가하고 있다. 림프절, 흉막, 근골격계, 중추신경계, 간 및 소화기계, 복막, 심막 등이 흔히 이환 되는 장기인데, 대개의 지역에서는 흉막이 림프절 다음으로 흔히 침범되는 곳이다. 결핵 유병률이 높은 지역에서는 흉수의 가장 흔한 원인이 결핵일 정도로 결핵성 흉막염의 흔하다[5].

발생 기전

결핵성 흉막염은 일차 결핵 혹은 재활성화 결핵, 모두에서 발생할 수 있다. 일차 결핵으로 발생한 경우, 흉막과 가까운 쪽에 발생한 병소가 흉막 쪽으로 터져 결핵균이 흉막강으로 들어가는 것이 근본 원인이다[6]. 흉막강의 결핵균에서 분비한 항원들이 지연형 과민반응을 유발하고, 그 결과 흉막의 투과도가 증가하거나[7] 배출능이 저하되어[8,9] 흉수가 고이게 된다. 한편 재활성화 결핵의 결과로 결핵성 흉막염이 발생하기도 하는데[10], 이 경우에는 대개 폐실질에 공동이나 섬유화가 동반되는 경우가 흔하다.

진단

항산균 도말 및 배양검사

결핵성 흉막염 환자의 흉수 항산균 도말 검사가 양성인 경우는 드물고, 우리나라에서 발표된 연구에 의하면 결핵균이 배양되는 경우도 20%에 불과하다[11]. 고체배지를 사용할 경우 배양 검사 결과를 확인하는 데 2개월 이상 걸린다는 점을 감안하면 항산균 도말 및 배양검사는 결핵성 흉막염의 진단에 큰 도움이 되지 않는다.
결핵성 흉막염이 의심되는 경우 흉수보다는 도리어 객담 결핵균 배양 검사의 양성률이 높다. 최근 106명의 결핵성 흉막염 환자를 대상으로 한 연구는 흉수 배양 양성률 20%, 객담 배양 양성률은 그보다 높은 31%로 보고하였는데, 이유는 결핵성 흉막염 환자들의 대부분이 폐실질에 결핵 병변을 가지고 있기 때문이라고 설명했다[11].

조직검사

흉수에서 결핵균 배양 양성률이 높지 않고 시간이 오래 걸린다는 점을 고려하면 결핵성 흉막염 진단의 중심은 흉막 조직검사이다. 흉막 조직에서 결핵 병변의 특징인 건락성 괴사를 동반한 육아종이 관찰되는 경우가 높게는 84%에 이르고, 67%의 조직에서 결핵균이 배양되었다는 보고를 감안하면[12], 적절한 흉막 조직을 얻을 수 있다면 결핵성 흉막염의 진단율이 향상될 수 있다.

Adenosine deaminase (ADA)

Adenosin을 inosine으로 전환시키는 효소인 ADA는 주로 활성화된 T림프구에서 분비되는데, 1978년 결핵성 흉막염에서 그 수치가 상승되었다고 보고된[13] 이후 결핵성 흉막염 진단의 필수 요소로 자리잡았다. 그간 세계 각지의 연구자들에 의해 40-71 U/L 사이를 기준으로 결핵성 흉막염 진단에 대한 민감도와 특이도가 연구되었는데, 종합적으로 분석해보면 민감도는 93.9%, 특이도는 91.3%에 이를 만큼[14] ADA의 측정은 결핵성 흉막염 진단에 큰 도움을 준다. 그러나 류마티스성 관절염에 동반된 흉수나 농흉의 경우에도 ADA가 상승할 수 있는데, 이 두 질환은 임상적으로 어렵지 않게 결핵성 흉막염과 구분해 낼 수 있다. 또한 ADA 농도가 높지 않으면 흉수의 원인이 결핵일 가능성은 매우 낮은데, ADA가 43 U/L 이하이면서 정확한 원인을 알 수 없는 흉수를 가진 환자 40명을 5년간 관찰하여도 결핵이 전혀 발생하지 않았다는 보고도 있었다[15].

Interferon-γ

ADA 다음으로 결핵성 흉막염 진단에 많이 연구된 것은 Interferon-γ이다. Interferon-γ는 흉수안의 CD4 T 림프구에서 분비되는데[16], 일반적으로 결핵성 흉막염에서 다른 원인의 흉막염에서보다 Interferon-γ의 농도가 더 높다[17]. 그러나 ADA 측정보다 Interferon-γ의 측정이 결핵성 흉막염 진단에 더 유용하다는 증거는 없는데다[18], 비용도 더 비싸 일반적으로 사용되지는 않는다.

진단의 실제

흉수가 삼출액의 기준을 만족하며, 백혈구 중 림프구가 50% 이상이면서, ADA가 70U/L 이상이라면 결핵성 흉막염일 가능성이 매우 높다. 같은 조건을 만족하면서 ADA가 40U/L 이상이라도 결핵성 흉막염일 가능성이 충분한데, 환자의 임상상이 결핵과 잘 맞지 않는다면 흉막 조직검사 등을 적극적으로 고려해야 한다(Fig. 1)

치료

항결핵약제의 투여

결핵성 흉막염 치료의 근간은 항결핵약제의 투여다. 폐결핵과 마찬가지로 2개월간 isoniazid, rifampicin, pyrazinamide, ethambutol을 사용한 후 다음 4개월 동안 isoniazid 및 rifampicin을 유지해야 한다[19]. 폐실질의 결핵 병변 없이 결핵성 흉막염만 존재하는 경우 결핵균의 숫자가 적으므로 isoniazid 와 rifampicin 만 6개월 동안 사용해도 충분하다는 보고들이 있어 왔지만[20,21], 단순흉부촬영에서 폐실질의 병변이 없는 것으로 보이는 경우라도 흉부 CT에서 결핵 병변이 발견되는 경우가 흔하다는 사실을 고려하면[11] 초기 2개월 동안은 pyrazinamide와 ethambutol을 함께 사용하는 것이 보다 안전할 것으로 판단된다.

스테로이드의 사용

치료 초기 항결핵약제와 함께 0.75 mg/kg의 prednisolone을 투여하는 것이 발열, 호흡곤란, 흉통 등의 증상 호전을 빠르게 한다는 소규모 연구가 있었지만[22], 추후 연구들에 의해 재확인되지 못했다[23,24]. 결국 지난 2007년에 발표된 메타분석을 통해 결핵성 흉막염 환자에서 전신스테로이드의 사용은 사망, 폐기능, 남은 흉수의 양, 흉막 비후를 줄이지 못하는 것으로 알려졌다[25].

흉수 배액

흉수의 양이 많아 호흡곤란을 호소하는 환자의 경우 당연히 흉수를 배액해야 하지만, 증상이 심하지 않은 환자에서 흉수를 빼주어야 하는지에 대해서는 논란이 있어왔다. 지난 2003년 61명의 결핵성 흉막염 환자를 대상으로 진단 후 카테터를 이용하여 흉수를 제거하는 것의 효과를 측정한 무작위배정 임상시험 결과, 카테터를 통한 흉수의 제거는 호흡곤란을 느끼는 기간을 단축시키기는 하지만, 흉막 비후 빈도를 줄이지는 못하는 것으로 확인되었다[26]. 그러나 국소적으로 고정된(loculated) 흉수의 경우 streptokinase 등을 이용하여 적극적으로 배액 하는 것이 이후 흉막 비후를 줄인다는 보고들도 있다[27,28].

합병증

결핵성 흉막염의 가장 흔한 합병증은 영구적인 흉막 비후인데, 호흡곤란 등의 증상이 없다면 추가 조치 없이 관찰할 수 있다. 결핵성 흉막염 환자에서 기관지-흉막 열공이나 농흉으로 진행하기도 하는데, 이 경우 수술적 치료가 필요한 경우가 대부분이다. 수술적 치료의 결과가 나쁘지는 않지만 합병증 역시 드물지 않다는 점을 유의해야 한다[29].
