항혈소판제제와 프로톤펌프억제제 간의 약물 상호 작용: Unsafe 측면과 대책

Abstract

Clopidogrel is a prodrug that is converted in the liver to an active thiol metabolite, which irreversibly inhibits the platelet P2Y12 adenosine diphosphate receptor. This mechanism requires cytochrome P450 2C19 (CYP 2C19) enzyme. Proton pump inhibitors (PPIs) competes against CYP 2C19 and inhibits the conversion of clopidogrel into its active metabolite, therefore, clopidogrel-PPIs drugs interaction may exist. These interactions could result in competitive inhibition decreasing the conversion of the clopidogrel pro-drug to the active metabolite and could potentially translate into an increased risk for cardiovascular events by inadequate platelet P2Y12 receptor inhibition. Many studies including retrospective cohort studies and studies using platelet function tests demonstrated the possible interactions between clopidogrel and PPIs leading to a decrease in the antiplatelet efficacy of clopidogrel and worse cardiovascular clinical outcomes than clopidogrel alone. In contrast, few comparative trials using clinical outcomes found no serious drug interactions between them. In this review, we introduce possible harmful effects of combined use of clopidogrel and PPIs on platelet function. In addition, we suggest how to overcome clopidogrel-PPIs interactions. 서 론

클로피도그렐(clopidogrel)은 티에노피리딘(thienopyridine) 계통의 약물로 경피적 관상동맥 성형술(percutaneous coronary intervention) 이후 혹은 급성 관동맥 증후군에서 심혈관 질환의 재발 위험을 감소시키기 위해 아스피린과 병합하여 표준치료로 쓰이는 대표적인 항혈소판(antiplatelet) 제재이다[1-5]. 최근 스텐트 시술 이후 스텐트 내강의 내막증식에 의한 재협착을 방지하기 위해 고안된 약물 방출형 스텐트(drug eluting stent)를 이용한 관상동맥 성형술을 시행 받는 환자가 많이 증가하면서 여러 약제들 간의 상호작용에 대한 우려가 증가하고 있다. 그중 위산과 관련된 소화성 궤양, 위식도 역류질환 등의 치료에 널리 처방되고 있는 프로톤펌프억제제(proton pump inhibitors, PPI)와의 상호 작용에 대해서 많은 의견들이 나오고 있으나 아직까지 명확하게 정립된 결과는 없는 상태이다. 이에 클로피도그렐과 프로톤펌프억제제 간의 약물 상호 작용, 그중 고려해야 할 위험성을 중심으로 고찰하였다.

허혈성 심질환에서 클로피도그렐의 역할 증대

클로피도그렐은 Percutaneous Coronary Intervention-Clopidogrel in Unstable angina to Prevent Recurrent Events (PCI-CURE), Clopidogrel for Reduction of Events During Observation (CREDO), PCI-the Clopidogrel as Adjunctive Reperfusion Therapy (CLARITY) 연구 등[1-3]에서 아스피린과의 병용 투여 시에 아스피린 단독투여에 비해 심혈관 사망률과 심근경색증, 뇌경색 발생율을 유의하게 감소시켰으며 현재 전 세계적으로 광범위하게 처방되고 있다. American College of Cardiology/American Heart Association 치료지침에서 급성관동맥 증후군 환자나 약물 방출형 스텐트 혹은 일반 금속 스텐트(Bare metal stent)를 사용한 관상동맥 성형술 이후에 아스피린과 클로피도그렐의 2제 병용요법을 12개월까지 유지하도록 권고하고 있다[4,5].

환자에 따라 다양한 클로피도그렐의 반응성

클로피도그렐에 대한 반응성이 환자들마다 다르다는 연구결과가 발표되었다[6,7]. 특히 클로피도그렐에 대한 반응성이 떨어져 있는 경우는 심혈관계 예후가 좋지 않은 것으로 증명되었다[8,9]. 클로피도그렐에 대한 낮은 반응성은 유전적인 요인, 약물 상호작용, 낮은 복약 순응도 등이 그 원인으로 알려져 있다[10]. 클로피도그렐은 그 자체가 직접적으로 작용하는 것이 아니라 cytochrome P450 (CYP450) 효소에 의해 활성화되는 물질로 전환된 후에 혈소판의 P2Y12 수용체를 비가역적으로 차단하여 adenosine diphosphate (ADP) 유발 혈소판 응고 과정을 억제하는 전구약물(prodrug)이다. 클로피도그렐의 활성화 과정을 매개하는 CYP450효소 중 2C19의 선천적인 유전자 결함이 있는 환자의 경우 클로피도그렐에 대한 반응성이 저하되고 예후도 좋지 않은 것으로 알려져 있다[11,12]. 위궤양 및 역류성 식도염 환자 등에서 널리 처방되고 있는 프로톤펌프억제제 또한 CYP450 대사 과정을 거치는데 이에 따른 클로피도그렐의 저항성 증가에 대한 우려가 널리 제기되고 있다.

프로톤펌프억제제

프로톤펌프억제제는 소장에서 흡수되어 혈류를 따라서 위벽세포(parietal cell)에 도달하고 H+, K+-ATPase라는 프로톤 펌프를 억제하여 위산 분비를 억제시키는 효과적이고 강력한 위산 분비 억제제이다. 이러한 기전으로 프로톤펌프억제제는 위산과 관련된 소화성 궤양, 위식도 역류질환, 졸링거-엘리슨 증후군(Zollinger-Ellison syndrome), 위나선균(Helicobacter pylori)의 제균치료, 비스테로이드 소염제(NSIADs)에 의한 위병증의 치료 등 다양한 질환에 전 세계적으로 널리 이용되고 있다. 이런 프로톤펌프억제제는 위벽세포의 산성환경 내에서 활성화된 대사물질로 전환되어 프로톤펌프에 비가역적으로 부착하여 위산 분비를 억제하는 기능이 있다. 이때 활성화된 대사물질이 되기 위하여 클로피도그렐과 마찬가지로 CYP450 효소 시스템을 이용하여야 한다.

클로피도그렐과 프로톤펌프억제제 간의 약물 상호작용

클로피도그렐과 프로톤펌프억제제는 두 약제 모두 활성화된 대사물질이 되기 위하여 CYP450 효소 시스템을 이용하는데 이 두 가지 약제를 동시에 복용하는 경우에 상호작용에 의해 약의 효과가 떨어질 가능성이 있다. 특히 프로톤펌프억제제인 경우에 CYP450 효소 시스템을 이용하여 활성화될 때 CYP2C19 동종효소(isoenzyme)를 감소시켜 클로피도그렐의 활성도를 낮출 것으로 추정되고 실제로도 혈소판 활성도를 측정해 보면 프로톤펌프억제제가 클로피도그렐의 반응성을 낮추는 것으로 증명되었다(Fig. 1) [13,14]. 대규모 코호트 연구나 후향적 연구들에서 프로톤펌프억제제와 클로피도그렐을 함께 복용한 군에서 심혈관계 예후가 좋지 않은 결과를 보여주었다(Fig. 2) [15-18]. 이러한 연구들은 전향적인 연구가 아니라서 통계의 치우침(bias)이 있을 수 있으나 몇 천명에서 만 명 이상의 대규모 연구라는 점이 그 특징이다. 최근 약 2만 명을 대상으로 네덜란드에서 시행한 대규모 인구기반 연구에서 프로톤펌프억제제와 클로피도그렐을 동시에 사용하게 되면 심혈관 질환, 사망률 그리고 합병증을 동반한 소화성 궤양의 위험이 모두 증가한다는 결과를 보여 주었다[15]. 또한 약 만 명의 환자를 대상으로 한 연구결과에서도 급성 관동맥 증후군으로 퇴원한 환자들의 경우에 프로톤펌프억제제와 클로피도그렐을 동시에 투여한 군에서 심혈관 질환의 예후가 좋지 않았다[18]. 이러한 결과들은 클로피도그렐과 프로톤펌프억제제를 동시에 투여하는 것이 해로울 것임을 나타낸다. 이전에 이 주제에 대한 전향적 대규모 연구가 없었기에 Clopidogrel with or without Omerazole in Coronary Artery Disease (COGENT) 연구가 진행되어 최근 그 결과가 발표되었다[19]. COGENT 연구는 2중 항혈소판제(dual anitplatelet agents, 아스피린과 클로피도그렐) 처방이 필요한 환자들에서 프로톤펌프억제제인 오메프라졸(omeprazol)과 클로피도그렐의 병합 치료에 대한 소화기계와 심혈관계의 유익성과 해로움을 전향적으로 분석하기 위한 대규모 연구이다. 2중 항혈소판제를 투여 받은 약 4,000명의 환자를 프로톤펌프억제제인 오메프라졸 투여군과 위약군, 2군으로 배정하여 결과를 분석하였다. 오메프라졸 투여군은 일차 연구 종착점인 경과관찰 6개월 시점의 위장관 사건(위출혈, 증상이 동반된 위십이지장 궤양 혹은 침식증, 폐색 혹은 천공) 발생률이 위약군에 비해 유의하게 감소되었으며(오메프라졸군 1.1% vs. 위약군 2.9%, 상대위험도 0.34, 95% 신뢰구간 0.18-0.63, p< 0.001) 또한 상부 위장관 출혈 발생도 유의하게 감소시켰다(상대위험도 0.13, 95% 신뢰구간 0.03-0.56, p= 0.001). 하지만 심혈관계 사건 발생율은 오메프라졸 군 4.9%로 위약군의 5.7%와 유의한 차이점을 보이지 않았다(상대위험도 0.99, 95% 신뢰구간 0.68-1.44, p= 0.96, Fig. 3). 이 연구는 잘 기획된 전향적 무작위 배정 연구였지만 회사의 사정으로 목표 환자수인 5,000명 중 약 4,000명의 환자만 등록되었다는 점과 연구기간이 상대적으로 짧았다는 점, CYP450 유전자의 CYP2C19의 기능 소실(mutant homozygote)이 2-3%로 대체로 낮은 백인을(대상 환자의 94%) 주요 대상으로 하였다는 점에서 그 결과를 우리나라 환자들에서 그대로 적용하기에는 무리가 따를 수 있는 연구라 할 수 있다. 최근 발표된 무작위배정 연구를 포함한 25개 연구의 약 16만 명을 대상으로 한 메타분석 결과에 따르면 클로피도그렐을 투여 중인 환자에게 프로톤펌프억제제를 병용하면 주요 심혈관계 사건(위험도 1.29, 95% 신뢰구간, 1.15-1.45), 심근경색의 위험도(위험도 1.31, 95% 신뢰구간, 1.12-1.53)가 유의하게 증가하였다[20]. 하지만 메타분석도 포함된 연구들의 질, 출판물, 연구대상자 수 및 위험인자가 균일하지 않고, 연구 목적이 다양하다는 여러 제한점이 있다.

클로피도그렐과의 약물 상호작용에 대해 프로톤펌프억제제들 간의 차이가 존재하는가?

프로톤펌프억제제 중 오메프라졸은 확실하게 클로피도그렐의 반응성을 낮추는 것으로 알려져 있다[13]. 판토프라졸(Pantoprazole)과 에소메프라졸(esomeprazole)은 클로피도그렐의 반응성을 낮추지 않는 것으로 발표되었다(Fig. 4) [21]. 기존 연구에서 프로톤펌프억제제들은 종류와 제형에 따라 클로피도그렐 대사에 미치는 영향이 조금씩 다르며 제제 간 공통효과(class effect)는 아닌것으로 판단되며 이것은 각 프로톤펌프억제제 간의 CYP450 2C19 억제효과의 차이에 의한 것으로 추정된다. 오메프라졸과 란소프라졸(lansoprazole)은 그 영향이 가장 크고 반면, 판토프라졸은 주로 CYP2C9으로 대사되어 그 영향이 가장 적은 것으로 보고되었다[22]. 이러한 연구 결과들은 판토프라졸이 클로피도그렐과의 약제 상호 작용이 가장 적어 클로피도그렐과 프로톤펌프억제제를 동시에 투여할 필요성이 있는 환자에서 가장 적합한 프로톤펌프억제제임을 제시한다.
하지만 네덜란드에서 진행된 대규모 후향적 연구[15]에서는 클로피도그렐과 프로톤펌프억제제들의 병용투여에 의한 유의한 심혈관계 사건의 증가가 증명되었고 또한, 각각의 프로톤펌프억제제들(오메프라졸, 판토프라졸, 에소메프라졸, 라베프라졸(rabeprazole) 간에 증가된 심혈관 질환 발생에 대한 유의한 차이점을 발견하지 못했으므로 프로톤펌프억제제들 간의 클로피도그렐과의 약제 상호 작용에 대한 실험적, 이론적 차이점을 실제 임상에 그대로 적용하기 위해서는 추가적인 연구 결과가 뒷받침 되어야 할 것이다.

어떻게 클로피도그렐과 프로톤펌프억제제 간의 해로운 약물 상호 작용을 극복할 수 있을까?

클로피도그렐과 프로톤펌프억제제는 혈장 내 반감기가 2시간 이내이기 때문에 두 가지 약제의 투여 시간을 조절하여 약제 상호 작용을 줄일 수 있다는 가설이 제기되었다. 실제로 프로톤펌프억제제는 식전 투여 시 가장 효과적이기 때문에 아침 식전에 복용하고 취침 시에 클로피도그렐을 복용하거나 저녁 식사 전에 프로톤펌프억제제를 점심 식사 시에 클로피도그렐을 복용하는 것을 권유하기도 한다[23]. 또한, 최근 히스타민2 (H2) 수용체 차단제를 프로톤펌프억제제 대신 사용하는 것의 유용성이 제안되었다 [17,24]. 하지만 최근 타이완에서 진행되었던 연구 결과에서는 그 유익성이 증명되지 못했다. 이 연구는 2002년부터 2005년까지 타이완에서 급성 관상동맥 증후군으로 입원 치료 후 퇴원한 약 7,000명의 코호트에 대해 진행된 후향적 연구로 일차 연구 종착점(급성관동맥 증후군에 의한 재입원율 혹은 재입원 3개월 이내의 모든 사망률)의 1년간 누적 발생률은 클로피도그렐과 히스타민 2 수용체 차단제 병용 투여군에서 26.8%, 클로피도그렐과 프로톤펌프억제제 병용 투여군에서 33.2%로 클로피도그렐 단독 투여군의 11.6%에 비해 유의하게 높았다(p < 0.0001). 다변량 분석에서도 클로피도그렐과 함께 히스타민 2 수용체 차단제의 병용 투여와 프로톤펌프억제제의 병용 투여는 그 위험률이 2.48 및 3.20로 일차 연구 종착점 발생의 독립적 위험 요인이었다. 따라서 프로톤펌프억제제 대신 히스타민2 수용체 차단제를 사용하는 것도 심혈관계 위험을 낮추지 못할 가능성이 높다[25]. 추가적으로 스텐트 시술 이후 스텐트 혈전의 발생 위험이 높거나 그로 인해 치명적일 수 있는 좌주간부 병변, 다제 혈관 병변, 미만성 병변 등의 환자군에서 위장관 합병증의 위험성이 높은 환자군은 아스피린과 더불어 항혈소판제의 투여와 프로톤펌프억제제의 병용이 불가피한데 이러한 환자에서는 새롭게 개발된 항혈소판제인 프라수그렐(Prasugrel) 혹은 티카그레롤(Ticagrelor)이 유용할 수 있다. 프라수그렐은 삼세대 티에노피리딘(Thienopyridine)계의 약제로 클로피도그렐보다 더 빠르고 강력하게 ADP를 통한 혈소판 응집을 억제한다. 또한 간에서 CYP450 활성 과정이 단순하고 주로 CYP3A4와 CYP2B6를 이용하므로 프로톤펌프억제제와의 약물 상호 작용의 가능성이 적다. 티카그레롤은 비-티에노피리딘 계열의 가역적 ADP 수용체 길항제로서 간대사 과정을 거치지 않으므로 프로톤펌프억제제와의 약제 상호 작용이 없을것으로 판단된다[26]. 프로톤펌프억제제들과 새로운 항혈소판제 들간의 약물 상호 작용에 대한 후속 연구가 진행되어야 할 것이다.

결 론

클로피도그렐과 일부 프로톤펌프억제제들 간에는 유의한 약물 상호작용에 의해 클로피도그렐의 활성도가 저하되어 심혈관계 위험도를 높이는 것으로 보고되었다. 이 주제에 대한 유일한 전향적 무작위 연구에서는 프로톤펌프억제제인 오메프라졸을 클로피도그렐과 병용 시에 위장관 합병증은 유의하게 감소시키고 심혈관계 위험도는 증가시키지 않는다는 고무적인 결과를 보였으나 연구의 여러 제한점들에 의해 우리나라 환자들에서 그 결과를 그대로 적용하기는 어려울 것으로 사료된다. 최근에 나온 일부 프로톤펌프억제제는 클로피도그렐과의 약물 상호작용이 적은 것으로 발표되었으나 임상적인 유용성에 대해서는 완전히 규명되지 않았다.
결론적으로 클로피도그렐을 투여 받는 환자에서 프로톤펌프억제제의 처방을 시작하거나 지속해야 하는 경우에 프로톤펌프억제제의 처방 필요성에 대해 보다 신중하게 재평가를 시행하고 처방하는 것이 필요하다. 불가피하게 처방해야 한다면 그 투여 시간을 클로피도그렐과 달리하여 처방하는 것이 합리적이라고 생각된다. 또한, 임상적인 유용성이 완전히 증명되지는 못했으나 약물 상호작용이 상대적으로 적은 프로톤펌프억제제(판토프라졸)를 선택하는 것도 고려해 볼 수 있다. 더불어 새로운 항혈소판제인 프라수그렐 혹은 티카그레롤을 클로피도그렐의 대용으로 고려할 수 있다. 향후 클로피도그렐과 프로톤펌프억제제 간의 약물 상호 작용 및 임상적 결과에 대한 추가적인 연구 결과의 확인이 필요하며 약제 상호작용이 상대적으로 적은 프로톤펌프억제제, 새로운 항혈소판 제제들의 유용성에 대한 임상 연구 결과들이 필요하리라 사료된다.
