위축성 위염 혹은 위암에 미치는 프로톤펌프억제제의 영향: Safe 측면

Abstract

Proton pump inhibitors (PPIs) are widely used over 20 years for management of symptoms due to acid related diseases such as peptic ulcer and reflux esophagitis. Serious adverse events are extremely rare for short-term PPIs use. Recently, as long-term PPIs use increase, diverse reports have been reported on adverse event related with long-term PPIs use. Long-term PPIs use is generally referred as use of PPIs more than 1 year. Secondary hypergastrinemia after long-term PPIs use is associated with development of fundic gland polyps (FGP) and hyperplasia of enterochromaffin-like cell (ECL) that might be concerned with gastric carcinoid tumor. Furthermore, several studies have posed the relationship between the risk of gastric cancer and long-term PPIs use with co-existing H. pylori infection. The present review summarize the recent accumulated evidence on neoplasm associated with secondary hypergastrinemia after long-term PPIs use. (Korean J Med 2011;81:1-5)

서 론

프로톤펌프억제제(proton pump inhibitors; PPIs)는 1988년 omeprazole이 처음 소개된 이후 약 20년 이상 소화성 궤양 및 역류성 식도염 등에서 점막치유와 더불어 증상 개선 효과가 입증되어 전 세계적으로 널리 사용되고 있다[1,2]. PPIs의 단기간 사용에 따른 중대한 부작용은 거의 없는 것으로 알려져 있고 두통, 설사, 오심, 피부 발적 등의 경미한 부작용이 1-2% 빈도로 나타날 수 있으며[3], 이는 cimetidine이나 ranitidine과 비슷한 정도이다[4]. 그러나 PPIs의 장기간 유지요법에 따른 여러 가지 부작용의 가능성이 제기되고 있다. PPIs의 장기간 사용에 대한 기간의 정의는 명확하지 않지만 대개 1년 이상으로 정의된다[5-13]. PPIs의 장기간 사용에 따라 최근 이슈가 되고 있는 위축성 위염 또는 위암과 관련하여 PPIs의 영향은 미미하며 장기간 사용에도 안전하다는 측면에서 이들과의 관련성에 대한 연구결과를 소개하고자 한다.

PPIs 약물의 사용과 고가스트린혈증

PPIs 약물을 복용하면 위산 분비를 감소시켜 위 내 산도가 올라간다. 이는 피드백 기전에 의해 전정부 D세포의 somatostatin 분비억제, G 세포의 가스트린 분비촉진을 유도하여 고가스트린혈증을 유발시킨다[14] (Fig. 1). PPIs 약물에 의해 유발된 고가스트린혈증은 대개 중등도 정도(50-400 pg/mL)로 대부분 약물을 중단하면 정상화되지만, Helicobacter pylori (H. pylori) 감염이나 위축성 위염이 동반된 경우 혈중 가스트린은 400-4000 pg/mL까지 증가할 수 있다[14]. 이러한 고가스트린혈증은 위체부에서 oxyntic cell hyperplasia와 parietal cell mass의 증가, 샘조직 확장과 enterochromaffin-like (ECL) 세포를 자극하여 혈중 chromogranin과 histamine의 농도가 상승한다[15]. 이러한 변화가 장기적으로 지속되면 위 내 다양한 세포의 생리적 변화를 야기하여 위저선 폴립, 유암종, 위암과 연관될 수 있다는 가능성이 동물이나 세포실험을 통해 제기되고 있다.

PPIs 약물의 사용과 위저선 폴립(fundic gland polyp, FGP)의 연관성

최근 서구에서 위저선 폴립은 증식성 용종보다 흔하게 위에서 발견되어 가장 흔한 폴립으로 보고되고 있다(Fig. 2) [16]. 가족력이 없는 산발성 위저선 폴립과 가족성 선종성 폴립증(familial adenomatous polyposis, FAP) 환자에서 발생하는 가족성 위저선 폴립의 두 가지 형태로 나눠진다[16,17]. 두 가지 모두에서 조직학적 차이는 없지만 산발성 위저선 폴립보다 FAP 연관 위저선 폴립에서 이형성증이 더 흔하다(sporadic FGP: 1%, FAP-associated FGP: 30-50%) [18-20]. 병인론적으로 산발성 위저선 폴립은 베타 카테닌 발현과 연관되고 FAP 연관 위저선 폴립의 경우는 APC 유전자의 변이에 따른 불활성화(mutational inactivation)와 연관된다[21,22]. 1992년 Graham 등이 omeprazole 사용 중 3명의 환자에게 발생한 위저선 폴립을 발표한 이후 PPIs 약물의 장기간 사용과 위저선 폴립의 연관성에 관한 여러 연구가 있었으며 장기간 PPIs 약물의 사용이 위저선 폴립을 약 4배 정도 증가시킨다고 보고하고 있다[18,22,23]. 이러한 PPIs 약물의 장기간 사용과 관련된 위저선 폴립은 H. pylori 감염이 없는 경우에 더 흔하게 발생되는 것으로 알려져 있어 흥미롭게도 H. pylori 감염이 위저선 폴립의 형성에 예방효과가 있다는 가능성도 제기되었다[14]. 이는 H. pylori의 단백 분해효소로 인해 위 점막의 효소가 분해되어 위저선의 유출을 촉진하고 이로써 위저선의 낭성 확장을 막는 것으로 설명되고 있다[24-26]. FAP와 관련 위저선 폴립은 이형성이나 선종이 동반되는 경우가 많은 반면에 PPIs 약물 사용과 관련된 산발성 위저선 폴립에서는 이형성의 빈도가 매우 적고 동반된 경우도 대부분은 저도 이형성이다. 아주 드물게 PPIs 약물의 사용과 연관된 산발성 위저선 폴립에서 조직검사에서 고도 이형성이 발견되었다는 증례보고가 있으나 아직까지 위암과 연관되었다는 보고는 없다[18,22,27]. 또한 몇몇 연구에서는 PPIs 약물 사용을 중지하였을 때 위저선 폴립의 크기가 감소하였다는 보고도 있다[28,29]. 따라서 장기간 PPIs사용에 따른 위저선 폴립 증가의 임상적인 의미는 미미하다고 판단되며 다만 FAP 환자에서 PPIs 약물을 장기간 사용 시에는 적절한 간격의 주기적인 추적 내시경 검사가 필요하다.

PPIs 약물의 사용은 위 유암종(carcinoid tumor) 발생을 증가시키는가?

혈중 가스트린은 강력한 산분비 자극뿐 아니라, ECL세포의 기능을 항진시키고 성장을 촉진하는 역할을 한다[30-33]. 고가스트린혈증에 따른 ECL세포의 과증식 상태에서 이형성이나 유암종으로의 변화는 서서히 진행되므로 PPIs 사용에 따른 유암종과의 직접적인 인과관계는 아직까지 밝혀진 바가 없다. 그렇지만 인체에서 PPIs의 장기 복용에 따른 ECL 세포 증식의 표지자인 chromogranin A가 증가한다는 연구 결과[15,30]와 쥐를 이용한 동물실험에서 PPIs와 연관된 장기간 산 분비 억제로 유암종이 발생이 보고되어[34] PPIs와 유암종과의 연관성이 제기되었다. 또한 고가스트린혈증을 보이는 졸링거엘리슨 증후군(Zollinger-Ellison syndrome, ZES)과 위축성 위염을 보이는 다발성 내분비종양(multiple endocrine neoplasm1, MEN1) 환자에서 유암종이 발생했다는 증례 보고[35]는 PPIs와 직접 관련된 것은 아니지만 장기간 PPIs 복용에 따른 고가스트린혈증으로 유암종 가능성을 높인다는 의견이 제시되었다. 하지만 이러한 보고는 ZES나 MEN-1과 같이 장기간 고가스트린혈증이 유지되는 유전적 소인을 가진 환자에서 매우 드물게 나타나고 PPIs 약물과 직접적인 인과관계가 증명되지 않았다. 또한 인체에서 일반적인 장기간 PPIs 약물 사용이 이형성이나 유암종을 유발한다는 보고도 없는 상태이다. 최근 대규모 역학연구를 통해 위장관 유암종이 증가하는 추세를 보이고 있다[36]. 특히 위 유암종은 근래 20여 년간 PPIs의 처방이 급속히 증가한 서구를 중심으로 유병률이 높아져 1970년도에 비해 2000년도에는 약 4배 가량 증가한 것이 보고되었다[37,38]. 이러한 유병률 증가가 내시경 등의 진단기기의 발전이나 암검진 등의 부산물인지 아니면 PPIs 약물의 장기사용과 인과 관계가 있는지는 향후 전향적인 대규모 연구가 필요할 것이다.

PPIs 약물의 사용은 위축성 위염이나 위암의 발생을 증가시키는가?

장기간 PPIs 약물을 사용하는 환자에게 있어서 H. pylori 감염이 동반되지 않을 때는 PPIs 자체가 이미 존재하는 위염을 악화시키거나 위축성 위염을 조장하지 않는다[39-41]. 오히려 이미 존재하던 위염을 호전시키기도 한다[42]. 하지만 H. pylori 감염이 동반된 경우 PPIs의 장기간 복용은 전정부에 국한되었던 위염을 위체부로 이동시킨다[43,44]. 1996년 Kuipers 등[45]은 최초로 장기간 omeprazole을 복용하는 환자에서 H. pylori 감염이 동반된 위축성 위염이 위암의 전구 병변이라 주장하였고, Uemura 등[46]은 H. pylori 감염과 함께 위체부에 위축성 위염이 존재할 때 위암의 빈도가 높다고 보고하였다. 아울러 PPIs 약물의 장기사용에 따른 위암의 연관성에 대한 추가 연구결과가 발표되었다[47,48]. 하지만 Lundell을 포함한 다른 연구자들은 장기간 PPIs 사용과 위 체부의 위축성 위염과의 연관성을 발견하지 못해[49-51] 현재도 논란이 되고 있다. 최근 Kuipers 등[52]은 무작위 비교 연구를 통해 장기간 PPIs 사용이 전정부 및 체부의 위염 활성도나 위점막 위축에 영향이 없으며, H. pylori를 제균함으로써 위점막 염증을 줄이고 위 체부 위선의 위축을 회복시킬 수 있다는 연구결과를 발표하였다. 따라서 아직까지 PPIs의 장기 사용이 위암을 유발시킨다는 근거는 명확하지 않다.

결 론

PPIs 약물은 지난 20년간 위산 관련 질환에서 위장관 점막 치유와 증상 완화를 통하여 삶의 질까지 향상시키는 매우 효과적이고 안전한 약물로 입증되어 왔다. 하지만 최근 PPIs의 복용 환자가 증가하면서 PPIs의 장기복용에 따른 고가스트린혈증과 관련된 위 종양의 가능성이 이슈화되고 있다. 하지만 이러한 PPIs 연관 고가스트린혈증과 연관된 위암은 인체에서 보고된 바 없으며 직접적인 인과관계가 증명되지 않았다. 따라서 향후 전향적 무작위비교 연구를 통한 근거가 제시되기까지는 위산 관련 질환에서 산분비 억제의 이점이 뚜렷한 만큼 안전하게 처방될 수 있는 약제라 판단된다. 단, PPIs 약물을 처방하는 의사는 장기복용에 따른 다양한 위 종양의 가능성을 고려하여 고위험의 장기 복용 환자에서 주기적인 내시경 검사와 더불어 최소한의 유효 용량 처방과 적절한 시기에 적절한 용법으로 tapering하려는 노력이 필요하다.
