Abstract |
A 40-year-old male who had been undergoing dialysis for 21 years was investigated for progressive hypercalcemia in the context of very high intact parathyroid hormone (PTH) levels. Three years earlier, he had developed secondary hyperparathyroidism and had undergone a partial parathyroidectomy. Imaging showed a functional parathyroid gland in the inferior portion of the left thyroid gland. Cervical exploration was performed with a total parathyroidectomy and autotransplantation of the normal gland into the forearm. The histology of the inferior parathyroid gland showed invasive parathyroid carcinoma. Parathyroid carcinoma is rare in the normal population and in hemodialysis patients; only 21 cases involving dialysis patients have been reported. This is the first report in Korea. We review the diagnostic and therapeutic information on the management of parathyroid carcinoma in the setting of hemodialysis. |
