A quadricuspid aortic valve associated with severe aortic stenosis and moderate aortic regurgitation

Abstract
A quadricuspid aortic valve (QAV) is a rare congenital malformation of the aortic valve first described in 1862 by Balington at necropsy. A 72-year-old woman presented with dyspnea on exertion (NYHA functional class II) that had begun several years earlier. Her chest X-ray showed mild cardiomegaly, but no pulmonary edema. The Doppler and color examination of the aortic valve at transthoracic echocardiography showed QAV with severe aortic stenosis (AV area=0.65 cm2 by the 2-D method) and moderate aortic regurgitation. Her coronary angiography was normal and she was referred for surgery. In the operating room, preoperative transesophageal echocardiography confirmed the QAV, and she underwent aortic valve replacement with a mechanical prosthesis. Although over 200 cases of QAV have been reported in other countries, this is the first reported case of QAV associated with severe aortic stenosis and moderate aortic regurgitation in Korea.
