A case of successful percutaneous stenting in spontaneous isolated dissection of the superior mesenteric artery |

Abstract |
Spontaneous, isolated dissection of the superior mesenteric artery (SMA) not associated with aortic dissection is a rare occurrence and is a cause of acute mesenteric ischemia. Early diagnosis is critical for a good outcome in this disease, because mortality is significant once bowel infarction has occurred. Because of the unreliability of the clinical symptoms and signs of isolated SMA dissection, various imaging modalities, including ultrasonography, contrast-enhanced computerized tomography (CT), and magnetic resonance angiography, are crucial to making an early diagnosis. The mainstay of treatment of isolated dissection of the SMA has been surgery. Recently, promising results have been reported with stenting in this disease. An 80-year-old male presented with postprandial epigastric pain, and isolated dissection of the SMA was shown on abdominal CT scan. After percutaneous stent placement, his symptoms were remarkably improved. We report a case of spontaneous isolated dissection of the SMA which was successfully treated by stent placement, along with a review of the literature. |
